  • Objectives: The aim of this study was to systematically evaluate the clinical therapeutic effects and safety of the Zeo lyung tang (ZLT) on primary glomerulonephritis (PGN). Methods: The MEDLINE, EMBASE, PubMed, CENTRAL, CNKI, RISS, NDSL, KISS, and OASIS databases were searched for randomized controlled trials (RCTs) testing the effects of ZLT on PGN. The Cochrane collaboration bias risk assessment scale was used to evaluate the methodological quality of the included studies. RevMan 5.3 software was used for data analysis. Results: Ten RCTs involving 781 patients were included in the review. Compared with conventional Western medicine (WM) therapy alone, a combination treatment of ZLT and WM improved the total effective rate (RR=1.24; 95%CI [1.16, 1.33]; p<0.00001), reduce the blood urea nitrogen (BUN; MD=-1.05; 95%CI [-1.32, -0.78]; p<0.00001) and the 24-hour urinary protein (MD=-0.38; 95%CI [-0.46, -0.29]; p<0.00001). Conclusions: The combination of ZLT with WM has therapeutic effects on PGN, and it has advantages over WM treatment alone in reducing BUN and 24-hour urinary protein. However, due to the low quality of the included studies and the small sample sizes, additional research is needed in this area.

  • Purpose: This study was to verify the effects of a fluid therapy education program for aged stroke patients for preventing dehydration, pyuria and bacteriuria. Methods: The study employed a non-equivalent control group pretest-posttest design in quasi experimental basis. Subjects were 38 stroke patients (20 in the experimental group and 18 in the control group) older than 65, hospitalized in a senior care center in S city, Korea. Results: The amount of daily average fluid intake, normal ratio of blood urea nitrogen-creatinine ratio, serum $Na^+$ and urine white blood cells were increased significantly in the experimental group. Normal ratio of bacteriuria increased in the experimental group, but not significantly. Conclusion: The education program is considered to be an effective nursing intervention tool for preventing dehydration and urinary tract infections which related to the secondary wellness of aged stroke patients. However, longer term study is necessary for better quality of nursing and developing more specific education programs for aged stroke patients usually hospitalized for a long period of time.

  • The renoprotective activities of white ginseng radix and rootlet were compared in spontaneously hypertensive rat (SHR) with diabetes. During oral administration of white ginseng radix (Ginseng Radix Alba, GRA) and white ginseng rootlet (Ginseng Radix Palva, GRP) for four weeks, arterial blood pressure and blood glucose levels were determined at every 10 days. In both GRA- and GRP-treatment groups, arterial blood pressures started to go down after 10 days of administration and maintained throughout the study period. After four weeks administrations of GRA and GRP, diastolic blood pressures were significantly decreased with 17% and 9%, respectively. GRA treatment also decreased blood glucose levels after 10 days of administration when compared with diabetic SHR group. At the end of the experiment, serum creatinine (Scr) and blood urea nitrogen (BUN) were not significantly different between the groups, except 62% higher value of BUN in diabetic SHR group when compared with SHR group. In the diabetic SHR group, the excretion of urinary albumin was increased significantly when compared with SHR. The level of urinary albumin in GRA treated group was markedly reduced when compared with diabetic SHR group $(67.8{\pm}4.7\;vs.\;131.3{\pm}13.5\;mg/24\;h).$ To examine the effects of ginseng radices on an overt diabetic nephropathy, index of kidney hypertrophy and transforming growth $factor-{\beta}1\;(TGF-{\beta}1)$ protein levels were evaluated. The glomerular and tubular cells stained positive for $TGF-{\beta}1$ seemed to be more abundant in diabetic SHR than in those with SHR, and GRA treated rats showed somewhat less $TGF-{\beta}1$ protein in glomerular and tubular cells when compared with diabetic SHR. Our results suggest that GRA might be a useful antihypertensive and antidiabetic agent with renoprotective effect.

  • this study was undertaken to compare the riboflavin status of rural women with different physical activity intensity and to determine factors influencing biochemical fiboflavin status. The study was carried out over three different farming seasons : planting (June), harvest(October) and interim(February) in two rural regions of Korea. One was a traditional farming region, the other a commercial farming region with heavier work intensity. Twenty women in the traditional region and eighteen women in the commercial region were involved. The intensity of physical activity was determined by a daily activity record. Body composition was assessed by bioelectrical impedence. Dietary riboflavin intake was measured by the food frequency method. Riboflavin biochemical status was assessed by erythrocyte glutathione reductase activity coefficient (EGR AC) and ruinary riboflavin excretion. The results from the EGR AC and urinary riboflavin excretion during the period showed the overall riboflavin status of the commercial farming women was significantly worse than that of the traditional farming women(EGR AC p<0.0001, urinary riboflavin excretion p<0.05). The traditional farming group had about 40% with risk of riboflavin deficiency, whereas the commercial farming group had about 70%. Overall mean nutrient intake was not significantly different between the two groups, however, overall mean percent lean body mass representing long term physical activity was significantly higher in the commercial farming group ( <0.005). It appears that the biochemical riboflavin status of traditional farming women was significantly influenced by riboflavin intake and crude nitrogen balance while the biochemical riboflavin status of the commercial farming women was significantly influenced by riboflavin intake and percent of lean body mass over the three seasons. • We investigated the effects of aqueous extract from talc for suppression in the process of cyclophosphamide-induced cystitis in the rat. The weight of urinary bladder was increased in the cyclophosphamide-injected rat compared with normal, but the increase of weight was arrested by intake of talc. More similar results showed in the uric test involving nitrate content and blood cell number and serum analysis involving the content of blood urea nitrogen and uric acid in the talc challenged rat compared with control one. More severe histological changes of urinary bladder such as edema, wall thickness, bleeding, vacuolation in mucosal epithelium were demonstrated in the rat injected with cyclophosphamide compared with normal. Fewer scores of these changes such as edema and bleeding were observed in rats treated with talc. In the immunohistochemical analysis, the expression of inflammatory-related protein examined tended to increase in the urinary bladder of cyclophosphamide-injected rat, especially COX-2 and $TNF-{\alpha}$ in mucosal epithelium and iNOS and $IL-1{\beta}$ in mucosal and muscular layer. The decline of these immunoreation were observed in the talc treated rat, significant decrease of COX-2 was detected in mucosal epithelium and iNOS in submucosal layer. These results suggest that talc may use as a useful therapeutic agent for noninfectious cystitis.

  • A 4.65 kg 13-month-old male Pekingese dog was referred to veterinary teaching hospital of Konkuk University for evaluation of dysuria. On physical examination, severe distention of urinary bladder was found in abdominal palpation. Urinary catheter could not be guided into urinary bladder. On serum biochemistry, blood urea nitrogen (35.6 mg/dl) and creatinine (1.9 mg/dl) were increased. Obstruction part of proximal membranous urethra was founded on urethrogram. The length (13 mm) of obstruction part was callipered by cystourethrogram and urethrogram on operation. Surgical resection of obstruction part of urethra was performed without pubic osteotomy, and anastomosis was performed with 5-0 polyglycolic acid. Omentum was placed around the urethral anastomosis. On first day after surgery, appetite was good. On day 5, complete blood count and serum biochemistry showed normal range but mild urinary incontinence was showed after removing catheter. On day 7, urinalysis showed normal condition. On day 14, no leakage of surgical site was observed in excretory urogram. On day 21, no more urinary incontinence and good micturition were found. On 1 year later, the patient showed healthy condition without recurrence.

  • This study was designed to determine the effects of protein sources on the curation of gastric ulceration, protein metabolism, and nitrogen balance in rats with gastric ulcer induce by restraint and water immersion stress. After the rats were fed 10% casein diet for 3 weeks, four groups of the rats were forced in 5$\times$5$\times$15cm plexiglass cage. The restraint and water immersion stress was carried at 20$\pm$2$^{\circ}C$ for 8-hour. The other one group(control group) was not exposed to stress. After stress 4 kinds of different diets containing 20% protein were given for 5 days. The protein sources were casein, whey protein, soy protein, gluten. The control group was fed to 10% casein diet. The results were as follows ; the weights of rats were not different among the diet groups During the experiment period follows ; the weights of rats were not different among the diet groups during the experiment period (for 5 days). The ulcer index of rats fed 10% gluten and soy protein diet was significantly higher than those of casein and whey protein diet groups(p<0.05). The level of serum albumin was not significantly different among diet groups. But hematocrit and the level of $\alpha$-amino-N, BU and UUN of plant protein diet groups were higher than animal diet groups, the urinary hydroxyproline of soy protein group was the highest and the whey protein was the lowest. The digestibility and BV of nitrogen of gluten diet group were significantly higher than those of casein and whey protein diet groups(p<0.05). The animal proteins had more curative effects of ulcer than plant animals. The results of this study provide useful information concerning diet therapy for the patients with gastrointestinal diseases and the field of enteral diet materials.

  • A metabolism trial was conducted with 28 Najdi rams allocated into seven dietary groups to evaluate the effect of dietary inclusion of Salicornia bigelovii Torr biomass on nutrient digestibility, rumina I fluid metabolites and nitrogen and mineral balances. Either the stems (ST) or spikes (SP) of this seawater-irrigated halophyte were incorporated into complete diets at rates of 0, 10, 20 and 30% levels, replacing equal amounts of rhodesgrass hay in a ground mixed control diet. Digestibility of DM, OM, EE, NFE and fecal and urinary nitrogen were not affected by increased level of ST in the diet. As level of ST increased from 0 to 20% in the diets, CP digestibility and nitrogen retention approached their maximum (p<0.01), whereas CF digestibility reached its minimum (p<0.01). On the other hand, except for EE, digestion of all nutrients and nitrogen retention were linearly depressed (p<0.01) as SP increased in the diets from 10 to 30% level. Concentration of ammonia-N, total VFA and pH values in the rumen fluid were lower (p<0.01) with the ST- or SP-fed diets than with the control diet. Increasing level of ST or SP in the diet was associated with an increase (p<0.01) in the proportion of acetate and a decline (p<0.01) in molar percentage of propionate in the ruminal fluid. Sodium absorption increased (p<0.01) with increased ST and SP in the diets up to the 10 and 20% level, respectively, followed by constant absorption values up to the 30% level. When the level of ST in the diet gradually increased to 30%, a concomitant increased (p<0:01) in Ca and P absorption were obvious; whereas, increased level of SP in the diets from 0 to 30% resulted in noticeable (p<0.01) depression in Ca and P apparent absorption.
