Association of Helicobacter pylori cagA Gene with Gastric Cancer and Peptic Ulcer in Saudi Patients

  • This study was conducted to assess the relationship between occurrence of gastric cancer and peptic ulcer, and the presence of H. pylori cagA gene and anti-CagA IgG, and to estimate the value of these antibodies in detecting infection by cagA gene-positive H. pylori strains in Saudi patients. The study included 180 patients who were subjected to upper gastrointestinal endoscopy in Taif province and Western region of Saudi Arabia (60 gastric cancer, 60 peptic ulcer, and 60 with non-ulcer dyspepsia). Gastric biopsy specimens were obtained and tested for H. pylori infection by rapid urease test and culture. PCR was performed on the isolated strains and biopsy specimens for detection of the cagA gene. Blood samples were collected and tested for CagA IgG by ELISA. H. pylori infection was detected among 72.8% of patients. The cagA gene and anti-CagA IgG were found in 63.4% and 61.8% of H. pylori-infected patients, respectively. They were significantly (p < 0.01) higher in patients with gastric cancer and peptic ulcer compared with those with non-ulcer dyspepsia. Detection of the CagA IgG was 91.6% sensitive, 89.6% specific, and 90.8% accurate compared with detection of the cagA gene. Its positive and negative predictive values were 93.8% and 86%, respectively. The study showed a significant association between the presence of the cagA gene and gastric cancer and peptic ulcer disease, and between anti-CagA IgG and the cagA gene in Saudi patients. However, a further larger study is required to confirm this finding.

Dynamics of Functional Genes and Bacterial Community during Bioremediation of Diesel-Contaminated Soil Amended with Compost

  • Compost is widely used as an organic additive to improve the bioremediation of diesel-contaminated soil. In this study, the effects of compost amendment on the remediation performance, functional genes, and bacterial community are evaluated during the bioremediation of diesel-contaminated soils with various ratios of compost (0-20%, w/w). The study reveals that the diesel removal efficiency, soil enzyme (dehydrogenase and urease) activity, soil CH4 oxidation potential, and soil N2O reduction potential have a positive correlation with the compost amendment (p < 0.05). The ratios of denitrifying genes (nosZI, cnorB and qnorB) to 16S rRNA genes each show a positive correlation with compost amendment, whereas the ratio of the CH4-oxidizing gene (pmoA) to the 16S rRNA genes shows a negative correlation. Interestingly, the genera Acidibacter, Blastochloris, Erythrobacter, Hyphomicrobium, Marinobacter, Parvibaculum, Pseudoxanthomonas, and Terrimonas are strongly associated with diesel degradation, and have a strong positive correlation with soil CH4 oxidation potential. Meanwhile, the genera Atopostipes, Bacillus, Halomonas, Oblitimonas, Pusillimonas, Truepera, and Wenahouziangella are found to be strongly associated with soil N2O reduction potential. These results provide useful data for developing technologies that improve diesel removal efficiency while minimizing greenhouse gas emissions in the bioremediation process of diesel-contaminated soil.

Helicobacter pylori 억제능 김치 유산균의 분리와 특성 규명 (Isolation and Characterization of Kimchi Lactic Acid Bacteria Showing Anti-Helicobacter pylori Activity)

  • 김치로부터 강력한 H. pylori 생육 저해활성을 보이는 균주를 분리, 동정하여 Lb. plantarum NO1으로 명명하였다. Lb. plantarum NO1은 H. pylori 뿐만 아니라 그람 양성균 및 그람 음성균들에 넓은 범위의 저해활성을 나타내었다. Lb. plantarum NO1의 배양 상징액을 H. pylori배양액에 첨가한 후 H. pylori의 urease 활성을 측정한 결과 Lb. plantarum NO1의 강한 urease 억제활성($40{\sim}60%$ 저하)을 확인할 수 있었다. AGS위암 세포주에 H. pylori를 부착시킨 후 Lb. plantarum NO1의 배양액을 첨가하여 AGS세포에서 H. pylori 탈착능을 측정한 결과 Lb. plantarum NO1은 유산균 배양액 무첨가보다 33% 이상 높은 H. pylori 탈착능을 나타내었으며, 비교구로 사용된 Lb. rhamnosus GG, Lb. sakei SI3에 비해 더 우수한 H. pylori 탈착능을 나타내었다. 분리균주의 장내 생존성 여부 확인을 위하여 내산성, 인공위액에서 2시간동안 처리한 결과 Lb. plantarum NO1이 초기균수$(10^9CFU/ml)$를 유지하면서 높은 저항성을 나타내었다. Oxgall 농도 0.3%와 0.5%의 인공담즙에서 24시간 처리한 후에도 초기균수$(10^9CFU/ml)$를 유지하였다. 뿐만 아니라 인공위액에서 생존한 균주를 연속적으로 인공담즙으로 처리하였을 때에도 높은 생존율$(10^8{\sim}10^9CFU/ml)$를 유지하였다. Lb. plantarum NO1의 용혈성 반응 유무 결과 용혈반응이 일어나지 않았으므로 인체에 안전하다는 것을 간접적으로 확인할 수 있었다. 본 연구에서 김치로부터 분리한 H. pylori억제 유산균 Lb. plantarum NO1은 장내에 생존 가능성도 높으며, 동시에 위에서 효과적으로 H. pylori를 억제 할 수 있을 것으로 기대되어진다.

Helicobacter pylori 감염의 치료와 Clarithromycin 내성간의 연관성 (Relationship between Eradication of Helicobacter pylori Infection and Clarithromycin Resistance)

  • 본 연구를 수행하기 전에 H. pylori에 대한 어떠한 치료도 받지 않은 114명의 소화기 궤양 환자들을 내시경 검사를 하는 동안, 114개의 H. pylori 균주를 위 전정부로부터 분리하였다. H. pylori를 검출하기 위하여 rapid urease test, SSA와 cagA 유전자의 PCR증폭을 수행하였고, CagA 발현 검출을 위하여 Western blot을 수행하였다. H. pylori에 감염된 환자들은 omeprazole. clarithromycin (a macrolide), amoxicillin을 모두 사용하는데 3제 요법(triple therapy)으로 치료하였다. 치료가 중단되고 6주 후에 내시경 검사에서 세균 박멸률을 측정하였다. 내성률은 각각 clarithromycin이 $20.2\%$. amoxicillin이 $0.0\%$였다. Clarithromycin 내성은 H. pylori의 23S rRNA 유전자에 있는 A2142G돌연변이에 의한 것이 $87\%$이었다. A2142G돌연변이의 clarithromycin의 MIC값($32\~>256\;{\mu}g\ml$)은 A2143G돌연변이의 MIC값($4\~128\;{\mu}g/ml$)보다 더 높았다. Clarithromycin에 감수성을 가진 H. pylori는 박멸되었으나 clarithromycin내성을 가진 H. pylori는 박멸되지 않았다(P = 0.0001). 이러한 결과들은 CagA 발현에는 어떠한 영향도 받지 않았으며 H. pylori의 clarithromycin 내성은 치료 실패의 가장 중요한 이유임을 제시하였다. 우선적으로 실시되는 생검 배양에 대한 H. pylori의 항생제 감수성 시험은 감염된 환자들에 대한 3제 요법을 선택하기 이전에 필히 실시되어야 하며 국내에서 clarithromycin에 대한 1차 내성의 높은 빈도는 H. pylori의 감염증 치료에 심각한 문제점을 야기시켰다.

Helicobacter pylori Infection and a P53 Codon 72 Single Nucleotide Polymorphism: a Reason for an Unexplained Asian Enigma

  • Aim: P53, the most commonly mutated tumor suppressor gene in all types of human cancer, is involved in cell cycle arrest and control of apoptosis. Although p53 contains several polymorphic sites, the codon 72 polymorphism is by far more common. There are divergent reports but many studies suggest p53 pro/pro SNP may be associated with susceptibility to developing various cancers in different regions of the world. The present study aimed to find any correlation between H. pylori infection and progression of carcinogenesis, by studying apoptosis and the p53 gene in gastric biopsies from north Indian population. Materials and Methods: A total of 921 biopsies were collected and tested for prevalence of H. pylori by rapid urease test (RUT), imprint cytology and histology. Apoptosis was studied by the TUNEL method. Analysis of p53 gene polymorphism at codon 72 was accomplished by PCR using restriction enzyme BstU1. Observation: Out of 921 samples tested 56.7% (543) were H. pylori positive by the three techniques. The mean apoptotic index (AI) in the normal group was 2.12, while gastritis had the maximum 4.24 followed by gastric ulcer 2.28, gastropathy 2.22 and duodenal ulcer 2.08. Mean AI in cases with gastric cancer (1.72) was less than the normal group. The analysis of p53 72 SNP revealed that p53 (Arg/Arg), (Pro /Arg) variant are higher (40.59% & 33.66%) as compared to p53 pro/pro variant (25.74%) inthe healthy population. Conclusions: The North Indian population harbors Arg or Pro/Arg SNP that is capable of withstanding stress conditions; this may be the reason of low incidence of gastric disease in spite of high infection with H. pylori. There was no significant association with H. pylori infection and AI. However, there is increased apoptosis in gastritis which may occur independent of H. pylori or p53 polymorphism.

부탄올 내성 미생물의 분리, 동정 및 변이주의 개발 (Isolation, Identification and Mutant Development of Butanol Tolerance Bacterium)

  • 부탄올 용매에서 생존하는 부탄올 내성 미생물을 분리하였다. 분리된 미생물들의 세포성장은 부탄올 농도가 증가함에 따라 감소하였으며, 그 중에서 BRS02가 12.5 g/L에서 가장 높은 내성도를 나타내었다. 또한, UV를 이용하여 BRS02균의 변이를 유도하여 고농도 부탄올 내성균 BRS251을 개발하였다. 부탄올 생산 모델균주로 대장균과 함께 부탄올, 프로판올 및 펜탄올에 대한 내성도를 비교한 결과, 대장균은 7.5 g/L 부탄올과 20 g/L 프로판올, 2 g/L 펜탄올 농도까지 생육이 가능한 반편, BRS251은 더 고농도인 17.5 g/L 부탄올과 32.5 g/L 프로판올, 6 g/L 펜탄올 농도까지 생육이 가능하였다. 분리된 세균을 동정하기 위해서 그람염색 후 광학현미경으로 관찰한 결과 그람양성의 구균으로 확인이 되었으며, 6.5% NaCl에서 생육이 가능하였다. 생화학적 특성을 분석한 결과, arginine dihydrolase, ${\alpha}$-glucosidase, urease 효소활성을 가지고 있었으며, 호기적인 조건에서 D-galactose, Dmaltose, D-mannitol, D-mannose, methyl-${\beta}$-D-glucopyranoside, D-ribose, sucrose, D-trehalose를 탄소원으로 자화하여 산을 생성할 수 있었으며, bacitracin, vibriostatic agent O/129 및 optochin에 대한 항생제 내성을 나타내었다. 16S rRNA 유전자 서열을 결정하고 계통발생도 분석을 통해 BRS02는 최종적으로 Staphylococcus sp.임을 동정하였다.

Investigation of Association between oipA and iceA1/iceA2 Genotypes of Helicobacter pylori and Gastric Cancer in Iran

  • Background: H pylori is the main causative agent of Gastric cancer and chronic gastritis. Genetic diversity of H. pylori has major contribution in its pathogenesis. We investigated the prevalence of oipA and iceA1/iceA2 positive strains of H. pylori among patients with gastric cancer and gastritis. Materials and Methods: Sampling performed by means of endoscopy from 86 patients. DNA was extracted from tissue samples using DNA extraction kit. PCR assay was performed and products were monitored by Agarose Gel Electrophoresis. Results: Urease Test and 16S rRNA PCR did not show significant differences in detection of H. pylori. The frequency of iceA1 allele in patients with gastric cancer was significantly higher than those with gastritis (p<0.05). However, there was no significant difference in prevalence of oipA and iceA2 genes among the two groups of patients (p>0.05). Conclusions: The iceA1 gene, but the oipA and iceA2 genes, is associated with H. pylori-induced gastric cancer. However, confirmatory studies must be performed in future.

논에서 분리한 메탄산화세균 Methylomonas sp. SM4의 특성과 메탄올 생합성 (Characterization and Methanol Biosynthesis of a Methane-Oxidizing Bacterium, Methylomonas sp. SM4, Isolated from Rice Paddy Field Soil)

  • A methane-oxidizing bacterium was isolated from rice paddy field soil around Jeollanam-do province, Korea, and characterized. The isolate was gram-negative, orange pigmented and short rod ($1.1-1.2{\times}1.6-1.9{\mu}m$). It was catalase and urease-negative but oxidase-positive. The strain utilized methane and methanol as sole carbon and energy sources. It had an ability to grow with an optimum pH 7.0 and an optimum growth temperature $30^{\circ}C$. The strain was resistant to antibiotic polymyxin B but sensitive to streptomycin, kanamycin, ampicillin, chloramphenicol and rifampicin. The isolate required copper for their growth with concentration range of $2-25{\mu}M$, with an optimum of $10{\mu}M$. Under optimal culture condition, specific cell growth rate and generation time were found to be $0.046hr^{-1}$ and 15.13 hr, respectively. Phylogenetic analysis based on 16S rDNA sequences indicated that the strain formed a tight phylogenetic lineage with Methylomonas koyamae with a value of 99.4% gene sequence homology. So, we named the isolate as Methylomonas sp. SM4. 8.6 mM methanol was accumulated in the reaction mixture containing 70 mM sodium formate and 40 mM $MgCl_2$ (MDH inhibitor) under atmosphere of methane:air (40:60) mixture for 24 hr at $30^{\circ}C$.

Characterization of Campylobacters Newly Isolated from Swine Gastric Mucosa

  • Campylobacter is a pathogen for both humans and animals that can be transferred from animals to humans. Four isolates, which grew under 5-10% $CO_2$ and had small and translucent colonies, were obtained from swine gastric mucosa and characterized using various methods. These bacteria were gram negative, spirally shaped with round ends. One or two non-sheathed polar flagella were observed under electron microscopy. A PCR with species-specific protein (SSP) primers for 16S rRNA gene in Campylobacter produced a typical 462 bp fragment. The isolates had various biochemical and molecular characteristics which differentiated them from other Campylobacters. The isolates were catalase and oxidase positive, urease (rapid) negative, nitrate reduction positive, indoxyl acetate hydrolysis positive, y-glutamyl transpeptidase negative, and alkaline phosphatase negative. All four isolates showed growth at $37^{\circ}C{\;}and{\;}42^{\circ}C{\;}but{\;}not{\;}at{\;}25^{\circ}C$, were resistant to cephalotin and cefoperazone, and susceptible to carbenicillin. The isolates showed various results in the reduction of chloride to triphenyl tetrazolium (TTC) and a susceptibility to nalidixic acid. Western blot analysis of these isolates with antiserum raised against one isolate showed different patterns from those of reference strains. A dendrogram drawn with the RAPD results showed that these isolates belonged to a new Campylobacter spp. group different from those of C. jejuni, C. doylei, C. lari, and C. coli.

A comparative study of the prevalence of Helicobacter pylori in the oral biofilms of a group of dental and non-dental undergraduates from Sri Lanka

  • Dental health care workers (DHCW) are at a risk of occupational exposure to Helicobacter pylori from the aerosolized oral biofilms and saliva of patients. We designed this study to investigate the prevalence of H. pylori in the oral biofilms of a group of dental and non-dental undergraduates from Sri Lanka. After obtaining informed consent, oral biofilms were collected from 38 dental undergraduates (19 males and 19 females) undergoing clinical training and 33 non-dental undergraduates (14 males and 19 females). The participants were in the age range of 22-27 years and had healthy periodontium. Total DNA from the oral biofilms were extracted, and H. pylori DNA was detected using polymerase chain reaction (PCR) amplification of 16S rRNA gene of H. pylori using JW22-JW23 primers, and the results were confirmed using PCR amplification of H. pylori-urease specific HPU1-HPU2 primers. Out of 71 participants, 11 (28.95%) dental and 3 (9.09%) non-dental undergraduates had H. pylori in their oral biofilms indicating an overall prevalence rate of 19.72% (14/71). Thus, the prevalence of H. pylori in oral biofilms was significantly higher in dental undergraduates than in non-dental undergraduates (p < 0.05). An odds ratio of 4.07 indicated that dental undergraduates were four times more likely to harbor H. pylori in their oral biofilms than non-dental undergraduates. Foregoing data support the fact that there may be greater occupational risk of exposure to H. pylori for dental undergraduates during clinical training than that for non-dental undergraduates, warranting meticulous infection control practices during clinical dentistry.
