Imaging Diagnosis of Emphysematous Pyelonephritis in a Non-Diabetic Dog

  • An 11-year-old, 4.3 kg, mixed breed, intact female dog exhibiting vomiting and a reduced appetite was presented. On physical examination, palpation of the right kidney elicited discomfort. Blood analysis revealed leukocytosis with neutrophilia, and elevation of alkaline phosphatase and blood urea nitrogen. Escherichia coli was detected in urine culture, whereas urine analysis showed absence of glucose. Radiography and ultrasonography suggested that there was gas in the right renal parenchyma, and computed tomography was performed while the dog was awake to investigate the suspected gas in the right kidney. After unilateral ureteronephrectomy, a histological diagnosis of right emphysematous pyelonephritis (EPN) was made. EPN refers to a severe necrotizing infection of the renal parenchyma with gas accumulation in the tissue. It is rare in both human and veterinary medicine, and almost all EPN patients have uncontrolled diabetes mellitus. This case report describes the imaging-based diagnosis of a rare occurrence of unilateral EPN in a non-diabetic dog.

Effects of Euphorbia Pekinensis RUPR. on Rats with Acute Renal Failure induced by Gentamicin Sulfate (대극전탕액(大戟煎湯液)이 Gentamicin sulfate로 유발(誘發)된 백서(白鼠)의 급성(急性) 신부전(腎不全)에 미치는 영향(影響))

  • This article is to investigate the effects of Euphorbia Pekinensis RUPR. on rats with acute renal failure induced by gentamicin sulfate. So this experimental research was focused on measuring (1)the levels of creatinine, blood urea nitrogen, glutamic oxaloacetic transaminase, glutamic pyruvic transaminase, total protein, sodium, potassium, chloride in the serum and (2)the levels of protein, blood, sodium, potassium, chloride, pH, specific gravity and volume in the urine and (3)intake water. The results were summarized as follows. 1. While the levels of creatinine and glutamic oxaloacetic transaminase, glutamic pyruvic transaminase in the serum were significantly increased, the level of BUN in the serum were partially increased and the levels of sodium and potassium in the serum were partially decreased on the Euphorbia Pekinensis $R_{UPR}$. group. 2. While the urine volume was significantly increased, the levels of protein, blood, sodium and chloride in the urine were partially increased and the specific gravity, pH in the urine were partially decreased. According to the above results, it is shown that Euphorbia Pekinensis $R_{UPR}$. without refinning had a diuretic effect on rats with acute renal failure induced by gentamicin sulfate, but aggravated the glomerular filtration rate and tubular reabsorption. So, it is supposed that Euphorbia Pekinensis $R_{UPR}$. without refinning needs to be used carefully on renal disease and futhermore research about the kinds of extracts form is necessary.

The Effect of Pyrrosiae Herba Herbal-acupuncture at KI10 on Lipopolysaccharide Induced Nephritis in Rats (음곡에 시술한 석위약침이 Lipopolysaccharide로 유도된 흰쥐의 신장염에 미치는 영향)

  • Objectives : The purpose of this study is to investigate the effects of Pyrrosiae Herba herbal-acupuncture(PH-HA) at $KI_{10}$(Umgok) on nephritis induced by lipopolysaccharide(LPS) in rats. Methods : Rats were assigned to four groups: normal, LPS, saline and PH-HA. Rats in the saline and PH-HA groups were treated with saline injection and PH-HA respectively at $KI_{10}$, three times over the period of one week. All animals, except those in the normal group, were injected intra-peritoneally with LPS to induce nephritis. WBC, in blood, tumor necrosis factor alpha(TNF-${\alpha}$), cytokine-induced neutrophil chemoattractant 1(CINC-1), blood urea nitrogen(BUN), creatinine in serum, urinal volume, total protein creatinine in urine, and renal myeloperoxidase (MPO) were analyzed. Results : 1. PH-HA group showed significantly reduced levels of serum BUN, serum creatinine, TNF-${\alpha}$, and CINC-1 compared to the LPS group. Furthermore, a significant increase in urine output and more significant decreases in total protein in urine and MPO in renal tissue were observed in the PH-HA group when compared to the LPS group. 2. The PH-HA group showed significantly reduced levels of serum creatinine and renal MPO, and a more significant increase in urine output compared to the saline group. Conclusions : According to these results, it is postulated that PH-HA at $KI_{10}$ has anti-inflammatory and renal-protective effects on LPS-induced nephritis in rats, and both acupoint $KI_{10}$ and the herb Pyrrosiae Herba made contributions to these effects. Further studies on the interaction between acupoint $KI_{10}$ and the herb Pyrrosiae Herba may be needed.

Effects of Okchunsan and Spinacia oleracea L.(Semen) on streptozotoicin-induced Diabetic Rats (실험적(實驗的) 당뇨병(糖尿病)에 대(對)한 옥천산(玉泉散) 및 파채자의 효과(效果))

  • In order to investigate the effects of Okchunsan (Sample A), Gamiokchunsan (Sample B) and Spinacia oleracea L. (semen: Sample C) on streptozotocin-induced diabetic rats, the experimal study was performed on the levels of glucose, insulin, total cholesterol, triglyceride, total protein, albumin, blood urea nitrogen (BUN), electrolytes $(Na^+,\;Ka^+,\;Cl^-)$, GOT and GPT in serum, hemoglobin, hematocrit, urine volume, glucose and protein in urine, body weight, liver and kidney weight. The results were obtained as follows: 1. Serum glucose levels were significantly decreased in Sample A. B and C group and serum insulin levels were increaseclin Sample A and B group. 2. Serum total cholesterol levels were recovered as the normal value in Sample A, B and C group and serum triglyceride levels were decreased in Sample A and C group. 3. Serum total protein levels were increased in Sample C group, BUN levels were decreased in Sample A and C group. 4. Serum $Na^+\;and\;Cl^-$ levels were increased, $K^+$ levels were decreased in Sample A, B and C group. 5. Serum GOT and GPT levels were remarkedly decreased in Sample A, B and C group. 6. Urine volume, urine glucose and protein levels were decreased in Sample A, B and C levles. 7. Body Weight showed a tendancy of increasing in Sample A, B and C group. 8. Levels of serum albumin, hemoglobin, nematocrit, liver and Kidney weight were not significantly changed. According to above results, Okchunsan, Gamiokchunsan and spinacia oleracea L. (Semen) were expected to be applied to the treatment of diabetes mellitus and its complications.

  • The effects of Soohwabunchungum on rats with nephrosis induced by a single tail-intravenous injection of PAN(puromycin aminonucleoside), 2.5mg/100g of body weight was evaluated in the present study. The effects of Soohwabunchungum on PAN nephrosis was evaluated by measuring the concentrations of albumin, total protein, total lipid, cholesterol, triglyceride, creatinine, BUN(blood urea nitrogen) and uric acid in the serum the amount of protein, creatinine, glucose, occult blood and volume of the 24 hours urine and the volume of intake water, To conclude, it can be inferred that Soohwabunchungum has the effects of improving proteinuria, hypoproteinemia, hyperlipidemia in nephrotic syndrome, and relieving azotemia when nephrotic syndrome is accompanied by the acute renal failure.

  • During the serial attempts to identify the chemical and biological characteristics of Ailanthi Cortex Radicis, the root bark of Ailanthus altissima (Simaroubaceae), we find out the serious toxic effect on kidney by chloroform fraction of the methanolic extract of the herb drug. The toxicities were revealed as the increase of urea nitrogen amount in blood and lactate dehydrogenase and ${\gamma}-glutamyltransferase$ activities in urine and the decrease of the concentration of glutathione and both of protein bound and non-protein bound -SH in kidney tissue.

The Change of Renal Function in Diabetic Nephropathy and Chronic Renal Failure Patients with Long Term Herb Medication by Frequently Prescribed Formular (상용 한약복합 처방의 장기간 연용 투여가 당뇨병성 신증 및 만성 신부전 환자의 신기능에 미치는 영향)

  • Twenty-one diabetic nephropathy patients with normal serum BUN(Blood Urea Nitrogen), creatinine levels and ten chronic renal failure patients with abnormal high BUN, creatinine levels were investigated to evaluate the renal function change after long term herb medicine administration. The hospitalized patients were administrated three times a day with herb medicine, which were prescribe frequently in practical oriental medicine such as many hospital and local clinics. Blood Urea Nitrogen, creatinine and glomerular filtration rate (GFR) were measured immediately after 7days medication. Serum BUN, creatinine levels in diabetic nephropathy patients changed from 17.63±4.38㎎/㎗, 1.09±0.26㎎/㎗(mean±SD) of pre-medication levels to 14.13±3.24 1,20±0.37, 14.75±2.21 1.23±0.55, 12.34±2.89 1.18±0.42 at 7th, 14th, 21th days after herb medicine administration respectively. Also 24hr urine total protein changed from 632.25±254.43㎎/㎗ of pre-medication levels to 623.18±231.56㎎/㎗ after herb medicine administration(P>0.05). Serum BUN, creatinine levels and GFR in chronic renal failure patients changed from 67.45±13.86㎎/㎗, 6.74±2.91㎎/㎗, 13.73±4.21㎖/min pre-medication levels to 61.23±17.75 6.43±2.29 15.49±3.56, 58.84±19.36 5.83±2.51 16.38±2.85, 56.39±20.33 5.64±2.52 16.73±3.40 at 7th, 14th, 21th days after herb medicine administration respectively. Therefore, there was not clinically remarkable difference in the serum BUN, creatinine, GFR levels between pre-medication and post-medication in both Group.

Effect of Dietary Protein Level on Renal Functions and Structure in Uninephrectomized Aging Model in Rat (식이 단백질 수준이 한쪽 신장을 절제한 흰쥐에서 나이 증가에 따른 신장의 기능 및 조직 변화에 미치는 영향)

  • This study was performed to investigate the effect of dietary protein level on renal senescence. Male rats of 337.8$\pm$5.7g body weight were underlateral nephrectomy or shamoperation. The rats were divided into high protein(40% casein), normal protein(15% casein) and low protein(8% casein)diets and fed experimental diets ad libitum for 24 weeks. The results are summarized as follows. There was a hypertophy of the remnant kidney of uninephrectomized rats of 40% or 15% protein group, coming up to the comparable weights of both kidneys of sham-operated rats. However, the hypertrophic effect was not seen in uninephrectomized rats of 8% protein group. Serum albumin was lower in uninephrectomized rats. With increasing dietary protein level blood urea nitrogen was increased, whereas, urinary urea nitrogen excretion was decreased. Urinary solute excretion was higher in uninephrectomized group than in sham-operated group. However, effect of dietary protein level on urinary solute excretion varied dpending on th solutes tested. GFR and urinary protein excretion, throughout experiment, increased with feeding period and with dietary protein level. Proteinuria was most severe in uninephrectomized rats fed 40% casein diet. Maximum urine concentration ability measured after dehydration was not different among the experimental groups. Light microscopic examination showed focal segmental glomerulosclerosis and mild increas of glomerular mesangial matrix in uninephrectomized rats fed 40% and 15% protein diet, however, which was not observed in uninephrectomized rats fed 8% protein diet and in sham-operated rats fed 40% diet. Immunofluorescence studies revealed segmental deposits of albumin in the mesangium and capillary loops in high protein and uninephrectomized groups. Minimal granular deposition of IgG was noted in the mesangium of all experimental groups. In conclusion, high protein intake accelerated deterioration of renal function and it was correlated with morphological change. Low protein intake was effective in preventing these changes.

Evaluation of antiproteinuric and hepato-renal protective activities of propolis in paracetamol toxicity in rats

  • BACKGROUND/OBJECTIVES: Propolis has a rich source of bioactive compounds and has renal and hepatic protective properties. The purpose of this study was to investigate the beneficial effect of hydro-ethanolic extract of propolis against paracetamol-induced liver damage and impairment of kidney function, as well as hematological changes in rats. MATERIALS/METHODS: Six groups of rats were used; the first group was served as a control; the second and third groups were treated by propolis extract at a dose of 50 and 100 mg/kg.B.WT. respectively; the fourth group was treated by paracetamol (200 mg/kg.B.WT.); the fifth group was treated by propolis (50 mg/kg.B.WT.) for eight days and then received similar dose of propolis for following seven days with paracetamol at a dose of 200 mg/kg.B.WT. daily for the seven days; and the sixth group was treated with propolis (100 mg/kg.B.WT.) for eight days and then received similar dose of propolis for following seven days with paracetamol at a dose of 200 mg/kg.B.WT. daily for the seven days. All the animals were treated for a period of 15 days. At the end of the experimental period, blood samples were collected for measurement of the liver enzymes, serum albumin, protein and creatinine, blood urea nitrogen, hematological parameters, and urine volume, protein and albumin. RESULTS: Paracetamol over dose significantly lowered hemoglobin, serum total protein, albumin, and uric acid, while it significantly increased blood creatinine, blood urea nitrogen, alanine aminotransferase, aspartate aminotransferase and lactate dehydrogenase activities, white blood cells, and platelet count as compared to the control. However, these alterations were significantly attenuated by the use of propolis extract and the effect was dose dependent. Interestingly, propolis prevented paracetamol induced proteinuria, low hemoglobin and body weight loss. CONCLUSIONS: Propolis significantly prevented paracetamol induced renal, hepatic and hematological toxicity and might be useful in the management of liver and renal diseases particularly proteinuria.

Evaluation of health screening data for factors associated with peri-implant bone loss

  • Purpose: Systemic health has a profound effect on dental treatment. The aim of this study was to evaluate peri-implant bone loss and health screening data to discover factors that may influence peri-implant diseases. Methods: This study analyzed the panoramic X-rays of patients undergoing health screenings at the Health Promotion Center at Seoul St. Mary's Hospital in 2018, to investigate the relationship between laboratory test results and dental data. The patients' physical data, such as height, weight, blood pressure, hematological and urine analysis data, smoking habits, number of remaining teeth, alveolar bone level, number of implants, and degree of bone loss around the implant, were analyzed for correlations. Their associations with glycated hemoglobin, glucose, blood urea nitrogen (BUN), creatinine, and severity of periodontitis were evaluated using univariate and multivariate regression analysis. Results: In total, 2,264 patients opted in for dental health examinations, of whom 752 (33.2%) had undergone dental implant treatment. These 752 patients had a total of 2,658 implants, and 129 (17.1%) had 1 or more implants with peri-implant bone loss of 2 mm or more. The number of these implants was 204 (7%). Body mass index and smoking were not correlated with peri-implant bone loss. Stepwise multivariate regression analysis revealed that the severity of periodontal bone loss (moderate bone loss: odds ratio [OR], 3.154; 95% confidence interval [CI], 1.175-8.475 and severe bone loss: OR, 7.751; 95% CI, 3.003-20) and BUN (OR, 1.082; 95% CI, 1.027-1.141) showed statistically significant predictive value. The severity of periodontitis showed greater predictive value than the biochemical parameters of blood glucose, renal function, and liver function. Conclusions: The results of this study showed that periodontal bone loss was a predictor of peri-implant bone loss, suggesting that periodontal disease should be controlled before dental treatment. Diligent maintenance care is recommended for patients with moderate to severe periodontal bone loss.
