The sphenopalatine vein: anatomical study of a rarely described structure

  • Although in counterpart, the sphenopalatine artery (SPA), has been well described in the medical literature, the sphenopalatine vein (SPV) has received scant attention. Therefore, the present anatomical study was performed. Additionally, we discuss the variations, embryology, and clinical significance of the SPV. Adult cadaveric specimens underwent dissection of the SPV. In addition, some specimens were submitted for histological analysis of this structure. The SPV was found to drain from the sphenoidal sinus and nasal septum. Small tributaries traveled through the nasal septum with the posterior septal branches of the SPA and nasopalatine nerve. The SPA and SPV were found to travel through the sphenopalatine foramen and another tributary was found to perforate the medial plate of the pterygoid process and to connect to the pterygoid venous plexus which traveled lateral to the medial plate of the pterygoid process. The vein traveled through the posterior part of the lateral wall of the nasal cavity with the posterior lateral nasal branches of the SPA and the lateral superior posterior nasal branches of the maxillary nerve. To our knowledge, this is the first anatomical study on the SPV in humans. Data on the SPV provides an improved anatomical understanding of the vascular network of the nasal cavity. Developing a more complete picture of the nasal cavity and its venous supply might help surgeons and clinicians better manage clinical entities such as posterior epistaxis, cavernous sinus infections, and perform endoscopic surgery with fewer complications.

Surgical Anatomy of the Sternal Median and Paramedian Approaches on the Junction of the Veins and the Arteries of the Pig Heart (흉골 정중선 절개와 방정중선 절개 접근술에 따른 돼지 심장 대혈관들의 외과적 해부구조의 육안적 비교)

  • 돼지 심장의 해부학적 구조에 대해서는 많은 연구가 보고되어 있으나 흉골 정중선 절개와 방정중선 절개 후 절개선에 나타나는 돼지 심장 대혈관들의 외과적 해부구조에 대한 보고는 아직 없고 심장적출 및 수술에 외과적 해부구조의 숙지가 필수적이므로, 체중 15kg - 25kg의 돼지 12마리를 4마리씩 3군, 즉 정중선절개군, 우측방정중선절개군, 좌측방정중선절개군으로 나누어 흉골열개 후 나타난 심장 대혈관들의 육안적 외과구조를 보고하고 심장적출술을 시도해 각 수술절개 접근법에 따른 수술의 용이성을 관찰하여 더 나은 수술접근법을 선택하는데 필요한 정보를 제공하고자 본 실험을 실시하였다. 그 결과, 우측과 좌측 방정중선절개법에 의해 흉골을 완전 열개시에는 제 1흉골과 갑상연골 부위에서 정중선으로 접근하는 초승달형 절개를 통해 internal mammary artery의 절개 위험성을 피할 수 있음을 발견하였고 흉골정중선절개에서는 상행대동맥의 정 중앙부에 aortic cannula를 쉽게 장착할 수 있었고 특히 비숙련자에게 더 적절한 수술 방법임을 알 수 있었다. 한편 숙련자가 심장적출이 아닌 동맥관개존증등과 같은 심장수술의 경우에는 방 정중선절개로써 환축의 수술 후 통증을 감소시키고 조기치유가 가능하다면, 초승달형 절개법을 응용한 방정중선 절개를 권할 수 있고 이에 따른 임상적 관찰이 요구되는 바이다.

Discovery of a trans-sellar vascular supply for the pituitary gland

  • The vasculature of the pituitary gland is discussed briefly and the details of an anatomical discovery of the vessels supplying the pituitary gland provided. Twenty latex injected cadaveric heads were dissected. Any vessels that were found to penetrate the sella turcica and travel to the pituitary gland were documented and measured. Additionally, 25 adult skulls were evaluated for the presence, size, and sites of bony foramina in the floor of sella turcica. Trans-sellar vessels were identified in 65% of specimens. There was a mean of 1.5 vessels per specimen consisting usually of a mixture of veins and arteries. The mean diameter of these vessels was 0.3 mm and the mean length from the sella turcica to the pituitary gland was 2.3 mm. These vessels were concentrated in the most concave part of the sella turcica. In bony specimens, the mean number of trans-sellar foramina was four. The diameter of these foramina ranged from 0.3 to 0.6 mm in size. The trans-sellar foramina were concentrated near the center part of the sella turcica and had no regular pattern. The pituitary gland receives at least some blood supply and drainage via vessels traveling along the septum of the sphenoidal sinuses and through the sella turcica. Knowledge of such vessels might lead to a better understanding of the vascular supply and drainage of the pituitary gland and would be useful during skull base approaches such as trans-nasal approaches to the pituitary gland.

Comparative Study of Cryostripping and Endovenous Laser Therapy for Varicose Veins: Mid-Term Results

  • Background: Conventional stripping is considered to be the standard procedure for great saphenous vein (GSV) varicosities, but many other alternative treatments such as cryostripping, endovenous laser therapy (EVLT), radiofrequency ablation, and ultrasound-guided foam sclerotherapy have been developed. Among them, both cryostripping and laser therapy have been reported to be less traumatic, with lower rates of complications and recurrences when compared to conventional stripping. To compare the efficacy of these treatments, we have analyzed and compared the mid-term clinical outcomes of cryostripping and EVLT. Methods: Patients diagnosed with varicose veins of the GSV and treated with cryostripping or laser therapy between September 2008 and April 2013 were enrolled in this study. Duplex ultrasonography was used for the diagnosis and evaluation of varicosity and reflux, and the clinical-etiology-anatomy-pathophysiology classification was used to measure the clinical severity. The symptoms, Venous Clinical Severity Score (VCSS), recurrence rates, and complication rates of the cryostripping and laser therapy groups were analyzed and compared. Results: A total of 68 patients were enrolled in this study. 32 patients were treated with cryostripping, and 36 patients were treated with laser therapy. The median follow-up period was 29.6 months. Recurrence was noted in three patients from the cryostripping group and in two patients from the EVLT group. There was no difference in the VCSS score, operative time, duration of hospital stay, and complication rate between the cryostripping group and the EVLT group. Conclusion: The mid-term clinical outcomes of cryostripping were not inferior to those of EVLT. Further, considering its cost-effectiveness, cryostripping seems to be a safe and feasible method for the treatment of varicose veins.

  • During dissection of the saphenofemoral junction, anatomical variations in the tributaries of the long saphenous vein and external pudendal artery are frequently encountered, but the transpositions of the femoral artery and vein have very rarely been reported. On operation of the varicose vein, the surgical complications can be minimized when the possible deviations in the normal anatomy are taken into the consideration.

Terminal bifurcation of the external jugular vein: a rare variation

  • Variations of external jugular vein are common. Here, we present a rare terminal bifurcation of the left external jugular vein. The left external jugular vein was formed by the union of entire retromandibular vein and posterior auricular vein. One inch above the clavicle, it bifurcated into medial and lateral divisions. The medial division terminated into the internal jugular vein and the lateral division terminated into the subclavian vein. Medial division received a common vein formed by the union of anterior jugular vein and an anonymous vein lying under the sternocleidomastoid muscle. The lateral division received a common vein formed by the union of suprascapular and transverse cervical veins. The knowledge about this variation could be useful to head and neck surgeons, radiologists and plastic surgeons.

Isolation of Endothelial Cells and Smooth Muscle Cells from Rat Aort (흰쥐 대동맥의 내피세포와 민무늬근육세포 분리)

  • Background: Atherosclerosis has emerged as the leading cause of death in developed countries. At present, human umbilical vein endothelial cells (HUVEC) are most commonly used for the investigation of Endothelial cells (EC). However, HUVEC are not found in arteries but only in veins. Currently there are many reports on methods used to isolate EC;, most of these methods require special equipment to remove contaminating smooth muscle cells (SMC). Materials and Methods: The method described here may be used to isolate not only ECs but also SMCs;,the approach presented here did not require special equipment. Rat aorta was treated with 2 mg/ml of type II collagenase solution for 45 minutes. The isolated cells from the aorta were incubated in medium G for a week;, only ECs could be separated. After the collagenase treatment, the rest of aorta was cut lengthwise, and left undisturbed to obtain SMCs in the culture dish for 10 days. To verify the purity of the isolated cells, we performed immunofluorescence and evaluated the results with transmission electron microscopy analysis. Results: The immunofluorescence study demonstrated specific expression of CD31 and ${\alpha}$-smooth muscle actin in the isolated ECs and SMCs, respectively. Cultured ECs and SMCs showed their own fine structure characteristics. Conclusion: These results suggest that this method for isolating ECs and SMCs may be especially useful for the study of atherosclerosis.

Evaluation of the morphology of the canalis sinuosus using cone-beam computed tomography in patients with maxillary impacted canines

  • Purpose: The nasopalatine canal is a well-known, important anatomical structure in the anterior maxilla, but this region contains many accessory canals. The canalis sinuosus (CS) is one of these canals; it contains the anterior superior alveolar nerve, along with veins and arteries. The purpose of this study was to evaluate the CS using cone-beam computed tomography (CBCT) in patients with maxillary impacted canines. Materials and Methods: A total of 111 patients admitted to the Istanbul Medipol University School of Dentistry for the exposure, orthodontic treatment, and/or extraction of an impacted canine were included in this study. CBCT images were obtained for these patients under standard conditions. Axial, coronal, and sagittal sections were evaluated to assess the prevalence of CS, the direction and diameter of the canal, its relation with the impacted canine, and its distance from the alveolar crest. Further, possible correlations with patient gender and age were analyzed. Results: The CS could be detected bilaterally in all the evaluated tomography images. The mean canal diameter was significantly larger in males than in females(P=.001). The CS ran significantly closer to the impacted canine when the canal was located horizontally (P=.03). Variations of the canal, such as accessory canals, were identified in 6 patients. Conclusion: CS is an anatomical entity that may resemble periapical lesions and other anatomical structures. Evaluation with CBCT prior to surgical procedures in the anterior maxilla will help to prevent overlooking such anatomical structures and to decrease possible surgical complications.

In Vitro Culture of Endothelial Cell and Smooth Muscle Cell for Studying Vascular Diseases

  • Endothelial cells play a key role in pathological processes such as cancer cell metastasis, atherosclerosis, and diabetic retinopathy. Vascular smooth muscle cells directly involve in the formation of atheroma in atherosclerosis. Some kinds of the endothelial cells are simply harvested from the umbilical veins, the tunica intima of aortic walls, the retina using various enzymes solutions. Those purely isolated cells provide a powerful tool in vitro studies of the endothelial cell related diseases. In this context, the cultured smooth muscle cells after the isolation from the tunica media of aortic walls are also used for elucidating the pathogenesis of atherosclerosis. Here, I briefly introduce articles that include the isolation of human umbilical vein endothelial cells(HUVEC), aortic endothelial and smooth muscle cells, retinal microvascular endothelial cells(RMEC), as well as the diseases' applications of these cells.

Patho-anatomy and treatment of complex congenital cardiovascular anomalies associated with left superior vena cava: report of 16 cases (좌상공정맥을 합병한 복합심혈관기형의 병리해부와 수술 성적: 16례 보)

  • From July 1975 to March 1984, 16 patients of complex congenital cardiovascular anomalies associated with left superior vena cava were seen at Han-Yang University Hospital. The age of patients was ranged from 2 to 15 years-old. The distribution of Sex was 7 patients in male, 9 patients in female. Persistent Left Superior Vena Cava [L.S.V.C.] was classified according to the proximal connection of L.S.V.C. into 3 groups. Group I which L.S.V.C. connected to coronary venous sinus was in 9 patients, Group II which L.S.V.C. connected to Left atrium was in 5 patients, Group III which L.S.V.C. hemodynamically connected to right atrium was in 2 patients. Pathoanatomical findings of complex congenital cardiovascular anomalies associated with L.S.V.C. in 16 cases were generally show unsystematic irregularity. In group I, A.S.D. were only in 3 cases, but in highest incidence and in group III, all two cases were supracardiac type of total anomalous drainage of pulmonary veins. Post-operatively, 3 patients among 14 patients of total correction, were died immediately, 1 patient of palliative shunt operation was died after 2 and half years, and Follow-up results of other remaining patients were excellent.
