Review on Efficacy and Safety of Manipulation Therapy including Chuna Manipulation (추나요법을 포함한 수기치료의 효과 및 안전성에 관한 문헌고찰)

  • Objectives : This study aims to educate clinical doctors of the valuable practice of Manipulative Therapy(MT) as an opportunity for evidence-based medicine. Consequently it also serves to review the effectiveness and safety of MT. Methods : The literature studies of overseas were done by Cochrane Library and Medline website; those of domestic researches were completed by utilizing the sources which are gained from KERIS, KISS, DBpia, Kisti, and domestic institutes related with MT. Results : Eight papers related to the effectiveness of MT were published in South Korea, which concluded with the positive effects of MT. None of them are, however, well-designed randomized controlled trials(RCT). On the other hand, fifteen cases of nine articles indicate the adverse reaction of MT, and numbers of the researches in overseas revealed the side effects of MT in order of cervical, lumbar, and thoracic vertebrae. A vascular adverse reaction such as vertebral and carotid artery dissection was a highly reported cervical adverse reaction; in the thoracic and lumbar regions, neurological adverse reaction in terms of disc hemiation was frequently discovered. Conclusions : Henceforth, highly qualitative studies are required developing the effective outcomes and preventing any possible complications of MT. Therefore, systemic curriculums in institutions and sufficient clinical training in the filed are strongly recommended.

Correlation Study between Cervical X-ray Sagittal Parameters and Chuna Posture Analysis Results (경추 X-ray 시상면 지표와 추나체형분석 결과와의 상관성 연구)

  • Objectives The purpose of this study was to examine how changes in the cervical spine correlate with body posture changes in the whole body, and to find out the significance of complementary clinical application X-ray and Chuna posture analysis. Methods From January 1, 2019 to October 31, 2019, the results of 27 patients with pain in the vertebral region were analysed in accordance with the results of cervical X-ray and Chuna posture analysis. In order to confirm the significance of Chuna posture analysis results, the survey of 187 Chuna standard curriculum instruction qualification certifiers was conducted and the responses of 47 of them were analyzed. Results The occiput-atlas cline angle increases in both hypolordosis/hyperlordosis groups based on cervical lordosis angle, and increases further than in the hyperlordosis group. There were significant correlations between the changes in the cervical spine and the body posture changes in the whole body. There were no significant differences between cervical X-ray sagittal parameters and the body posture analysis parameters based on the patient's major disease codes. Conclusions As a result of conducting a survey on the clinical importance of the body posture analysis parameters, the importance of cervical parameters was verified. Changes in the cervical spine may not only cause other changes in the cervical region, but also affect the body posture changes in the whole body. The complementary application of X-ray and Chuna posture analysis results is helpful in clinical diagnosis and treatment of musculoskeletal disorders.

  • Recently, the number of patients with habitual vertebrae is increasing due to inappropriate postures and habits while sitting on a chair for a long time, because modern people have a great deal of work and studying due to a lot of work and study. Therefore, it is necessary to prevent such a disease through real-time monitoring and to present a guide for the user to recognize the importance of correct posture. In this study, we implemented a posture discrimination system which can help users to calibrate posture by judging the user's sitting position and feedback by using a pressure sensor based weight distribution system. We analyzed the distribution pattern of body weight to determine one normal posture and six abnormal postures. If the abnormal posture is maintained constantly, the implementation of the application that induces the right posture by ringing the notification is described.

Research of Body Pressure Distribution Change with the Use of BackJoy and Satisfaction of Human Sensibility

  • Objective: The aim of this study is to investigate the effect of BackJoy on how it effectively reduces the physical load generated in a posture in which the user sits, increases user's comfort and satisfaction, and maintains the correct posture. Background: Because of development of science and economic development, most office workers and students spend about 75% of their working time in chairs. However most of them have a poor posture. Method: This experiment conducted measurement using a pressure mat and surveyed to evaluate fatigue level, satisfaction and comfort of sit. The study is experimented 20 male and 14 female participants. The participants carried out four different types of tasks and each task took 20 minutes long. Results: In the case of experiment results before and after the use of BackJoy, average pressure, contact area and pressure per unit area appear to prove that using BackJoy is more effective. Conclusion: Through this study, the BackJoy's effects for the maintenance of good posture and loads that occur in the body are reduced. In the future study, there are some researches needed for various verifications using an EMG sensor that shows loads of vertebrae and we need to analyze each group of the participants by dividing them. Application: The evaluation method used in this study can be applied to evaluating ergonomic chairs.

  • A 9-year old boy suffering from cleidocranial dysplasia associated with impacted 4 supernumerary teeth and unerupted all permanent teeth is presented with his mother. The pedigree showed autosomal dominant pattern of inheritance, and the raiographic features of them were very similar in clavicle, skull, vertebrae, peivis and extremities. Although almost of the skeleton was involved with this syndrome, they did not recognize any other problem but except dental problem. In mother, who was wearing removable partial dentures leaving 24 impacted teeth in her jaws, the radiographic abnormalities like cystic lesion were not detected. And in the son, who showed impacted 4 supernumerary and all permanent teeth, we have attempted surgical extraction of the supernumerary teeth and periodic surgical opening of the alveolar bone covering the permanent dentition to induce the eruption of permanent teeth at the proper position, Orthodontic treatment has also been combined to correct class III malocclusion state.

TREATMENT OF BASAL CELL NEVUS SYNDROME WITH ENUCLEATION FOLLOWING MARSUPIALIZATION : A CASE REPORT (다발성 악골의 치성 각화낭을 동반한조대술 후 적출술을 이용한 기저세포모반증후군의 치험례)

  • Basal cell nevus syndrome is a hereditary disease of an autosomal dominant trait with variable conditions such as basal cell carcinomas of the skin, deformity of rib, fusion of vertebrae, mental retardation, hypertelorism, and multiple odontogenic keratocysts. A 32 years old man with pus discharge from fistula on the vestibule of left upper 1st molar visited to Chosun University Dental Hospital. Radiographic evaluation revealed multiple maxillary and mandibular cysts that had multilocular radiolucency on left mandibular body area, thining of inferior border of left border of ramus and well defined unilocular radiolucency above right upper 1st and 2nd molar and from left upper 1st premolar to 2nd molar. In chest PA view, he had a forked rib in the left 4th rib and in skull PA view the calcification of falx cerebri was observed. There was not any skin lesion. After the preliminary evaluation, the patient was diagnosed with basal cell nevus syndrome and he underwent marsupialization for decreasing the size of cystic lesion and came to hospital for dressing 3days a week. As time goes by, the size of lesion decreased. So, one and half year after marsupialization, he underwent cyst enucleation and iliac bone graft for the mandibular lesion and buccal fat pad grafts for the maxillary lesions. After the surgery, the patient experienced normal healing without any complications and he is on long-term follow-up.

  • Anatomy: Facet joint syndrome most often affects the lower back and neck and refers to pain that occurs in the facet joints, which are the connections between the vertebrae in the spine that enable the spine to bend and twist. Many physicians have believed that the usual lesion of facet syndrome was an anatomical impairments of facet joint itself.. Facet joint injection using local anesthetics is a reliable method for the diagnosis and treatment for facet syndrome. Etiology: One of many possible causes is imbalances that can occur in stress levels, hormone levels, and nutritional levels. These imbalances can adversely affect posture, which can lead to neck and back pain. The common disorder called facet syndrome exhibits lower back pain, with or without, radiating pain to buttock and thigh due to facet joint arthropathy. Pain in the facet joint is supposedly the secondary effect of narrowing of joint space by sustained muscle contracture around joints. Syndrome: Facet joint syndrome tends to produce pain or tenderness in the lower back that increases with twisting or arching the body, as well as pain that moves to the buttocks or the back of the thighs. Other symptoms include stiffness or difficulty standing up straight or getting out of a chair. Pain can be felt in other areas such as the shoulders or mid-back area. Treatment: Non-drug treatments include hot packs, ultrasound, electrical stimulation, and therapeutic exercises. Stimulating blood flow using massage or a hot tub may also help. Alternative treatments include yoga and relaxation therapy. If your pain persists after trying these treatments, a surgical procedure called radiofrequency rhizotomy, which destroys the sensory nerves of the joint, may bring relief. Facet joint injection has been helpful in diagnosis and therapy for this facet syndrome. Radiofrequency thermocoagulation of medial branches is known to be an effective method of relieving pain caused by facet joint problems. We conclude that spasmolytic treatment of muscles connecting the two vertebral articular space would be better for treatment and diagnosis of facet syndrome rather than facet block with local anesthetic and steroid only.

The Vertebrae of Some Freshwater Fishes Collected in T$\v{o}$kyusan National Park (덕유산 국립공원 일대에서 확인된 몇 종의 담수어 척추골수에 관하여)

  • The freshwater fish fauna of the Tokyusan National Park was censused during June 1993 to November 1993 at 9 sampling sites to establish baseline data for natural conservation program on the National Park. Seventeen species included 7 Korean endemic species and/or subspecies(Acheilognaths koreunus, Squazidus gracilis majimae, Microphysogobio yaluensis, Phoxinus kumganensis, Cobitis koreensis koreensis, Silurus microdorsalis and Odontobutis platycephala) belonging to 5 families were collected in this region. Sixteen species were found in Kumkang and 8 species were found in Naktongkang, however, common species were Squalidus gracilis majimae, Microphysogobio yaluensis, Puntungia herzi, Moroco oxycephalus, Zucco temminckii, Misgurnus anguillicaudatus and Odontobutis platycephala. Dominant species of this region were Moroco oxycephalus, Zacco temminckii, Odontobutis platycephala. The final aims of this study were not only accumulate baseline data on the osteology of Korean freshwater fishes which were collected at those streams during June 1993-November 1993 with soft X-ray radiography, but also evaluate the suitability of osteological analyses on the environmental damage to fish.

  • The Korean ricefish, Oryzias, consisted of 2 species, O. latipes and O. sinensis. Through investigation on their distributions, it was confirmed that O. sinensis was restricted to western areas, and O. latipes was distributed throughout Korea. Interestingly, Daecheon region near the West Sea was sympatric, existing O. latipes and O. sinensis. In addition to their distributions, O. latipes was well distinguished from O. sinensis as the following morphological characteristics: mean 31 vertebrae (vs. 30), mean 15 gill rakers (vs. 12), lots of dots in both sides of body (vs. absent), and yellowish ventral and anal fins (vs. blackish).

  • Primary lung cancer frequently metastasizes to distant organs. But breast is a very rare site of metastasis. So, accurate diagnosis is essential to rule out primary breast cancer. A 62-year old woman who had complained of cough was diagnosed as small cell lung carcinoma of the right lower lobe. She had a 2.5cm sized lobular mass in left upper mid portion of breast. Sonoguided breast biopsy revealed small cell carcinoma consistent with metastatis from primary lung cancer. She also had bone metastses to the scapula and the vertebrae. We report the case of metastatic breast cancer from small cell lung carcinoma.
