승마시뮬레이터, 슬링과 Kendall 운동이 머리전방자세를 가진 젊은 성인의 머리척추각과 목 통증에 미치는 영향 (The effect of Horseback riding simulat or, Sling and Kendall Exercise on a cranio-vertebral angle and neck pain in Young Adults with Forward Head Posture)

  • 본 연구는 승마시뮬레이터, 슬링과 Kendall 운동이 머리전방자세의 머리척추각과 목 통증에 미치는 영향에 대해 알아보고자 연구를 실시하였다. 연구의 대상자는 머리전방자세를 가진 20대 성인 30명을 대상으로 실시하였다. 대상자는 무작위로 승마시뮬레이터 운동그룹 10명, 슬링 운동그룹 10명, Kendall 운동그룹으로 분류되어 2018년 9월에서 11월 까지 주 3회 6주간 운동을 실시하였다. 운동 전•후 머리척추각을 측정하기 위해 사진 촬영 후 Image J를 사용하였으며, 목통증의 측정을 위해 시각적 상사 척도를 사용하였다. 또한 그룹 내 운동 전•후의 변화를 검증을 위하여 대응 t-검정을 실시하고, 세 그룹 간 운동 전•후 변화의 차이를 검증하기 위하여 일원배치분산분석을 실시하였다. 연구 결과, 그룹 내 운동 전•후의 변화는 머리척추각, 목통증 모두 유의한 변화를 보였으나(p < .05), 그룹 간 운동 전•후 변화의 차이는 유의한 변화를 보이지 않았다(p > .05). 비록 운동그룹간의 차이는 없었지만 각 운동은 목 통증 감소와 목뼈각도의 증가에 효과적인 운동방법이므로 주어진 환경과 대상자의 선호도에 따라 적절한 운동을 하는 것이 효과적인 방법이라고 사료된다.

Segmental Deformity Correction after Balloon Kyphoplasty in the Osteoporotic Vertebral Compression Fracture

  • Objective : Balloon kyphoplasty can effectively relieve the symptomatic pain and correct the segmental deformity of osteoporotic vertebral compression fractures. While many articles have reported on the effectiveness of the procedure, there has not been any research on the factors affecting the deformity correction. Here, we evaluated both the relationship between postoperative pain relief and restoration of the vertebral height, and segmental kyphosis, as well as the various factors affecting segmental deformity correction after balloon kyphoplasty. Methods : Between January 2004 and December 2006, 137 patients (158 vertebral levels) underwent balloon kyphoplasty. We analyzed various factors such as the age and sex of the patient, preoperative compression ratio, kyphotic angle of compressed segment, injected PMMA volume, configuration of compression, preoperative bone mineral density (BMD) score, time interval between onset of symptom and the procedure, visual analogue scale (VAS) score for pain rating and surgery-related complications. Results : The mean postoperative VAS score improvement was $4.93{\pm}0.17$. The mean postoperative height restoration rate was $17.8{\pm}1.57%$ and the kyphotic angle reduction was $1.94{\pm}0.38^{\circ}$. However, there were no significant statistical correlations among VAS score improvement, height restoration rate, and kyphotic angle reduction. Among the various factors, the configuration of the compressed vertebral body (p=0.002) was related to the height restoration rate and the direction of the compression (p=0.006) was related with the kyphotic angle reduction. The preoperative compression ratio (p=0.023, p=0.006) and injected PMMA volume (p<0.001, p=0.035) affected both the height restoration and kyphotic angle reduction. Only the preoperative compression ratio was found to be as an independent affecting factor (95% CI : 1.064-5.068). Conclusion : The two major benefits of balloon kyphoplasty are immediate pain relief and local deformity correction, but segmental deformity correction achieved by balloon kyphoplasty does not result in additional pain relief. Among the factors that were shown to affect the segmental deformity correction, configuration of the compressed vertebral body, direction of the most compressed area, and preoperative compression ratio were not modifiable. However, careful preoperative consideration about the modifiable factor, the PMMA volume to inject, may contribute to the dynamic correction of the segmental deformity.

  • Purpose : To assess therapeutic effects of percutaneous polymethylmethacrylate(PMMA) vertebroplasty on the pain caused by osteoporotic thoracic and lumbar vertebral body compression fractures in a large scale of a prospective clinical design, and to determine clinical factors influencing its therapeutic effects. Methods : A prospective clinical study was carried out in 349 vertebral levels of 159 patients between April 1998 and July 1999. The compression fractures were confirmed with bone scan and spine CT, and bone marrow density was measured. Visual analogue scale(VAS) score was used for pre- and post-operative assessments of the pain. All 159 patients were assessed immediately after surgery, and 140 patients of them were followed-up for about 6 months in average. Results : Partial and complete pain relief was sustained immediately after operation in 73%, through follow-up period in 88% of the patients. Pain relief was not proportional to the amount of PMMA or the rate of increase in the height of the compressed vertebral body. It appears that 3 to 6cc of PMMA was proper enough to sustain pain relief. Better clinical improvement was achieved in the patients treated within 6 months after occurrence of vertebral body fracture. The most frequent surgical complication was epidural leakage of PMMA, and the most serious complication was extravertebral leakage into the paravertebral muscles, which appeared to exert the worst influence on the outcome. However, surgery was not required in these patients. Conclusion : Therapeutic effects of PMMA percutaneous vertebroplasty on osteoporotic vertebral body compression fractures were confirmed in a relatively large scale of prospective clinical study. It appears that good outcome can be achieved in patients treated within 6 months after fracture, treated each level with 3 to 6cc of PMMA in amount. without serious complications.

Foramen transversarium enlargement caused by vertebral artery tortuosity: Diagnosis with cone-beam computed tomography and magnetic resonance angiography

  • A markedly enlarged foramen transversarium was discovered incidentally on a cone-beam computed tomography scan of a 72-year-old male patient who was referred for dental implant placement. Further evaluation with magnetic resonance angiography revealed that the foramen enlargement was caused by a tortuosity in the course of the vertebral artery. This case report highlights the importance of recognizing significant incidental findings on diagnostic images and the potential need for additional imaging as part of the complete interpretative process.

골다공증성 흉요추부 압박 골절에서의 보존적 치료 (Conservative treatment of Osteoporotic Compression Fractures in Thoracolumbar spine)

  • Backgrounds: To evaluate the effectiveness of conservative treatment in osteoporotic thoraco-lumbar compression fractures and to identify the factors influencing the progression of compression. Materials and Methods: From January 2003 to October 2004, Patients who were admitted to our hospital for osteoporotic thoraco-lumbar compression fractures were reviewed retrospectively with follow-up more than 12 months (ave. 14.6 months). With simple x-ray lateral view, we evaluate wedge compression ratio (WCR) and kyphotic angle (KA) at initial and final follow-up. We separate the patients into two groups baesed on the amount of progression of vertebral compression and evaluate a relation with BMD, vertebral fracture level, initial WCR, initial KA. All datas were statistically analyzed. Results: An average of T-score was -3.5 and the changes of KA between initial and final follow-up were average $3.5^{\circ}$. Compression of anterior column were progressed to 8.5%. The changes between initial and final WCR in Group I (N=24) was 17.8%, and Group II (N=18) was 3.3%. T-score in group I was -3.4 and group II was -3.8 (p=0.228). vertebral fracture level were 10 T12, 12 L1, 2 L2 in group I; 6 T12, 6 L1, 6 L2 in group II (p=0.156). Initail WCR was 0.74 in group I, 0.63 in group II, and there was statistical difference between two groups(p=0002). Initial kyphotic angle was $13.9^{\circ}$in group I, $16.2^{\circ}$in group II repectively (p=0.392). Conclusion: The conservative treatment with short-term bed rest and early embulation is effective and valuable method to patients who have osteoporotic thoraco-lumbar compression fractures. There was no statistical difference between two groups according to BMD, vertebral fracture level, KA. But in comparison with initail WCR between two groups, there was statistical difference. That means, in the case of small initial compression of anterior column, the progression of compression was bigger than else. In these patients, more strict use of appropriate brace and careful follow-up should be needed.

Foraminoplastic Superior Vertebral Notch Approach with Reamers in Percutaneous Endoscopic Lumbar Discectomy : Technical Note and Clinical Outcome in Limited Indications of Percutaneous Endoscopic Lumbar Discectomy

  • To describe the details of the foraminoplastic superior vertebral notch approach (FSVNA) with reamers in percutaneous endoscopic lumbar discectomy (PELD) and to demonstrate the clinical outcomes in limited indications of PELD. Retrospective data were collected from 64 patients who underwent PELD with FSVNA from August 2012 to April 2014. Inclusion criteria were high grade migrated disc, high canal compromised disc, and disc protrusion combined with foraminal stenosis. The clinical outcomes were assessed using by the visual analogue scale (VAS), Oswestry Disability Index (ODI) and modified MacNab criteria. Complications related to the surgery were reviewed. The procedure used a unique approach, using the superior vertebral notch as the target and performing foraminoplasty with only reamers under C-arm control. The mean age of the 55 female and 32 male patients was 52.73 years. The mean F/U period was $12.2{\pm}4.2$ months. Preoperative VAS ($8.24{\pm}1.25$) and ODI ($67.8{\pm}15.4$) score improved significantly at the last follow-up (VAS, $1.93{\pm}1.78$; ODI, $17.14{\pm}15.7$). Based on the modified MacNab criteria, excellent or good results were obtained in 95.3% of the patients. Postoperative transient dysthesia (n=2) and reoperation (n=1) due to recurred disc were reported. PELD with FSVNA could be a good method for treating lumbar disc herniation. This procedure may offer safe and efficacious results, especially in the relatively limited indications for PELD.

우리나라 폐경전 여성에서 칼슘, 단백질, 인의 섭취상태가 골밀도에 미치는 영향 (Effects of Dietary Calcium, Protein, and Phosphorus Intakes on Bone Mineral Density in Korean Premenopausal Women)

  • Effects of dietary calcium(Ca), protein, and phosphorus(P) intake on bone mineral density (BMD) were investigated in 129 Korean premenopausal women(age 31-54 years) without diagnosed disease. BMD was measured at the spine(vertebrae L2-4) and femur(neck, Ward's triangle and trochanter). By stepwise multiple regression analysis it was shown that protein, Ca, and P intakes affected most significantly on BMD at the vertebrae L2-4, protein and P intakes affected most significantly on BMD at the femoral neck and Ward's triangle, and body mass index(BMI) affected most significantly on BMD at the trochanteric region. When ate-matched BMD % at the vertebrae L2-4 and all femoral sites was grouped by three levels(<90%, 90-99%, >=100%), only at the vertebrae L2-4>=100% and 90-99% groups had higher Ca intakes than <90% groups. When Ca, protein and P intakes of the recommended level for Korean(RDA) were grouped by three levels (Ca or P ; <=650mg/d, 650-750mg/d, >=750mg/d, Protein ; <=55g/d, 55-60g/d, >=65g/d), only at the vertebrae L2-4>55g/d of protein intake had higher age-matched BMD % than <=55g/d intake, >=750mg/d of Ca and P intakes, age-matched BMD % than <=650mg/d. In RDA range of Ca, protein, and P intakes, age-matched BMD % of the vertebrae L2-4 and all femoral sites was greater than 90%. Correlation between Ca intake and vertebral BMD was examined closer. There was more significant linear correlation between vertebral BMD and Ca intake below 800mg/d(r=0.346, p<0.0001)than above(r=0.376, p<0.019), implying a threshold effect and vertebral BMD was better expressed as a function of the logarithm of calcium intake(r=0.3881, p<0.0001). These results suggest that Ca, protein, and P intakes greater than RDA help to maintain proper BMD in middle-aged prementopausal women. Especially dietary Ca have important role in increasing the vertebral BMD and 800mg/d of Ca intake is optimum amount.

Endovascular Treatment of Vertebral Artery Dissecting Aneurysms That Cause Subarachnoid Hemorrhage : Consideration of Therapeutic Approaches Relevant to the Angioarchitecture

  • Objective : Intracranial ruptured vertebral artery dissecting aneurysms (VADAns) are associated with high morbidity and mortality when left untreated due to the high likelihood of rebleeding. The present study aimed to establish an endovascular therapeutic strategy that focuses specifically on the angioarchitecture of ruptured VADAns. Methods : Twenty-three patients with ruptured VADAn received endovascular treatment (EVT) over 7 years. The patient group included 14 women (60.9%) and 9 men (39.1%) between the ages of 39 and 72 years (mean age 54.2 years). Clinical data and radiologic findings were retrospectively analyzed. Results : Four patients had aneurysms on the dominant vertebral artery. Fourteen (61%) aneurysms were located distal to the posterior inferior cerebellar artery (PICA). Six (26%) patients had an extracranial origin of the PICA on the ruptured VA, and 2 patients (9%) had bilateral VADAns. Eighteen patients (78%) were treated with internal coil trapping. Two patients (9%) required an adjunctive bypass procedure. Seven patients (30%) required stent-supported endovascular procedures. Two patients experienced intra-procedural rupture during EVT, one of which was associated with a focal medullary infarction. Two patients (9%) exhibited recanalization of the VADAn during follow-up, which required additional coiling. No recurrent hemorrhage was observed during the follow-up period. Conclusion : EVT of ruptured VADAns based on angioarchitecture is a feasible and effective armamentarium to prevent fatal hemorrhage recurrence with an acceptable low risk of procedural complications. Clinical outcomes depend mainly on the pre-procedural clinical state of the patient. Radiologic follow-up is necessary to prevent hemorrhage recurrence after EVT.
