Objective: To analyze differential diagnostic accuracy of urinary human epidydimis secretory protein 4 (HE4) in patients with ovarian tumors. Materials and methods: In the case-control study 23 patients with ovarian cancer, 37 patients with benign ovarian tumors and 18 women in the control group were included. Serum CA125 values and urinary concentrations of HE4were assessed quantitatively. Urinary creatinine concentrations and glomerular filtration rate were also determined and used to calculate ratios to HE4. Results: Higher urinary HE4 concentrations were observed in patients with late stage ovarian cancer (p=0.001) and also in patients with early stage ovarian cancer when compared to patients with benign ovarian tumors (p=0.044). On analysis where all ovarian cancer patients were included, higher diagnostic accuracy was observed with calculated ratio of HE4 to glomerular filtration rate (GFR) to unchanged urinary HE4 concentrations -AUC 0.861 vs. 0.858. When discriminatory accuracy was calculated for urinary HE4/GFR ratio and unchanged urinary HE4 concentrations, the last demonstrated a higher area under the curve - 0.701 vs. 0.602. The urinary HE4/creatinine ratio had lower discriminatory characteristics than unchanged concentrations of urinary HE4. However, HE4 serum concentration was more accurate for discrimination of patients with benign and malignant ovarian tumors when compared to urinary HE4 and CA125 in sera (AUCs were 0.868 for serum HE4 and 0.856 and 0.653 for urinary HE4 and CA125, respectively). Conclusions: Ovarian cancer patients have higher urinary concentrations of human epidydimis secretory protein 4 than patients with benign ovarian tumors. Urinary HE4 has comparable discriminatory accuracy with serum HE4 for benign and malignant ovarian tumors and can be recommended as a non-invasive ovarian cancer risk assessment method.
This study was performed to evaluate the effect of dietary protein source and sulfur amino acid content on bone metabolism in ra. Thirty male rats (body weight 145$\pm$2g) were divided into three groups. The rats in the first group were fed on casein 20% diet as animal protein source and those in the second group were fed on soy 20% diet as plant protein source. Sulfur amino acid ratio of these group was 1.07:1. The rats in the third group were fed on soy 20% diet and the sulfur amino acid were supplemented with the amount contained as much in the soy 20% diet. All rats were fed on experimental diet and deionized water ad libitum for 9 weeks, The total body, spine, femur bone mineral density and bone mineral content were measured using Dual Energy X-ray Absorptiometry Calcium, phosphate, pyridinoline, creatinine in urine and calcium, phosphate, alkaline phosphatase, osteocalcin in serum were measured. During the experimental period, plant protein (soy protein) group had a lower urinary Ca excretion, urine pyridinoline & crosslinks value and had a higher Ca efficiency in total bone and femur bone mineral density than animal protein (casein) group. There were no significant differences in serum calcium, phosphate, alkaline phosphatase and osteocalcin among the three groups of the rats. The findings from this study demonstrated that plant protein (soy protein) is beneficial of bone mineral density because it had a higher Ca efficiency in total bone and femur bone mineral density than animal protein (casein). However, the supplementation of sulfur amino acid on soy results were consistent with prior studies that dietary sulfur amino acid load had a negative effect on calcium balance. The rats fed sulfur amino acid supplementation diet increased urinary calcium excretion and decreased calcium efficiency for total and femur mineral density. To investigate recovery effects of Insambackhotang, which have been used clinically in diabetes therapny, on kidney failure of a diabetes-induced mouse by Alloxan administration, body and kidney weight changes of mice, BUN, creatinine, glucose and MDA level in serum, MDA level in kidney tissue. 1. A hyperglycemia(250-400mg/dl) mouse induced by Alloxan(75mg/kg) showed significant decline of kidney function: increase of BUN and creatinine in serum, excretion of glucose, protein, ketone in urine were observed at 4 days after the treatment. 2. Increase of the mouse body and kidney weight and a ratio of the kidney/body weight of Insambackhotang treated group as compared to the control group was significantly inhibited. 3. The BUN, creatinine level in serum of Insambackhotang treated group as compared to the control group were significantly inhibited. 4. The MDA level in serum and kidney tissue of Insambackhotang treated group as compared to the control group were significantly inhibited.
Objective: This study was conducted to investigate whether acupuncture and moxibustion treatment could improve proteinuria in immunoglobulin A nephropathy. Methods: A 25-year-old man diagnosed with immunoglobulin A nephropathy had severely increased proteinuria. We conducted acupuncture treatment and moxibustion treatment on the abdominal area between the abdominal aorta and kidney at level L2. Results: After five months of acupuncture and moxibustion treatment, the patient's urine albumin/creatinine ratio and urine protein/creatinine ratio had improved from 0.571 to 0.28 and 0.79 to 0.47, respectively. Conclusions: This case report suggests that acupuncture and moxibustion treatment may ameliorate proteinuria in immunoglobulin A nephropathy.
BACKGROUND/OBJECTIVES: Chronic renal failure (CRF) is a complex pathological condition that lacks a cure. Certain Chinese medicines, such as melittin, a major component in bee venom, have shown efficacy in treating CRF patients. On the other hand, the mechanisms underlying the therapeutic effects of melittin are unclear. MATERIALS/METHODS: A 5/6 nephrectomy model (5/6 Nx) of renal failure was established on rats for in vivo assays, and mouse podocyte clone 5 (MPC5) mouse podocyte cells were treated with angiotensin II (AngII) to establish an in vitro podocyte damage model. The 24-h urine protein, serum creatinine, and blood urea nitrogen levels were evaluated after one, 2, and 4 weeks. Hematoxylin and eosin staining, Masson staining, and periodic acid-Schiff staining were used to examine the pathological changes in kidney tissues. A cell counting kit 8 assay was used to assess the cell viability. Reverse transcription polymerase chain reaction and Western blot were used to assess the mRNA and protein levels in the cells, respectively. RESULTS: In the rat 5/6 Nx, melittin reduced the 24-h urinary protein excretion and the serum creatinine and blood urea nitrogen levels. Furthermore, the renal pathology was improved in the melittin-treated 5/6 Nx rats. Melittin promoted podocin, nephrin, Beclin 1, and the LC3II/LC3I ratio and inhibited phosphorylated mammalian target of rapamycin (mTOR)/mTOR in 5/6 Nx-induced rats and AngII-induced MPC5 mouse podocyte cells. Moreover, inhibiting autophagy with 3-MA weakened the effects of melittin on podocin, nephrin, and the LC3II/LC3I ratio in podocytes. CONCLUSION: Melittin may offer protection against kidney injury, probably by regulating podocyte autophagy. These results provide the theoretical basis for applying melittin in CRF therapy.
목적: 본 연구는 지난 3여년간 연쇄상구균 감염후 사구체신염으로 진단 받은 환아들 중 초기에 육안적 혈뇨를 보인 환아들의 임상양상을 관찰하고 현미경적 혈뇨만을 보인 환아들과의 차이를 비교하여 경과 예측에 도움이 되고자 한다. 방법: 2000년 1월부터 2003년 4월까지 인제대학교부속 일산 백병원과 국민건강보험공단 일산병원에서 연쇄상구균 감염후 사구체신염으로 진단받은 환아 39례를 대상으로 하였다. 결과: 대상 환아들의 평균 발병 연령은 8.85세 남녀 비는 1.1:1이었으며, 육안적 혈뇨를 보인 환아들(A군)은 17례, 현미경적 혈뇨만을 보인 환아들(B군)은 22례이었다. 두 군간의 부종, 핍뇨, 상기도 감염의 기왕력등의 빈도의 차이는 없었으나, 고혈압의 빈도는 A군 17례중 3례(17.6%), B군 22례중 11례(50.0%)로 B군에서 높았다. 검사 소견상 백혈구수, BUN, creatinine, ASO치는 차이가 없었으나 혈청 보체치 C3와 C4는 B군에서 낮게 측정되었고 단백뇨의 정도를 나타내는 소변 내 단백/creatinine의 비와 24시간 소변내 단백량은 A군에서 높은 수치를 보였다. 그러나 질환에 의한 합병증의 발생이나 사망은 없었다. 결론: 초기에 육안적 혈뇨를 가진 환아들(A군)은 현미경적 혈뇨만을 보였던 환아들(B군)보다 고혈압의 빈도는 낮았으며, 단백뇨의 양은 많았다. 그러나 두 군간의 임상경과 및 예후의 유의한 차이는 없었으며 대상 환아 전례에서 임상적인 호전을 보였다.
목적 : IgA 신병증의 임상 및 병리학적 소견의 비교를 통해서 IgA 신병증의 신조직 소견을 예측할 수 있는 인자를 찾아봄으로써 신생검 시기를 결정하는데 도움이 될 수 있는 지표를 찾아본다. 방법 : 원주기독병원에서 신생검상 IgA 신병증으로 확인된 40례를 대상으로 후향적 연구를 시행하였다. 결과 : 40명의 환자 중 크레아티닌이 1.5 mg/dL 이상을 보인 1명은 추적 관찰이 안되었고 나머지 39명 중 2명에서는 혈중크레아티닌이 1.5mg/dL 이상으로 진행하였으며 그 중의 한명은 진단 후 2년 7개월만에 말기신부전으로 진행하였다. WHO 분류에 따르면 class I이 15명, class II가 14명, class III가 7명, classs IV가 3명, class V 0명이었다. 육안적 혈뇨군에서 현미경적 혈뇨군보다 class I과 class II의 비율이 의미있게 높았다(P<0.02). 세뇨관간질 소견만을 분류해 보았을 때 grade 1은 24명, grade 2는 4명, grade 3는 8명, grade 4는 3명이었다. 세뇨관간질의 변화가 심해질수록 24시간 요단백/알부민 비율이 증가하였다(r=-0.32, P<0.05) 혈청 크레아티닌치가 0.79 mg/dL 이하인 경우 세뇨관간질소견이 경하였으며, 크레아티닌치가 1.13 mg/dL 보다 큰 경우 세뇨관간질병변이 심하였다. 육안적 혈뇨군에서 혈청 크레아티닌치는 현미경적 혈뇨군에 비해 낮았고(0.78 vs 1.09 mg/dL)(P=0.027), 혈청 IgA 치가 높았으며(316.3 vs 198.8mg/dL)(P=0.027), class I과 II의 낮은 WHO 분류 class를 보였다(23 vs 4)(P=0.029). 결론 : IgA 신병증 환자에서 혈청크레아티닌이 0.79 mg/dL 이하, 육안적 혈뇨, 높은 24시간 urine protein/albumin 비율을 보이는 경우 경한 신조직 소견을 보임으로 이런 소견을 보이는 경우 신조직 생검 시기를 미룰 수 있을 것으로 사료된다.
This study was done to investigate recovery effects of YJT, which has been used clinically in diabetes therapy. Mice were administerd Alloxan to induce diabetes. There body weight and kidney weight changes, BUN, creatinine, glucose, total protein and ALP activity in serum, urinalysis, excretion volume of Na+, K+ in urine were measured. The results were as follows: 1. Increase in body weight and kidney weight and the ratio of kidney to body weight of YJT treated group showed similar results with those of normal group. 2 The BUN, creatinine, ALP activity levels in serum of YJT treated group were similar with those of normal group. 3. The glucose level in serum of YJT treated group was better than that of control group 4. The result of the urinalysis in the group of YJT was almost same as that of control group The above results suggest that YJT partially improves the function of the kidney.

Purpose: Angiotensin-converting enzyme inhibitors and angiotensin II receptor blockers (ARBs) are frequently employed to counteract the detrimental effects of proteinuria on glomerular diseases. However, the effects of ARBs remain poorly examined in pediatric patients with immunoglobulin A (IgA) nephropathy. Herein, we evaluated the efficacy and safety of losartan, an ARB, in pediatric IgA nephropathy with proteinuria. Methods: This prospective, single-arm, multicenter study included children with IgA nephropathy exhibiting proteinuria. Changes in proteinuria, blood pressure, and kidney function were prospectively evaluated before and 4 and 24 weeks after losartan administration. The primary endpoint was the difference in proteinuria between baseline and 24 weeks. Results: In total, 29 patients were enrolled and received losartan treatment. The full analysis set included 28 patients who received losartan at least once and had pre- and post-urinary protein to creatinine ratio measurements (n=28). The per-protocol analysis group included 22 patients who completed all scheduled visits without any serious violations during the study period. In both groups, the mean log (urine protein to creatinine ratio) value decreased significantly at 6 months. After 24 weeks, the urinary protein to creatinine ratio decreased by more than 50% in approximately 40% of the patients. The glomerular filtration rate was not significantly altered during the observation period. Conclusions: Losartan decreased proteinuria without decreasing kidney function in patients with IgA nephropathy over 24 weeks. Losartan could be safely employed to reduce proteinuria in this patient population. ClinicalTrials.gov trial registration (NCT0223277)
To assess calcium and sodium and urinary excretion of preschool children in Busan and to evaluate the relationship of intakes of food and nutrient with urinary calcium excretion, calcium and sodium food frequencies of 25 common foods affecting intakes of calcium and sodium per week, nutrient intake by 24hr recall and 24hr urinary calcium and sodium excretion were measured with 97 preschool children. The mean calcium intake was 436.11mg and below RDA. The mean sodium intake was 1890.11mg. The mean urinary calcium and sodium excretion were 42.88mg and 735.25mg respectivery. The mean urinary calcium/creatinine ratio was 0.20. The urinary calcium excretion showed positive significant correlations with weight, intake frequency of pizza consumed per week and urinary sodium excretion (p<0.05, p<0.05, p<0.001). The urinary calcium excretion per milligram of creatinine showed positive significant correlations with intake frequencies of pizza and common squid consumed per week(p<0.01, p<0.05) and negative correlation with intake frequencies of pizza and common squid consumed per week(p<0.01, p<0.05) and negative correlation with age(p<0.05). No significant relations were found between urinary calcium and intakes of calcium, protein and phosphorus. Urinary sodium was found to be the most important determinant of urinary calcium excretion. Intake frequency of pizza consumed per week was found to be the most important determinant of urinary calcium excretion per milligram of creatinine. Based on the results, urinary calcium excretion was related to intake frequency of pizza consumed per week and urinary sodium excretion. Low calcium intake and increase of calcium loss in the urine potentiated by sodium intake during growth may reduce peak bone mass. So nutritional education is needed in order to increase calcium intake and decrease sodium intake, especially from food like pizza.
