  • We scrutinized the preoperative electrocardiographic and hemodynamic findings in adult atrial septal defects older than 15 years, and then followed up the postoperative electrocardiographic changes sequentially. In preoperative electrocardiographs, the mean PR interval [0.17 sec] was prolonged than normal adults [M;0.13,F;0.15], and the mean QRS axis [93.1*] was deviated to rightward than normal [M;63.7*,F;64.4*], and 122 cases of all 159 patients [77.8%] were in RAD quadrant. The QRS morphology was classified into three groups; a] crista supraventricularis hypertrophy, 25 cases, b] right ventricular outflow tract hypertrophy, 89 cases, c] right ventricular hypertrophy, 44 cases, and normal rs pattern, 1 case. Comparing the QP/QS, Pp/Ps, Rp/Rs in these three groups, Qp/Qs increased a] 2.65 to b] 2.97 and decreased b] 2.97 to c] 2.55, Pp/Ps increased a] 0.27 to b] 0.35 to c] 0.44, and Rp/Rs increased a] 0.1 to b] 0.14 to c] 0.2. In comparing the atrial fibrillation with sinus rhythm, the patient`s mean age was increased [26.4 to 45.7], the mean Qp/Qs was decreased [2.97 to 2.7], the mean Pp/Ps was increased [0.35 to 0.46], the mean Rp/Rs increased [0.14 to 0.2], and the QRS morphology was RVOT hypertrophy;7 cases, RVH;2 cases in all 11 cases. Therefore, the atrial fibrillation was appeared in progressed status. Increasing the mean pulmonary arterial pressure, size of the R` wave in Vl lead increased, and the QRS morphology tended to become severe patterns. Postoperatively, the PR interval shortened and QRS axis tended to normal axis quadrant, and size of R` wave decreased sequentially, atrial fibrillation disappeared in 4 cases. Conclusively, by use of the conventional surface electrocardiography, we could anticipate the hemodynamic changes and the prognosis at outpatient department.

  • Premature contraction arrhythmia is the most common disease among arrhythmia and it may cause serious situations such as ventricular fibrillation and ventricular tachycardia. Most of arrhythmia clasification methods have been developed with the primary objective of the high detection performance without taking into account the computational complexity. Also, personalized difference of ECG signal exist, performance degradation occurs because of carrying out diagnosis by general classification rule. Therefore it is necessary to design efficient method that classifies arrhythmia by analyzing the persons's physical condition and decreases computational cost by accurately detecting minimal feature point based on only QRS features. We propose method for personalized specific classification of premature contraction arrhythmia based on QRS features in smart healthcare environments. For this purpose, we detected R wave through the preprocessing method and SOM and selected abnormal signal sets.. Also, we developed algorithm to classify premature contraction arrhythmia using QRS pattern, RR interval, threshold for amplitude of R wave. The performance of R wave detection, Premature ventricular contraction classification is evaluated by using of MIT-BIH arrhythmia database that included over 30 PVC(Premature Ventricular Contraction) and PAC(Premature Atrial Contraction). The achieved scores indicate the average of 98.24% in R wave detection and the rate of 97.31% in Premature ventricular contraction classification.

  • Background and Objectives: Brugada syndrome (BrS) is an inherited arrhythmia syndrome that presents as sudden cardiac death (SCD) without structural heart disease. One of the mechanisms of SCD has been suggested to be related to the uneven dispersion of transient outward potassium current (Ito) channels between the epicardium and endocardium, thus inducing ventricular tachyarrhythmia. Artemisinin is widely used as an antimalarial drug. Its antiarrhythmic effect, which includes suppression of Ito channels, has been previously reported. We investigated the effect of artemisinin on the suppression of electrocardiographic manifestations in a canine experimental model of BrS. Methods: Transmural pseudo-electrocardiograms and epicardial/endocardial transmembrane action potentials (APs) were recorded from coronary-perfused canine right ventricular wedge preparations (n=8). To mimic the BrS phenotypes, acetylcholine (3 μM), calcium channel blocker verapamil (1 μM), and Ito agonist NS5806 (6-10 μM) were used. Artemisinin (100-150 μM) was then perfused to ameliorate the ventricular tachyarrhythmia in the BrS models. Results: The provocation agents induced prominent J waves in all the models on the pseudo-electrocardiograms. The epicardial AP dome was attenuated. Ventricular tachyarrhythmia was induced in six out of 8 preparations. Artemisinin suppressed ventricular tachyarrhythmia in all 6 of these preparations and recovered the AP dome of the right ventricular epicardium in all preparations (n=8). J wave areas and epicardial notch indexes were also significantly decreased after artemisinin perfusion. Conclusions: Our findings suggest that artemisinin has an antiarrhythmic effect on wedge preparation models of BrS. It might work by inhibition of potassium channels including Ito channels, subsequently suppressing ventricular tachycardia/ventricular fibrillation.

  • Premature ventricular contractions are the most common of all arrhythmias and may cause more serious situation like ventricular fibrillation and ventricular tachycardia in some patients. Therefore, the detection of this arrhythmia becomes crucial in the early diagnosis and the prevention of possible life threatening cardiac diseases. Most methods for detecting arrhythmia require pp interval, or the diversity of P wave morphology, but they are difficult to detect the p wave signal because of various noise types. Thus, it is necessary to use noise-free R wave. So, the new approach for the detection of PVC is presented based on the rhythm analysis and the beat matching in this paper. For this purpose, we removed baseline wandering of low frequency band and made summed signals that are composed of two high frequency bands including the frequency component of QRS complex using the wavelet filter. And then we designed R wave detection algorithm using the adaptive threshold and window through RR interval. Also, we developed algorithm to classify PVC using RR interval. The performance of R wave and PVC detection is evaluated by using MIT-BIH arrhythmia database. The achieved scores indicate average detection rate of 99.76%, sensitivity of 99.30% and specificity of 98.66%; accuracy respectively for R wave and PVC detection.

  • Purpose: In this study, relationships between symptom experience and quality of life in a cross-sectional sample of patients with Atrial Fibrillation (AF) were investigated. Methods: This descriptive study involved a convenience sample of AF patients from S university hospital, C city. One hundred and two AF patients completed psychometric validated measures of AF related symptoms and quality of life. Descriptive statistics and Pearson correlation coefficients with SPSS WIN 14.0 were used for data analysis. Results: Of 16 atrial arrhythmia-related symptoms, the patients reported 'tiredness' as the most frequent and 'shortness of breath' as the most severe. The level of overall quality of life for patients with AF was 53.92. There were significant differences in symptom frequency according to religion, New York Heart Association (NYHA) classification and left ventricular ejection fraction ; symptom severity according to monthly income and stroke ; quality of life according to age, job, alcohol intake, NYHA class and stroke. Quality of life for these patients was positively correlated with symptom frequency and symptom severity. Conclusions: This study demonstrated that patients with more frequent and severe symptoms perceive poorer quality of life than patients with less frequent and less severe symptoms. Symptom experience should be assessed early to improve quality of life for patients.

  • We showed the effects of the traditional herbal medicine, Jukyeoondam-tang (JO-T, Zhu-ru-Wen-Dan-Tang in Chinese), on ventricular arrhythmia induced by aconitine. Electrophysiological experiments with conventional microelectrode techniques revealed that JO-T potently suppressed the aconitine-induced arrhythmias in ventricular strips of the rat. In the aconitine-induced arrhythmia model of the rat, pretreatment with JO-T $(100\;{\mu}g/ml)$ completely occluded the appearance of ventricular tachyarrhythmia (VT) or ventricular fibrillation (VF) induced by aconitine. Furthermore, the aconitine-induced ventricular arrhythmia was occluded by $Na^+$ channel blocker quinidine but was not occluded by $K^+$ channel blocker glibenclamide $(3\;{\mu}mol/L)\;and\;Ca^{2+}$ channel blocker nifedipine $(10\;{\mu}mol/L)$. We also confirmed the effect of JO-T in the ischemia-reperfusion (I/R)-induced arrhythmia model of the rat. JO-T did not affect the I/R-induced arrhythmias in rats. JO-T may alleviate the risk of ventricular arrhythmias following aconitine. These results suggest that JO-T is a potent antiarrhythmic drug having a$Na^+$ channel-blocking action.

  • This paper proposes a fuzzy support vector machine ($FSVM_n$) pattern classifier to classify the arrhythmia patterns of an electrocardiograph (ECG). The $FSVM_n$ is a pattern classifier which combines n-dimensional fuzzy membership functions with a slack variable of SVM. To evaluate the performance of the proposed classifier, the MIT/BIH ECG database, which is a standard database for evaluating arrhythmia detection, was used. The pattern classification experiment showed that, when classifying ECG into four patterns - NSR, VT, VF, and NSR, VT, and VF classification rate resulted in 99.42%, 99.00%, and 99.79%, respectively. As a result, the $FSVM_n$ shows better pattern classification performance than the existing SVM and FSVM algorithms.
