한국인의 요중 크레아티닌 농도에 관한 연구 -국민건강영양조사 4기 자료 이용- (Urinary Creatinine Concentration in the Korean Population in KNHANES IV, 2009)

  • Objectives: Biomonitoring is used to assess human environmental exposures. Urinary biomonitoring data are typically adjusted to a constant creatinine concentration to correct for variable dilutions among spot samples. Our objective was to investigate urinary creatinine concentrations by region, gender, age and lifestyle. Methods: We studied urinary creatinine concentrations in 6,286 Koreans aged 20 years old and older who participated the fourth Korea National Health and Nutrition Survey (KNHANES IV) in 2009. Urinary creatinine concentration analysis used the Jaffe method. Results: The average urinary creatinine concentration of 6,286 Koreans was 158.99 mg/dl. The urinary creatinine concentration was significantly higher among men (184.97 mg/dl) than women (130.02 mg/dl). In both men and women, urinary creatinine concentrations were significantly different according to age group (p < 0.01). Whereas the urinary creatinine concentration of men significantly differed with smoking in adjusted comparison analysis, that of women significantly differed by education level. Ninety-two percent of urinary creatinine concentration was included within WHO guidelines effective range(30 mg/dl ${\leq}$ urinary creatinine concentration ${\leq}$ 300 mg/dl). In multiple regression analysis, creatinine was influenced by gender, age and body mass index (BMI). Conclusions: The urinary creatinine value obtained from representative samples of adult Koreans aged 20 year and older in KNHANES IV 2009 could be used as a reference value for other nationally studied surveys, such as abandoned metal mine surveys and surveys for industrial complexes.

한국 성인의 요중 크레아티닌 농도 변화에 대한 성, 연령 그리고 체질량지수(BMI)의 관련성 연구 (Relevance of Gender, Age and the Body Mass Index to Changes in Urinary Creatinine Concentration in Korean Adults)

  • The purpose of this study was to investigate the relevance of gender, age, and BMI (Body Mass Index) to changes in the urinary creatinine concentration in Korean adults. We recruited and surveyed 2,156 persons $\geq$20 years of age from 98 districts across the country, and collected urine samples for analyzing the creatinine concentration. Participants were 41.6% men and 58.4% women. In terms of age, the percentage of the population in their 20's and $\geq$60 years of age was relatively similar, with 13.6% and 17.8%, respectively, while the percentages in their 30's, 40's and 50's were also relatively similar, with 22.0%, 24.0%, and 22.6%, respectively. The proportion of participants in their 20's and 30's who were underweight (according to the BMI) was 25% and 19.2%, respectively, but only 6.2~6.4% of those $\geq$40 years of age were underweight. This showed that the underweight proportion in elder generations was less than that in younger generations. Meanwhile, the proportion who were overweight was 13.7%, 18.8%, 28.6%, 27.9%, 32.0% when the participants were in their 20's, 30's, 40's, 50's and $\geq$60 years of age, respectively (p<0.000). This showed that the overweight proportion increased along with increasing age. The overall urinary creatinine concentration was 10 mg/dl (arithmetic mean: AM), and 92.2 mg/dl (geometric mean: GM). The urinary creatinine concentration in men (132.6 mg/dl, AM) was significantly higher than that in women (93.3 mg/dl, AM (p<0.000)). Showing a similar trend in men and women, urinary creatinine concentrations were highest when the participants were in their 20's (135.6 mg/dl, AM), and tended to decrease with increasing age. Urinary creatinine concentrations in overweight and obese subjects (AM of 117.9 mg/dl and 118.0 mg/dl, respectively) were significantly higher than in other groups, and this trend was similar in men and women. In conclusion, we found that urinary concentrations were significantly affected by gender, age, and BMI, and that care should therefore be exercised when correcting urinary metabolites according to the urinary creatinine concentration.

치과위생사들의 요중 수은함량과 관련요인 분석 (Determinants of Urinary Mercury Concentration among Dental Hygienists)

  • Objectives: The aim of this study was to evaluate the level of urinary mercury and analyze which factors would affect urinary mercury concentration among dental hygienists in dental clinics. Methods: This study conducted by questionnaire and detection of urinary mercury concentration of 268 dental hygienists working in dental clinics from July to August of 2009. Data collected from two hundred and thirty-five dental hygienists were analyzed by the geometric mean (GM). Analytical results of urine samples with less than 0.3 g creatinine/L and greater than 3g creatinine/L were excluded from statistical analysis. Results: Urinary mercury concentration of 235 dental hygienists showed the geometric distribution. The arithmetic and geometric means of urinary mercury concentration were $0.996{\mu}g/g$ creatinine and $0.755{\mu}g/g$ creatinine, respectively. From multiple regression analysis, the number of amalgam filling, the consumption frequency of raw fish and the number of amalgam handling in current workplace was revealed as increasing factors of urinary mercury concentration. Conclusions: The level of urinary mercury in dental hygienists was higher than in general Korean population. The number of amalgam filling, the consumption frequency of raw fish and the number of amalgam handling in current workplace was revealed as increasing factors of urinary mercury concentration. Therefore using resin materials instead of amalgam in dental clinics is highly desirable.

  • The effect of exercise on serum cholesterol and triglyceride concentration and on urinary creatinine and 3-methylhistidine excretion was studied in eight healthy women, aged 21 to 25yr. Subjects were tested before and after a 3-day experimental period, during which 1000 times rope-jumping a day were performed. Serum cholesterol and triglyceride levels decreased significantly after exercise. Urinary excretion of creatinine and 3-methylhistidine slightly increased after exorcise.

벤젠 노출 근로자의 생물학적 모니터링 지표로서의 요중 S-Phenylmercapturic Acid에 관한 연구 (Urinary S-Phenylmercapturic Acid as a Biomarker for Biological Monitoring in Workers Exposed to Benzene)

  • This study was to evaluate the associations between urinary S-Phenyl-mercapturic acid(S-PMA) as a new indicator of biological monitoring for low level of exposure to benzene and independent variables such as the air concentration of benzene in the breathing zone of workers, the years of work, and smoking. In this study the subjects were the total of 145 drawn from 53 workers who were occupationally exposed to benzene and 92 workers who were not. The results were as follows: 1. In the workplace geometric mean concentration of benzene in the breathing zone of workers was 0.31 ppm(0.02 - 3.26 ppm) for the spraying workers and 0.25 ppm(0.02 - 3.95 ppm) for the printing workers. 2. The geometric mean of uninary S-PMA for non exposed group was $8.9{\mu}g/g$ creatinine($0.6-72.3{\mu}g/g$ creatinine), 80.3% (74 workers) of the total non-exposed workers indicated less than $20{\mu}g/g$ creatinine of uninary S-PMA. The difference of uninary S-PMA by sex, age, smoking was not significant. 3. The geometric mean of urinary S-PMA for workers who were exposed to benzene was $37.2{\mu}g/g$ creatinine, and was four times higher than that of workers who were not exposed. And 79.3% (42 workers) of the total exposed workers indicated more than $20{\mu}g/g$ creatinine of urinary S-PMA. 4. Regarding the level of benzene in the air, urinary S-PMA was the highest level of $147.9{\mu}g/g$ creatinine in the workers who were exposed to air concentration of 0.5 ppm of benzene and was higher as the level in the air was increased. 5. The correlation coefficient between log urinary S-PMA and log benzene concentration in the breathing zone was 0.80, and the following linear equation was found between urinary log S-PMA and log benzene concentration in the breathing zone : log S-PMA(${\mu}g/g$ creatinine) = 0.564 log benzene in air(ppm) + 0.192 (n=53, r=0.80, p=0.000) In conclusion, the concentration of S-PMA in urine proved to be good parameter for biological monitoring benzene exposure at the workplace even at low level of benzene in air.

정상 신생아에서 용질배설 측정 의의에 관한 연구 (A Study on Evaluating Solute Excretion in the Normal Neonate)

  • 목 적 : 출생 후 며칠간의 신생아의 creatinine수치는 모체의 영향을 받으므로 소변의 creatinine을 상수로 용질 배설 측정을 하는 것은 의미가 없을 수도 있다는 보고가 있어, 저자들은 신생아에서 일정한 간격으로 측정된 소변 creatinine이 차이가 있는가에 대하여 연구를 시행하게 되었다. 방 법 : 1998년 7월부터 8월까지 출생한 건강한 신생아를 대상으로 하여 두 번의 소변을 측정하였으며, 대조군으로는 그 기간 동안 입원한 소아과 병동의 환아를 대상으로 하였다. 신생아군은 49명이고 대조군은 33명이었으며, 신생아군의 첫 소변은 출생 첫날 시행하였고 비교군의 첫 소변시 나이는 $5.7{\pm}4.3$세였다. 신생아군과 대조군에서 처음과 두 번째 소변간의 간격은 2-3일이었다. 통계분석은 sigmastat 2.0을 사용하여 단순 선형 회귀 분석, t-test, paired t-test를 이용하였다. 결 과 : Creatinine mg/dL(Mean${\pm}$SD)은 첫 소변, 두 번째 소변에서 각각 신생아군은 $32.5{\pm}29,\;29.2{\pm}23$이고 대조군은 $57.7{\pm}51,\;58.8{\pm}51$으로 두 군 모두 첫 소변과 두 번째 소변의 소변 creatinine치는 차이가 없으며, 두 군간의 평균치의 비교분석은 유의한 차이를 보였다(P<0.01). 첫 소변과 두 번째 소변간의 creatinine평균치의 상관성에서 신생아군은 r=0.504(P<0.001)이고 대조군은 r=0.631(P<0.001)이었으며, creatinine/osmolality로 보정 후에도 각각 상관계수 0.506(P<0.001), 0.386(P<0.001)으로 유의한 상관관계를 보였다. 결 론 :신생아의 첫 소변과 두 번째 소변에서 creatinine의 비교분석에서는 유의한 차이가 없었으며 따라서 신생아에서도 용질배설 측정시 creatinine을 기준치로 사용할 수 있을 것으로 사료된다.

방향족 유기용제 폭로근로자들의 요중 N-Acetyl-β-Glucosaminidase Activity와 마뇨산농도 (NAG Activity and Urinary Excretion of Hippuric Acid Among Workers Exposed to Aromatic Organic Solvents)

  • In order to investingate the renal dysfunction in workers exposed to aromatic organic solvents, 105 male exposed workers and 109 controls were participated in this study. This study was conducted to examine the correlation among the concentration of aromatic organic solvents in air, worker's urinary NAG activity and urinary hippuric acid. The results were as follows : 1. The exposure level of aromatic organic solvent mixture caculated by the equation proposed by ACGIH was ranged from 0.27 to 0.54 and toluene concentration was ranged from 18.3ppm to 48.3ppm. 2. NAG activity in the urine of control and exposed group were $36.1{\pm}14.2nmolMU/mg$ creatinine and $52.4{\pm}26.1nmolMU/mg$ creatinine, respectively. Hippuric acid concentration in the urine of control and exposed group were $191.1{\pm}194.2mg/g$ creatinine and $789.3{\pm}784.1mg/g$ creatinine, respectively. 3. Correlation coefficent between the exposure level of aromatic organic solvent mixture was significantly related to urinary NAG activity(r=0.542) and urinary hippuric acid(r=0.752). 4. Correlation coefficient between NAG activity and hippuric acid concentration was 0.357. This study suggested that urinary NAG activity was a good renal function index for aromatic organic solvents exposure and urinary hippuric acid was an index for the biological monitoring of toluene exposure.

디젤 연소물질에 노출된 광산 근로자에서 소변 중 1-hydroxypyrene을 이용한 생물학적 모니터링 (Biological monitoring of miners exposed to diesel exhaust using urinary 1-hydroxypyrene)

  • Diesel vehicles are a significant source of fine carbon particle emissions including polynuclear aromatic hydrocarbons (PAHs). Urinary 1-hydroxypyrene (1-OHP) is firmly established as a useful biomarker of PAHs uptake in human. To investigate the exposure effect of PAHs in miners according to using diesel truck which was for transportation of ore, we measured urinary 1-OHP as the PAHs exposure biomarker, and analyzed the relationship between urinary 1-OHP concentration and using diesel truck. The study was performed on 118 workers (56 miners in factories using diesel truck, 62 miners in factories non-using diesel truck) and 21 controls. Urine samples were obtained at the end of shift on the survey day. There was no significance in comparison with the mean concentrations on urinary 1-OHP by age, BMI, work duration, smoking, drinking and ventilation type. But significant difference were found among urinary 1-OHP concentrations on factories according to using diesel truck (p=0.000). The urinary 1-OHP mean concentration on underground miners using diesel truck ($0.54{\mu}mol/mol$ creatinine) was higher than those of surface miners using diesel truck ($0.33{\mu}mol/mol$ creatinine, p=0.028), underground miners non-using diesel truck ($0.32{\mu}mol/mol$ creatinine, p=0.001) and controls ($0.22{\mu}mol/mol$ creatinine, p=0.000). In comparison with using status diesel truck, the urinary 1-OHP mean concentration of underground miners using diesel trucks was higher than those of other mine status. The study results would be beneficial to future environmental and biological studies of PAHs exposure to diesel exhaust in mines.

Urinary albumin excretion rate and puberty in non-diabetic children and adolescents

  • Slightly elevated urinary albumin excretion rate (microalhuminuria) is a marker of early diabectic nephropathy, but it is unclear if the established definition of microalbuminuria ($20-200{\mu}g/min$) is correct for children and adolescents. We investigated th.: albumin excretion rate, albumin/creatinine ratio and urinary albumin concentration in 150 healthy schoolchildren and adolescents to (a) obtain a reterence value for albumin excretion rate, (b) relate albumin excretion to pubertal stages and (c) evaluate albumin/creatinine ratio and morning albumin concentration as screening methods for elevated albumin excretion rate. Albumin concentration was measured by immunoturbidimetry in timed overnight urine samples. The albumin excretion showed a skewed distribution (geometric mean $3.2{\mu}g/min$, 95 percentile ($15.1{\mu}g/min$). In girls. a peak in the albumin excretion rate was found at the pubertal stage 4 (Tanner) and in boys at stage 5. Albumin/creatinine ratio of 2.5 mg/mmol as a screening level for elevated albumin excretion ($15{\mu}g/min$) showed a high positive (0.88) and negative (0.99) predictive value.

Monitoring the Reproductive Status of Dairy Cows by Urinary Pregnanediol Glucuronide

  • This study was undertaken with the aim to establish a reliable radioimmunoassay (RIA) system for urinary pregnanediol glucuronide (PdG) and to employ it for monitoring the reproductive status of dairy cows. Urine and blood samples were collected from the Holstein cows both pregnant and non-pregnant. The samples were then investigated for evaluating the relationship between progesterone ($P_{4}$) in blood and PdG in urine adjusted with or without urinary creatinine basis. Biweekly urine collection was employed for three cows in estrous and those artificially inseminated, while urine from pregnant cows was collected on a monthly basis. P_{4}$ and PdG levels were measured by enzymeimmunoassay (EIA) and RIA techniques, respectively. Our results indicated the sensitivity of PdG for RIA being 35 pg/tube and the recovery rate of 100%. Urinary creatinine concentrations also fluctuated within a day, but change at midday was not noteworthy. Regardless of the time of urination the change in concentrations of PdG was relatively smaller and did not vary significantly. The urinary PdG concentration showed periodic changes as that with serum P_{4}$ levels during the cow's estrus cycle. The correlation coefficient rose when creatinine level in urine was adjusted but the change was also not significant. The concentrations of PdG during the luteal phase were detected between 8.2 and 17.4 ng/ml, three to five times higher than that in the follicular phase. The concentration of PdG from pregnant cows (21 days after conception) was three to four times higher than in the nonpregnant cows. Our finding suggests that the determination of urinary PdG could be reliably employed for early pregnancy detection. The urinary PdG level continued to raise until 30 days pre-partum while the concentration reached its peak at 30 ng/ml, after which it started to fall 18 to 30 days before parturition and finally fell to its nadir value one week after parturition. As the correlation coefficient between the urinary PdG and serum P_{4}$ was higher than that corrected by urinary creatinine it can be suggested that the adjustment is not needed. The concentrations of urinary PdG could be maintained stably for 2 days in urine samples stored at room temperature and extended to 8 days when the samples were pretreated by boiling for 30 minutes. In conclusion urinary PdG concentration even without the need for creatinine basis adjustment can be used directly for monitoring the reproductive status of dairy cows.
