익수사고자에 대한 효과적인 응급처치 방법 (An effective emergency care of a person from water submersion)

  • This study was to exhibit the effective emergency care method for the drowning and non-drowning who are reached two-thousand peoples every year in our country. For investigate the effective emergency care, this study was discussed as follows ; Pathophysiology of the water submersion, Fresh-water & sea-water drowning, Factors affecting survival, and Prehospital management. The conclusions from this study were summarized as follows; 1. Remove the patient from the water. If you suspect neck or spinal injuries, Always support the head and neck level with the back and, begin rescue breathing. 2. Maintain the airway and support ventilation in the water use the jaw-thrust technique to avoid farther injury to the neck or spine. We might encounter more resistance to ventilations than you expect because of water in the airway. Once you have determined that there are no foreign objects in the airway, apply ventilations with more force; adjust ventilations until you see the patient's chest rise and fall but not until you see gastric distention. Do not attempt to remove water from the patient's lungs or stomach. 3. If there is no pulse, begin CPR. 4. Administer high-flow supplemental oxygen; suction as needed. 5. Once the patient is breathing and has a pulse, assess for hemorrhage; control any serious bleeding that you find. 6. Cover the patient to conserve body heat, Handle the patient very gently, and, Transport the patient as quickly as possible to Emergency Department, Continuing resuscitative measures during transport. If the patient have the hypothermia, follow hypothermia management.

풍동실험을 통한 공동주택 지하주차장의 자연환기 성능 연구 (Experimental Study on Wind-driven Ventilation in Basement Parking Lots of Apartment)

  • This paper aims for evaluating the wind-driven ventilation in basement parking lots of apartment. Wind tunnel tests coupled with tracer gas method were conducted, and classified by wind directions and opening types. The test results showed that, as for wind-driven ventilations, stack type openings were more successful than scuttle vent. Finally, according to Weibull distribution in Seoul, yearly averaged wind-driven ventilation rate was calculated.

  • In this paper, we simulated a new apartment building by using radon emission test values from various building materials used as interior finishing materials. The simulations evaluated the radon concentration in the room according to the radon emissions and the ventilations for each type of finishing material (gypsum board, stone, tile and concrete). Overall concrete finish simulation case showed the highest concentration than the case using other materials due to the effect of wall area at the center of each room and the mean radon concentration at 1.5 m above the floor was slightly lower than the mean value at each center. In the case of the porch, pantry and bathroom, the radon concentration was high even when the same materials were used as in the other rooms.

  • Several papers on respiratory and critical care published from March 2012 to February 2013 were reviewed. From these, this study selected and summarized ten articles, in which the findings were notable, new, and interesting: effects of high-frequency oscillation ventilation on acute respiratory distress syndrome (ARDS); safety and efficacy of hydroxyethyl starch as a resuscitation fluid; long-term psychological impairments after ARDS; safety and efficacy of dexmedetomidine for sedation; B-type natriuretic peptide-guided fluid management during weaning from mechanical ventilation; adding of daily sedation interruptions to protocolized sedations for mechanical ventilation; unassisted tracheostomy collar of weaning from prolonged mechanical ventilations; and effects of nighttime intensivist staffing on the hospital mortality rates.

  • Numerical study were performed to analyze for fire safety in railway tunnel with forced ventilation vents. For the condition of train fires with heat release rate of 30 MW, unsteady three dimensional analysis were carried out to investigate the effects of smoke movements, the heat transfer and $CO_2$ concentrations and in double track tunnel with two vents. Among three operation modes of forced ventilations at two vents, the exhaust-exhaust mode of the vent represents the best performance for the evacuation of passengers to avoid the fire.

  • Vehicle's airtight integrity is a very important factor that greatly affects passenger's habitability. However, when a door is closed, the pressure in the passenger compartment increases due to the vehicle's airtight integrity. That pressurizes the eardrums of the passenger, and makes passenger unpleasant. Thus, in this study, the configurations of air ventilation hasve been investigated to reduce pressure in the passenger compartment. Truck cab is utilized to measure the pressure in the passenger compartment. Various kinds of air ventilations are considered to find out optimized pressure in truck cab when a door is closed.

  • This paper is how to improve contaminated air by the pollutant from vehicles through ventilators and entrance at the underground station. We are looking for the environment management to take care of customers. There is two ventilation systems. One is natural ventilation system, the other is forced ventilation system. Usually, subway ventilators were installed low on the sidewalk. There are lots of craps on the ventilators, so these things interrupt influx of outside air. But the gas from the vehicles comes into the station through entrance. There is lots of noise while ventilations run. So we install the supply air vents away from the road for the customers. If it's difficult, we cover around the ventilator with clear plastic plates more than 2M heights. We also install silencer on the ventilators. We install the air curtains on the entrances to prevent dust from outside. Seoul Metropolitan has a plan to make 60M deep underground road. To improve underground road air quality, ventilators should be installed that consider the above information.

  • Severe sepsis is the most common cause of death among critically ill patients in non-coronary intensive care units. In 2002, the guideline titled "Surviving Sepsis Campaign" was published by American and European Critical Care Medicine to decrease the mortality of severe sepsis and septic shock patients, which has been the basis of the treatment for those patients. After the first revised guidelines were published on 2008, the most current version was published in 2013 based on the updated literature of until fall 2012. Other important revised guidelines in critical care field such as 'Clinical Practice Guidelines for the Management of Pain, Agitation, and Delirium in Adult Patients in the Intensive Care Unit' were revised in 2013. This article will review the revised guidelines and several additional interesting published papers of until March 2014, including the part of ventilator-induced lung injury and the preventive strategies.
