A Study on Sleep in Rheumatoid Arthritis Patients (류마티스 관절염 환자의 수면 및 관련 요인에 관한 연구)

  • This study is to investigate sleep patterns of rheumatoid arthritis patients through a survey research. The subjects for this study were 97 patients registered in Hanyang University Hospital Rheumatoid Arthritis Center, and the period of data collection was from July 15, 1998 to August 30, 1998. The research instruments used in this study were the measures of sleep, pain, and fatigue, and SPSSWIN 8.0 Program was used for data analysis. The research results are as follows ; The patients went to bed between 11 and 12 p.m., but many of them found difficulty in falling asleep within 5 minutes. They woke up quite early at around 4 to 6 o'clock in the morning and remained in bed about 1 hour. Only 39 percent of the subjects reported satisfaction with their sleep. Fifty six percent of the subjects took a nap, generally did in the afternoon and 22.7 percent of them napped for half an hour. They suffered sleep disturbance, but their sleep environments were calm and comfortable, and they turn off the light when they went to sleep. As for the quality of sleep, over 50 percent of the subjects reported not being able to sleep deeply, 30 percent of the subjects woke up frequently during sleep, 60 percent experienced frequent arousal after sleep onset. Over 90 subjects slept for 6 to 8 hours. This shows that even though they had rheumatoid arthritis, the patients remained in bed for a sufficient period of time. They also reported waking up or turning frequently during sleep. The sense of fatigue from sleep disturbance scored a relatively high 35.84 points on average against the possible score of 64 points. Behavior for sleep promotion was very active. Sleep disturbance occurred in proportion to the sense of fatigue and pain, and was negatively correlated with quality of sleep. The pain had positive correlations with the illness duration, sleep disturbance and had a negative correlation with the quality of sleep.

Development and Effect of Learning Materials of Earth Science Using Simplifying Condition Method (단순화 조건법을 이용한 지질 연대 분야의 학습 자료 개발과 그 효과)

  • The purpose of this study was three-folded to suggest the Simplifying Conditions Method (SCM) as a means of task analysis and sequencing of instructional content, to develop teaching-learning materials by analyzing part of the geological time scale of the earth science and finally to analyze the effectiveness of this method. SCM began by simplifying a complex task into the basic components by eliminating various complexities, which produced a simple representative of the entire task. The next step was to relax conditions on the basic version one by one, thereby gradually introducing progressively more complex tasks to the students. This sequential strategy enabled students to understand the task holistically and to acquire authentic skills from very onset of the course. Moreover, Early mastery of skills enhances the effectiveness and efficiency of instruction. The result of this study revealed that instruction through SCM was more effective in developing students' self-directed learning characteristics and academic achievement than instruction through sequential task analysis methodology.

  • 1. King Hyeonjong(1641-1674) mainly suffered from eye disease and abscess. He specially took a hot spring bath for cure of eye and skin problems. He probably died of septicemia and the following gastro-intestinal infection at the age of 34, quite early for his age. 2. King Sukjong(1661-1720) was not very well all through his life, but lived quite longer than other Kings in the Joseon Dynasty. He suffered from various diseases like heart-based heat, abscess, edema, upper respiratory infections, etc. He frequently took the treatments of acupuncture and moxibustion. He presumably died of dysfunction of liver and kidney at the age of 60. 3. King Gyeongjong(1688-1724) suffered from political problems from birth to death, so he may have had excessive mental stress for his poor health. He mainly suffered from heart-based heat and abscess. It is quite not sure why he died in only one month from the onset of his symptoms, so many people thought that he was poisoned to death. He probably died of septicemia and the following gastro-intestinal infection at the age of 37. 4. King Yeongjo(1694-1776) lived for 83 years, which is the longest of all the Kings in the Joseon Dynasty. He mainly suffered from hypofunction of gastro-intestinal system with cold symptoms, coughs, uneasiness. He took various kinds of herbal medicine, of which he took ginseng the most. He is supposed to be dead due to his old age at 83.

Clinicopathological and p53 Gene Alteration Comparison between Young and Older Patients with Gastric Cancer

  • Background: Differences in clinicopathological characteristics of gastric cancer (GC) between young and older patients are controversial and a matter of debate. Determining the statistical significance of clinicopathological information with respect to age might provide clues for better management and treatment ofGC. Materials and Methods: A total ofl03 Indiao GC patients were enrolled for study and specimens were classified according to the AjCC-TNM system. Patients were grouped into two age-wise categories, young patients (<40 years; n=13) and older patients (${\geq}40$ years, n=90). The clinicopathological features of both groups were retrospectively examined and compared. p53 alterations were analyzed by polymerase chain reaction-single strand conformational polymorphism and immunohistochemistry methods at gene and protein levels respectively. The cases were considered p53 over-expressed if it was present in more than 25% of the tumor cells and p53 alterations was correlated with the clinicopathological characteristics of the patients as well as etiological factors for GC in both groups. Results: We found significant association of young patients with cancer stage (p=0.01), and very strong association with histology grade (p=0.064) and poorly differentiated (p=0.051) state of GC. However, neither young nor elderly patients showed associations with location, gender, etiological factors and p53 expression and alteration. Overall the male-to-female ratio of GC patients was 3.12 and the value was higher in the young (5.5) than in the older group (2.91). Conclusions: Clinicopathological features of GC like caocer stage, cell differentiation and histological grades were significantly different among young and old age cohorts. We observed a male predominance among the young group that decreased significantly with advancing age. More awareness of GC onset is required to detect cancer at an early stage for successful treatment.

Treatment of Deep Venous Thromboses of Lower Leg with Thrombolysis (혈전용해술을 이용한 하지 심부정맥 혈전증의 치료)

  • Background: Deep venous thrombosis(DVT) is a curable disease when it is appropriately treated in the early stages of onset. The long term follow up of chronic DVT shows poor prognosis with serious complications such as venous valvular insufficiency, venous claudication, venous ulcer and leg swelling. Thrombolytic therapy is a very active treatment that delivers thrombolytic agents via catheter to the target thrombi. The aim of this study is to evaluate the effect of catheter directed thrombolysis using urokinase to acute DVT. Material and Method: We studied 5 patients, who were diagnosed as acute DVT and had no contraindication for selective hemolysis using urokinase. Result: All the patients were successfully recanalized. Total infusion time of urokinase was 2.0$\pm$0.6 days, and the amount was 5.9$\pm$2.45 million IU. In 4 patients, who were diagnosed as May-Therner syndrome, we performed the balloon angioplasty and inserted the stent at the stenotic portion. There were minor complications such as hematuria, hematoma at puncture site, and all of them are self limited. Conclusion: Catheter induced thrombolysis is an effective treatment in acute DVT.

Cotyledon and Leaf Development Associated with Seeding Vigor of Six Forage Lerumes (여섯가지 두과목초의 유식물 활력과 관련된 자엽과 엽의 발달)

  • Cotyledon and leaf development of six important forage legums were compared to study their contribution to the seedling growth, Cotyledons of forage legumes expanded their size rapidly during one week maximum size was reached and entered senescence. Larger seeds produced greater colyledon area, and speed of colyledon expansion was closely associated with seedling growth. Earlier onset leaf production and earlier leaf development were major determinant factor initial seedling growth. Alfalfa and red clover which have larger seeds were good in seedling vigor with larger photosynthetic area. Alsike clover and white clover with small seeds showed also good seedling vigor, as they developed leaves early with relatively high photosynthetic rate. On the other hand, though lespedeza has heavy seeds, its seedling vigor was very poor with slow photosynthetic area development and low photosynthetic rate.

Risk Factors of Catheter-related Bloodstream Infection Due to Methicillin-resistant Staphylococcus aureus in Very Low Birth Weight Infants (극소저체중 출생아에서 메티실린 내성 포도상구균으로 인한 카테터 연관 혈류감염의 위험인자)

  • Purpose: The incidence of methicillin-resistant Staphylococcus aureus (MRSA) infection has increased in children and in neonates, and is particularly associated with frequent use of central venous catheter in very low birth weight (VLBW) infants. It is known that the morbidity and mortality of MRSA infection are low in neonates, as compared with adults. The objective of this study was to examine the difference in clinical characteristics between VLBW infants that survived and those that did not, a catheter-related bloodstream infection (CRBSI) of MRSA. Methods: Thirty-four VLBW infants had laboratory-confirmed bloodstream infection with S. aureus. We examined the incidence, mortality and morbidity of CRBSI, and predictive factors associated with mortality. Results: Twenty-six infants had same pathogen (24 MRSA, 2 Methicillin-sensitive Staphylococcus aureus) in the blood and in the catheter tip. Eight infants (25.8%) died in the CRBSI and they all had MRSA blood infections. Sex ratio, gestational age, duration between blood collection and identification of pathogens, and WBC and platelet count were not significantly different between patients that died from and patients that survived CRBSI of MRSA. C-reactive protein (CRP) was significantly higher in VLBW infants that died. Mean age of onset and hospital day was earlier (9.1${\pm}$6.6 vs. 26.9${\pm}$20.2; P=0.005) and shorter for patients that died (10.1${\pm}$7.0 vs. 73.0${\pm}$32.4; P=0.000). Two survivors had complications of pyogenic arthritis of the lower extremities and soft tissue infection, respectively. Conclusion: Mortality of CRBSI was likely to be high in VLBW infants and might be anticipated by CRP and early onset of disease.

Changes of Fruit Characteristics and Cell Wall Component during Maturation and Ripening in Asian Pear 'Hanareum', 'Manpungbae', and 'Niitaka' (Pyrus pyrifolia Nakai) ('한아름', '만풍배' 및 '신고' 배의 성숙 기간 중 과실특성 및 세포벽 관련물질의 변화)

  • We investigated the changes of fruit quality parameters, polysaccharide contents and cell wall components during maturation and ripening of two Korean pear cultivar 'Hanareum' and 'Manpungbae' compared with 'Niitaka' pear (Pyrus pyrifolia Nakai) which showed different physiological maturity based on days after full bloom (DAFB). Flesh firmness decreased continuously with fruit development and maturation, reaching a final level of 29.4, 33.5, and 27.4N at maturity in 'Hanareum' (127 DAFB), 'Manpungbae' (163 DAFB), and 'Niitaka' (170 DAFB), respectively. The level of ethylene production was very low in early season 'Hanareum' pear which showed at most 0.39 ${\mu}L{\cdot}L^{-1}$ at maturity and no ethylene was detected in 'Manpungbae' and 'Niitaka' at maturity. Fructose was the most abundant soluble sugar during fruit maturation in the pears tested and an increase of sucrose was observed during fruit ripening in the Asian pears commonly. Ethanol insoluble solids (EIS) content decreased gradually with different levels among the pear cultivars as fruit ripens consisted of 10.79, 12.72, and 12.75 $mg{\cdot}g^{-1}$ FW. The amount of total soluble polyuronides was higher in early season cultivars 'Hanareum' than those of mid-season cultivar 'Manpungbae' and 'Niitaka'. In 'Niitaka' which harvested most late season, the level of 4% KOH soluble hemicelluloses was lower than 'Hanareum' and 'Manpungbae' and maintained constantly during fruit ripening period. Cellulosic residues were determined high level in 'Niitaka' which showed 612.33 ${\mu}g{\cdot}mg^{-1}$ EIS at maturity when compared with 'Hanareum' (408.0 ${\mu}g{\cdot}mg^{-1}$ EIS) and 'Manpungbae' (538.67 ${\mu}g{\cdot}mg^{-1}$ EIS). The main constituents of cell wall neutral sugars which consisted of arabinose, xylose, galactose, and glucose were decreased gradually with onset of fruit ripening regardless of cultivar. Arabinose which was predominant in 'Hanareum' pear decreased at the last stage of ripening, but the changes of cell wall neutral sugar during ripening were not occurred in 'Niitaka' pear. The change of molecular mass distribution in water soluble pectin observed dominantly at the early stage of fruit development. Depolymerization of 4% KOH-soluble hemicelluloses and degradation of xyloglucan showed in early-season cultivar 'Hanareum' during fruit maturation, and degradation of those fractions were detected only at the early stage fruit development in mid-season cultivar 'Manpungbae' and 'Niitaka'. The molecular mass profile of CDTA soluble pectin, $Na_2CO_3$-SP and 24% KOH soluble hemicelluloses showed no significant change during fruit maturation regardless of cultivar.

Clinical Features of Dietary Protein Induced Proctocolitis (식이 단백 유발 직결장염의 임상적 고찰)

  • Purpose: Dietary protein induced proctocolitis (DPIPC) can be considered as a cause of rectal bleeding or blood streaked stool in otherwise healthy-looking infants in the first several months of life. Failure to appreciate this entity may lead to inappropriate diagnostic or therapeutic intervention. This study aimed to ascertain the clinical features, treatment and prognosis of DPIPC. Methods: We reviewed 13 infants retrospectively, presented with bloody stool in early infancy. They were diagnosed as DPIPC clinically in Pusan National University Hospital from May 2002 to June 2004. Results: Seven males and six females were included. The mean age at onset of bleeding was $96.8{\pm}58.8days$. The mean frequency of hematochezia was $2.6{\pm}2.5$ times a day. Duration from onset of symptom to diagnosis was $35.5{\pm}55.0days$ and duration from onset of symptom to resolution of bleeding was $58.7{\pm}67.0days$. Nine (69.2%) were exclusively breast-fed infants and two (15.4%) were formula-fed infants. All but one infant did not have family history of other allergic diseases. A dietary history of ingestion of cow's milk, nut or shellfish was present in three mothers. Peripheral eosinophil count was normal to slightly elevated (total WBC count $10,555{\pm}3,145/mm^3$, relative eosinophil count $6.3{\pm}3.0%$, absolute eosinophil count $659.0{\pm}532.2/mm^3$). Sigmoidoscopy revealed lymphonodular hyperplasia with surrounding hemorrhagic spots in the rectosigmoid colon in 6 infants. Histopathologic finding of colonic biopsies in 5 infants showed chronic inflammation with lymphoid follicular hyperplasia (5 infants), crypt abscess (3 infants), or mild infiltration of eosinophils (less than 20/high power field) in the lamina propria. Spontaneous resolution of rectal bleeding occurred in all infants without dietary change or medicine. Conclusion: Most infants with DPIPC experience a very benign course and have spontaneous resolution of rectal bleeding without changes in the mother's diet. In the case of strong evidence for DPIPC we suggest deferring further invasive investigation and continuing breast feeding.

Surgical Experience of Ascending Aorta and Aortic Valve Replacement in Patient with Calcified Aorta

  • Background: The conventional method of aortic cross-clamping is very difficult and increases the risk of cerebral infarct due to embolism of the calcified aorta in these patients. Accordingly, we analyzed our experience with 11 cases of ascending aorta and aortic valve replacement with hypothermic circulatory arrest. Materials and Methods: From January 2002 to December 2009, 11 patients had ascending aorta and aortic valve replacement with hypothermic arrest at our hospital. We performed a retrospective study. Results: There were 5 males and 6 females, with a mean age of 68 years (range, 44 to 82 years). Eight patients had aortic stenosis, and 3 patients had aortic regurgitation. An aortic cannula was inserted into the right axillary artery in 3 patients and ascending aorta in 6 patients. Two patients with aortic regurgitation had a remote access perfusion catheter inserted though the right femoral artery. The mean cardiopulmonary bypass time was 180 minutes (range, 110 to 306 minutes) and mean hypothermic circulatory arrest time was 30 minutes (range, 20 to 48 minutes). The mean rectal temperature during hypothermic circulatory arrest was $21^{\circ}C$ (range, $19^{\circ}C$ to $23^{\circ}C$). No patient had any new onset of cerebral infarct or cardiovascular accident after surgery. There was no hospital mortality. Early complications occurred in 1 patient who needed reoperation due to postoperative bleeding. Late complications occurred in 1 patient who underwent a Bentall operation due to prosthetic valve endocarditis. The mean follow-up duration was 32 months (range, 1 month to 8 years) and 1 patient died suddenly due to unknown causes after 5 years. Conclusion: Patients with a calcified aorta can be safely treated with a technique based on aorta and aortic valve replacement under hypothermic circulatory arrest.
