Hemifacial Spasm Developed after Contralateral Vertebral Artery Ligation

  • Although the mechanism of hemifacial spasm (HFS) is not yet well established, vascular compression of the facial nerve root exit zone and hyperexcitability of the facial nucleus have been suggested. We report a case of HFS in the setting of coinciding intracranial hemorrhage (ICH) of the pons and proximal ligation of the contralateral vertebral artery (VA) for the treatment of a fusiform aneurysm of the distal VA and discuss the possible etiologies of HFS in this patient. A 51-year-old male with an ICH of the pons was admitted to our hospital. Neuroimaging studies revealed an incidental fusiform aneurysm of the right VA distal to the origin of the posterior inferior cerebellar artery. Eight months after proximal ligation of the VA the patient presented with intermittent spasm of the left side of his face. Pre- and post-ligation magnetic resonance angiography revealed an enlarged diameter of the VA. The spasm completely disappeared after microvascular decompression.

Cervical Compressive Myelopathy due to Anomalous Bilateral Vertebral Artery

  • We report a very rare case of cervical compressive myelopathy by an anomalous bilateral vertebral artery (VA) entering the spinal canal at the C1 level and compressing the spinal cord. A 70-year-old woman had been suffering from progressive gait disturbance. Magnetic resonance imaging revealed that a bilateral VA at the V4 segment had abnormal courses and caused compression to the high cervical cord. VA repositioning was performed by anchoring a suture between the artery and around the arachnoid membrane and dentate ligament, and then, microvascular decompression using a Teflon sponge was done between the VA and the spinal cord. The weakness in the patient improved in the lower extremity after the operation. Anomalous VA could be one of the rare causes of cervical compressive myelopathy. Additionally, an anchoring suture and microvascular decompression around the VA could be a sufficient and safe method to indirectly decompress the spinal canal.

Inadvertent Complication of a Pipeline Embolization Device for Treatment with Vertebral Artery Dissecting Aneurysm : Distal Tip Fracture of Delivery Wire

  • Use of the Pipeline embolization device (PED) has increased based on studies about its safety and effectiveness, and new reports that describe perior postprocedural complications are now emerging. We report a rare periprocedural device-related complication that occurred during endovascular treatment with the pipeline embolization device for a dissecting aneurysm on the vertebral artery. A 55-year old woman was admitted due to left medullary infarction, and angiography showed a fusiform dilatation in the left vertebral artery that was suspicious for dissecting aneurysm. Endovascular treatment with PED was planned. Under general anesthesia, the procedure was performed without significant problems and a PED was deployed in an appropriate position. However, in the final step of the procedure, the distal tip of the PED delivery wire became engaged within a small branch of the posterior cerebral artery and fractured. Fortunately, imaging studies after the procedure revealed neither hemorrhagic nor ischemic stroke, and the patient recovered without neurological morbidities except initial symptoms.

유체상과 변형율속도를 고려한 해면골의 거동해석 (Behavior of trabecular bone considered by fluid phase and strain rate)

  • The pressure variation of interstitial fluid is one of the most important factors in bone physiology. In order to understand the role of interstitial fluid and the biomechanical interactions between fluid and solid constituents within bone, poroelastic theory was applied. The purpose of this study is to describe the behavior of calf vertebral trabecular bone composed of the porous solid trabeculae and the viscous bone marrow by using a commercial finite element analysis program based on the poroelasticity. In this study, the model was numerically tested for 5 different strain rates, i. e., 0.001, 0.01, 0.1, 1.0, and 10 per second. The material properties of the calf vertebral trabecular bone were utilized from the previous experimental study. Two asymptotic poroelastic response, the drained and undrained deformation, were predicted. From the predicted results for the simulated five strain rate, it was found that the pore pressure generation has a linearly increasing behavior when the strain rate is the highest at 10 per second, other wise it showed a nonlinear the strain rate Increased. Based on the results of the present study, it was suggested that the calf vertebral trabecular bone could be modeled as a porous material and its strain rate dependent material behavior could be predicted.

Change of Pain, Lumbar Sagittal Alignment and Multifidus after Sling Exercise Therapy for Patients with Chronic Low Back Pain

  • Purpose: This study examined the effects of sling exercise therapy on vertebral alignment, VAS, muscle activity, and multifidus of patients with chronic low back pain. Methods: Simple random sampling was used to divide the patients (n=116) into the sling exercise therapy group (SETG) and conservative physical therapy group (CPTG), with each group provided a intervention program in 3 sessions a week for 12 weeks. The lumbar lordosis angle (LLA), lumbar intervertebral disc angle (LIVDA) for vertebral alignment, lumbar muscle activity, and multifidus atrophy were measured before and after the intervention. Results: SETG showed significant changes in LLA, LIVDA of rate of change (delta score), and in relieving pain. The right-left balance gap for the lumbar dynamic muscle activity decreased after the intervention. The SETG showed significant changes in the grade of lumbar multifidus atrophy. Conclusion: The sling exercise therapy program is an effective exercise therapy method on vertebral alignment, muscle activity, recovery from multifidus atrophy, and pain relief for patients with chronic low back pain.

요추 추체의 골 -재형성에 대한 준분자 동력학적 접근 방법 (Analysis of Bone-Remodeling Process Using Quasi-molecular Dynamics)

  • 준분자 동력학 기법을 적용한 새로운 골-재형성 해석 방법을 제안 하였다. 본 연구에서는 기존의 연구에서 고려하지 못 하였던 해변골 내의 유체 압력이 골-재형성에 미치는 영향 까지를 고려한 해석 모델이 제안 되었다. 골-재형성의 대상 모델로 추체 시상면상의 2-D 모델을 개발 하였으며, 약 2kN의 수직 압축력에 해당되는 1.8564mm의 일정 변위 조건에 따른 모델내의 준분자들의 재배치 과정을 해석 하였다. 수렴된 피질골 및 해면골 모델의 준 분자의 형상은 실제 추체의 모양과 일치된 형태를 보이고 있었으며 계산된 수핵 압력 변화도 기존의 in-Vitro 실험 결과와 비교적 일치 하여 본 모델의 타당성을 입증 할 수 있었다.

A Case of Lumbar Metastasis of Choriocarcinoma Masquerading as an Extraosseous Extension of Vertebral Hemangioma

  • We report here on an uncommon case of metastatic choriocarcinoma to the lung, brain and lumbar spine. A 33-year-old woman was admitted to the pulmonary department with headache, dyspnea and hemoptysis. There was a history of cesarean section due to intrauterine fetal death at 37-weeks gestation and this occurred 2 weeks before admission to the pulmonary department. The radiological studies revealed a nodular lung mass with hypervascularity in the left upper lobe and also a brain parenchymal lesion in the parietal lobe with marginal bleeding and surrounding edema. She underwent embolization for the lung lesion, which was suspected to be an arteriovenous malformation according to the pulmonary arteriogram. Approximately 10 days after discharge from the pulmonary department, she was readmitted due to back pain and progressive paraparesis. The neuroradiological studies revealed a hypervascular tumor occupying the entire L3 vertebral body and pedicle, and the tumor extended to the epidural area. She underwent embolization of the hypervascular lesion of the lumbar spine, and after which injection of polymethylmethacrylate in the L3 vertebral body, total laminectomy of L3, subtotal removal of the epidural mass and screw fixation of L2 and L4 were performed. The result of biopsy was a choriocarcinoma.

  • 본 논문에서는 골다공증성 척추압박골절에 대한 척추성형술시 골강화제인 PMMA 주입방법을 제안하였다. 척추성형술의 정량적 분석을 위해서는 유한요소해석을 사용하였으며 1m간격의 척추체 CT를 바탕으로 보다 정교한 척추체 모델을 제시함으로써, 단순화된 척추의 기하학적 형상과 간단한 재료 물성치를 사용하는 기존의 연구의 한계를 극복하고자 하였다. 본 연구 결과를 토대로 추체내 망상골에 주입되는 PMMA의 주입형태, 주입위치를 정량적으로 평가하여 이상적인 수술방법을 제시하였다. PMMA의 구입위치는 망상골의 앞쪽-위쪽에 많은 양이 집중하여 주입되었을 때, 가장 효율적으로 추체를 보완해 주는 것으로 나타났다.

인체 요추해면골의 배류계수, 겉보기밀도, 공극비 사이의 상관관계에 관한 연구 (Correlation Among Permeability, Apparent Density, And Porosity of Human Lumbar Vertebral Trabecular Bone)

  • Abnormal intraosseous flow and pressure in trabecular bone could cause various pathological conditions such as osteonecrosis and osteoarthritis. Characteristics of intraosseous fluid flow and pressure generation in porous trabecular bone can be significantly affected by the permeability. Factors which determine the permeability could be the porosity and apparent density of trabecular bone. However, there is little data on the permeability and the relationship among the permeability. porosity, and apparent density of trabecular bone. In this study. the permeability. porosity, and apparent density of human lumbar vertebral trabecular bone were experimentally measured. Also, a power relationship among the permeability, porosity, and apparent density was investigated to understand effects of the porosity and apparent density variations on the permeability of trabecular bone based on Kozeny-Carman equation. A near linear relationship between intraosseous fluid flow and time indicated that the fluid phase flowed through the pores in trabecular bone is governed by the permeability. The permeability of trybecular bone was found to have a significant power relationship with the porosity and apparent density (r: 0.84 and $\textit{p}$< 0.0005). The power relationship could be useful to determine the permeability of trybecular bone after measuring the apparent density and porosity.

Combination Therapy of Conventional and Korean Medicine for Multiple Vertebral Compression Fractures Associated with Postpartum Osteoporosis: A Case Report

  • Multiple vertebral compression fractures are a rare condition and are a serious consequence of postpartum osteoporosis. This report describes the case of a 35-year-old woman who had given birth to her first child 2 months before the onset of pain. Magnetic resonance imaging showed compression fractures of 7 vertebrae. The patient was treated with both conventional, and Korean medicine methods including acupuncture and herbal prescriptions. The patient's progress was assessed using self-reported symptoms, scale scores and laboratory test results. Her pain was gradually alleviated and biochemical inflammation marker levels improved, but her functional status remained severely impaired. Clinical practitioners who treat women in the postpartum period must be aware of osteoporosis and potential vertebral fractures and need to consider Korean medicine as an alternative therapy to help such patients.
