Development of a vestibulo-ocular reflex measurement system for the study of cybersickness (사이버멀미 경감 연구를 위한 전정안구반사 측정 시스템 개발)

  • Vestibulo-ocular reflex (VOR) is a compensatory response of the extraocular muscles generated by vestibular signals to stabilize images on the retina during head/body movements. It has been reported that mismatches between retinal and vestibular information, which cause motion sickness or cybersickness, modify VOR. To investigate the characteristic changes of VOR in subjects experiencing cybersickness, we developed a low-cost, multi-purpose VOR measurement system using LabVIEW and Arduino. To test the applicability of the system, we performed two experiments. In Experiment 1, horizontal and vertical VORs of four participants were measured using a vestibular autorotation task. In Experiment 2, eight participants were exposed to a virtual navigation to measure changes of VORs as an index of cybersickness. We observed significantly greater head rotations and eye movements while the participants were exposed to the virtual navigation than to a static image. The results suggest that the present system can help understand the psychophysiological mechanisms of cybersickness symptoms.

The Effects of Sensory Integrative Therapy on Vestibulo-Proprioceptive Sensory Processing of Children With Asperger Syndrome (감각통합치료가 아스퍼거 아동의 전정.고유감각 처리능력에 미치는 효과)

  • Objective : This study verifies the effects of sensory integrative (SI) therapy on vestibular- and proprioceptive sensory (BPS) processing ability of a child with Asperger Syndrome (AS). Method : A boy who is 11 years and 2 months old took the Functional Independence Measure for Children (Wee-FIM), Canadian Occupational Performance Measure (COPM), Short Sensory Profile, Bruininks-Oserestky Test of Motor Proficiency-2 (BOT-2), and Test of Playfulness (ToP) for the baseline. The child participated in 3 evaluation sessions and 8 therapy sessions based on the AB research design. Duration of each session is 50 min and the therapy session is divided into 40 minutes for treatment and 10 minutes for evaluation. Since the vestibular sense and proprioception build up one's ability of postural control, several tests were employed to evaluate the child' postural control as outcome measure; distance from front leg of chair to heel of the child with sitting (C-H distance), angle between trunk and thigh (hip joint angle) with sitting, and the 'prone-extension posture' which is a subtest of Clinical Observation of Motor and Postural Skill (COMPS) to examine postural control embodied with integration of reflex and BPS processing. Result : During the therapy, average data of the C-H distance is decreased from 27.33cm to 11.69cm, average data of the hip joint angle is also decreased from $43.3^{\circ}$ to $20^{\circ}$, and average time for the prone-extension posture is increased from 13.15seconds to 24.84seconds. Conclusion : This result indicates that the ability to postural control in sitting and to maintain the prone-extension posture can be improved by sensory integrative therapy, with enhanced BPS processing.

Effects of [D-$Pen^2$, D-$Pen^5$]-enkephalin on the Neuronal Activity of Medial Vestibular Nuclear Neurons

  • This study was designed to investigate direct effects of [D-$Pen^2$, D-$Pen^5$]-enkephalin, a $\delta$-opioid receptor agonist on the neuronal activity of medial vestibular nuclear (MVN) neurons by whole-cell configuration patch clamp experiments. The spike frequency of MVN neuron was increased to $9.50{\pm}0.55$ (P<0.05) and $10.56{\pm}0.66$ (P<0.05) by 5 and $10{\mu}M$ [D-$Pen^2$, D-$Pen^5$]-enkephalin from the control level of $8.05{\pm}0.55$ spikes/sec, respectively (n=18). The resting membrane potential of the neurons was increased to $-37.86{\pm}0.92$ and $-36.97{\pm}0.97$ (P<0.05) from $-38.74{\pm}1.13\;mV$ by 5 and $10{\mu}M$ [D-$Pen^2$, D-$Pen^5$]-enkephalin, respectively. The amplitude of afterhyperpolarization was decreased to $23.78{\pm}0.65$ and $21.67{\pm}0.89$ (P<0.05) from $23.73{\pm}0.53\;mV$ by 5 and $10{\mu}M$ [D-$Pen^2$, D-$Pen^5$]-enkephalin, respectively. The spike width was changed to $2.22{\pm}0.08$ and $2.24{\pm}0.07$ from $2.20{\pm}0.08\;mV$ by 5 and $10{\mu}M$ [D-$Pen^2$, D-$Pen^5$]-enkephalin, respectively. After pretreatment of naltrindole, a highly selective 8-opioid receptor antagonist, [D-$Pen^2$, D-$Pen^5$]-enkephalin did not change firing rate, resting membrane potential, afterhyperpolarization amplitude, and spike width of MVN neurons. The above experimental results suggest that [D-$Pen^2$, D-$Pen^5$]-enkephalin increases the neuronal activity of MVN neurons via inhibition of calcium-dependent potassium currents underlying the afterhyperpolarization.

Factors Related to Balance Ability in Healthy Elderly (노인의 균형유지 능력에 영향을 미치는 요인)

  • The purposes of this study were to ascertain differences of the sensory system, central processing system, effector system, and balance ability according to general characteristics (sex, fall experience, and age), and to identify the contributions of the sensory, central processing, and effector system to balance ability in 83 healthy elderly subjects. The subjects were elderly (over 60 years) who live in Wonju City. All subjects participated in six tests (position sense test, visual acuity, vestibular stepping test, Mini-Mental Status Exam-Korea, strength of dorsiflexors, and Berg Balance Test). Collected data were analyzed by the independent t-test, one-way ANOVA, and stepwise multiple regression. The results were as follows: 1. The results of the six tests according to sex showed that vestibular function was significantly better in women than in men. But visual acuity, cognitive function, strength of dorsiflexors, and balance ability were significantly better in men than in women (p<0.05). 2. There were no significant differences in position sense, visual acuity, vestibular function, cognitive function, strength of dorsiflexors, and balance ability between fallers and non-fallers (p>0.05). 3. However, the results of the six tests according to age (group A: 60-69 years, group B: 70-79 years, group C: 80-89 years) showed significant differences in five tests. In the Scheff$\acute{e}$ test, position sense, visual acuity, and cognitive function showed significantly different results between in group A and group C: the strength of the dorsiflexors was significantly different between in group A and group B, and balance ability was significantly different between in group A and group C, and between in group B and group C (p<0.05). 4. Cognitive function and position sense were positively associated with balance ability. Their power of explanation regarding balance ability was 38% (p<0.05). These results suggest that cognitive function and position sense play an important role in balance ability. This would seem to suggest that programs for evaluation or improvement of elderly's balance ability in the elderly should consider cognitive function and position sense.

TREATMENT OF HEAVY MANDIBULAR BUCCAL FRENUM USING APICALLY POSITIONED PARTIAL-THICKNESS FLAP IN CHILD (치아 교환기 어린이에서 근단변위부분층 판막술을 이용한 하악 거대 협소대의 처치)

  • The mandibular buccal frenum becomes a problem when its attachment is too close to the marginal gingival. Heavy buccal frenum mucogingivally results in insufficient attached gingiva, inadequate vestibular depth and also difficulty in eruption of mandibular premolar. The aim of this study is to demonstrate the effectiveness of apically positioned partial-thickness flap as adequate method in order to remove heavy buccal frenum and expose impacted teeth. The author presents the case of a 12-year-old boy. He had problems that was prolonged eruption of #34, #35 due to high pull buccal frenum. After space regaining for normal eruption of #34, #35, we performed apically positioned partial-thickness flap on #34, 35 area. As a results, we confirmed that eruption of #34, #35 has been processed normally And vestibular depth, position of buccal frenum and width of attached gingival was within a normal range. Decrease in muscle pull, adequate width of attached gingiva and increased vestibular depth can be expected from this treatment.

  • Streptococcus vestibularis is a urease-producing oral bacterium. frequently isolated from vestibular mucosa of human oral cavity. Ureolysis by S. vestibularis and other ureolytic oral bacteria is believed to be crucially involved in oral microbial ecology and oral health. Genomic library of the S. vestibularis ATCC49124 was constructed in an E. coli plasmid vector and the urease-positive transformants harboring the urease gene cluster were isolated on Christensen-urea agar plates. The minimal DNA region required for the urease activity was located on a 5.6 kb DNA fragment. (omitted)

  • In the treatment of maxillary lesion including a maxillary sinuses, most of oral and maxillofacial surgeons have used the vestibular incision or the Weber-Fergusson incision. However, the vestibular incision has disadvantage which it provides a rather limited exposure and the Weber-Fergusson incision leaves visible scar in the midface. Furthermore, because the scar is confined on unilateral side only, the technique is hesitated to apply especially in children, younger patients and women. Since Casson first introduced midfacial degloving technique, this approach has been used frequently to treat the lesion on nasal cavity, nasopharynx, skull base and paranasal sinuses by ENT surgeons. But, we think this technique can be used usefully in oral and maxillofacial regions. So, we experienced favorable results which it provided a proper exposure, no visible facial scar and it could be used on bilateral midfacial lesions.

Two Cases of Improved Dizziness and Ataxia due to Pontine Stroke in Bojungikki-tang-gamibang (보중익기탕가미방(補中益氣湯加味方) 투여를 통한 뇌교병변 현훈과 보행실조 환자 치험 2례)

  • Bojungikki-tang-gamibang is a widely used herbal prescription in traditional medicine in Korea. The aim of this study is to investigate the effectiveness of Bojungikki-tang-gamibang for patients who have pontine stroke. They have problems with type of Qi deficiency(氣虛) in the rehabilitation stage. The type of Qi deficiency caused the dizziness and ataxia for the patients. In this study there are two types of patients. The first case is a patient with dizziness and ataxia because of potine hemorrhage. The second case is a patient who has the same symptoms as the first one but caused by pontine infraction. We treated the two patients with Bojungikki-tang-gamibang. The Improvement of symptom was evaluated by Korean vestibular disorders activities of daily living scale(K-VADL). As a result the degree of dizziness decreased noticeably while other symptoms improved as well. This study suggests that Bojungikki-tang-gamibang may be an effective treatment for patients who suffer stroke with type of Qi deficiency(氣虛) in the rehabilitations stage. However, more cases and methods of diagnosis are required to prove that the oriental medicine, Bojungikki-tang-gamibang can apply to patients universally.

A Review on the Mechanism of Human Postural Control (인간의 자세조절 메커니즘에 대한 연구)

  • Stance is defined as any state in which the total mass of the body is supported by the feet. In order to maintain stance, the sum of gravito-inertial forces acting on the body must be registered by equal and opposite forces at the region of contact between the organism and the support surface. Balance is controlled by applying forces to the surface of support so as to maintain the body's center of mass vertically above the feet. for a muIti-segment organism, there can be a variety of ways in which balance can be controlled, since movements of different body segments can have similar effects on the control of balance. In general, the organism tends to have a body configuration that is aligned with gravito-inertial force when there are no external forces acting on it. If any segments of the body are not aligned with gravito-inertial force vector, a torque on that segment would tend to move the body's center of mass. The maintenance of postural stability is accomplished in humans by a complex neural control system. This requires organizing integrating and acting upon visual, vestibular, and somatosensory input, providing orientation information to the postural control system. The information necessary to control and coordinate movement is provided by the visual sense of eye position with respect to the surrounding surface layout, the vestibular sense of head orientation in the gravito-inertial space, and the somatic sense of body segment position relative to one another and to the support surface. In this study, perception and action capability was examined from various points of view. The underlying assumption of the study was that the change of postural configuration could be effected by organism, environment and task goal.

Comparison of the Static Balance Ability according to the Subjective Visual Vertical in Healthy Adults

  • Purpose: The subjective visual vertical (SVV) test is used to evaluate the otolith function in the inner ear. This study compared the different balance ability according to the results of the SVV in healthy adults. Methods: This study recruited 30 normal healthy subjects who did not have neurological and musculoskeletal disorders. The subjects were divided into experimental and control groups based on the results of SVV: experimental group, >2°; control group, <2°. The static balance ability was evaluated using the Fourier Index, which could evaluate the balance capacity objectively and quantitatively. Results: The mean angle of the SVV in the experimental and control groups was 4.44° and 0.59°, respectively. In the result of the Fourier series, the F1 frequency band in the experimental group showed a significantly higher value under one condition compared to the control group (p<0.05). In the F2-4 and F5-6 frequency bands, the experimental group showed a significant increase in the Fourier series value under the four conditions compared to the control group (p<0.05). In the F7-8 frequency band, significantly higher values of the Fourier series were observed in the experimental group under the three different conditions (p<0.05). Conclusion: These results showed increased trunk sway while maintaining static balance in the experimental group who showed a larger SVV angle compared to the control group. The SVV can be applied to evaluate the vestibular system and balance ability in normal adults.
