Persistent right aortic arch with aberrant left subclavian artery originating from the patent ductus arteriosus in a dog: a case report

  • A 4-month-old intact male Sapsaree dog was referred due to a history of postprandial regurgitation following consumption of solid food. Thoracic radiography revealed focal leftward displacement of the thoracic trachea at T1 to T4 vertebrae levels. Barium contrast radiography revealed focal dilation of the cranial thoracic esophagus at the heart base level. Persistent right aortic arch (PRAA) with an aberrant left subclavian artery branching from the patent ductus arteriosus was diagnosed by computed tomography angiography (CTA). Although barium contrast radiography can presumptive diagnose PRAA, CTA should be considered for identifying additional vascular anomalies, specific types, and surgical planning.

  • The normal larynx locates to the front of the neck symmetrically and the thyroid notch lies in the center of the neck, but practically the larynx is not symmetric in all people. From a clinical point of views, there are vague cases to decide whether a disordered laryngeal structure is within normal variations or a pathologic condition. The purpose of this study is to investigate the anatomy of the laryngeal framework in normal population. Authors investigate various measures of normal laryngeal framework, such as symmetry and length of the larynx, levels of the hyoid bone and vocal cord and angle of thyroid cartilage by using calipers and protractor on 45 cases of neck CT. The results are summerized as follows. 1. The laryngeal framework was asymmetric to a greater or lesser extent in most cases with directional preponderance to the right side. The degree of asymmetry did not differ among different age groups and between seres. 2. The level of the hyoid bone ranged from C2-C3 vertebrae to C5-C6 intervertebral space with most frequent level of C5. 3. The level of the vocal cord ranged from 01 vertebra to C6-C7 intervertebral space with most frequent level of C5. 4. The angle of thyroid cartilage ranged from 58 degree to 100 degree with average of 81.5 and mean angle between both thyroid alae were 77.24 degree in male and 87.88 degree in female.

Prediction of osteoporosis using fractal analysis on periapical radiographs (구내방사선사진의 프랙탈 분석을 이용한 골다공증 예측)

  • Purpose : The purpose of this study was to investigate whether the fractal dimension and radiographic image brightness of periapical radiograph were useful in predicting osteoporosis. Materials and Methods : Ninety-two postmenopausal women were classified as normal, osteopenia and osteoporosis group according to the bone mineral density of lumbar vertebrae and periapical radiographs of both mandibular molar areas were taken. The ROIs of 358 areas were selected at periapical and interdental areas and fractal dimension and radiographic image brightness were measured. Results : The fractal dimension in normal group was significantly higher than that in osteoporosis group at periapical ROI (P < 0.05). The radiographic image brightness in normal group was higher than that in osteopenia and osteoporosis group. There was significant difference not only between normal and osteopenia group (P < 0.05) but also within osteopenia and osteoporosis group (P< 0.01) at periapical ROI. Significant difference was observed not only between normal and osteopenia group but also between normal and osteoporosis group at interdental ROI (P< 0.01). Positive linear relationship was weakly shown at Pearson correlation analysis between fractal dimension and radiographic image brightness. BMD significantly correlated with fractal dimension at periapical ROI (P< 0.01), and BMD and radiographic image brightness significantly correlated at both periapical and interdental ROIs (P< 0.01). Conclusion : This study suggests that the fractal dimension and radiographic image brightness of periapical ROI may predict BMD. (Korean J Oral Maxillofac Radiol 2005: 35 : 41-6)

A Taxonomic Review of Four Sillaginid Fishes (Perciformes) from the Adjacent Waters of Korea (한국 주변해역 보리멸과(농어목) 어류 4종의 분류학적 재검토)

  • A taxonomic review of the family Sillaginidae was carried out based on two species (Sillago japonica and S. parvisquamis) collected in the coastal waters of Korea from 2008 to 2009 and three species (S. parvisquamis, S. aeolus and S. sihama) kept in the museum. Sillago japonica was easily distinguished from S. parvisquamis and S. sihama by lateral line scales (70~73 in S. japonica vs. 77~81 in S. parvisquamis vs. 67~70 in S. sihama) and scales above lateral line (3~4 vs. 7 vs. 5~6, respectively). Sillago aeolus differed from the other three species in having dark brown blotches on the side of body. We newly found morphological differences in some measurements (snout length, pectoral fin length and $2^{nd}$ anal spine length) and the number of vertebrae between Korean and Japanese S. parvisquamis, suggesting the two may be different populations. Although four sillaginid species are known from Korea, only two species (S. japonica and S. parvisquamis) were found in this study; accordingly, the distribution of the remaining two species (S. sihama and S. aeolus) may be restricted to subtropical waters.

The Thoracic Radiculopathy in SAPHO Syndrome - A Case Report - (SAPHO 증후근에서의 신경근병증 - 증 례 보 고 -)

  • SAPHO syndrome is an acronym for an increasingly recognized syndrome of synovitis, acne, pustulosis, hyperostosis, and osteitis. Most of the previously reported case are from Japan and Europe. The authors report a case of SAPHO syndrome in Korean female who presented with extrasternal neurologic symptoms. A 60-year-old female with thoracic and right chest wall pain presented with the sternocostoclavicular hyperostosis, and recurrent aseptic osteitis. Previously, she had hystrectomy and thyroidectomy due to uterine malignancy 25 years ago. Also, she started to take medications for palm and sole pustulosis 7 days prior to admission. Through evaluation of clinical, radiological, serological studies, studies was done along with bone needle biopsy for the biological reassessment. The hyperostosis was found in the pedicles of thoracic 8, 9, 10 vertebrae and sternoclavicular joint. Radio-isotope bone scan showed an accumulation of tracer in thoracic vertebra. The bone biopsy from these site showed increased osseous turnover, thickening of trabeculae accompanied by mild acculuation of granulation tissue and round cell infiltration, compatible with mild chronic inflammation with marrow fibrosis. The patient showed good response to conservative management. The authors report a case of SAPHO syndrome with thoracic radiculopathy. It is considered that SAPHO syndrome is related to spondyloarthropathy, and appears to have benign disease process with good prognosis.

Extra-spinal sciatica and sciatica mimics: a scoping review

  • Not all sciatica-like manifestations are of lumbar spine origin. Some of them are caused at points along the extra-spinal course of the sciatic nerve, making diagnosis difficult for the treating physician and delaying adequate treatment. While evaluating a patient with sciatica, straightforward diagnostic conclusions are impossible without first excluding sciatica mimics. Examples of benign extra-spinal sciatica are: piriformis syndrome, walletosis, quadratus lumborum myofascial pain syndrome, cluneal nerve disorder, and osteitis condensans ilii. In some cases, extra-spinal sciatica may have a catastrophic course when the sciatic nerve is involved in cyclical sciatica, or the piriformis muscle in piriformis pyomyositis. In addition to cases of sciatica with clear spinal or extra-spinal origin, some cases can be a product of both origins; the same could be true for pseudo-sciatica or sciatica mimics, we simply don't know how prevalent extra-spinal sciatica is among total sciatica cases. As treatment regimens differ for spinal, extra-spinal sciatica, and sciatica-mimics, their precise diagnosis will help physicians to make a targeted treatment plan. As published works regarding extra-spinal sciatica and sciatica mimics include only a few case reports and case series, and systematic reviews addressing them are hardly feasible at this stage, a scoping review in the field can be an eye-opener for the scientific community to do larger-scale prospective research.

A Patient with Symptoms Caused by Electric Shock Treated with Traditional Korean Medicine

  • This case report describes a 60-year-old female patient diagnosed with intercostal neuropathy and vertebral compression fractures which occurred following an electric shock injury. The patient received acupuncture, pharmacopuncture, and herbal medicine administration between February 10th, 2020 and April 25th, 2020. The pain level in the thoracic and left intercostal areas was assessed using the Numerical Rating Scale. The Self-report of the Leeds Assessment of Neuropathic Symptoms and Signs Pain Scale were used to diagnose neuropathic pain. The Neuropathic-Pain -Scale was used to evaluate the degree of neuropathic symptoms. The Oswestry Disability Index and the European Quality of Life-5 Dimensions were used to assess quality of life scales and functional disorder. Following combined Korean medicine treatment, the patient exhibited reduced levels of pain and significant improvement in functional disorder symptoms and quality of life.

A Revision of the Genus Microphysogobio in Korea with Description of a New Species (Cypriniformes, Cyprinidae) (한국산 모래주사속(Genus Microphysogobio) 어류의 분류학적 연구)

  • The five species of the genus Microphysogobio in Korea are reviewed with the keys to species and their illustrations: M. longidorsalis, M. jeoni sp. nov., M. koreeneis, M. yaluensis, and M. rapidus. Microphysogobio tungtingensis uchidai was treated as a junior synonym of M. yaluensis based on having the distinct papillae of upper lip and the color pattern on body sides by the examination of the type specimens. Microphysogobio jeoni is described as a new species from the specimens collected in the Naktong River, Han River and Keum River, Korea. it is well distinguished from the related species by the slender body, undeveloped papillae of upper lip and unspotted fins. Microphysogobio yaluensis was showed the clinal variations in the number of lateral line scales and vertebrae among the populations in the western drainages of Korea. It is remarked biogeographically that the five species of Microphysogobio are all endemics to Korea with the restricted ranges.

  • The morphological characteristics of chum salmon Oncorhynchus keta, masou salmon. Oncorhynchus masou and rainbow trout, Oncorhynchus mykiss in Korea were studied from 1989 to 1990. It was found that 13 factors of the female and 11 factors of the male in body form, number of pyloric ceaca and gill rakers has significant specific differences. There were some differences among species in the ratios of body depth, caudal peduncle depth, dorsal fin length and anal fin length to standard length (PBL), and snout length, eye diameter, upper jaw length, and cheek to head length. In the ratio of head length to fork length of the male, chum salmon showed the highest(24.7%) value and rainbow trout the lowest(21.6%). No difference was found in the female. It was proved that number of vertebrae, gill rakers, pyloric ceaca and lateral line scales were useful taxonomical characteristics of the salmon species. In particular, the ratio of eye diameter (ED) to head length (HL) appeared to be a new taxonomic criterion for the salmons in Korean waters.

  • Sexual dimorphism of the matured chum salmon, Onchorhynchus keta, have been studied based on biometrical measurements and osteological characters. Male chum salmon has an elongated and hooked upper jaw and canine-like teeth on the upper and lower jaw. The ratios of snout length (SnL) and upper jaw length (UJ) to head length of male were larger than those of the female, whereas eye diameter (ED) and postorbital part of head (CK) of the male were smaller than those of the female's. The ratios of body depth, caudal peduncle depth, anal fin length and adipose fin length to the standard length (PBL) of the male were also larger than those of the female. On sexual dimolrphism was detected in number of fin rays, gill rakers and pyloric ceaca, where as number of lateral line scales and caudal vertebrae of the female was larger than those of the male. Glossohyal and vomer bones has no sexual difference. The ratio of width to length of supraethmoid bone of the male was larger than that of the female. The ratio of frontal part to posterior part of parasphenoid bone of the male was smaller than that of the female.
