Comparison of medical history based diagnosis and urine test using ultra-performance liquid chromatography-tandem mass spectrometry in drug overdose (약물중독 환자에서 병력 기반 진단과 요 고성능 액체색층분석 탄뎀 질량 분광분석의 비교)

  • Purpose: In patients with acute drug overdose, identification of drugs ingested is crucial to make a precise diagnosis. In most cases, the diagnoses are made on the medical history and physical examination findings. This study was undertaken to determine the concordance of diagnosis made on the basis of patient history by comparing it with urine toxicology analysis. Methods: This was a retrospective study of drug intoxicated patients over 18 years old who presented to the emergency center from 2017 to 2019. Specimens from urine were tested using ultra-performance liquid chromatography-tandem mass spectrometry (UPLC-TMS). The test results were compared with information obtained from patients. Diagnostic concordances for drug detection in intoxicated patients were calculated. Logistic regression analysis was used to examine the association between clinical characteristics and diagnostic discrepancy. Results: Totally, 370 patients were included in the analysis. Overall, 66 types of drugs were detected by UPLC-TMS. The drugs detected most frequently were zolpidem (104, 27.8%), citalopram (70, 18.7%), and paracetamol (66, 17.6%). The mean diagnostic concordance of patients was 52.7%. There were statistically significant diagnostic discrepancies in patients with underlying depression and patients intoxicated with multiple types of drugs. Conclusion: In ED patients with acute drug overdose, the diagnoses made on history alone were often inaccurate. It is essential to perform urine toxicology tests such as UPLC-TMS as a confirmatory instrument to improve accuracy in evaluating patients with drug intoxication.

A Clinical Study of Sleeping, Stool, Urine According to Taeyangsoyangin(Yangin) and Taeumsoeumin(Eumin) (태양소양인(太陽少陽人)과 태음소음인(太陰少陰人)의 수면(睡眠), 대편(大便), 소편(小便)에 관한 임상적 고찰)

  • 1. Objectives : There are many studies on the ordinary symptoms based on Sasang constitution. But there are not quite satisfactory between the types of Sasang constitution. So This study is for learning the characteristics of ordinary symptoms depending on Taeyangin Soyangin(the rest Yangin) and Taeumin Soeumin(the rest Eumin) of the Sasang constitution. 2. Methods : We classified them into the each type of Sasang Constitutional Medicine(SCM) by the well-trained SCM specialist, and assessed their ordinary features by the questionnaire. Binary logistic regression analysis was applied to evaluate the influence of ordinary features to the diagnosis of SCM. 3. Results : There are a result of the binary logistic analysis on the observed questionnaire. 1) Regarding sleeping, Yangin do not dream much as Eumin do when they sleep. The time they are sleeping is not longer, and they usually do not sleep well. 2) Regarding stools, Yangin go to stool more than Eumin do. The constipation does not occur when they are not in a good condition, and they do not feel uncomfortable when they do not go to stool for a day. The length of time taken for emptying the bowels is much longer, and the hardness of their stools is much more, but the hardness does not mean that they have the constipation. 3) Regarding urine, Yangin have much more foam than Eumin. 4. Conclusions: We found that Yangin and Eumin have characteristics of ordinary symptoms, but partly there are not in accordance with ones what Lee Je-ma said in his book. So in future we hope clinical studies are required steadily.

Urinary tract infections in pediatric oncology patients with febrile neutropenia (호중구 감소성 발열을 보이는 소아 암 환자에서의 요로감염에 대한 연구)

  • Background: Neutropenic fever is one of the most common and potentially severe complications of chemotherapy in pediatric oncology patients, while urinary tract infection (UTI) is one of the most prevalent bacterial infections in these patients. Therefore, this study was conducted to investigate features of UTI with neutropenic fever in pediatric oncology patients. Methods: We retrospectively reviewed and analyzed the medical records, laboratory results and image findings of cases of neutropenic fever in the Department of Pediatrics of Yeungnam University Medical Center, South Korea between November 2013 and May 2015. Episodes were divided into two groups, UTI vs. non-UTI group according to the results of urine culture. The results were then compared between groups. The analysis was performed using IBM SPSS 23.0. A p-value <0.05 was considered to indicate a significant difference between groups. Results: Overall, 112 episodes of neutropenic fever were analyzed, among which 22 episodes (19.6%) showed organisms on urine culture and were classified as UTI. The remaining 90 episodes were classified as non-UTI. Only four episodes (18.2%) of the UTI group showed pyuria on urine analysis. In the UTI group, 76.5% were sensitive to the first line antibiotics and showed higher clinical response than the non-UTI group. Among hematologic malignancy patients, the UTI group revealed higher serum ${\beta}2$-microglobulin levels than the non-UTI group ($1.56{\pm}0.43mg/L$ vs. $1.2{\pm}0.43mg/L$, p<0.028). Conclusion: UTI in pediatric neutropenic fever responds well to antibiotics. Hematologic malignancy cases with UTI reveal increased serum ${\beta}2$-microglobulin level. These results will be helpful to early phase diagnosis of UTI.

  • This paper describes a methodology for the development of models of discrete event system. The methodology is based on transformation of continuous state space into discrete one to homomorphically represent dynamics of continuous processes in discrete events. This paper proposes a formal structure which can coupled discrete event system models within a framework. The structure employs the discrete event specification formalism for the discrete event system models. The proposed formal structure has been applied to develop a discrete event specification model for the complex spectral density analysis of strip for urin analyzer system. For this, spectral density data of strip is partitioned into a set of Phases based on events identified through urine spectrophotometry. For each phase, a continuous system of the continuous model for the urine spectral density analysis has been simulated by programmed C++. To validate this model, first develop the discrets event specification model, then simulate the model in the DEVSIM++ environment. It has the similar simulation results for the data obtained from the continuous system simulation. The comparison shows that the discrete event specification model represents dynamics of the urine spectral density at each phase.

Hematologic Findings & Urinalysis of Workers Exposed to Mixed Organic Solvents (복합 유기용제 취급 근로자의 혈액 및 뇨검사 결과 분석)

  • To evaluate the effects on health by exposure to mixed organic solvents, the author analyzed some labortory findings of blood (hemoglobin, hematocrit RBC, WBC, SGOT, SGPT) and urine (protein, blood, sugar) that were obtained from 722 workers (male:446, female:276) who worked in plywood, painting and shoes factories in Pusan area from January to December, 1990. The result were as follows ; 1. The mean values of each hematologic findings were all within normal limit but there were significant differences at hematocrit, RBC, WBC, SGOT in male and at WBC, SGPT in female at the comparison of the mean values and the distribution according to the normal criteria by type of work. 2. There were no differences in urinalysis(protein, blood, sugar) by type of work. 3. In stepwise multiple regression, analysis of hematologic findings on age, working duration and type of work, regression coefficients on age and type of work were relatively high in male and female sub1, respectively. As a result of this study, it is considered that the measurement of WBC with morphology is significant in hematologic findings and instead of spot urine and urinstix, 24 hours urine and quantitative analysis is required in urinalysis in screening test for healthy status of workers who deal with the organic solvents.

  • This study evaluates the pretreatment for analysis of benzidine metabolites in urine by measuring the recovery rates according to the temperature and periods of storage of the urine. By the solid phas e extraction, the recovery rates of basic hydrolysis are benzidine 67.4 %, monoacetylbenzidine 105.1 %, and diacetylbenzidine 115.8 %, respectively. By the liquid extraction, the recovery rates of back-extraction into 0.1 M perchloric acid are benzidine 105.7%, monoacetylbenzidine 94.2 %, diacetylbenzidine 72.8 %, respectively. The difference of the recovery rates between the back-extraction into 0.1 M HCl and 0.1 M perchloic acid after basic hydrolysis are 101 % and 98.8 %, respectively. When the recovery rates of the urinary s amples of pH 3, pH 7, pH 12 at $25^{\circ}C$ and $-76^{\circ}C$ are compared for four weeks, there are no differences according to the temperature and the periods of storage. The above results show that the solid phase extraction and back-extraction by 0.1 M perchloric acid after basic hydrolys is are suitable for the analysis of benzidine metabolites. There are no difference of the recovery rates of the urinary samples stored at $25^{\circ}C$ and $-76^{\circ}C$ at pH 3, pH 7, pH 12, respectively for 28 days.

The association between dietary sodium intake and obesity in adults by sodium intake assessment methods: a review of systematic reviews and re-meta-analysis

  • BACKGROUND/OBJECTIVES: The scientific evidence of a sodium-obesity association is limited by sodium intake assessments. Our specific aim is to synthesize the association between dietary sodium intake and obesity across the sodium intake assessments as evidenced by systematic reviews in adults. SUBJECTS/METHODS: A systematic search identified systematic reviews comparing the association of dietary sodium intakes with obesity-related outcomes such as body mass index (BMI), body weight, waist circumference, and risk of (abdominal) obesity. We searched PubMed on October 24, 2022. To assess the Risk of Bias in Systematic Reviews (ROBIS), we employed the ROBIS tool. RESULTS: This review included 3 systematic reviews, consisting of 39 unique observational studies (35 cross-sectional studies and 4 longitudinal studies) and 15 randomized controlled trials (RCTs). We found consistently positive associations between dietary sodium intake and obesity-related outcomes in cross-sectional studies. Studies that used 24-h urine collection indicated a greater BMI for those with higher sodium intake (mean difference = 2.27 kg/m2; 95% confidence interval [CI], 1.59-2.51; P < 0.001; I2 = 77%) compared to studies that used spot urine (mean difference = 1.34 kg/m2; 95% CI, 1.13-1.55; P < 0.001; I2 = 95%) and dietary methods (mean difference = 0.85 kg/m2; 95% CI, 0.1-1.51; P < 0.05; I2 = 95%). CONCLUSIONS: Quantitative synthesis of the systematic reviews has shown that cross-sectional associations between dietary sodium intake and obesity outcomes were substantially different across the sodium intake assessments. We need more high-quality prospective cohort studies and RCTs using 24-h urine collection to examine the causal effects of sodium intake on obesity.

Inherited metabolic diseases in the urine organic acid analysis of complex febrile seizure patients (복합 열성경련 환자의 소변 유기산 분석에서 나타난 유전대사질환)

  • Purpose : Seizure associated with fever may indicate the presence of underlying inherited metabolic diseases. The present study was performed to investigate the presence of underlying metabolic diseases in patients with complex febrile seizures, using analyses of urine organic acids. Method : We retrospectively analyzed and compared the results of urine organic acid analysis with routine laboratory findings in 278 patients referred for complex febrile seizure. Results : Of 278 patients, 132 had no abnormal laboratory findings, and 146 patients had at least one of the following abnormal laboratory findings: acidosis (n=58), hyperammonemia (n=55), hypoglycemia (n=21), ketosis (n=12). Twenty-six (19.7 %) of the 132 patients with no abnormal findings and 104 (71.2%) of the 146 patients with statistically significant abnormalities showed abnormalities on the organic acid analysis (P<0.05). Mitochondrial respiratory chain disorders (n=23) were the most common diseases found in the normal routine laboratory group, followed by PDH deficiency (n=2) and ketolytic defect (n=1). In the abnormal routine laboratory group, mitochondrial respiratory chain disorder (n=29) was the most common disease, followed by ketolytic defects (n=27), PDH deficiency (n=9), glutaric aciduria type II (n=9), 3-methylglutaconic aciduria type III (n=6), biotinidase deficiency (n=5), propionic acidemia (n=4), methylmalonic acidemia (n=2), 3-hydroxyisobutyric aciduria (n=2), orotic aciduria (n=2), fatty acid oxidation disorders (n=2), 2-methylbranched chain acyl CoA dehydrogenase deficiency (n=2), 3-methylglutaconic aciduria type I (n=1), maple syrup urine disease (n=1), isovaleric acidemia (n=1), HMG-CoA lyase deficiency (n=1), L-2-hydroxyglutaric aciduria (n=1), and pyruvate carboxylase deficiency (n=1). Conclusion : These findings suggest that urine organic acid analysis should be performed in all patients with complex febrile seizure and other risk factors for early detection of inherited metabolic diseases.

  • Principles and advantages of neutron actiation analysis which is one of widely using nuclear techniques are introduced. The importance of neutron activation analysis in occupational health study is discussed. The indusrial hygienic study of the samples like human hair, blood, urine, organs, tissues and airborne contamination of the working environment can be enhanced by the technique. Statistical treatments of the acquired data are also emphasized.

  • After oral administration of $^{14}$ C-labelled $N^{G}$-mono[$^{14}$ C-methyl]-L-arginine into rats, 66.3% of the administered radioactivity has been recovered in the urine, and 86.2% of the total of the recovered radioactivity is recovered in the first 24-hr urine. Distributions of the radioactivity of the acidic, basic, and neutral portions and unmetabolized $N^{G}$-monomethyl-L-arginine are 33.3%, 40.2%, 12.5% , and 0.3%, respectively. The radioactivity corresponding N-monomethylurea is about 50% of the neutral portion and 6% of the administered radioactivity.ity.
