A Study of the Relationship between Straight Leg Raising, Valsalva Test and Size, Position of Lumbar Disc Herniation (요추부 추간판 탈출 정도와 SLR, valsalva test의 관계)

  • Objectives : This study investigated the relationship between straight leg raising(SLR), valsalva test and size, position of lumbar disc herniation. Methods : We took SLR and valsalva test on 105 patients with lumbar disc herniation. According to the result of MRI findings, this study classified three groups of 105 patients with lumbar disc herniation, bulging, protrusion and extrusion. According to the position of lumbar disc herniation, 72 patients that were diagnosed protrusion and extrsuion were sorted 4 groups, lateral, lateral postero-lateral, central postero-lateral, central. The association size, position of lumbar disc herniation and SLR, valsalva test were analysed. Results : The bigger size of disc herniation, the more positive result of SLR and valsalva test, the lower angle of SLR test. There was not significant association between the position of lumbar disc herniation and the angle of SLR test. Conclusions : The SLR and valsalva test is an useful physical examination to speculate about the degree of lumbar disc herniation.

A Study on the Characteristics of Lumbar Disc Herniation Being Classified Blood Stasis and Phlegm-Retained Fluid (어혈, 담음으로 변증된 요추부 추간판 탈출증 환자들의 특성에 관한 연구)

  • Objectives In this study, we wanted to find out the characteristics - gender, SLR test, valsalva test, duration, ESR, CRP and night pain - of lumbar disc herniation being classified as blood stasis and phlegm-retained fluid. Methods We surveyed 42 patients suffering from lumbar disc herniation using the diagnosis blood stasis syndrome, water retention syndrome of the comprehensive diagnosis of QI blood water. Blood stasis and phlegm-retained fluid are identical with blood stasis syndrome, water retention syndrome. Then we analyzed the characteristics - gender, SLR test, valsalva test, duration, ESR, CRP and night pain - of the patients suffering from lumbar disc herniation who were diagnosed as blood stasis and phlegm-retained fluid. Results Patients were sorted into two groups: 18 blood stasis patients and 30 phlegm-retained fluid patients. Gender (Woman), acute phase and night pain are related to blood stasis. Acute phase, positive of SLR test is related to phlegm-retained fluid. There was no correlation between blood stasis and SLR test, valsalva test, duration, ESR, CRP. Also no correlation between phlegm-fluid and gender, valsalva test, duration, ESR, CRP and night pain. Conclusions By Considering the characteristics - gender, SLR test, valsalva test, duration, ESR, CRP and night pain - of lumbar disc herniation patients, it can help to analyze the pattern of its symptoms.

An effect of the Valsalva maneuver on resistance exercise of trunk flexion and extension (발살바 기법이 몸통 굴곡과 신전에 대한 저항운동에 미치는 영향)

  • Background: The effect of the Valsalva maneuver applied in resistance exercise has shown contradictory results. This study aims to investigate the effect of the Valsalva maneuver on resistance exercise by examining the changes that occur by applying the Valsalva maneuver during trunk flexion and extension resistance exercise in healthy adults. Design: Randomized controlled trial. Methods: 34 healthy adults were conveniently recruited. According to the with or without Valsalva maneuver, the flexion and extension of the trunk were measured and evaluated. Results: Paired t test showed that there was a statistically significant difference in maximum torque of trunk flexion in the resistance exercise with Valsalva maneuver. However, there was no statistically significant difference in trunk extension (p<.05), in the total and average work of trunk flexion and extension (p<.05). Conclusion: The results of this study in which the difference in the amount of exercise according to the application of the Valsalva maneuver was not significant, show that the practice of the Valsalva maneuver, which has a risk of injury, should be avoided in rehabilitation exercises.

  • The object of this study is to report a clinical effect regarding the conservative treatment of the patients who were diagnosed as the herniated intervertebral lumbar disc(HIVD). The patients were treated by the oriental medical conservative treatment including acupunture, Bee venom therapy and Chu-na therapy. We estimated by visual analogue scale and pain rating score subjectively. And we examined the patients with SLR test and valsalva test. Oriental medical conservative treatment improved HIVD subjuctively and objectively.

Autonomic dysfunction in patients with orthostatic dizziness

  • Orthostatic dizziness is feeling dizzy or lightheaded when standing up. Hemodynamic orthostatic dizziness can be caused by autonomic dysfunction such as orthostatic hypotension or postural tachycardia syndrome. The interpretation of the autonomic function test results in patients with orthostatic dizziness is crucial for diagnosing and managing the underlying condition. The head-up tilt and Valsalva tests are especially important for evaluating adrenergic function in patients with hemodynamic orthostatic dizziness. However, it is important to note that autonomic function tests do not cover the entire diagnostic process, since their findings need to be considered along with the detailed history and physical examination results of the patient because various differential diagnoses exist for orthostatic dizziness. Ensuring appropriate treatment by interpreting the autonomic function test results can help to determine the improvement of and prevents falls from orthostatic dizziness.

A Case Report on Cervical Myelopathy Applied Chuna Treatment (추나 요법을 적용한 경수부 척수 손상 환자의 경과관찰 1례)

  • Objects : This study was to report a clinical effect of Chuna Treatment for a patient diagnosed by Magnetic Resonance Imaging(MRI) as Cervical Myelopathy. Methods: In order to alleviate shoulder pain and neck stiffness, the patient was treated by acupuncture therapy, herbal medicine, phamacopucture, cervical traction technique of Chuna treatment and conservative managements. To evaluate the effect of the treatment, Recovery rate of Hirabayashi, VAS(Visual Anlaogue Scale), NDI and ROM(Range of Motion) were used. Lhermitte's sign, Valsalva test, Distraction test, Compression test and Spurling test were carried out. Results: VAS, NDI and ROM were improved and Recovery rate was 100%. Conclusions: Korean Treatment can be effectively used for a patient with Cervical Myelopathy. Further clinical studies are needed to verify the findings.

  • Objects : This study was to report a clinical effect of Korean Treatment for a patient diagnosed by Computed Tomography as Cervical Spinal Stenosis with Ossification of posterior Longitudinal Ligament. Methods : In order to alleviate left shoulder pain and neck stiffness, the patient was treated by acupuncture therapy, herbal medicine, distilled herbal injection, cervical traction technique of Chuna treatment and conservative managements. To evaluate the effect of the treatment, Recovery rate of Hirabayashi, VAS(Visual Anlaogue Scale), NDI and ROM(Range of Motion) were used. Lhermitte's sign, Valsalva test, Distraction test, Compression test and Spurling test were carried out. Results : VAS, NDI and ROM were improved and Recovery rate was 100%. Conclusions : Korean Treatment can be effectively used for a patient with Cervical Spinal Stenosis with OPLL. Further clinical studies are needed to verify the findings.

  • Syncope in children and adolescents have a common occurrence according for up to 15% before adulthood. Micturition syncope, a kind of situational syncope, can be considered a form of reflex syncope. It can typically occur in healthy young men after rising from bed in the early morning who experience sudden loss of consciousness during or immediately after urination. The mechanism of micturition syncope is not completely understood, but it has been suggested that vasovagal reflex mediated bradycardia and peripheral vasodilation and decreased venous return due to Valsalva effect and standing position lead to the decrease in cerebral blood flow resulting in syncope. The causes of syncope are variable. So complete history taking, physical examination, electrocardiography, exercise stress test, echocardiography, head-up tilt table test, electroencephalography(EEG), brain magnetic resonance image and urodynamic study should be required for the diagnosis of micturition syncope. There were several reports about micturition syncope. However, literature of micturition syncope at the pediatric age has rarely been reported in Korea so far. Therefore, we report a case of a 9-year-old boy with micturition syncope with typical EEG findings of high amplitude delta wave and flattening during syncope.

  • Using transcranial doppler ultrasonography (TCD) to measure the presence or absence of abnormalities in patent foramen ovale (PFO) is more effective than transesophageal echocardiography (TEE) because it is simple, inexpensive, and non-invasive. Thus, it can be a good alternative test in cases where it is difficult to perform TEE or in groups at a high risk of complications. Therefore, in this study, we sought to assist in arriving at a diagnosis by suggesting an appropriate method through the comparison of empirical results according to the presence or absence of blood use and varying patient posture. The results were compared depending on the patient's posture and whether blood was used. Patients in whom both middle cerebral arteries were observed were targeted. In cases where the temporal window was defective, it was replaced with basilar artery and compared. There were higher positivity rate and accuracy for sitting position using valsalva maneuver (VM) than for Resting and supine position using VM. Therefore, if it is possible to perform a sitting position using VM with a mixture of blood, it would be a good idea to perform it.
