Effects on the Adjacent Motion Segments according to the Artificial Disc Insertion (인공 추간판 적용으로 인한 인접 운동 분절의 영향)

  • To evaluate the effect of artificial disc implantation and fusion on the biomechanics of adjacent motion segment, a nonlinear three-dimensional finite element model of whole lumbar spine (L1-S1) was developed. Biomechanical analysis was performed for two different types of artificial disc, ProDisc and SB $Charit{\acute{e}}$ III model, inserted at L4-L5 level and these results were also compared with fusion case. Angular motion of vertebral body, forces on the spinal ligaments and facet joint under sagittal plane loading with a compressive preload of 150 N at a nonlinear three-dimensional finite element model of Ll-S1 were compared. The implant did not significantly alter the kinematics of the motion segment adjacent to the instrumented level. However, $Charit{\acute{e}}$ III model tend to decrease its motion on the adjacent levels, especially in extension motion. Contrast to motion and ligament force changes, facet contact forces were increased in the adjacent levels as well as implanted level for constrained instantaneous center of rotation model, i.e. ProDisc model.

Cytologic Findings of Fine Needle Aspiration Biopsy of Ancient Schwannoma (퇴행성 신경초종의 세침흡인 생검에 대한 세포학적 소견)

  • Ancient (degenerated) schwannomas are benign tumors that display pronounced degenerative changes including cyst formation, calcification, hemorrhage, and hyalinization. The tumors are usually infiltrated by large numbers of inflammatory cells such as neutrophils, lymphocytes, siderophages, and histiocytes. They are located in deep locations such as the retroperitoneum and must be differentiated from malignant soft tissue tumors because spindle cells with nuclear atypia may be present. In the fine needle aspiration biopsy. The cytologic findings of two cases of ancient schwannoma occurring in posterior mediastinum and retroperitoneum, respectively, are described. Computerized tomographic findings of both cases showed changes of cystic degeneration or necrosis. A case occurred in retroperitoneum revealed features of destruction of vertebral body to suggest a malignant soft tussue tumor radiologically. The cytologic findings of aspiration biopsy of both tumors revealed that the tumor cells were spindle in shape with elongated nuclei. Some of tumor cells were arranged in a palisading fashion. Cell structures that resemble the Verocay bodies were observed. Some of the tumor cells showed pleomorphic bizarre nuclei, but no mitotic activity or chromatin clumping was seen. Inflammatory cells, siderophages and histiocytes were scattered in fibrillar material. These cytologic findings are important in the diagnosis of ancient schwannoma and in the differentiation of this tumor from the malignant spindle cell tumor.

Intraesophageal spread of esophageal cancer - case report - (식도 종양의 식도내 전이 - 수술 치험 1례 -)

  • Esophageal cancer is relatively uncommon except in isolated endemic areas, but it generally devastating to the patient. Usually, by the time the disease becomes clinically evident, it is incurable. The aim of treatment is then relegated to attempting to palliate the symptoms in the best possible manner with the least morbidity and mortality. Squamous cell carcinoma in by far the commonest type of malignancy involving the body of the esophagus, accounting for more than 95 percent of all esophageal malignancies. Because the tumor’s microscopic spread is much greater than its macroscopic extent, it is necessary to resect a sufficiently long segment of the esophagus. And second tumors may occur either in the esophagus as a manifestation of a field change or in other organs. Recently we had experienced a case with in situ carcinoma away from the invasive squamous cell carcinoma of the esophagus. A 58 year-old male was admitted with the chief complaint of swallowing difficulty for a month prior to admission. While we studied the esophagogram and chest CT, we found that the mass was protruded to the lumen of esophagus at the level of the 7th-9th thoracic vertebral columns. We performed esophagectomy with lymph node dissection and esophagogastrostomy by thoracic and abdominal approaches. The pathologic result showed separation of another in situ carcinoma away from the invasive squamous cell carcinoma of esophagus at the level of esophagogastric junctions. Postoperative course was uneventful. Now he is taking the postoperative irradiation at out patient department.

A Clinical Study on the Outcome of Chuna Treatment of Adolescent Structural Scoliosis (청소년기의 구조적 척추측만증에 대한 추나치료 결과 고찰)

  • Objectives The present study retrospectively investigated clinical outcome of patients with structual scoliosis during Chuna treatment. Methods : The study population consisted of 34 patients(14 patients of adolescent and 20 patients of adult) who were diagnosised as scoliosis with radiological finding. All patients were treated with Chuna treatment, and after treatment, evaluated with radiological measurement by calculating the scoliosis correction angle from the Initial and follow-up spine AP and lateral views. Also the evaluation of clinical outcome was done twice pre and post treatment during this study. Results : The results were summarized as follows ; 1) The 85.3 percent of patients complained physical pain. and adult group had various painful lesion compared with adolescent group. 2) Adult group had treated $18.0{\pm}7.2$ times and adolescent group treated $13.5{\pm}5.7$ times with Chuna treatment. 3) Scoliotic angle is reduced in 92.9 percent of adolescent group and had all of adult group after Chuna treatment, especially, statistically significance appeared in adult group. 4) Adult group was superior to adolescent group in correctability and scoliotic angle change. 5) Correction effect was better in the group which initial scoliotic angle was lessor, or had more time of treatment. 6) According to grade of vertebral body rotation, scoliotic angle is larger and correctability was lower. Conclusions : Chuna treatment was efficacious against scoliosis. And it is necessary for adolescent scoliosis patient to be more carefully treated and observed.

A Case of Recurrent Intracranial Hypotension after Successful Epidural Blood Patch (경막외강 혈액첩포 시술로 두통이 호전된 후 반복적으로 발생한 두개내 저압증 1예)

  • Intracranial hypotension is a benign disorder characterized by orthostatic headache. It is caused by CSF leakage, therefore its treatment of choice has been epidural blood patch when initial conservative treatments were not effective. We would like to report a 26-year-old patient with recurrent orthostatic intracranial hypotension in spite of the successful epidural blood patch several times. Her headache was caused by myelography 8 month ago. Cisternography showed a CSF leakage at the level of L1 vertebral body. Headache was resolved completely after epidural blood patch and then recurred 3 times several months after blood patch. Now, she is free from headache for 5 months after last blood patch.

Reference Levels for Radiation Dose in Angiography and Interventional Radiology : In the Cerebrum (혈관 조영 검사 및 중재적 방사선 시술시 방사선량에 대한 참고 기준치 : 대뇌 중심으로)

  • The aim of this study is to compare reference levels for radiation dose in angiography and interventional radiology. Proposed reference levels for various procedures and classification of diseases are provided by fluoroscopy time and kerma area product(KAP) rate normalizing the body habitus focusing the cerebrum. Subarachnoid hemorrhage(SAH) represents the highest KAP-rates and aneurysm represents the lowest KAP-rates. According to these types of procedures, internal carotid artery(ICA), common carotid artery(CCA), and vertebral artery(VA) show the highest KAP-rates and guglielmi detachable coil shows the lowest KAP-rates. Therefore, the present study can suggested reference levels for patient radiation dose and is expected to be further useful in the field of radiation dose education and management of angiography and interventional radiology.

  • PURPOSE: This paper aims to present the available angle to evaluate the severity of forward head posture (FHP) with the observation method and photographic method. METHODS: A cross-sectional observation research design study consisted of 29 subjects who was divided two groups (slight FHP group, moderate FHP group) in Eulji university was used. We evaluated the FHP and the angles including CranioVertebral Angle (CVA), Head Tilting Angle (HTA), Head Position Angle (HPA) and Forward Shoulder Angle (FSA) with the Body style S-8.0 (South Korea, LU Commerce). RESULTS: The mean of CVA, FSA from the slight FHP group was shown higher than moderate severe FHP group. According to independent t-test result, but there was no difference among all angles in two groups. The linear discriminate analysis showed the size of distinction of FSA was the biggest, and then CVA, HTA and HPA were in the order. 55.6% of FSA is properly classified in the slight FHP group. CONCLUSION: The FSA is the best to distinguish the severity of FHP and then CVA as the second best. Therefore, FSA is recommended to check the FHP.

Clinical Experience of Pectus Excavatum Corrected by Ravitch Method (Ravitch 술식으로 교정한 누두흉 치험)

  • Background: The purpose of this study is to analyze the clinical results ad operative compatability by Ravitch methods with pectus excavatum. Material and Method: From 1984 to 1997, were performed Ravitch operations in 40 patients of pectus excavatum and analyzed the effects of correction and postoperative complication according to time of operation retrospectively. The group comprised 35 mem and women whose mean age was 8.9$\pm$7.9 years(between 2 to 35 years). All patients had corrected with Ravitch operation or its modified operation. Modified fixations were accomplished with K-wire in 15 patients and internal plate in 2 and the materials were removed 3 months after operation. The result of correction was estimated with the degree of changed distance between inner surface of sternal body and vertebral in 3 months after correction. Result: Postoperative complications were wound disruption in 14 patients. Estimated distances after correction ranged 0.4 cm to 4 cm. The acceptability for chest wall correction was high(6 excellent, 29 good in criteria of Humphreys). Conclusion: We concluded that the benefits of Ravitch operation were a high acceptability of patients and their family, and sufficient correction with no specific complications related operation.

Modified Approach through the Sacrococcygeal Junction to Block the Ganglion Impar (천미골 접합부를 이용한 외톨이 신경절 차단법)

  • Ganglion impar lies immediately anterior to the sacrococcygeal junction and blockade of the ganglion is used to treat anorectal and perineal pain. Although the technique introduced by Plancarte et at is widely practised, the bent needle is sometimes difficult to position precisely and patients find the procedure painful. We modified this approach of block of ganglion impar by positioning the needle into the sacrococcygeal junction and using the loss of resistance technique. With the patient in the lateral position, a skin wheal was raised at 1-1.5cm below the sacral hiatus. Twenty-three gauge short needle was directly placed into the sacrococcygeal junction with aid of fluoroscopic guidance. From 1 cm behind the anterior margin of the vertebral body in lateral view, we used the loss of resistance technique to confirm the retroperitoneal space. We found this modified approach easier to perform during six blocks for three patients with anorectal or perineal pain. Our modified approach through the sacrococcygeal junction may provide opportunity for wider administration of this procedure because of its simple technique, reduced pain during procedure and decreased risk of infection.

Analysis of Dural-sac Cross Sectional Area Changes According to Vertical Impact rate (수직 충격률에 따른 척추 경막 단면적 변화 해석)

  • In this study the occlusion of dural-sac. the outer membrane of spinal cord in the lumbar region. was quantitatively analyzed using one motion segment finite element model. Occlusion was quantified by calculating cross sectional area change of dural-sac for different compressive impact duration (loading rate) due to bony fragment at the posterior wall of the cortical shell in vertebral body. Dural-sac was occluded most highly in the range of 8∼12 msec impact duration by the bony fragment intruding into the spinal canal. $\Delta$t = 400 msec case 4 % cross sectional area change was calculated. which is the same as the cross sectional area change under 6 kN of static compressive loading.
