Effects of Uwhangchungsim-won(Niuhuangqingxin-yuan) on Systemic Blood Pressure, Pulse Rate, Cerebral Blood Flow, and Cerebrovascular Reactivity in Humans (우황청심원(牛黃淸心元)이 정상인의 혈압(血壓), 맥박수(脈搏數), 뇌혈류(腦血流) 및 뇌혈관반응도(腦血管反應度)에 미치는 영향(影響))

  • Objectives: Uwhangchungsim-won(UC) has been used in the treatment of a wide variety of conditions including stroke, hypertension, arterosclerosis, autonomic imbalance, and mental instability, in Korean traditional hospitals. The aim of this study was to evaluate the effect of DC on cerebral hemodynamics and to determine the appropriate dosage. Methods: We studied changes in hyperventilation-induced cerebrovascular reactivity and mean blood flow velocity of the middle cerebral arteries(MCAs) were studied by means of transcranial Doppler ultrasound. Changes in mean blood pressure, pulse rate and expiratory CO2(PECO2) were observed using Cardiocap TM/5. Six healthy young volunteers who were administrated with full doses of DC for group A, and half doses for group B. Six other healthy subjects comprised the control group. The evaluation was performed during basal condition, and repeated at 20, 40, 60, 120, and 180 minutes after administration. Results: Increases of cerebrovascular reactivity and mean blood flow velocity in the middle cerebral artery in group A were significantly different compared with group B and the control group (p<0.1). Mean blood pressure, pulse rate and expiratory CO2 did not change during the observation and were not different among these three groups. We observed that in cerebrovascular reactivity induced hyperventilation, group A was most effective at 40 minutes after administration, and its effectiveness lasted for 120 minutes. Conclusions: This study provides evidence for UC, in full doses, as an agent for dilation of the cerebral arteriols to increase hyperventilation-induced cerebrovascular reactivity as a consequence of faster recovery of blood flow velocity.

Hypertension Treatment: A 118-Case Series in a Korean Medicine Clinic (일개 한의원의 고혈압 한의치료 118례에 대한 고찰)

  • Objectives: The aim of this study was to report a 118-case series demonstrating the treatment of hypertension in a Korean medicine clinic and to evaluate the effect of Korean medicine. Materials & Methods: From 2006 to 2018, patients who visited a Korean medicine clinic for hypertension were investigated by studying changes in blood pressure before and after treatment with herbal medicine, acupuncture, and Uwhangchungsim-won. The average treatment period was 81.6 days. Results: After treatment with Korean medicine, the mean systolic blood pressure (SBP) and diastolic blood pressure (DBP) of all patients decreased significantly (p<0.001). The SBP decreased from $148.9{\pm}10.3$ to $133.8{\pm}13.9$, and the DBP decreased from $91.3{\pm}7.5$ to $82.5{\pm}9.0$. After treatment with acupuncture or Uwhangchungsim-won, the patient showed decreased blood pressure but the differences were not statistically significant when compared with the non-treated group. Conclusions: This study shows the real situation of hypertension treatment in Korean medicine and indicates that Korean medicine could be one of the primary treatment for hypertension. However, this study had limitations, such as variations in the treatment periods, the frequency of acupuncture treatments, and the type of antihypertensive drugs. For further evaluation of the effectiveness of treatment for hypertension using Korean medicine, a well-designed study should be undertaken.

Delayed Neuropsychiatric Sequalae with Reversible Quadriplegia after Carbon Monoxide Intoxication : A Case Report (일산화탄소에 기인한 Delayed Neuropsychiatric Sequalae 환자에서 발생한 사지 불완전마비 증례보고 1례)

  • Carbon monoxide poisoning leads to hypoxia, which eventually leads to tissue ischemia. The delayed neuropsychiatric syndrome can arise in the period from 4 days to 5 weeks following carbon monoxide poisoning. This case report describes a male who suffered from delayed sequelae after self-inflicted carbon monoxide poisoning. At the time of admission, he had symptoms of quadriplegia, both upper limb ankylosis, gait disturbance, and dysuria. He was treated with acupuncture, electro-acupuncture, physical therapy, and Uwhangchungsim-won. Paraplegia improved considerably after 7 days in hospital, and paralysis of the both upper extremities improved after 14 days in hospital. He was able to walk holding on to his wheelchair after 28 days in hospital. He was hospitalized for 55 days and was discharged from the hospital in a fine condition for everyday life. This case report suggests the possibility that korean medicine for delayed neuropsychiatric syndrome.

A Comparative Study about Cerebrovascular Reactivity from a Single Medication and Continuous Medication on Healthy Subjects (우황청심원(牛黃淸心元)의 단독투여 및 연속투여에 따른 정상인의 뇌혈관반응도의 비교연구)

  • Objectives: Uwhangchungsim-won (DC) has been used in various medical fields such as stroke, hypertension, atherosclerosis, autonomic imbalance and mental instability, etc. The aim of this study was to evaluate the effect of UC on cerebral hemodynamics and estimate the appropriate dose of UC. Methods: We studied changes of hyperventilation-induced cerebrovascular reactivity and mean blood flow velocity of middle cerebral arteries (MCA) using transcranial Doppler. We observed the changes of mean blood pressure, pulse rate and expiratory CO2 using S/5 Compact Anesthesia Monitor from 10 healthy young volunteers who were administered UC twice a day in the 1 st section and then once a day in the 2nd section. Results: Mean blood pressure tended to decrease at 1 hour and pulse rate tended to decrease at 2 hours after second administration. After 2 hours, mean blood pressure rose to state before administration, but pulse rate maintained from 2 hours to 4 hours. The changes were not statistically significant. Cerebral blood flow velocity in middle cerebral artery was not statistically significant after second administration. Cerebrovascular reactivity increased from 2 hours to 4 hours after second administration. Conclusions: This study provides that administration of UC twice a day is more effective on hyperventilation-induced cerebrovascular reactivity than administration of UC once a day.

A Case Report of Oculomotor Palsy Associated with Stent-Assisted Coil Embolization of Cerebral Aneurysm (뇌동맥류의 스텐트 보조 코일색전술 후에 발생한 동안신경 마비에 대한 한방치험 1례)

  • Posterior communicating artery aneurysms (PcomAAs) are the second most common type of aneurysm. Large cerebral aneurysms show various neurological symptoms, especially oculomotor nerve palsy (ONP), due to PcomAAs. Recent research has shown that stent-assisted coil embolization has many side effects. We report the effectiveness of Korean medicine in the treatment of ONP due to PcomAAs after stent-assisted coil embolization. A patient with ptosis and limitation of eye movement was treated with Korean medicine, including herbal medicine (Samlyeongbaegchul-san-byeonbang [參苓白朮散變方] and Uwhangchungsim-won [牛黃淸心元]), acupuncture, electroacupuncture, cupping, and moxibustion. Clinical symptoms were observed by images of the inter-palpebral fissure and eyeball movement. After the treatment, the inter-palpebral fissure and eyeball movement were noticeably improved. These findings suggest that treatment with Korean medicine can be an effective option for the treatment of ONP due to PcomAAs after stent-assisted coil embolization.
