Effect of Testosterone Cypionate for Impotent Petients with Low or Low Normal Serum Testosterone Level (혈청테스토스테론치가 감소된 발기부전증환자(저성선자극호르몬성 성선기능저하증 제외)에서 Testosterone cypionate의 치료효과)

  • Clinical effects of intramuscular injection of 250mg testosterone cypionate every 2-3 weeks were investigated in 15 impotent patients with low or low normal serum testosterone level (Hypo-gonadotrophic hypogonadism was excluded). The results were obtained as follows. 1. Among 15 patients, 7(46.7%) showed markedly improved potency, 6(40%) partially improved potency and 2(13.7%) no improvement of potency. There was no correlation between effec-tiveness of testosterone replacement and age, testicular size, serum testosterone level or LH, FSH level. 2. Among 12 patients who had showed improved potency, 8(66.7%) complained of redeveloped decrease in potency during testosterone replacement. In conclusion, testosterone cypionate for treatment of the impotent patients with low or low normal serum testosterone level was effective, but further studies are necessary to investigate cause of redeveloped decrease in potency during testosterone replacement.

Malignant Fibrous Histiocytoma of the Spermatic Cord: A Case Report (정삭에 발생한 악성섬유조직구종 -1예 보고-)

  • Malignant fibrous histiocytoma (MFH) of the spermatic cord represents an uncommon location for the most common soft tissue tumor in adults. MFH of the spermatic cord is extremely rare. No case report has been described in the Korean literature. We report a case of malignant fibrous histiocytoma, myxoid type, arising from left spermatic cord. A 77-year-old male presented with a painless left upper scrotal mass for 5 months. Simple mass excision was performed for a diagnosis. Grossly, the mass closely contacted with the left spermatic cord. Since a myxoid type of malignant fibrous histiocytoma was diagnosed from histopathological findings, left radical orchiectomy with high ligation of the spermatic cord was performed additionally. There were no evidence of local recurrence or metastasis at 6 months after surgery.

  • The three homologous members of the p160 SRC family (SRC-1, SRC-2 and SRC-3) mediate the transcriptional functions of nuclear receptors and other transcription factors, and are the most studied of all the transcriptional co-activators. Recent work has indicated that the SRC-3 gene is subject to amplification and overexpression in various human cancers. Some of the molecular mechanisms responsible for SRC overexpression, along with the mechanisms by which SRC-3 promotes breast and prostate cancer cell proliferation and survival, have been identified. However, the function of SRC-3 in bladder cancer remains poorly understood. In the present study, our results indicate that overexpression of SRC-3 promotes bladder cancer cell proliferation whereas knockdown of SRC-3 results in inhibition. At the molecular level, we further established that CXCR4 is a transcriptional target of SRC-3. Therefore, our study first identified that SRC-3 plays a critical role in the bladder cancer, which may be a target beneficial for its prevention and treatment.

  • Male urethral diverticulum is uncommon lesion, furthermore calculus formation within the male urethral diverticulum is very rare. Generally, urethral diverticula are classified as congenital and acquired. The majority of male urethral diverticula are acquired and approximately 10 to 20 per cent are congenital. Acquired urethral diverticula in the male may arise from many sources, including infection(prostatic abscess, infection of periurethral glands, hematoma or schistosomiasis), obstruction (stricutre, impacted stone, Cunningham clamp or condom catheter) and trauma(instrumentation, external injury and pelvic fracture). Calculi formation is more common in the acquired diverticulum owing to stagnation of urine and infection. These calculi in the diverticulum usually are solitary and may attain considerable size with predisposing factors. 1) a ureteral or bladder calculus that is lodged in the urethra, 2) urethral trauma or stricture, 3) calcification around a foreign body or hair. The treatment of urethral diverticulum conbined with stone is excision of the diverticula with removal of stone. We treated two cases of urethral diverticulum combined with stone in the male, and report with review of literature.

Can Recurrence and Progression be Predicted by HYAL-1 Expression in Primary T1 Bladder Cancer?

  • Background: Molecular prognostic markers have been under investigation for the last decade and no validated marker to date has been proven to be used in daily clinical practice for urinary bladder cancers. The aim of the present study is to evaluate the significance of HYAL-1 expression in prediction of recurrence and progression in pT1 urothelial carcinomas. Materials and Methods: Eighty-nine urothelial carcinoma cases staged as T1 according to 2004 WHO classification were studied. Representative sections from every case were stained immunohistochemically for HYAL-1 and scored between 0 and +3, according to staining density, and graded as low and high for the scores 0-1 and 2-3, respectively. Results: Of the 89 pT1 bladder cancer patients, HYAL-1 expression was high in 92.1% (82 patients; 72 patients +3 and 10 patients +2) and low in 7.9% (only 7 patients; 6 patients +1 and 1 patient 0) of the cases. Of the 89 patients, 38 (42.7%) had recurrence and 22 (24.7%) showed progression. HYAL-1 staining did not show significant characteristics for tumor grade, accompanying CIS, multiplicity, tumor size, age and sex. HYAL-1 expression did not have any prognostic value in estimating recurrence or progression. Conclusions: HYAL-1 expression was found to be high, but did not have any prognostic importance in T1 bladder urothelial carcinomas.

The Value of Diagnostic Laparoscopy for Impalpable Testes (비촉지성 정류고환에서 진단적 복강경검사의 유용성)

  • There have been no definitive preoperative diagnostic imaging studies for impalpable testes. We observed the effectiveness of laparoscopy for detecting impalpable testes not identified with ultrasonography (USG) or careful physical examination under general anesthesia. We retrospectively reviewed 117 patients (118 testes) who were operated upon for undescended testes from January 1998 to December 2004. The testes of these patients were palpable in 97(82 %) and impalpable in 21 (18 %). We analyzed the preoperative diagnostic method, site of the testes, operative method and operative findings of the 21 impalpable testes. Preoperative USG and physical examination under general anesthesia were performed on 20 patients, and 12 patients' testes could be localized. Eight patients whose testes could not be localized with USG and physical examination underwent laparoscopy. Seven of the 8 patients had testes in inguinal canal and 4 of these were atrophied and underwent orchiectomy because of atrophy (2) and vanishing (2). Only 1 patient had bilateral intraabdominal testes and one of the testes was atrophied. Laparoscopy was a useful method for detecting impalpable testes, but the clinical application might be limited because the location of atrophic or vanishing testes was mainly inferior to internal inguinal ring.

A Correlation of Testicular Size with Testicular Function in Non-Obstructive Infertile Male (비폐쇄성 남성불임증환자의 고환용적과 고환기능)

  • The testicular volume measured by a Prader orchidometer was compared with sperm count in semen, the levels of serum FSH, LH, testosterone, prolactin, estradiol and progesterone in 59 nonobstructive infertile men. 1. The causes of infertility were primary hypogonadisms in 50 patients (35 unknown, 9 Klinefelter syndromes, 6 varicocels) and secondary hypogonadism in 9 (5 isolated FSH deficiencies, 1 hyperprolactinerriia, 3 pituitary hypogonadisms). 2. Decreased levels of serum FSH (less than 4 mIU/ml) did not correlate with testis volume but increased level of serum FSH (more than 20 mIU/ml) were mostly noted in the testis less than 10ml. 3. Decreased level of serum testosterone (less than 3 ng/ml) were distinguishably noted in the atrophied testis less than 5 ml. 4. There was no correlation between the testicular volume and the levels of serum prolactin, estradiol, and progesterone. Coclusively, testicular volume less than 11 ml suggests poor spermatogenesis, but normal testicular volume dose not nessarily rule out poor spermatogenesis. Function of Leydig cell is relatively well preserved in atrophied testis of 5 to 10 ml comparing with that of seminiferous tubule.

Suprameatal Transvaginal Urethrolysis in Urethral Obstruction Associated with Anti-incontinence Surgery: A Case Report (요실금 수술후 발생한 요도 폐색에서 외요도구 상방 질벽을 통한 요도 박리술: 치험 1례 보고)

  • We report our experience with a case of urethrolysis using a transvaginal suprameatal approach without lateral perforation of the urethropelvic ligament. A 43-year-old woman suffered from voiding difficulties such as hesitancy, frequency, urgency, decreased urinary flow, residual urine sensation after Marshall-Marchetti-Krantz operation concurrent with hysterectomy. The results of multidisciplinary work-ups of urethral obstruction such as history, vaginal examination, voiding cystourethrography, urodynamic study, showed that she had urethral obstruction due to a previous operation. Since clean intermittent catheterization and alpha-blocker therapy did not improve her symptoms, suprameatal transvaginal urethrolysis was performed to resolve the symptoms. Postoperative follow-up for 5 months showed that the patient remained free from voiding difficulty in their life. We believe that suprameatal transvaginal urethrolysis is worth attempting for urethral obstruction associated with anti-incontinence surgery.

Allium Vegetables and Risk of Prostate Cancer: Evidence from 132,192 Subjects

  • Objective: To evaluate the relationship between allium vegetable intake and risk of prostate cancer. Methods: A systematic literature search up to May 2013 was carried out in PubMed, EMBASE, Scopus, Web of Science, Cochrane register, and Chinese National Knowledge Infrastructure (CNKI) databases, and the references of retrieved articles were also screened. The summary relative risks with 95% confidence interval for the highest versus the lowest intake of allium vegetables were calculated. Heterogeneity and publication bias were also evaluated. Results: A total of nine epidemiological studies consisting of six case-control and three prospective cohort studies were included. We found a significantly decreased risk of prostate cancer for intake of allium vegetables (OR=0.82, 95% CI 0.70, 0.97). Moreover, in the subgroup analysis stratified by allium vegetable types, significant associations were observed for garlic (OR=0.77, 95% CI 0.64-0.91) but not onions (OR=0.84, 95% CI 0.62-1.13). Conclusions: Allium vegetables, especially garlic intake, are related to decreased risk of prostate cancer. Because of the limited number of studies, further well-designed prospective studies are warranted to confirm the findings of our study.
