  • Background: The vascularity of the peri-implant tissue is a very important parameter in establishment and maintenance of a healthy tissue after dental implant insertion. Objective: The purpose of this study was to compare the vascularity of the peri-implant mucosa between flap and flapless implant surgeries by using a canine mandible model. Study Design: In six mongrel dogs, bilateral, edentulated, flat alveolar ridges were created in the mandible. After 3 months of healing, two implants were placed in each side by either the flap or flapless procedures. After another healing period of 3 months, biopsies were obtained, prepared for light microscopy and exposed to morphometric measurements. Results: The supracrestal connective tissue lateral to the implant was found to be more richly vascularized in the flapless group than in the flap group. Conclusion: These results suggest that the flapless procedure may have an effect on increasing the vascularity of the peri-implant mucosa.

초냉동 보관된 백서의 동종 기관 이식편의 대망 내 이식에 따른 조직 생육성 및 혈관 형성 (The Viability & Vascularization of the Cryopreserved Rat Tracheal Allografts with Omental Implantation)

  • 배경: 대망의 가장 중요한 성질 중 하나인 혈관 형성 촉진 기능을 이용하여 초냉동 보관된 기관 이식편의 대망 내 이식이 기관 이식편의 생육성이나 혈관 형성에 미치는 영향을 연구하고자 한다. 대상 및 방법: 8주령의 Sprague Dawley rat 수컷의 초냉동 보관된 기관 이식편을 백서의 복강 내 대망에 이식하였다. 연구군은 냉동 보관기간과 대망 내 이식기간에 따라 4개의 군으로 분류하였다(n=52). 이식된 기관 이식편을 획득하여 기관 평활근 및 주변 결합 조직의 섬유화 및 염증 정도, 기관 연골의 석회화 정도, 기관 내 상피세포의 변화 및 연골간 간격에서의 혈관 형성 정도 등을 검사하였다. 결과: 기관 평활근의 염증 정도는 냉동 보관기간이나 대망 내 이식기간에 따라 유의한 차이를 보이지 않았고, 기관 연골의 석회화 정도는 냉동 보관기간과 상관없이 대부분 심하게 진행되어 있었다. 혈관 형성은 기관 이식편의 양끝뿐만 아니라 중간 부위에서도 충분히 이루어져 있었다. 결론: 초냉동 보관된 백서의 장분절 기관 이식편을 2주간 대망 내 이식을 시행한 결과 기관 이식편의 조직 생육성이 적절히 유지되면서 새로운 혈관이 형성되었다. 향후 동종 기관 이식 시 초냉동 보관된 기관 이식편을 대망 내 이식하여 새로운 혈관이 형성된 후 단계적으로 기관 이식을 시행하는 데 도움이 될 수 있다고 생각한다.

  • Carotid body tumor is rare tumor in the neck. Among the pathologic conditions affecting paraganglionic tissue. the carotid body is most frequently involved. There are controversies in terms of natural history. biological behaviors, technique of excision, risks of the operation. Carotid angiography is the most valuable diagnostic aid and important for the planning of therapy. Definite treatment of carotid body tumor is surgical excision. Considerable degree of caution and vascular surgical armamentation are required because of its anatomical location and profuse vascularity. Surgical removal of this kind of paraganglioma must be predicated upon several factors such as tumor character. location. symptom, vascularity, and surgeon's ability.

  • Chronic Achilles tendon rupture is likely to result in functional impairment in gait and sports activity. The presence of a large defect secondary to retraction of the tendon ends, atrophy of the calf muscles, and vulnerable vascularity of the soft tissue envelope make it a challenging problem to treat. Surgical reconstruction aims to restore the length and tension of the gastrocnemius-soleus complex. Various surgical treatment options have been described, depending on several factors, including residual gap size after scar tissue removal, remaining tissue quality, and vascularity. Despite good results being reported, there is a lack of high-level, evidence-based clinical guidelines available to select the first-line surgical procedure. This paper overviews the current available surgical options for patients with chronic Achilles tendon rupture.

Efficacy of Lauromacrogol Injection for Ablation of Benign Predominantly Cystic Thyroid Nodules and Related Factors: A Prospective Study

  • Objective: To prospectively evaluate the efficacy of lauromacrogol injection for ablation (LIA) of benign predominantly cystic thyroid nodules and its related factors. Materials and Methods: A total of 142 benign predominantly cystic thyroid nodules (median volume, 12.5 mL; range, 0.4-156 mL) in 137 patients (male:female sex ratio, 36:101; mean age ± standard deviation [SD], 49 ± 13 years) were treated with LIA after being confirmed as benign via cytology. The volume reduction rate (VRR) of the nodules and cosmetic score were evaluated during follow-up at 1, 3, and 6 months after treatment and every 6 months thereafter. A VRR of ≥ 50% at the 12-month follow-up was considered to indicate effective treatment. The associations between the clinical factors and nodular ultrasound features, including the initial nodule volume, proportion of solid components, vascularity grade and ineffective treatment (VRR of < 50% at the 12-month follow-up), and regrowth were analyzed. Results: All patients completed follow-up for at least 12 months. The average ± SD follow-up period was 32 ± 11 months (range, 12-54 months). The effective treatment rate was 73.2% (104/142), while the regrowth rate was 12.0% (17/142) at the last follow-up. Grade 2-3 intranodular vascularity in the solid components of the nodules was the only independent factor associated with ineffective treatment, with an odds ratio (reference category, grade 0-1) of 3.054 (95% confidence interval, 1.148-8.127) (p = 0.025). Conclusion: LIA is an effective treatment for predominantly cystic thyroid nodules. Grade 2-3 intranodular vascularity in the solid components of nodules is the only independent risk factor for ineffective LIA.

기관지천식환자에서 기도내 혈관분포정도(vascularity) 및 다른 기도 재구성 소견과의 연관성 (Relationship Between Vascularity and Other Remodeling Parameters in Asthmatic Airway)

  • 연구배경 : 기관지천식에서 기도 재구성과 관련되는 병리학적 소견은 다양하다. 본 연구의 목적은 기관지천식 환자의 기도내 혈관분포정도 및 다른 기도 재구성 소견과의 관련성을 일아보는데 있다. 방 법 : 기관지천식환자(n=34)와 대조군(n=6)을 대상으로 기관지점막조직생검을 시행하였다. HE 염색으로 기저막 및 기저막하부의 두께를, type IV collagen 면역염색을 하여 혈관분포정도(점막하 단위면적당 혈관의 수와 혈관면적)를 image analyzer를 이용 하여 측정하였다. 결 과 : 1) 기관지천식환자에서 대조군에 비해 기저막 두께 ($6.92{\pm}2.01{\mu}m$ vs $9.67{\pm}2.84{\mu}m$, p<0.05) 및 기저막하부의 두께 ($44.49{\pm}31.92{\mu}m$ vs $121.22{\pm}72.79{\mu}m$, p<0.05) 가 유의하게 높았다. 2) 기관지천식환자에서 대조군에 비해 점막하 단위면적당 혈관면적은 유의하게 높았으며 ($4.51{\pm}2.13%$ vs $10.32{\pm}6.08%$, p<0.05) 점막하 단위면적당 혈관수는 높은 경향을 보였으나 통계적인 유의성은 없었다. 3) 기관지천식환자에서 점막하 단위면적당 혈관면적 및 혈관수는 기저막 두께, 기저막하부의 두께, 기관지천식의 중증도, $FEV_1$, $PC_{20}$ 등과 유의한 상관관계가 없었다. 결 론 : 본 연구결과 혈관분포정도가 기관지천식환자의 기도 재구성에 중요한 요소임을 확인할 수 있었으나 다른 기도 재구성의 요소들과는 직접적인 관련성은 없었다. 따라서 각각의 기도 재구성의 요소들은 서로 다른 임상적인 의의를 가칠 것으로 사료되며 이러한 요소들에 대한 추후 연구가 필요할 것으로 생각된다.

Sonographic Diagnosis of Cervical Lymph Node Metastasis in Patients with Thyroid Cancer and Comparison of European and Korean Guidelines for Stratifying the Risk of Malignant Lymph Node

  • Objective: To evaluate the ultrasonography (US) features for diagnosing metastasis in cervical lymph nodes (LNs) in patients with thyroid cancer and compare the US classification of risk of LN metastasis between European and Korean guidelines. Materials and Methods: From January 2014 to December 2018, US-guided fine-needle aspiration was performed on 836 LNs from 714 patients for the preoperative nodal staging of thyroid cancer. The US features of LNs were retrospectively reviewed for the following features: size, presence of hilum, margin, orientation, cystic change, punctate echogenic foci (PEF), large echogenic foci, eccentric cortical thickening, abnormal vascularity, and cortical hyperechogenicity. A multiple logistic regression analysis was performed to identify the independent US features for the diagnosis of metastatic LNs. The diagnostic performance of independent US features was subsequently evaluated. LNs were categorized according to the Korean Thyroid Imaging Reporting and Data System (K-TIRADS) and European Thyroid Association (ETA) guidelines, and the correlation between the two sets of classifications was assessed. Results: Absence of the hilum, presence of cystic changes, PEF, abnormal vascularity, and cortical hyperechogenicity were independent US features of metastatic LNs. Cystic changes, PEF, abnormal vascularity, and cortical hyperechogenicity showed high specificity (86.8%-99.6%). The absence of the hilum had the highest sensitivity yet low specificity (66.4%). When LNs were classified according to the ETA guidelines and K-TIRADS, they yielded similar categorizations of malignancy risks and were strongly correlated (Spearman coefficient, 0.9766 [95% confidence interval, 0.973-0.979]). According to the ETA guidelines, 9.8% (82/836) of LNs were classified as "not specified." Conclusion: Absence of hilum, cystic changes, PEF, abnormal vascularity, and cortical hyperechogenicity were independent US features suggestive of metastatic LNs in thyroid cancer. Both K-TIRADS and the ETA guidelines provided similar risk stratification for metastatic LNs with a high correlation; however, the ETA guidelines failed to classify 9.8% of LNs into a specific risk stratum. These results may provide a basis for revising LN classification in future guidelines.

Surgery of a Solid Hemangioblastoma at the Cervicomedullary Junction

  • The surgical removal of solid deep-seated hemangioblastomas remains challenging, because treatment of these lesions is often complicated by severe bleeding associated with the rich vascularity of this tumor, and by severe neural tissue injury associated with the difficulty of en bloc resection, especially when the tumor is located at the cervicomedullary junction. Therefore, preoperative embolization of deep-seated solid hemangioblastomas may play an important role in successful surgical removal by reducing major bleeding and neural tissue damage. A 24-year-old woman, 28-weeks pregnant, was admitted to our hospital for the evaluation of quadriparesis, and brain magnetic resonance imaging[MRI] revealed intra-axial mass lesion in the cervicomedullary junction. After delivery, her neurologic symptoms became aggravated, and we decided to operate. Preoperative angiography revealed a hypervascular tumor in the posterior fossa, and embolization of the main feeding artery using gelfoam and microcoil, resulted in marked reduction of tumor vascularity. She underwent a midline suboccipital craniotomy involving the removal of the arch of C-1. The tumor was totally removed through a midline myelotomy, and at her 6-month follow-up she walked independently. We report on the combined use of the preoperative embolization of feeding vessels and subsequent operative resection in a patient with a solid hemangioblastoma at the cervicomedullary junction immediately after delivery.

변형된 공장-장간막 복합 유리피판을 이용한 인두식도 재건 (Pharyngoesophageal Reconstruction Using Modified Jejunomesenteric Composite Free Flap)

  • Purpose: The jejunal free flap is the most standard and reliable procedure of reconstruction of the circumferential pharyngoesophageal defect because it provides pliable, elastic, secreting mucosa and posses reliable vascular anatomy. In this report, the authors introduce the modification of jejunal free flap for decreasing the complications in fatty complicated patients. Method: After harvesting the jejunum with mesentery and mesenteric vessels, both ends of jejunum were excised remaining the mesenteric portion. The jejunal portion of this composite flap was placed to reconstruct esophagopharyngeal defect area and the mesenteric portion was used to obliterate the dead space at paratracheal region and to cover the vital structure and the vascular anastomotic region. Result: A 72 year-old man with recurrent hypopharyngeal cancer who had about 15 cm sized circumferential pharyngoesophageal defect after total pharyngectomy was reconstructed with jejunomesenteric composite free flap without any complications. Conclusion: The mesenteric flaps at both side of jejunomesenteric composite free flap provide the advantages that could obliterate dead space, that could provide cover for the vital cervical vascular structure in case of vascularity was compromised due to previous radiation therapy, and that could preserve as much vascularity at both ends of jejunal flap as possible.

Transcortical Endoscopic Surgery for Intraventricular Lesions

  • To review recent advances in endoscopic techniques for treating intraventricular lesions via transcortical passage. Articles in PubMed published since 2000 were searched using the keywords 'endoscopy,' 'endoscopic,' and 'neuroendoscopic.' Of these articles, those describing intraventricular lesions were reviewed. Suprasellar arachnoid cysts (SACs) can be treated with ventriculo-cystostomy (VC) or ventriculo-cysto-cisternostomy (VCC). VCC showed better results compared to VC. Procedure type, fenestration size, stent placement, and aqueductal patency may affect SAC prognosis. Colloid cysts can be managed using a transforaminal approach (TA) or a transforaminal-transchoroidal approach (TTA). However, TTA may result in better exposure compared to TA. Intraventricular cysticercosis can be cured with an endoscopic procedure alone, but if pericystic inflammation and/or ependymal reaction are seen, third ventriculostomy may be recommended. Tumor biopsies have yielded successful diagnosis rates of up to 100%, but tumor location, total specimen size, endoscope type, and vigorous coagulation on the tumor surface may affect diagnostic accuracy. An ideal indication for tumor excision is a small tumor with friable consistency and little vascularity. Tumor size, composition, and vascularity may influence a complete resection. SACs and intraventricular cysticercosis can be treated successfully using endoscopic procedures. Endoscopic procedures may represent an alternative to surgical options for colloid cyst removal. Solid tumors can be safely biopsied using endoscopic techniques, but endoscopy for tumor resection still results in considerable challenges.
