하공정맥 폐색증에 의한 Budd-Chiari 증후군의 수술치험 -1례 보고- (Cavoatrial bypass for Budd-Chiari Syndrome Associated with Obstruction of the Iinferior Vena Cava -Report of One Case-)

  • We report a case of a 45-year-old woman with Budd-Chiari syndrome caused by the obstruction of the inferior vena cava just below the diaphragm. Transatrial dilatation or membranotomy was not possible due to the severe fibrotic obliteration of the inferior vena cava. Instead, cavoatrial bypass with a Dacron graft[20 mm-Vascutek] was performed under the median sternotomy and median abdominal incision.The postoperative course was uneventful and generalized symptoms were much improved. During the following period[6 month] the graft patency was maintained with no recurrence of symptoms.

간내 하공정맥 폐쇄증: 수술 3례 보고 (Surgical Treatment of Intrahepatic Inferior Vena Cava Obstruction: Three Cases Report)

  • We operated three cases of intrahepatic inferior vena caval obstruction from January, 1969 to February, 1978 at the department of Thoracic and Cardiovascular Surgery in Seoul National University Hospital. They were all female. And their ages were 33, 38 and 32 respectively. The lesions were the same site; just above the opening of the hepatic vein of inferior vena cava. In first case, segmental narrowing of the inferior vena cava without thrombosis was noted. In second case; thrombus occluded the inferior vena cava and left hepatic vein. In third case, we observed membrane of 4mm thickness which occluded the lumen completely. They were totally operated by using cardiopulmonary bypass. Their post-operative courses were good in the last two cases and re-obstruction was suspected in the first case.

  • Hypothermia and circulatory arrest is efficatious adjunct in the surgical treatment of conventionally difficult or otherwise inoperable lesion. This technique was utilized in 5 patients, 3 with membraneous obstruction of inferior vena cava[MOVC] and 1 with giant middle cerebral artery aneurysm and 1 with renal cell carcinoma invading inferior vena cava. All membraneous obstruction of inferior vena cava patients had excellent results but the others died of operative complications. The rationale for the use of complete cardiac arrest with hypothermia is reviewed and the use of these technique in selected patients is warrented.

  • Torso stab injuries near the cardiac box may present unique challenges due to difficulties in hemorrhage control. For a stab injury to the heart, the repair is straightforwardly performed via median sternotomy. In contrast, injuries to the inferior vena cava are challenging to repair, especially when they are close to the diaphragm, and the bleeding can be torrential. Herein, we describe a case of a self-inflicted stab wound within the "cardiac box." The trajectory of the stab injuries went below the diaphragm and injured the infradiaphragmatic inferior vena cava. Successful emergent repair via the thoraco-laparotomy approach revived the young man. In this report, we revisit and discuss previous large series of patients with this rare vena cava injury.

  • 신세포암은 환자의 약 5∼10%에서 하대정맥을 침범한다. 이러한 경우 수술적으로 함께 제거하는 것이 가장 좋은 치료로 되어있다. 최근에는 간정맥 유입로 상부에 위치한 하대정맥내의 종양 혈전 제거술의 경우 체외 순환을 통한 순환 정지 방법이 사용되고 있다. 본 교실에서는 근치적 신적출술과 함께 하대정맥내의 종양 혈전 제거술을 4례 치험하였다. 모든 환자에서 간정맥이하의 하대정맥 혈전으로 체외 순환의 도움 없이 일반적인 방법으로 시행되었으나, 그 중 한 환자에서 간정맥의 유입로보다 상부에 위치한 하대정맥혈전이 재발되어 순환정지후 혈전제거술을 시행하였다. 네 명의 환자 모두 특별한 문제없이 회복되었다.

Budd-Chiari Syndrome 수술치험 -1례 보고- (Surgical Treatment of Budd-Chiari Syndrome -1 Cases Report-)

  • The Budd-Chiari syndrome is a rare type of portal hypertension caused by complete or incomplete obstruction of the hepatic vein or the corresponding portion of the inferior vena cava or both. In this case, the obstruction was located just beneath the diaphragm, above the right hepatic vein opening, which was confirmed by vena cavography preoperatively. Budd-Chiari syndrome with stenosis or thrombosis of the inferior vena cava may be cured by prosthetic bypass to the right atrium. This case is caused by thrombus of unknowed primary origin. Combined mesoatrial and cavoatrial shunt should be encouraged in this specific situation. Postoperatively, there were marked fall of venous pressure and symptoms and signs improved remarkably.

  • 저자들은 폐색전증과 심부정맥 혈전증을 이전에 진단받았던 말기 전립선암환자에서 외상성 뇌출혈로 인해 항응고제 치료를 시행하지 못하여 하대정맥 필터를 삽입 후 하지 부종, 하지 맥압 소실, 급성 신부전 및 DIC 등으로 사망한 phlegmasia cerulea dolens 1예를 경험하였기에 문헌고찰과 함께 보고하는 바이다.

  • Renal cell carcinoma frequently extends into the vena cava and occasionally, even into the right atrium. We experienced 2 cases of renal cell carcinoma extending into the inferior vena cava by radical nephrectomy and complete removal of thrombi in the cava by joint approach with urologic surgeons. In the literature, improvement of survival by complete removal of tumor thrombi in the vena cava was documented and joint approach of cardiovascular surgeons and urologic surgeons result in appropriate surgical approach.

  • Because of the. rise in the incidence of high speed automobile accident and various gun shot wound, complicated vascular injuries are becoming more frequent. Inferior vena caval injury seems to be also in high incidence, but reports in the literature were rare. because of potentially lethal. Recently we have experienced a case of inferior vena caval injury due to stab wound on the posterior aspect of the right abdomen. This was successfully treated with inferior caval ligation on the both, proximal and distal of the injured infrarenal vena cava.

A Case of Budd-Chiari Syndrome Associated with Alveolar Echinococcosis

  • Although alveolar echinococcosis (AE) can cause a serious disease with high mortality and morbidity similar to malign neoplasms. A 62-year-old woman admitted to a hospital located in Sivas, Turkey, with the complaints of fatigue and right upper abdominal pain. On contrast abdominal CT, a $54{\times}70{\times}45$ mm sized cystic lesion was detected in the left lobe of the liver that was seen to extend to the posterior mediastinum and invade the diaphragm, esophagus, and pericardium. The cystic lesion was seen to be occluding the inferior vena cava and left hepatic vein at the level where the hepatic veins poured into the inferior vena cava. Bilateral pleural effusion was also detected. We discussed this secondary Budd-Chiari Syndrome (BCS) case, resulting from the AE occlusion of the left hepatic vein and inferior vena cava, in light of the information in literature.
