  • $Br{\aa}nemark^{\prime}s$ original protocol required 4 to 6 months for implant osseointegration before placement of the definitive prosthesis. Although this approach gave very predictable results, it had certain drawbacks. The main disadvantages of this approach were prolonged treatment time, two surgical procedures, placement of a removable prosthesis that required modifications during the course of treatment, and a greater number of appointments. Immediate implant loading is a viable treatment method for selected cases. One of the greatest advantages of this method is the virtual surgery, which precedes the actual clinical treatment and eliminates any need for last minute decisions. The actual surgery time is decreased, since all steps are predetermined. These advantages aren't only more useful for normal patients but also for the mental retardation patients whose cooperation is difficult. This article presents a clinical approach made possible due to the guided implant surgery and CAD/CAM technique.

  • Medical implant devices are one of the targets of the US's Food & Drug Administration (FDA) for tracking in case of a serious adverse event since they are directly connected to the lives of patients. The US law stipulates that the public health agency shall order implantable device makers to track their product down to the patient level if a serious adverse event has occurred or defects have been discovered; in reality, however, the agency can pass on the responsibility for tracking or recalling faulty devices to the manufacturers or use mass media. This article proposes an efficient tracking and recall management system and examines four main virtual scenarios based on such. This research seeks to suggest a system that enables FDA to perform accurate and prompt tracking and recall management for patients' enhanced safety.

Psycho-therapeutic Elements in Serious Game for Therapy (치료용 기능성 게임에서의 심리 치료적 요소)

  • The purpose of serious game for therapy is not to cure patients directly, but to have them cause psychological change, and get positive treatment will. In this study, we extracted 5 elements to be considered to design efficient serious games for psychological therapy through precedent data's analysis. For verification of the extracted elements, we intensively analyzed 2 elements. Those are 'Re-Mission' of which the effect was already proven, and 'SIMS2' which was designed to get the effect of treatment which patients who may experience difficulties in real-world can obtain in virtual space through previous experiences. As a result, material stability, reality, consistency with characters and self-determination of game progress are applied to all the analyzed games, But interactivity with reality is not applied. However, previous researches and validated questionnaire show that interactivity with reality is appropriate element. Consequently, the results of this study will be important materials in designing more efficient serious games for therapy.

Korean Clinic Based Outcome Measure Studies (한방 병-의원에서 하는 임상지표 연구)

  • Background: Evidence based medicine has become main tools for medical practice. However, conducting a highly ranked in the evidence hierarchy pyramid is not easy or feasible at all times and places. There remains a room for descriptive clinical outcome measure studies with admitting the limit of the intepretation. Aims: Presents three Korean clinic based outcome measure studies with a view to encouraging Korean clinicians to conduct similar studies. Methods: Three studies are presented briefly here in치uding 1) Quality of Life of liver cancer patients after 8 Constitutional acupuncture; 2) Developing a Korean version of Measuring yourself Medical Outcome profile (MYMOP); and 3) Survey on 5 Shu points: a pilot In the first study, we have included 4 primary or secondary liver cancer patients collecting their diagnostic X-ray film and clinical data f개m their hospital, and asked them to fill in the European Organization Research and Treatment of Cancer, Quality of Life Questionnaire before the commencement of the treatment. The acupuncture treatment is set up format but not disclosed yet. The translation and developing a Korean version of outcome measures that is Korean clinician friendly has been sought for MYMOP is one of the most appropriate one. The permission was granted, the translation into Korean was done, then back translated into English only based on the Korean translation by the researcher who is bilingual in both languages. The back translation was compared by the original developer of MYMOP and confirmed usable. In order to test the existence of acupoints and meridians through popular forms of Korean acupuncture regimes, we aim at collecting opinions from 101 Korean clinicians that have used those forms. The questions asked include most effective symptoms, 5 Shu points, points those are least likely to use due to either adverse events or the lack of effectiveness, theoretical reasons for the above proposals, proposing outcome measures, and the time from insertion to be effective. The questionnaire is posted on the virtual notice board of the homepage of the communication KOMA (The official tool of Association of Korean Oriental Medicine).

Communication Aid System For Dementia Patients (치매환자를 위한 대화 보조 시스템)

  • The goat of the present research is to improve the quality of life of both the elderly patients with dementia and their caregivers. For this Purpose, we developed a communication aid system that is consisted of three modules such as speech recognition engine, graphical agent. and database classified by a nursing schedule. The system was evaluated in an actual environment of nursing facility by introducing the system to an older mail patient with dementia. The comparison study was then carried out with and without system, respectively. The occupational therapists then evaluated subject"s reaction to the system by photographing his behaviors. The evaluation results revealed that the proposed system was more responsive in catering to needs of subject than professional caregivers. Moreover we could see that the frequency of causing the utterances of subject increased by introducing the system.

A pilot study of augmented reality-based postural control training in stroke rehabilitation

  • Objective: The purpose of this study was to determine the effects of Augmented Reality-based Postural Control (ARPC) training on balance and gait function in patients with stroke. Design: Single-blind randomized controlled trial. Methods: Twenty participants who experienced a stroke were enrolled in the study and randomly assigned to the ARPC (n=10) or control group (n=10). Subjects in both groups received conventional physical therapy for 60 min per session, 5 days per week, for 4 weeks. In addition, subjects in the ARPC group received ARPC training for 30 min per day, 3 days per week, for 4 weeks. The participants watched established normal postural control patterns on a head-mounted display and repeated the movements in ARPC training. Outcome measurements were assessed using the Berg Balance Scale (BBS) and 10-Meter Walk Test (10MWT) before and after 4 weeks of training. Results: Of the 20 randomized participants, only 18 completed the 4-week training program. The ARPC group showed significant improvement in the BBS and 10MWT after training (p<0.05). Meanwhile, the control group did not exhibit improvement in either variable. In addition, the ARPC group showed significantly greater improvement than the control group in the 10MWT (p<0.05), whereas no significant difference was observed between the groups for the BBS. Conclusions: The results of this study confirmed the benefits of ARPC training on dynamic balance and functional gait ability. Additionally, this study may provide evidence supporting the use of an ARPC training program for improving balance and gait ability in patients after a chronic stroke.

  • A systematic review was conducted to identify components and convergent effects of simulation program using schizophrenia scenario in nursing education. Using 4 different databases, 226 articles were identified and 11 studies met the inclusion criteria. There were 5 qualitative studies, 5 quantitative studies and 1 study used mixed method design. The simulation incorporated various methods including standardized patients, role playing, simulator and virtual reality that majority studies(63.6%) used standardized patients. For the evaluation, studies examined diverse variables including knowledge, learning self competency, learning satisfaction and self directed learning. Considering complexity and difficulty of nursing for schizophrenia, future studies with well designed simulation program are required to prove its effectiveness.

  • To verify internal movements of the body, a DICOM file obtained from CT and a Geant4 code were used to simulate lung cancer patients. In addition, the method is applied to measure the movement of tumor when the movement of t he tumor is located inhale and exhale by creating a virtual tumor in the self-produced moving phantom, and to check the distribution of dose in the treatment plan and the accuracy of tumor in PTV for respiratory and lung cancer patients. It was confirmed that 97% or more respiratory control radiation therapy was effective even if the moving area was more than 3cm, in the 40% to 70% range. Dose distribution with respiratory radiation therapy applied to moving targets, measured by film in the actuation phantom, was shown to be within a 3mm margin of error for dose distribution containing 90%. It was confirmed that for actual patient breathing curves, the treatment time may be shorter than that due to the longer expiratory time.

  • Advancements in CAD-CAM technology have allowed for a variety of options in implant-supported fixed prosthesis restorations for completely edentulous patients. For patients with severe alveolar bone resorption, zirconia prosthetic restorations that include the gingival portion, such as implant-supported hybrid dentures, are possible, as well as monolithic zirconia prosthetic restorations in the form of crowns. The integration of digital technology in fixed prosthetic restoration using implants enables predictable treatment through diagnosis and virtual surgery by integrating the patient's clinical and radiological information. In this case, the utilization of digital software allowed for the establishment of implants in appropriate positions, considering both the remaining alveolar bone and the final prosthesis. By designing both the maxillary removable prosthesis and the mandibular fixed prosthesis simultaneously in CAD software, bilateral balanced occlusion could be easily achieved. Furthermore, maintaining the initial interocclusal relationship from provisional restorations to final prostheses allowed for convenient transition while ensuring functional and aesthetic satisfaction.

Evaluation of the combination of Bone Scan Image and Pelvic X-ray Image (뼈 검사 영상과 골반 X선 영상의 결합 유용성 평가)

  • Purpose The introduction of bone scan has been reported as a useful tool in the diagnosis, treatment, and treatment response of skeletal disease. The purpose of this study is to improve the anatomical information and tolerance of the bone by combining bone scan and pelvic X-ray without additional radiation exposure. Materials and Methods From November 2015 to August 2016, 236 patients(64 men and 172 women, average age $50.96{\pm}15.39years$) take Bone scan and Pelvis AP(Anteroposterior) X-ray scan at the National Cancer Center. The scan equipment was a gamma camera, Symbia Ecam (SIEMENS, Germany), and a digital x-ray, DRS-800 (Listem, Korea). Osirix version 3.8.1 (Osirix, USA) and Stata/SE version 14.0 (StataCorp, USA) were used for image combination and analysis. The patient was intravenously injected with $^{99m}Tc-DPD$ (740 MBq), and the scan was performed 2 to 4 hours later. Gamma camera image acquisition were Matrix size $256{\times}1024$, Zoom 1.00, and scan speed 17 cm/min. The digital X-ray was made with a collimator size of $14^{{\prime}{\prime}}{\times}17^{{\prime}{\prime}}$, 77 kVp (60 to 97 kVp) and an average of 30 mAs (20 to 48). ASIS and pubic symphysis Select virtual points then Combine three virtual points and pelvic contour lines. The acquired images were evaluated by three radiologists who worked for more than 5 years in the nuclear medicine department. Results Of the total 236 patients, 216 (91.53%) were matched. The median and range (min~max) of the age were 67 (46~81) years old in the unmatched group and 52 (22~87) years old in the matched group, The Wilcoxon rank-sum test was performed to determine whether age was different between the two groups. As a result, the age difference between the two groups was statistically significant at p < 0.0001. Of the 64 men, 60 (93.75%) were match and of the 172 women, 156 (93.75%) were match. There was no statistically significant difference according to gender(p = 0.4542). Of the 54 patients without pelvic lesions, 54 (100.00%) were match, and 162 (89.01%) of 182 patients with pelvic lesions were match. There was a statistically significant difference according to the presence of pelvic lesions. Conclusion There are many variables in the combination of bone scan and pelvic X-ray imaging, and the patient's age and pelvic lesion may have some effect on the image combination. This study is expected to be useful for the diagnosis of pelvic osteosarcoma of children without radiation exposure. It is expected that this combination of images will help to develop the nuclear medicine image.
