Imaging Diagnosis of Emphysematous Pyelonephritis in a Non-Diabetic Dog

  • An 11-year-old, 4.3 kg, mixed breed, intact female dog exhibiting vomiting and a reduced appetite was presented. On physical examination, palpation of the right kidney elicited discomfort. Blood analysis revealed leukocytosis with neutrophilia, and elevation of alkaline phosphatase and blood urea nitrogen. Escherichia coli was detected in urine culture, whereas urine analysis showed absence of glucose. Radiography and ultrasonography suggested that there was gas in the right renal parenchyma, and computed tomography was performed while the dog was awake to investigate the suspected gas in the right kidney. After unilateral ureteronephrectomy, a histological diagnosis of right emphysematous pyelonephritis (EPN) was made. EPN refers to a severe necrotizing infection of the renal parenchyma with gas accumulation in the tissue. It is rare in both human and veterinary medicine, and almost all EPN patients have uncontrolled diabetes mellitus. This case report describes the imaging-based diagnosis of a rare occurrence of unilateral EPN in a non-diabetic dog.

Repeated Dose 90-Day Oral Toxicity Study of Modified Samjung-Hwan in Sprague-Dawley Rats (삼정환의 랫드를 이용한 90일 반복 경구투여 독성시험)

  • Objectives: The study is aimed at evaluating the possible toxicity in 90-day repeated oral administration of modified Samjung-hwan (mSJH) in Sprague-Dawley (SD) rats. This study was conducted to detect the no-observed adverse effect level (NOAEL). Methods: Modified SJH extract was administered orally in male and female SD rats at dose of 0, 1,000, 2,000, 4,000 mg/kg. Each group consisted of 10 rats of each gender. The modified SJH extract was given once a day for 90 days. We monitored the changes of mortalities, clinical signs, body weight changes, food consumption, ophthalmologic findings, urine analysis, hematology, serum biochemistry, necropsy findings, organ weights, histological markers of all animals treated with modified SJH extract during the study period. Results: There were no toxicologically significant changes in mortalities, clinical signs, body weight gains, food consumption, ophthalmologic findings, urine analysis, hematology, serum biochemistry, necropsy findings, organ weights, histological markers in any of rats tested. Conclusions: The NOAEL of the modified SJH extract in male rats and no observed effect level (NOEL) in female rats are considered 4,000 mg/kg.

Uncertainty Evaluation of the Analysis of 11-Nor-9-carboxy-${\Delta}^9$-tetrahydrocannabinol in Human Urine by GC/MS (GC/MS를 이용한 소변 중 대마 대사체 분석의 측정불확도 평가)

  • We described an estimation of measurement uncertainty in quantitative analysis of 11-nor-9-carboxy-${\Delta}^9$-tetrahydrocannabinol (THCCOOH), the major metabolite of ${\Delta}^9$-tetrahydrocannabinol, in urine sample by solid-phase extraction (SPE) and GC/MS detection. The analytical results were compared and the different contributions to the uncertainty were evaluated. Inter-day and inter-person validation were performed using statistical analysis of several indicative factors. Measurement uncertainty associated with target analyte in real forensic samples was estimated using quality control (QC) data. Traceability of measurement was established through traceable standards, calibrated volumetric glassware and volume measuring device. The major factors of contribution to combined standard uncertainty, were calibration linearity, inter-day repeatability and inter-person reproducibility, while those associated with preparation of analytical standards and sampling volume were not so important considering the degree of contribution. Relative combined standard uncertainties associated with the described method was 12.05% for THCCOOH.

Analysis of urine β2-microglobulin in pediatric renal disease (소아 신장질환에서 요 β2-microglobulin검사의 분석)

  • Purpose : There have been numerous researches on urine ${\beta}_2$-microglobulin (${\beta}_2$-M) concerned with primary nephrotic syndrome and other glomerular diseases, but not much has been done in relation to pediatric age groups. Thus, our hospital decided to study the relations between the analysis of the test results we have conducted on pediatric patients and renal functions. Methods : Retrospective data analysis was done to 102 patients of ages 0 to 4 with renal diseases with symptoms such as hematuria, edema, and proteinuria who were admitted to Chung-Ang Yongsan Hospital and who participated in 24-hour urine and urine ${\beta}_2$-M excretion test between January of 2003 and January of 2006. Each disease was differentiated as independent variables, and the statistical difference of the results of urine ${\beta}_2$-M excretion of several groups of renal diseases was analyzed with student T-test by using test results as dependent variables. Results : Levels of urine ${\beta}_2$-M excretion of the 102 patients were as follows : 52 had primary nephrotic syndrome [MCNS (n=45, $72{\pm}45{\mu}g/g$ creatinine, ${\mu}g/g-Cr$), MPGN (n=3, $154{\pm}415{\mu}g/g-Cr$), FSGS (n=4, $188{\pm}46{\mu}g/-Cr$], six had APSGN ($93{\pm}404{\mu}g/g-Cr$), seven had IgA nephropathy ($3,414{\pm}106{\mu}g/g-Cr$), 9 had APN ($742{\pm}160{\mu}g/g-Cr$), 16 had cystitis ($179{\pm}168{\mu}g/g-Cr$), and 12 had HSP nephritis ($109{\pm}898{\mu}g/g-Cr$). IgA nephropathy (P<0.05) and APN (P<0.05) were significantly higher than in other renal diseases. Among primary nephrotic syndrome, FSGS with higher results of ${\beta}_2$-microglobulin test had longer treatment period (P<0.01) when compared to the lower groups, but no significant differences in Ccr, BUN, or Cr were observed. Conclusion : IgA nephropathy and APN groups showed significantly higher level of ${\beta}_2$-M excretion value than other groups. Although ${\beta}_2$-microglobulin value is not appropriate as an indicator of general renal function and pathology, it seems to be sufficient in the differential diagnosis of the UTI and in the prediction of the treat-ment period of nephrotic syndrome patients.

Analysis of some pesticides in urine by GC/MS (GC/MS를 이용한 뇨중 몇가지 농약의 분석)

  • The purpose of this study is to establish the assessment techniques of hazardous chemicals by the development of analytical method of biological samples. In this study, we have developed an extraction method of nine pesticides used for rice paddy that resulted in high recovery from the spiked human urine by the liquid-liquid extraction with diethyl ether at pH 7.0. Calibration curve obtained from each pesticide standard using by gas chromatography/mass spectrometry/selected ion monitoring has shown good linearity and detection limits were the range of $0.4{\sim}2.0$ ng/mL in urine. As a biological monitoring, urine samples of local farmers exposed directly to nine pesticides in the field were collected and analyzed by GC/MS. Of the tested pesticides, metabolites of phenthoate assumed were identified by GC/MS analysis. No parent compound was detected.

Analysis Method of N-Nitrosamines in Human Urine by LC-MS/MS System (LC-MS/MS 시스템을 이용한 소변 중 N-니트로사민류 분석법 확립)

  • N-nitrosamines are the nitroso compounds which are produced by nitrosation reactions of the secondary amine and nitrite under acidic conditions. Approximately 300 species of N-nitrosamine have been tested for carcinogenicity in laboratory experiments, with 90% of them demonstrated carcinogenic effects different animal species, including higher primates. In 1978, IARC classified NDMA and NDEA as Group 2A, and NDPA, NDBA, NPIP, NPYR and NMOR as Group 2B. In this study, we established pretreatment and analytical method for N-nitrosamines (NDMA, NDEA, NMEA, NDPA, NDBA, NPIP, NPYR and NMOR) in human urine for biological monitoring of N-nitrosamines. The analytes were extracted using solid phase extraction (SPE), then quantitative analysis was performed by LC-(APCI)-MS/MS. The accuracies of the established method were between 85.8~108.7% and precisions were lower than 20%. The limit of detection (LOD) were between 0.0002 (NDBA) and 0.0793 (NDMA) ng/ml. The linearity obtained was satisfying for the 8 N-nitrosamines, with a coefficient of determination ($r^2$) higher than 0.999. The mean concentrations of N-nitrosamines in the urine were 2.645 mg/g creatinine for NDMA, 0.067 mg/g creatinine for NDEA, 0.009 mg/g creatinine for NMEA, 0.011 mg/g creatinine for NDBA, 0.271 mg/g creatinine for NPIP and 0.413 mg/g creatinine for NPYR. NDPA and NMOR were not detected. It can be used as a instrumental methodology for evaluation and risk assessment of human exposure to N-nitrosamines for the further research.

The Horse Manure Fertilizers and Pollutants Characteristics Analysis Research (말 분뇨의 비료성분 및 오염물질 특성 조사 연구)

  • Horse manure general composition and contaminants, heavy metals content analysis was investigated as follows. The moisture content of horse feces was 76.1% and 96% for horse urine. The average T-N, T-P, T-K concentrations of the racing horse were 0.18%, 0.10%, 0.22% respectively and 2.88%, 0.0015%, 0.84% for urine. And the average BODs, COD, SS concentrations were 26,906 $mg/{\ell}$, 36,642 $mg/{\ell}$, 89,375 $mg/{\ell}$ respectively and 14,298 $mg/{\ell}$, 7,484 $mg/{\ell}$, 6,987 $mg/{\ell}$ for urine. In addition, the Cr, Cu, Ni, Cd, Pb, Zn concentrations of feces were 2.65 mg/kg, 7.05 mg/kg, 1.24 mg/kg, 0.07 mg/kg, 0.01 mg/kg, 45.11 mg/kg respectively and 1.52 mg/kg, 1.62 mg/kg, 0.00 mg/kg, 0.03 mg/kg, 0.03 mg/kg, 4.49 mg/kg for urine on the fresh matter basis. The end of the survey was that the quality of the compost can be used as a raw material, and manure is necessary to constantly driven forward horse manure composting research.

Analysis of Uranium Concentrations in Urine Samples Using Alpha Spectrometry and Dose Assessment (알파분광분석법을 이용한 소변시료 중 우라늄 농도 분석 및 선량평가)

  • This study was conducted to measure the uranium concentrations in urine of some members of the general public in Busan and Daejeon and to assess the annual committed effective doses from uranium analysis of daily excretion. As a result, the ranges of total uranium concentrations in the urine for the residents in Busan and Daejeon were found to be 0.556 - 1.53 $mBq\;L^{-1}$ and 2.18 - 4.55 $mBq\;L^{-1}$, respectively. It was noted that the uranium concentrations for the residents in Daejeon were observed to be higher than those for the residents in Busan. This result assumes that the uranium concentrations in the urines for the residents in Daejeon are probably related to the high uranium concentrations contained in the drinking water of Daejeon city. The bedrock of Daejeon, known as granitic rocks formed in the Jurassic period of the Mesozoic Era, contains high uranium contents. Also, results showed no significant correlation with age or sex. The ranges of annual committed effective doses from ingestion of uranium for the residents in Busan and Daejeon were calculated to be 0.472-1.41 ${\mu}Sv$ and 1.99-4.15 ${\mu}Sv$, respectively.

Urinalysis and Imaging Studies in Children with Urinary Tract Infection (소아 요로감염증으로 진단된 환아에서 요분석 검사와 신영상 검사)

  • Purpose : We studied the correlation between urinalysis and radiologic findings in infants and children with urinary tract infection. Method : Urine Dipstick test and unstained urine microscopic examination were carried out in 56 infants and children who were hospitallized with the diagnosis of urinary infection by pocitive urine culture at Seoul Adventist from September 1996 through August 1998. Urine was collected by midsream, catheter, urine bag after cleansing or bladder puncture. Renal sonography and $^{99m}Tc-DMSA$ renal scan or voiding cystoureterography were studied. Results : 1)In dipstick analysis, leukocyte esterase(LE) were positive in 30 cases of 56 patients(54%) and Nitrite were positive in 20 cases of 56 patients(36%) and LE or Nitrite were positive in 38 cases of 56 children with UTI(68%). 2)In microscopic analysis, WBC were positive in 38 cases of patients(68%) and bacteria were positive in 23 cases of 56 patients(41%) and WBC or bacteria were positive in 41 cases of 56 children with UTI(71%). 3)Ten cases of 56 UTI patients(18%) showed negative finding in Dipstick and microscopic analysis. 4)There was no significant difference in positive rate of dipstick(71% vs 66%) and microscopia analysis(83% vs 66%) between two group with or without renal scar on $^{99m}Tc-DMSA$ scan (P=0.117). 5)There was no significnt difference in positive rate of dipstick(100% vs 91%) and microscopic analysis(100% vs 100%) between two groups with or without vesicoureteral reflux on VCUG. Conclusion : There was no specific relationships between the abnormal urinalysis and the abnormal findings on $^{99m}Tc-DMSA$ scan or VCUG.

Microanalysis of Pancuronium Bromide in Urine and Blood by HPLC (HPLC를 이용한 뇨 및 혈액중의 Pancuronium Bromide의 미량분석)

  • HPLC/fluorescence detection method for the analysis of pancuronium bromide in biological fluids was developed. The method depends on the formation of insoluble red complex between pancuronium bromide and rose bengal in aqueous layer. This complex is quantitatively extracted from aqueous layer into chloroform layer. The complex is stable for 1 day in chloroform layer at room temperature. It was possible to analyze pancuronium bromide in the range of 0.05~0.5 $\mu\textrm{g}$/ml without the effect of co-prescribed drugs.
