  • Published data have shown that the levels of vascular endothelial growth factor (VEGF) and soluble VEGF receptor-1 (sVEGFR-1) in plasma and pleural effusion might be usefulness for lung cancer diagnosis. Here, we performed a prospective study to investigate the utility of VEGF and sVEGFR-1 in bronchoalveolar lavage fluid (BALF) for differential diagnosis of primary lung cancer. A total of 56 patients with solitary pulmonary massed by chest radiograph or CT screening were enrolled in this study. BALF and plasma samples were obtained from all patients and analyzed for VEGF and sVEGFR-1 using a commercially available sandwich ELISA kit. The results showed that the levels of VEGF in BALF were significantly higher in patients with a malignant pulmonary mass compared with patients with a benign mass (P < 0.001). However, no significant difference of sVEGFR-1 in BALF was found between malignant and non-malignant groups (P = 0.43). With a cut-off value of 214 pg/ml, VEGF showed a sensitivity and specificity of 81.8% and 84.2%, respectively, in predicting the malignant nature of a solitary pulmonary mass. Our study suggests that VEGF is significantly increased in BALF among patients with lung cancer than in benign diseases. Measurement of VEGF in BALF might be helpful for differential diagnosis of primary lung cancer.

  • Background: The benefits of epicardial ultrasound scanning (EUS) in coronary artery bypass grafting (CABG) have not yet been established. The aim of this study was to evaluate the usefulness of EUS in CABG, including in the assessment of the quality of distal anastomoses, the identification of epicardial target vessels, and the evaluation of any graft issues other than the distal anastomoses. Methods: Fifty-three patients undergoing CABG were enrolled between March 2018 and February 2019. Intraoperative EUS was performed along with transit-time flow measurement (TTFM). Graft evaluations were performed early (shortly after surgery) and 1 year after surgery for 53 (100%) and 47 (88.7%) patients, respectively. Results: EUS was applied to assess the quality of all distal anastomoses, 32 target vessels, and 2 conduit trunks. Insufficient TTFM findings were obtained for 18 grafts. However, graft revision was performed for only 3 distal anastomoses; based on the EUS findings, the remaining 15 sites were not revised. The early and 1-year overall graft patency rates were 100% (141 anastomoses) and 96.1% (122 of 127 anastomoses), respectively. All 15 of the distal anastomoses that were not revised despite insufficient TTFM results were patent at the 1-year mark. Conclusion: The routine application of EUS in CABG could be beneficial by confirming the quality of surgery and reducing unnecessary procedures.

  • Nuclear imaging is one of the most powerful measures for non-invasive diagnosis of myocardial vascular disease. Radionuclide such as 13N, 15O, 201Tl and 82Rb is used for the measurement of cardiac blood flow. 13N, 15O and 201Tl are produced in cyclotrons while 82Rb is obtained from generator. Rubidium (Rb), an alkali ion, behaves biologically like potassium, and accumulates in myocardial tissue. Rb has rapid blood clearance profile which allows the use of 82Rb with a short physical half-life of 75 s for non-invasive evaluation of regional myocardial perfusion. There are several advantages of 82Rb over other radioisotopes. An ultra-short half-life significantly reduces the exposure of patients to radiation and allows to repeat injections for studying the effects of medical intervention. As a positron emitter, 82Rb allows positron emission tomography (PET) imaging which have shown superior diagnostic performances. 82Rb can be produced from generator by decay of its parent 82Sr. However, the preparation of 82Sr is difficult, because appropriate purity is required to meet the specification of the product. Recently reported procedure from ARRONAX research institute showed that a Chelex-100 resin is sufficient for this purpose and additional column is not necessary. Whereas Brookhaven National Laboratory (BNL) procedure contains three ion exchange resin separation, including Chelex-100 resin. Currently, since 82Sr production site is non-existent in Korea, Korea Atomic Energy Research Institute (KAERI) has plan to produce 82Sr within specifications. We compared 82Sr purification procedures reported from ARRONAX and BNL to investigate the most suitable procedure for our conditions.

  • Objectives: The basis research of aged people's healthcare technology is very important, because our nation goes into an aged society rapidly. Especially in traditional medicine, pulse shapes of aged people were supposed to weak and empty pulse until now, but any objective measurement and compare research do not taken. Methods and Results: Therefore in this study we design observation study to find characteristic of pulse between younger group (age $20{\sim}30$) and elder group (age $43{\sim}80$) in Busan area. We used 3-dimensional pulse analyzer '3-D MAC' (Daeyomedi Co., Ltd.). And major parameters for comparison are 1) pulse height (h1), 2) applied pressure, 3) pulse power (energy), 4) high pressure duration (w) and 5) R-AI. Conclusions: According to analysis result, elder people's pulse has more large value in pulse height, applied pressure and pulse power than younger's that. It reveals that elder people in modernized lifestyle has Yang symptoms as well Yin symptoms. Consequently we need new method to care elder people's health, and this means more important direction than old text book. From now on verification research with objective diagnosis device like 3-D MAC, must be activated to build of evidence in traditional medicine. • This study was done to examine the home care service provided by home care nurses and the level of client satisfaction. Data were collected from 110 clients who received the home care service at the P-hospital in Pusan from 23th April, 2001 to 30th November, 2002. The tool for measurement of satisfaction was composed of 16 items and was 4 score scale. Data was analyzed by using SPSS/WIN 10.5 program, the results of this study are summarized as follows; 1) 77.3% among 110 clients were over sixty years old. By the disease pattern. subjects were distributed into cancer(52.7%), cerebro-vascular disease(22.7%) and the others. 36.4% of clients were received home visits from 10 to 20 times by home care nurse. 2) The total number of home care services was 20,828. And most of the clients received the health education and training. 3) The mean score of satisfaction on provided home care services was $3.36\pm0.45$, out of 4. among 16 items. 'The home care nurses were kind enough' was highest ($3.59\pm0.49$), the total level of satisfaction of home care services was very high. with total mean score $53.84\pm7.16$. As mentioned above. the satisfaction level of home care services was very high. so we can except that the prospect of home care services is very challengeable. Therefore. we should try to expand the service recipients with promotional education to the home care clients under the cooperation with medical staff and make more efforts to develop the management system about clients' information. to improve the quality of the service, to assure close ties with the public health centers for the continuous home care service linking.

  • The aim of the present study was to evaluate the relative contribution of CYP1A2 isoforms (-3860 G/A, -2467T/delT and -163C/A) in control subjects and breast cancer patients to the metabolism of caffeine in human liver. Restriction fragment length polymorphism analysis of PCR-amplified Fragments (PCR-RFLP) was used for the genotyping of CYP1A2 SNPs and HPLC allowed the phenotyping through the measurement of CYP1A2 activity using the 17X + 13X + 37X/137X urinary metabolite ratio (CMR) and plasma caffeine half life (T1/2). The CYP1A2 -3860A genotype was associated with a decreased risk of breast cancer. In contrast, distributions of the CYP1A2 -2467T/delT or -2467delT/delT and -163A/C or A/A genotypes among breast cancer patients and controls were similar. When the genotype and phenotype relationship was measured by comparing the mean CMR ratios and caffeine half life within the genotype groups between subjects and breast cancer patients, there were no significant differences except for -3860 A, most of them being homozygous for the -3860 G/G SNP and had a significant higher mean CMR ratio and half life than those with -3860 G/A (P=0.02). The results of this preliminary study show a significant association between CP1A2 -3860 G variant and CYP1A2 phenotype which must be confirmed by further large-size case-control studies.
