Unusual Cardiac Metastasis of Nonvisceral Soft Tissue Leiomyosarcoma in the Right Ventricle: A Case Report and Literature Review (우심실로 전이된 내장 외 연부조직 평활근육종: 증례 보고 및 문헌 고찰)

  • Leiomyosarcoma (LMS) is a soft tissue sarcoma that originates from smooth muscle cells. It is commonly found in the uterus but can occur throughout the body, including the retroperitoneal space, abdominal cavity, and any vascular structure. Although there are many case reports of uterine or vascular LMS metastasizing to the heart, cardiac metastasis from nonvisceral lesions has only been reported in two cases. Herein we report a rare case of a patient presenting metastatic LMS from the left flank in the right ventricle observed with echocardiography and enhanced computed tomography.

Clinical Course of Suspected Diagnosis of Pulmonary Tumor Thrombotic Microangiopathy: A 10-Year Experience of Rapid Progressive Right Ventricular Failure Syndrome in Advanced Cancer Patients

  • Background and Objectives: Several cases involving severe right ventricular (RV) failure in advanced cancer patients have been found to be pulmonary tumor thrombotic microangiopathies (PTTMs). This study aimed to discover the nature of rapid RV failure syndrome with a suspected diagnosis of PTTM for better diagnosis, treatment, and prognosis prediction in clinical practice. Methods: From 2011 to 2021, all patients with clinically suspected PTTM were derived from the one tertiary cancer hospital with more than 2000 in-hospital bed. Results: A total of 28 cases of clinically suspected PTTM with one biopsy confirmed case were included. The most common cancer types were breast (9/28, 32%) and the most common tissue type was adenocarcinoma (22/26, 85%). The time interval from dyspnea New York Heart Association (NYHA) Grade 2, 3, 4 to death, thrombocytopenia to death, desaturation to death, admission to death, RV failure to death, cardiogenic shock to death were 33.5 days, 14.5 days, 7.4 days, 6.4 days, 6.1 days, 6.0 days, 3.8 days and 1.2 days, respectively. The NYHA Grade 4 to death time was 7 days longer in those who received chemotherapy (7.1 days vs. 13.8 days, p value=0.030). However, anticoagulation, vasopressors or intensive care could not change clinical course. Conclusions: Rapid RV failure syndrome with a suspected diagnosis of PTTM showed a rapid progressive course from symptom onset to death. Although chemotherapy was effective, increased life survival was negligible, and treatments other than chemotherapy did not help to improve the patient's prognosis.

  • Background: Osteoradionecrosis is the most dreadful complication after head and neck irradiation. Orocutaneous fistula makes patients difficult to eat food. Fibular free flap is the choice of the flap for mandibular reconstruction. Osteocutaneous flap can reconstruct both hard and soft tissues simultaneously. This study was to investigate the success rate and results of the free fibular flap for osteoradionecrosis of the mandible and which side of the flap should be harvested for better reconstruction. Methods: A total of eight consecutive patients who underwent fibula reconstruction due to jaw necrosis from March 2008 to December 2015 were included in this study. Patients were classified according to stages, primary sites, radiation dose, survival, and quality of life. Results: Five male and three female patients underwent operation. The mean age of the patients was 60.1 years old. Two male patients died of recurred disease of oral squamous cell carcinoma. The mean dose of radiation was 70.5 Gy. All fibular free flaps were survived. Five patients could eat normal diet after operation; however, three patients could eat only soft diet due to loss of teeth. Five patients reported no change of speech after operation, two reported worse speech ability, and one patient reported improved speech after operation. The ipsilateral side of the fibular flap was used when intraoral soft tissue defect with proximal side of the vascular pedicle is required. The contralateral side of the fibular flap was used when extraoral skin defect with proximal side of the vascular pedicle is required. Conclusions: Osteonecrosis of the jaw is hard to treat because of poor healing process and lack of vascularity. Free fibular flap is the choice of the surgery for jaw bone reconstruction and soft tissue fistula repair. The design and selection of the right or left fibular is dependent on the available vascular pedicle and soft tissue defect sites.

  • A 64-year-old Korean woman, presented a hemangiopericytoma of the sublingual fossa and was of 10 years' duration. It was encapsulated and considered benign. The hemangiopericytoma is an uncommon tumor which is a vascular neoplasm characterized by the proliferation of capillaries surrounded by masses of round or spindle-shaped cells. The treatment of most reported cases of hemangiopericytoma has been surgical excision because of no recurrence and the excellent results obtained.

  • Lipoma is a benign neoplasm composed of mature fat cells and usually circumscribed by a fibrous capsule. The fat cells are arranged in irregular lobules, partitioned by fibrous septa with supportive vascular channels. Lipoma of the oral cavity is uncommon, and has been reported to be infrequent in the literature in the world so far. An oral lipoma mainly occurs in the cheek and tongue. Surgical excision is the only treatment recommended, and prognosis is uniformly excellent. Recently we experienced two cases of lipoma of the oral cavity and removed the tumor completely by surgical excision.

  • Orofacial pain can be caused by intracranial disorders or can be musculoskeletal, vascular, internal derangemental, and neurologic in origin. The neurologic pain is derived from structural and functional disorders of nerve, and the trigeminal neuralgia is the typical manifestation. Trigeminal neuralgia is known from centuries ago, and is one of the most common pains in human. We present our experience with three patients who have trigeminal neuralgia. The first case is a 50-year-old female who had no specific evidence radiographically. Second is a 50-year-old male with microvascular compression on right trigeminal nerve. The third case is a 60-year-old female who had a neoplasm in cerebellopontine angle with associated mass effect.

  • Hemangiopericytoma is a rare neoplasm of vascular origin and arises in almost any part of the body. We experienced a 26 year-old man of primary pulmonary hemangiopericytoma with 4 months history of hemoptysis. Pulmonary hemangiopericytoma has no characteristic clinical or radiological features to distinguish it from other tumor of the lung, and because of that reason, wide surgical excision appears to be the treatment of choice. The hemangiopericytoma is usually encapsulated, and is composed of capillary pericytes histologically. The prognosis of the tumor is poor. Our case was dead 1 year after surgery because of the recurrence.

  • Trigeminal neuralgia (TN) is generally characterized by lancinating, unilateral, paroxysmal pain occurring in the distribution of the fifth cranial nerve. TN is diagnosed clinically based on the typical patient history, negative findings in a neurologic examination, and the response to medication. Idiopathic TN is the most common type, but TN can result from vascular malformation, compression, trauma, neoplasm, multiple sclerosis, or inflammation. We report a TN case diagnosed as recurrent trigeminal neuritis of the maxillary branch confirmed by magnetic resonance imaging.

  • Adrenal cortical tumors are rare in adults and children. Most are malignant and functional. The principal clinical features are virilization, Cushing's syndrome, hyperaldosteronism and feminization. Recently, we treated a case of virilizing adrenal cortical tumor in a 26 month-old boy. The diagnosis was made by hormone assay, abdominal CT and tissue pathology. Right adrenalectomy was successful performed. Pathologic examination revealed an adrenal cortical adenoma with vascular invasion.

Primary Intimal Sarcoma Originating from Pulmonary Valve. (폐동백 판막에 발생한 폐동맥 내막육종)

  • Primary intimal sarcoma of the pulmonary artery is a rare disease and there has been no report of any case originating from the pulmonary valve. Recently we experienced a 62 year-old female patient who had a primary intimal sarcoma of the pulmonary valve with distal metastasis. She was brought to medical attention due to exertional dyspnea facial edema productive coughing and general weakness for 1 month. Chest CT and echocardi-ography suggest an acute pulmonary thromboembolism or tumor. Exploration showed a large polypoid mass arising from the pulmonary leaflets and multiple masses on distal pulmonary arteries. We replaced the pulmonary valve and reconstructed the pulmonary artery. She received radiotherapy 1 month postoperatively and now 4 months after surgery she has begun receiving chemotherapy.
