Korean red ginseng extract alleviates advanced glycation end product-mediated renal injury

  • The effect of Korean red ginseng (KRG) on diabetic renal damage was investigated using streptozotocin (STZ)-induced diabetic rats. The diabetic rats showed loss of body weight gain, and increases in kidney weight and urine volume, whereas the oral administration of KRG at a dose of 100 or 250 mg/kg of body weight per day for 28 d prevented these diabetes-induced physiological abnormalities. Among the kidney function parameters, elevated plasma levels of urea nitrogen and creatinine in diabetic control rats tended to be lowered in KRG-treated rats. In addition, administration of KRG at a dose of 100 mg/kg body weight in the diabetic rats showed significant decreases in serum glucose and tumor necrosis factor-${\alpha}$ (TNF-${\alpha}$), implying that KRG might prevent the pathogenesis of diabetic complications caused by impaired glucose metabolism and oxidative stress. KRG also significantly reduced advanced glycation end product (AGE) formation and secretion from kidney of diabetic rats. Furthermore, KRG decreased the levels of N-(carboxymethyl) lysine and expression of AGE receptor. KRG also reduced the overexpression of cyclooxygenase-2 and inducible nitric oxide synthase in the kidney via deactivation of nuclear factor-kappa B. We also found that KRG prevented STZ-induced destruction of glomerular structure and significantly suppressed high glucose-induced fibronectin production. Taken together, KRG ameliorates abnormalities associated with diabetic nephropathy through suppression of inflammatory pathways activated by TNF-${\alpha}$ and AGEs. These findings indicate that KRG has a beneficial effect on pathological conditions associated with diabetic nephropathy.

Clinical Observation on C.V.A with Diabetes Mellitus (당뇨병(糖尿病) 환자(患者)에 병발(倂發)된 뇌졸중(腦卒中)의 임상적(臨床的) 고찰(考察))

  • Clinical observations were done on 67 cases with Diabetes Mellitus in CVA patients who were confirmed by CT scan and observed for over 1 week, admitted to the Dept. of Internal Medicine in Oriental Medical Hospital of Dongguk University from January 1992 to December 1993. The results were as follows; 1. 86 patients (15.3%) with Diabetes Mellitus were found in 561 CVA patients, the 6th decade of age was 40.2%, the ratio of male to female was 0.72:1. 2. The local distribution of CVA was similar to common CVA, and occlusive CVD was 83.6%, cerebral hemorrhage was 16.4% in this study. 3. The association between blood glucose and years were not significant. The largest ratio of fasting blood glucose were 140-199 mg/dl (44.6%) in admission, below 139 mg/dl (51.8%) in discharge in case of occlusive CVD. In cerebral hemorrhage, that were 140-199 mg/dl(45.5%) in dmission, below 139 mg/dl (45.5%) in discharge. The largest ratio of pp2hrs blood glucose were 200-299mg/dl in admission and discharge both occlusive CVD and cerebral hemorrhage. 4. The total sensitivity of urine glucose was 71.6%, and sensitivity of urine glucose in cerebral hemorrhage (81.8%) was more higher than that of occlusive CVD (69.6%). 5. Below 4 years had the highest prevalence(44.8%) in duration of diabetes mellitus. Patients usually used oral hypoglycemic agents(41.8%), insulin injection(23.9%) treatment and non-therapeutic was 17.9% in this study. 6. Predisposing factors and symptoms in admissin were similar to common CVA. The conscious disturbance on attacck was 41.1% in occlusive CVD, and that was 63.7% in cerebral hemorrhage. 7. The most common ratio of the season's attack was spring (44.8%), 8. The frequency of post history was as follows, hypertension (44.8%), heart disease (10.4%), and they were in below 199 mg/dl (83.3%) of fasting blood glucose. 9. The family history of CVA was 46.3%, and they was higher than nondiabetic patients. 10. The recurrence rate of CVA was 28.4%, and that of occlusive CVD(28.6%) was higer than cerebral hemorrhage's (18.2%). 11. The smoker was 52.2%, the drinker was 38.9%. 12. The complications was occured in 10 cases (14.9%) after admission, and they frequently occured than common CVA. 13. In admission, the ratio of systolic blood pressure in over l60mmHg was 42.9%, that of diastolic blood pressure in over l00mmHg was 12.5% in occlusive CVD. In cerebral hemorrhage, the ratio of systolic blood pressure in over l60mmHg was 54.5%, that of diastolic blood pressure in over l00mmHg was 27.3%. 14. The average beginned time of physical theraphy was, generally lated, 8.3 days in occlusive CVD, 11.2 days in cerebral hemorrhage. Average admitted period was longer than common CVA, and was 29.2 days in occlusive CVD, 11.2 days in cerebral hemorrhage. 15. The degree of recovery were 82.1% in occlusive CVD, 72.7% in cerebral hemorrhage. 16. The herb medications were various Sunghyanggeonggisans, Sopungtang, Ganghwalyupungtang, Yanggyuksan etc. were used most frequently, and Yukmijihwangtang, Gamidaebotang, Mangeumtang etc. were used as discharge.

  • The effects of Soohwabunchungum on rats with nephrosis induced by a single tail-intravenous injection of PAN(puromycin aminonucleoside), 2.5mg/100g of body weight was evaluated in the present study. The effects of Soohwabunchungum on PAN nephrosis was evaluated by measuring the concentrations of albumin, total protein, total lipid, cholesterol, triglyceride, creatinine, BUN(blood urea nitrogen) and uric acid in the serum the amount of protein, creatinine, glucose, occult blood and volume of the 24 hours urine and the volume of intake water, To conclude, it can be inferred that Soohwabunchungum has the effects of improving proteinuria, hypoproteinemia, hyperlipidemia in nephrotic syndrome, and relieving azotemia when nephrotic syndrome is accompanied by the acute renal failure.

  • Male Sprague Dawley rats were exposed to 0, 0.04, 0.2, and 0.8% Pb acetate in drinking water for 13 weeks and fed a commercial diet. Dose-related adverse effects observed at the end of the Pb acetate exposure in the drinking water were as follows: decrease in body weight gain, decrease in hemoglobin, hematocrit(HCT), mean corpuscular volume (MCV) and mean corpuscular hemoglobin (MCH), increase in serum glucose, decrease in serum testosterone, increase in lead accumulation and $\delta$-ALA release in urine, and decrease in $\delta$-ALAD activities DNA content and histopathlogy (intranuclear inclusion body in kidney proximal tubule cell). Taken together, repeated exposure of lead acetate induced toxicities in hematopoietic system, especially testis and kidney.

  • In order to elucidate the mechanism of toxic action of a pisonous mushroom, Amanita pantherina, biochemical effects of the mushroom extracts on mice were studied. A hotwater extract of Amanita pantherina injected intraperitoneally into male ICR mice evoked signs similar to those observed clinically upon acute poisoning by the mushroom and also changed the levels of component enzyme activities in blood, liver and urine. The serum cholinesterase activity was decreased significantly during 1-3 h after injection.

Dietary Effect of Hemicellulose from Soy Fiber on Blood Glucose and Cholesterol Content in Streptozotocin-Induced Diabetic Rats (콩 식이섬유로부터 추출한 헤미셀룰로즈의 식이가 Streptozotocin유도 당뇨 흰쥐의 혈당과 혈청 콜레스테롤 함량에 미치는 영향)

  • The effect of hemicellulose extracted from soy fiber on the level of blood glucose and serum cholesterol in the streptozotocin-induced diabetic rat were investigated. The experimental plots were divided to cellulose group (control, 0.5% hemicellulose group (H-l) and 1% hemicellulose group (H-2) group. The groups were fed for 6 weeks, then fed for 1 week more after streptozotocin injection. Food intakes, weight gain and food efficiency ratio of H-2 group were higher, while water intakes and liver weight were lower than those of control and H-l group. The content of blood glucose and urine glucose were 212.8 mg/dL, 0.97 mg/dL in the control group, 160.5 mg/dL, 0.53 mg/dL in the H-l group, 141.0 mg/dL, 0.35 mg/dL in the H-2 group, respectively. There was no significantly difference in the content of neutral lipid, while the content of total serum cholesterol was 101.6 mg/dL in the control group, 73.8-78.4 mg/dL in the hemicellulose groups. The content of serum HDL-cholesterol in the all experimental groups showed no significantly difference showing 39.8-44.7 mg/dL. HTR and atherogenic index were 0.44 and 1.27 in the control group, but 0.54 and 0.46-0.85 in the hemicellulose groups, respectively.

Effects of Onion Kimchi Extract Supplementation on Blood Glucose and Serum Lipid Contents in Streptozotocin-induced Diabetic Rats (양파김치 추출물 투여가 Streptozotocin 유발 당뇨병 흰쥐의 혈당강하 및 혈중지질 함량에 미치는 영향)

  • The purpose of this study was carried out to examine the effects of onion kimchi extract supplementation on blood glucose level and serum lipid components in streptozotocin (STZ)-induced diabetic rats for 4 weeks. STZ was administered as a single dose (50 mg/kg BW) to induce diabetes, and the diabetic rats were divided into eight groups (normal, diabetic control, and six treatment groups). The dose of onion kimchi extract 100 (OK-100), 200 (OK-200), and 400 (OK-400) mg/kg/day or quercetin as a main compound of onion 5 (Q-5), 10 (Q-10), and 20 (Q-20) mg/kg/day were orally administered daily to STZ-induced diabetic rats for 4 weeks after STZ injection. The diabetic control rats (465.6 mg/dL) showed significantly higher blood glucose level than the normal rats (76.3 mg/dL) after 4 weeks, but was significantly reduced with onion kimchi extract and quercetin supplementation (p<0.001). Changes in body weight, kidney weight and urine volume were not significantly different in diabetic control rats, and in onion kimchi extract and quercetin treated rats. The serum total cholesterol levels of control were significantly decreased in onion kimchi extract and quercetin supplementation groups, respectively (p<0.001). The blood urea nitrogen level and urinary protein excretion in diabetic rats were not significant different among the groups. These results suggest that onion kimchi extract supplementation in STZ-induced diabetic rats may be a very important factor for the reduction of blood glucose and serum cholesterol profiles.

The Effects of Gamioryung-san Extracts on Streptozotocin-induced Diabetic Nephropathy Rats (가미오령산(加味五苓散)이 Streptozotocin으로 유발된 흰쥐의 당뇨병성(糖尿病性) 신증(腎症)에 미치는 영향)

  • Objectives : The object of this study was to observe the effects of Gamioryung-san (GOS), which consists of 22 types of herbs, on streptozotocin (STZ)-induced diabetic nephropathy rats. Methods : Three different dosages of GOS were orally administered once a day for 28 days from 3 weeks after STZ treatment. Six groups, each of 8 rats per group were used. Changes on the body weights, blood glucose levels, serum BUN and creatinine levels, urine volumes, and UAER were observed with changes on the kidney malondialdehyde contents and glutathione, dismutase and catalase contents. In addition, histopathology of kidney, pancreas, thymus and spleen were observed. The results were compared with antioxidant silymarin 100 mg/kg, of which the effects on STZ -induced diabetes and related complications are already confirmed. Results : As a result of treatment of GOS 800, 400 or 200 mg/kg for 28 days, STZ-induced decreases of body weights, hyperglycemia, atrophic changes of pancreatic islets with decreases of insulin-immunoreactive cells and decreases of glucagon -immunoreactive cells were inhibited dose-dependently. Increases in kidney weight, serum BUN and creatinine levels, urine volumes, UAER, vasodilated atrophic glomerulus and abnormal tubules were inhibited dose-dependently. Also increases of kidney MDA contents and decreases of GSH contents, SOD and CAT activities, decreases of thymus and spleen weights, and atrophic changes at histopathological observation were also inhibited. The effects of GOS 400 mg/kg showed similar effects to silymarin 100 mg/kg. Conclusions : These results suggest that 400 mg/kg of GOS retarded the STZ-induced diabetic nephropathies as similarly to silymarin 100 mg/kg, through modulations of oxidative stress and immune systems.

Burkholderia Cepacia Causing Nosocomial Urinary Tract Infection in Children

  • Purpose: Burkholderia cepacia is an aerobic, glucose-non-fermenting, gram-negative bacillus that mainly affects immunocompromised and hospitalized patients. Burkholderia cepacia has high levels of resistance to many antimicrobial agents, and therapeutic options are limited. The authors sought to analyze the incidence, clinical manifestation, risk factors, antimicrobial sensitivity and outcomes of B. cepacia urinary tract infection (UTI) in pediatric patients. Methods: Pediatric patients with urine culture-proven B. cepacia UTI between January 2000 and December 2014 at Samsung Medical Center, a tertiary referral hospital in Seoul, Republic of Korea, were included in a retrospective analysis of medical records. Results: Over 14 years, 14 patients (male-to-female ratio of 1:1) were diagnosed with B. cepacia UTI. Of 14 patients with UTI, 11 patients were admitted to the intensive care unit, and a bladder catheter was present in 9 patients when urine culture was positive for B. cepacia. Patients had multiple predisposing factors for UTI, including double-J catheter insertion (14.2%), vesico-ureteral reflux (28.6%), congenital heart disease (28.6%), or malignancy (21.4%). Burkholderia cepacia isolates were sensitive to piperacillin-tazobactam and sulfamethoxazole-trimethoprim, and resistant to amikacin and colistin. Treatment with parenteral or oral antimicrobial agents including piperacillin-tazobactam, ceftazidime, meropenem, and sulfamethoxazole-trimethoprim resulted in complete recovery from UTI. Conclusion: Burkholderia cepacia may be a causative pathogen for nosocomial UTI in pediatric patients with predisposing factors, and appropriate selection of antimicrobial therapy is necessary because of high levels of resistance to empirical therapy, including aminoglycosides.

Beneficial Effect of the Combination of Oral Administration and Herbal-acupuncture Stimulation with Anti-diabetic Prescriptiom 1(AD-1) on Streptozotocin-induced Diabetic Nephropathy Rats (당뇨1호방(糖尿1號方)의 약침(藥鍼) 및 구강투여(口腔投與)가 Streptozotocin에 의한 흰쥐의 당뇨병성(糖尿病性) 신증(腎症)에 미치는 영향)

  • Objectives : The Present study was carried out to investigate the preventive effect of Anti-diabetic prescription 1(AD-1) on streptozotocin(STZ)-induced diabetic nephropathy. Methods : AD-1 consists of eleven herbs that have an effect on diabetes mellitus. AD-1 was given to rats with the combination of oral administration and herbal-acupuncture stimulation. The experimental animals were divided into 3 groups : normal group of rats, control group of STZ-induced diabetic rats, sample group with AD-1 treatment. Experimental diabetes was induced by the injection of STZ(60mg/kg) to e rat via the peritoneum. The effect of AD-1 on STZ-induced diabetic nephropathy was observed by measuring the serum level of creatinine and BUN. Urine secretion of albumin for 24 hours and urine level of glucose measures too. Anti-oxidative stress of AD-1 administration in living body was estimated by measuring lipid peroxide and GSH content in cortex of kidneys. Results : STZ induced increase of serum creatinine, BUN and albumin secretion were lowered by AD-1 treatment. Conclusion : The AD-1 treatment showed protective effect on rat diabetic nephropathy model, and action mechanism of the effect was thought to be concerned with anti-oxidative stress.
