경부 식도협착 재건술에 있어서 유리공장 이식편의 이용 (Reconstruction of the Cervical Esophagus Using the Free Jejunal Graft)

  • The cervical esophageal stricture has various surgical modalities and difficulties in reconstruction. We had experienced a case of successful reconstruction of the cervical esophageal restenosis using the free jejunal graft, on 30 year old man had had esophageal stricture after ingestion of lye. He had undergone colon interposition[esophagocologastrostomy] with left colon feeding gastrostomy. But restenosis was occurred just above of the cervical esophagocolostomy site several times of balloon dilatation were failed. So, we decided to use of the free jejunal graft. The free jejunal graft was isolated about 15cm length with it`s vascular arcades. The graft was irrigated with the mixed solution as isotonic saline, heparin and papaverine chloride. The artery of graft was anastomosed to the branch of the external carotid artery in end to side with continuous sutures of the 8.0 Prolene. The vein of the graft was anastomosed to the branch of the anterior facial vein in end to end with continuous sutures of the 8.0 prolene. Postoperative course was uneventful and the patient was discharged after removal of the tracheostomy cannula and gastrostomy tube.

Comparison of Outcomes of Hybrid and Surgical Correction for De Novo Arteriovenous Graft Occlusion

  • Purpose: This study aimed to compare surgical revisions and balloon angioplasty after surgical thrombectomy on thrombosed dialysis access as a first event. Materials and Methods: Records of patients undergoing creation of arteriovenous grafts (AVGs) at the Gachon University Gil Medical Center between March 2008 and February 2016 were reviewed. Among them, patients who underwent treatment on first-time thrombotic occlusion after AVG creation were identified. Outcomes were primary, primary-assisted, and secondary patency. The patency was generated using the Kaplan-Meier method, and patency rates were compared by log-rank test. Results: A total of 59 de novo interventions (n=26, hybrid interventions; n=33, surgical revisions) for occlusive AVGs were identified. The estimated 1-year primary patency rates were 47% and 30% in the surgery and hybrid groups, respectively. The estimated primary patency rates were not different between the two groups (log-rank test, P=0.73). The Kaplan-Meier estimates of 6 and 12 months for primary-assisted patency rates were 68% and 57% in the surgery group and 56% and 56% in the hybrid group. The Kaplan-Meier estimates of 12 and 24 months secondary patency rates were 90% and 71% in the surgery group and 79% and 62% in the hybrid group. There were no differences in the estimated primary-assisted and secondary patency rates between the two groups. Conclusion: Our results showed no significant difference between the two groups in terms of primary patency (P=0.73), primary-assisted patency (P=0.85), and secondary patency (P=0.78). However, percutaneous transluminal angioplasty can give more therapeutic options for both surgeons and patients.

Paclitaxel Coating Inhibits Inflammation Surrounding Subcutaneously Implanted Expanded Polytetrafluoroethylene (ePTFE) Hemodialysis Grafts in Rabbit Model

  • Hemodialysis vascular access dysfunction (HVAD) due to the aggressive development of venous neointimal hyperplasia remains a major complication for patients with synthetic arteriovenous grafts. Paclitaxel-coated expanded polytetrafluoroethylene (ePTFE) grafts effectively prevent neointimal hyperplasia and stenosis. However, perigraft inflammation or edema can be another complication of ePTFE grafts, preventing early cannulation. Three different types of ePTFE grafts, including grafts without paclitaxel coating (control group, n = 12), grafts with paclitaxel coating at a dose density of $0.61ug/mm^2$ (low concentration group, n = 12), and grafts with paclitaxel coating at a dose density of $1.15ug/mm^2$ (high concentration group, n = 12) were placed in the backs of 12 rabbits, simultaneously. Six rabbits were euthanized after one week and the remaining six were euthanized two weeks after implantation. Perigraft inflammation, graft wall inflammation, stromal cell proliferation, blood vessel formation, tissue necrosis and edema were analyzed for the grafts in each animal. Inflammation surrounding the paclitaxel-coated grafts was significantly reduced compared to the control group. Stromal cell layers were detected at the interface between the graft and the surrounding tissue in the control group, infiltrated into the graft interstices, and differentiated into myofibroblasts for graft healing. Paclitaxel-coated grafts inhibited stromal cell proliferation and infiltration into the graft wall. Tissue necrosis and edema were not detected in either of the paclitaxel-coated graft groups.

  • Bentall`s operation for repair of annuloaortic ectasia has been associated with postoperative bleeding and with false aneurysm of the anastomotic site between the coronary orifice and composite graft.Among 5 cases, 2 cases have been operated direct anastomosis between coronary artery and vascular graft.Remained 3 cases have been operated with doughnutlike Teflon felt buttress.The technique of sandwiching the freed button of aortic wall bearing the coronary artery ostium between an outer Teflon felt doughnutlike buttress and the inner composite graft provides a leak-proof anastomosis.We experienced one case reoperation for bleeding at coronary anastomotic site above method.

잡견에서의 알진 코팅 인조혈관 삽입실험 (Experimental Evaluation of Algin-coated Vascular Grafts in Dogs)

  • Microvel knitted double velour vascular grafts coated with biodegradable algin were evaluated in the canine experimental model as a new biologically coated Dacron graft. Three series of implantations were conducted involving the insertion of 6 mm diameter grafts in the abdominal aortae of mongrel dogs. The first series used the regular Microvel vascular grafts coated with algin,whereas the second and third series used Hemashield [collagen-coated grafts and the regular Microvel grafts with preclotting,respectively as control groups. Each series involved the implantation of one prosthesis for each of 2 preselected periods,namely 3 months and 6 months. In addition,algin-impregnated grafts were implanted for 4 hours,72 hours,2 weeks,and 4 weeks. All grafts were patent when the animals were sacrificed at intervals ranging from 4 hours to 6 months. Histological examinations revealed no obvious or significant differences in the healing characteristics of the algin-coated grafts and the control grafts after 3 months and 6 months of implantation. Endothelial cell-like cells were present on the midsegments of all grafts explanted from animals sacrificed after 3 months and 6 months,except a suspicious finding in the 3 month-implantation animal of a preclotted graft. With special stains,the algin became invisible between the polyester filaments during the first 3 months of implantation. This study has demonstrated that the use of a biodegradable algin coating is a feasible approach as biological sealants for textile arterial prostheses.

Management of Proximal Iliac Artery Injury during Lumbar Discectomy with Stent Graft

  • Iatrogenic vascular injuries during lumbar disc surgery may occur rarely but they are serious complications, which can be fatal without appropriate management Prompt diagnosis and management of these complications are imperative to prevent a desperate outcome. A 72-year-Old female with proximal left common iliac artery iatrogenic injury during lumbar discectomy was successfully treated by percutaneous deployment of a stent graft in an emergency setting. Postprocedural angiogram demonstrated complete exclusion of the iliac artery laceration. The patient became hemodynamically stable. Two weeks later she complained of vascular claudication. Follow-up angiography revealed decreased arterial flow in the opposite common iliac artery. An additional kissing stent was inserted into the right common iliac artery and the symptoms of vascular claudication disappeared. Endovascular stenting offers a safe and effective method for the treatment of an iatrogenic arterial laceration, particularly in a critical condition. But, the contralateral iliac arterial flow should be kept intact in case of proximal iliac artery injury. Otherwise, additional treatments may be needed.

조직공학적 인조혈관의 생체 내 이식 실험 (In Vivo Experiment of Tissue-Engineered Artificial Vessel)

  • 관상동맥 질환과 말초혈관 질환의 증가에 따라 직경 6 mm 이하의 소구경 혈관의 필요성이 증가하고 있다. 저자들은 조직공학적 방법을 이용하여 소구경 인공혈관을 제작하여 생체 실험을 시행하였다. 동종 판막을 얻어 이를 탈세포화시킨 후 피실험동물의 골수를 채취하여 탈세포화시킨 혈관용 지지체(scaffold) 위에 이식하였다. 이와 같이 하여 제작된 인공 혈관을 잡견의 양측 경동맥에 이식한 후 혈관이 막히거나, 8주가 되었을 때 이를 제거하여 조직학적 검사를 시행하였다. 자가 세포를 이식하지 않고 지지체만을 이식하였던 대조군 4마리 중 3마리의 혈관은 2주 이내에 모두 막힌 것을 확인하였고 나머지 한 마리의 혈관은 혈관류(aneurysm)가 발생하였다. 그러나 자가 세포를 이식한 실험군 4마리 중 2마리는 각각 4주와 6주까지 혈관의 개통성을 유지하였고, 나머지 2마리는 8주까지 혈관의 개통성을 유지하였다. 조직학적 검사 결과, 8주까지 개통성을 유지하였던 혈관은 정상의 혈관과 거의 유사한 조직학적 구조를 나타내었다. 자가 세포와 탈세포화된 지지체를 이용하여 제작한 인공혈관은 조직학적 검사 결과 정상과 유사한 구조로 재생이 가능함을 보여주었다.

유리 장 이식편을 이용한 식도 재건 (Free jejunal graft for replacement of cervical esophagus)

  • Reconstruction of the pharynx and cervical esophagus is a difficult surgical problem. A successful case of reconstruction of cervical esophagus by free jejunal graft upon a 23-year-old female who had stricture in the pharynx and cervical esophagus after ingestion of hydrochloric acid is presented. This procedure was done after the primary traditional reconstruction with right sided colon had resulted in restenosis due to necrosis of the cervical portion of the graft. A proximal jejunal segment, about 12 cm in length was isolated for free graft preserving its vascular arcade. Both superior thyroid artery and vein were anastomosed to the graft vessels in end to end by continuous suture of 8-0 monophil. Nylon. The postoperative course was uneventful. The patient has been followed for 4 months after operation and she can eat every kind of food without dysphagia. We think free jejunal graft offers an excellent and safe method of reconstructing cervical esophagus and pharynx with definitive advantages over other traditional techniques.

혈류의 유동이 혈관-인조혈관 접속부 혈관 내막 세포증식에 미치는 영향 (Hemodynamic Effects on Artery-Graft Anastomotic Intimal Hyperplasia)

  • Wall shear rate or stress is believed to be a major hemodynamic variable influencing atherosclerosis and artery-graft anastomic intimal hyperplasia. The purpose of this study is to verify the effects of radial wall motion, artery-graft compliance and diameter mismatch, and impedance phase angle on the wall shear rate distribution near an end-to-end artery-graft anastomosis model. The results show that radial wall motion of the elastic artery model lowers the mean wall shear rates under pulsatile flow condition by 15 to 20 % comparing to those under steady flow condition at the same mean flow rate. Impedance phase angle seems to have small effects on the mean and amplitude of the wall shear rate distribution. In order to study the effects of compliance and diameter mismatch on the wall shear rates, two models are studied-Model I has 6% and Model I has 6% and Model II has 11% smaller graft diameter. Divergent geometry caused by diameter mismatch near the distal sites reduces the mean wall shear rates significantly, and this low shear region is believed to be prone to intimal hyperplasia.

Indocyanine Green Videoangiography for Confirmation of Bypass Graft Patency

  • Objective : The aim of the study is to determine the efficacy of indocyanine green (ICG) videoangiography for confirmation of vascular anastomosis patency in both extracranial-intracranial and intracranial-intracranial bypasses. Methods : Intraoperative ICG videoangiography was used as a surgical adjunct for 56 bypasses in 47 patients to assay the patency of intracranial vascular anastomosis. These patients underwent a bypass for cerebral ischemia in 31 instances and as an adjunct to intracranial aneurysm surgery in 25. After completion of the bypass, ICG was administered to assess the patency of the graft. The findings on ICG videoangiography were then compared to intraoperative and/or postoperative imaging. Results : ICG provided an excellent visualization of all cerebral arteries and grafts at the time of surgery. Four grafts were determined to be suboptimal and were revised at the time of surgery. Findings on ICG videoangiography correlated with intraoperative and/or postoperative imaging. Conclusion : ICG videoangiography is rapid, effective, and reliable in determining the intraoperative patency of bypass grafts. It provides intraoperative information allowing revision to reduce the incidence of technical errors that may lead to early graft thrombosis.
