We analysed 60 consecutive patients who got Senning operation for transposition of the great arteries [TGA] with or without ventricular septal defects [VSD]. There were 41 simple TGA [group I] and 19 TGA with VSD [Group II], the operative mortality was 20 % [in group I 4.9 %, group II 52.6 %]. Among the survivors [n=48], the mean follow-up period was 7 years [range, 1 year to 13.5 years] and the actuarial survival rate at 13 years were 95 % in group I and 42 % in group II. Preoperative high left ventricular pressure and high pulmonary arterial pressure affected the surviving [p<0.01]. There occurred various type of arrhythmia like junctional rhythm, first degree atrioventricular [AV] block, sick sinus syndrome and complete AV block, and we inserted 2 permanent pacemakers for these patients. The incidence of arrhythmia were 28.2 % [11/39] in group I and 55.6 % [5/9] in group II, and the actuarial freedom from arrhythmia at 13 years after operation was 66 % [71 % in group I, 44 % in group II]. Increased aortic cross clamping time had affected the development of arrhythmia [p<0.05] which meant the complexity of the operation. The total incidence of left ventricular outflow tract obstruction [LVOTO] was 31.3 % [15/48], but only 3 patients [6.25 %] showed the significant gradient requiring reoperation. The pulmonary venous pathway obstruction [PVO] were found in 3 patients, all in group I, and among them only one required the reoperation. The estimated freedom from PVO was 89 % at 13 years [87 % in group I, 100 % in group II], but we couldn`t find any significant systemic venous obstruction in our series. There occurred 27.1 % [13/48] mild degree tricuspid valve regurgitation without necessary surgical correction. We experienced 14.6 % [7/48] reoperation rate: 3 residual VSD, 3 LVOTO, 1 PVO, 3 atrial baffle leakage. For this high incidence of complication rate after Senning operation and high mortality in TGA with VSD, We do not use this kind of surgical modality any more and do the Jatene operation for all the TGA patients since several years ago.
We present one case of 26-year-old male having saddle block combined with mitral valvular disease [NYHA Class IV] with auricular fibrillation. The most common cause of emboli is atrial fibrillation. The clinical manifestations of saddle emboli are relatively slow due to development of collateral circulation and large size of lumen of the aorta. The 5month duration of saddle emboli in this case led to severe atrophic changes, coldness, peripheral cyanosis on the both lower extremities, and flexion deformity on the knee and ankle joint of the left lower extremity. We planned staged operation for the saddle block and for mitral stenoinsufficiency and tricuspid insufficiency, because of poor general condition of the patient. The thromboembolectomy of aortic bifurcation was performed through the transabdominal approach without trial of Fogarthy catheter embolectomy, because of expectation of the secondary inflammatory changes of the vessel wall and thrombi which was 3 cm X 1 cm X 0.5 cm in size with irregular surfaced solid in consistency. 1 month later, after thromboembolectomy, mitral valve replacement and tricuspid annuloplasty were performed, with successful early operative result. During operation organized thrombi [1 cm X 0.5 cm] in the left auricle was removed. We wonder if simple management using Fogarthy catheter might be possible to remove the thromboemboli instead of thromboembolectomy by aortotomy in this case.

In this study, a BMW plant process control system model which produces BMW is suggested and the BMW plant process controller with the following functions is developed. The first function is to operate the electronic overload relays to stop the blower for a certain period of time and to re-operate it again when the blower is overloaded. The second function is to close the motor operated valve automatically in case of power failure to prevent the circulation from the guided tank to the compost throwing tank and to block leak from the compost throwing tank due to the failure of ball valve. The third function is to transfer produced BMW from the concentration tank to 4 storage tanks for automatic managing of the BMW output. A device to measure the signal of the BMW plant process controller and a test equipment are developed. The designed BMW plant process controller is checked to see if it operates correctly according to the design specifications. The sequence control method based on BMW plant process controller is developed at a low cost in this study, so it is expected to bring improvements in the stability and the efficiency of system and to cause reductions in the operation and the management costs in the future.

Oil hydraulic piston pumps are being extensively used in the world, because of simple design, light weight and effective cost etc. An oil hydraulic pump is likely to have serious problems of high leakage, friction and low energy efficiency according to large time use. In the oil hydraulic piston pumps the clearance between the valve block and piston plays an important role for volumetric and overall efficiency. In this paper, the wear property of the SACM645 material used the hydraulic piston pump has been work out by experimentation with variable heat treatment. To investigate the effect according to the piston surface condition, seven different types specimen were prepared. From the wear test results, induction hardening and nitration were definitely superior to the others. On the whole, nitration was estimated for high strength material to wear resistance.

A Coolant core for Range Extender engine has been developed using CFD technique. Coolant by-pass has been added to the improved model to reduce temperature near and between exhaust valve. Due to the increased coolant flow-rate both around the second cylinder block and between exhaust valves, improved model shows no significant stagnant flow compared with base model. Finally, the improved model shows improved heat transfer coefficients near exhaust valves, and 5% reduced pressure-drop through the coolant core. Reduced pressure-drop may increase the fuel efficiency by reducing the load of a coolant pump.

This study presents an HFC152a refrigerant air conditioner as an alternative to HFC134a, which is currently used in mobile air conditioning systems. Cool-down performance tests of an HFC152a air conditioning system were conducted and compared to a baseline HFC134a air conditioner. The experimental set-up consisted of a belt-driven compressor, a sub-cooled type condenser, an evaporator, and a block-type thermal expansion valve (TXV). A drop-in test was carried out on the mobile air conditioning system under various vehicle running speeds in a climate-controlled wind tunnel (CWT). Additionally, to optimize the HFC152a air conditioning system, the effects of the TXVs on the performance were studied. The results show that compared to the HFC134a air conditioning system, the refrigerant charge quantity was reduced by approximately 20%, the discharge pressure was reduced by about 350~430 kPa, and the air discharge temperature at vehicle running conditions was $0.5{\sim}1.5^{\circ}C$ lower. In addition, good compressor durability was expected due to the lower compression ratio.

Automatic power transmission systems consisted of a torque converter and several planetary gear sets, clutches and brakes are controlled by a hydraulic shift control circuit and an electronic transmission control unit. The hydraulic circuit serves for the operation of the torque converter and lubrication oil supply of the transmission system as well as for the actuation of clutches for the automatic gear shift. The complicated hydraulic control circuit constructed by many spools, solenoids, orifices and flow passages are integrated into one small valve block and it is powered by one hydraulic pump. In this paper, a test equipment was developed to measure the oil consumption of each component at various wide operating conditions. Test data about 730 sets acquired from five test items are analyzed and discussed on the oil capacity of the circuit.

In this study, the performance of mobile air conditioner(MAC) systems to which the refrigerants R134a and R1234yf were used was evaluated to compare the characteristic of automotive refrigeration cycles with refrigerant. The experimental setup of a MAC consists of an belt driven compressor, a condenser, an evaporator and a block type thermal expansion valve. The drop-in test on MAC were carried out under variable compressor speed from 800 to 2500 rpm. Performance test by using R1234yf and R134a in the same system revealed low the charge amount and mass flow rates for using R1234yf, that is, up to 10% and 17%, respectively. The compressor discharge temperature of R1234yf is $8^{\circ}C$ lower than that of R134a. The cooling capacity with R1234yf system decreased by 4~7% compared with R134a system. In addition, The COP of R1234yf system is lower 3~4% than that of R134a system.
Purpose : The prognosis of patients with corrected transposition of the great arteries(C-TGA) is variably affected by associated intracardiac defects, systemic right ventricular function, tricuspid valve competence, and conduction disturbances. This study aims to evaluate the importance of those factors at mid-term follow-up. Methods : Medical records of 94 patients(males 58, females 36; mean age at last follow-up, $12{\pm}9$ years; mean follow-up duration, $9{\pm}6.4$ years) diagnosed between January 1980 and May 2002 at Seoul National University Children's Hospital were studied retrospectively. Results : Among 94 patients, operations were performed in 72 patients(classic operations in 55; double switch operations in 17). Among prognostic factors including associated intracardiac anomalies(at least moderately severe tricuspid insufficiency(TI), ventricular septal defect, pulmonary stenosis and pulmonary atresia), intracardiac operation and complete atrioventricular block, TI was the only significant factor for death(P=0.001), and in turn, Ebstein anomaly and high grade atrioventricular block predicted TI. 20-year survival without TI was 77%, but only 35% with TI(P=0.0002); excluding perioperative death, the 20-year survival rates with and without TI were 48% and 87% respectively(P=0.008). There was no statistical difference in 20-year survival rate or association with TI between classic and double switch operation. Conclusion : TI was the major prognostic factor for C-TGA and was associated with Ebstein anomaly and high grade atrioventricular block at mid-term follow-up. Long-term follow-up is required to evaluate other factors, including double switch operations and associated intracardiac defects more exactly.
Background: Sixty five cases with congenitally corrected transposition of the great arteries (CCTGA) indicated for biventricular repair were operated on between 1984 and september 1998. Comparison between the results of the conventional(classic) connection(LV-PA) and the anatomic repair was done. Material and Method: Retrospective review was carried out based on the medical records of the patients. Operative procedures, complications and the long-term results accoding to the combining anomalies were analysed. Result: Mean age was 5.5$\pm$4.8 years(range, 2 months to 18years). Thirty nine were male and 26 were female. Situs solitus {S,L,L} was in 53 and situs inversus{I,D,D} in 12. There was no left ventricular outflow tract obstruction(LVOTO) in 13(20%) cases. The LVOTO was resulted from pulmonary stenosis(PS) in 26(40%)patients and from pulmonary atresia(PA) in 26(40%) patients. Twenty-five(38.5%) patients had tricuspid valve regurgitation(TR) greater than the mild degree that was present preoperatively. Twenty two patients previously underwent 24 systemic- pulmonary shunts previously. In the 13 patients without LVOTO, 7 simple closure of VSD or ASD, 3 tricuspid valve replacements(TVR), and 3 anatomic corrections(3 double switch operations: 1 Senning+ Rastelli, 1 Senning+REV-type, and 1 Senning+Arterial switch opera tion) were performed. As to the 26 patients with CCTGA+VSD or ASD+LVOTO(PS), 24 classic repairs and 2 double switch operations(1 Senning+Rastelli, 1 Mustard+REV-type) were done. In the 26 cases with CCTGA+VSD+LVOTO(PA), 19 classic repairs(18 Rastelli, 1 REV-type), and 7 double switch operations(7 Senning+Rastelli) were done. The degree of tricuspid regurgitation increased during the follow-up periods from 1.3$\pm$1.4 to 2.2$\pm$1.0 in the classic repair group(p<0.05), but not in the double switch group. Two patients had complete AV block preoperatively, and additional 7(10.8%) had newly developed complete AV block after the operation. Other complications were recurrent LVOTO(10), thromboembolism(4), persistent chest tube drainage over 2 weeks(4), chylothorax(3), bleeding(3), acute renal failure(2), and mediastinitis(2). Mean follow-up was 54$\pm$49 months(0-177 months). Thirteen patients died after the operation(operative mortality rate: 20.0%(13/65)), and there were 3 additional deaths during the follow up period(overall mortality: 24.6%(16/65)). The operative mortality in patients underwent anatomic repair was 33.3%(4/12). The actuarial survival rates at 1, 5, and 10 years were 75.0$\pm$5.6%, 75.0$\pm$5.6%, and 69.2$\pm$7.6%. Common causes of death were low cardiac output syndrome(8) and heart failure from TR(5). Conclusion: Although our study could not demonstrate the superiority of each classic or anatomic repair, we found that the anatomic repair has a merit of preventing the deterioration of tricuspid valve regurgitations. Meticulous selection of the patients and longer follow-up terms are mandatory to establish the selective advantages of both strategies.
