Effects of Krill (Euphausia superba) on Free Fatty Acid and Electrolyte Concentrations in Rats (해양생물 크릴(Euphausia superba)이 흰쥐의 혈청 유리지방산 및 전해질 농도에 미치는 영향)

  • The purpose of this study was to investigate the change of hematology and serum chemistry values on Sprague-Dawley rats, while used krill (Euphausia superba) meal diet for 5 weeks. Seven-week-old male rats were divided into four groups (n=6) and fed experimental diets containing three different krill meal contents and control group; 10.0% krill meal (G10), 20.0% krill meal (G20), 30.0% krill meal (G30), and control group (GC). Concentrations of non-esterified fatty acid (NEFA), blood urea nitrogen (BUN) in serum were significantly lower in the G10, G20, G30 than GC group (p<0.05). Statistically significant differences, at the confidence level of 95%, for the creatinine, uric acid, electrolyte (T-Ca) parameters in the sera were observed in G20 group, G30 group. The concentration of electrolyte (Pi) in serum was no significant difference among the groups (p<0.05). The results indicate that a krill meal diet was effectively reduce the NEFA.

Therapeutic Efficacy and Complications of Automated Peritoneal Dialyzer in Dogs with Renal Failure (신부전 개에서 자동 복막투석기를 이용한 복막투석에 대한 평가)

  • Peritoneal dialysis (PD) is a treatment for renal failure and acute poisoning, and uses the patient's peritoneum in the abdomen as a membrane across which fluids and dissolved substances are exchanged from the blood. In this study, we evaluated the therapeutic efficacy and complications of automated peritoneal dialyzer (APD) in dogs with renal failure. PD was performed in 10 dogs using a swan neck catheter (Neonatal, Coviden) and automatic APD. The efficacy for each dog was assessed by calculating urea reduction ratio (URR) and creatinine reduction ratio (CRR). Mean concentrations of pre-dialysis creatinine and blood urea (BUN) were $7.09{\pm}3.84$ and $145.8{\pm}48.5$, respectively. The mean number of peritoneal dialysis cycles applied was $6{\pm}1$ cycles. Peritoneal dialysis resulted in a significant decrease in BUN concentration in 7/10 dogs, while a significant decrease in creatinine concentration in 9/10 dogs. The mean of URR was higher than that of CRR ($0.39{\pm}0.16$ vs $0.38{\pm}0.13$). The mean CRR and URR per dialysis cycles were $0.064{\pm}0.023$ and $0.065{\pm}0.023$, respectively. Complications found in this study were catheter occlusion, subcutaneous dialysate leakage, septic peritonitis, hypoalbuminemia and overhydration. This study found PD using a swan neck catheter and APD machine showed acceptable efficacy for successful peritoneal dialysis in dogs. However, close monitoring is required to minimize the risk of complication.

Therapeutic Plasma Exchange in Pediatric Kidney Disease: 23-year Experience at the Severance Children's Hospital in Korea

  • Purpose: The American Society for Apheresis provides clinical guidelines for therapeutic apheresis in adults, but there are no guidelines for children. This study aimed to analyze the effect of therapeutic plasma exchange (TPE) in pediatric patients with various kidney diseases in Korea. Methods: We retrospectively reviewed the data of 16 children (up to 18 years of age) who were admitted to Severance Children's Hospital with refractory kidney disease. All patients received TPE between 1994 and 2016. Clinical and laboratory characteristics such as age, weight, sex, change in blood urea nitrogen (BUN), and creatinine level before and after TPE, and complications after TPE were analyzed. Results: The mean age and weight of the 16 patients at the time of TPE was $11.3{\pm}4.0$ years and $34.6{\pm}17.5$ kg, respectively. The BUN level was 35.4 mg/dL before TPE and significantly decreased to 21.5 mg/dL (P=0.025) at 1 week and 20.5 mg/dL (P= 0.01) at 1 month after TPE. The creatinine level significantly decreased from 1.20 mg/dL before TPE to 0.90 mg/dL (P=0.02) at 1 week after TPE. Four complications (hypovolemia, anemia, hypocalcemia, and thrombocytopenia) were reported, but were not fatal. Conclusion: Our findings suggest that TPE is an effective therapeutic modality in children with refractory kidney disease and can be indicated for the treatment of various kidney diseases.

Analysis of Clinical Biochemical Components in Sera of Tsutsugamushi Disease Patients

  • The factors and mechanisms by infection of Oriental Tsutsugamushi caused disease are not well understood. The onset of tsutsugamushi disease is characterized by chilliness, fever, malaise, headache and generalized aching. Infection of tsutsugamushi is the cause of impairment of function of a major organ often complicate the picture and immediately change the prognosis for the worse. Tsutsugamushi disease is reported that this disease is characterized by the histopathogenesis of liver, kidney, heart, and lung, but the variation of biochemical components in serum of tsutsugamushi disease patient are not clear. We analyzed total protein (TP), albumin (AL), aspartic aminotranferase (AST), alanine aminotransferase (ALT), alkaline phosphotase (ALP), urea nitrogen (UN), creatinine (CRE), glucose (GLD), cholesterol (CHOL) and total bilirubin (TB) in sera of patients with tsutsugamushi disease. In comparison with reference, total protein and albumin were abnormally decreased in 19.6% and 39.2% of patients, respectively. AST, ALT, ALP, creatinine, UN, glucose, cholesterol and total bilirubin were abnormally increased in 94.1 %, 72.5%, 25.5%, 15.7%, 9.8%, 62.7%, 25.5% and 6.0% of patients, respectively. The patients showed abnormal relative rate of protein electrophoretic fractions to total protein in serum compared to them of reference were 43.1% (albumin), 12.9% ($\alpha_1$-globulin), 58.8% ($\alpha_2$-globulin), 60.8% ($\beta$-globulin) and 70.6% ($\gamma$-globulin), respectively. These data suggest that infection of Oriental Tsutsugamushi causes impairment of function of a major organ and abnormal serum protein electrophoresis fractions to tsutsugamushi patients.

Correlation Analysis of Serum Hormones and Components in Hanwoo during Slaughtering Process (한우 도축시 혈청 호르몬과 혈액 성분의 상관 관계 분석)

  • Concentration of hormones and blood components at the last fatting stage was changed before slaughter in Hanwoo steers and bulls. Two months before slaughter and shipment, concentration of cortisol and creatinine was increased, but that of calcium was decreased. Concentration of insulin growth factor-1 (IGF-1) was decreased after shipment, and inorganic phosphorus (IP) was decreased at slaughter. It is unclear that changes of concentration in between 2 months before slaughter and shipment were either caused by aging or stresses (abstinence, environmental change, blood drawing, and shipment). Changes of blood concentration between shipment and slaughter may be accounted for overall responses from abstinence, shipment, and unfamiliar environment. A positive correlation between 2 months before slaughter and before shipment was detected for IGF-1, total protein (TP), albumin, creatinine, high density lipoprotein cholesterol (HDLC), and globulin in steers, and creatinine and globulin in bulls. A positive correlation between 2 month before slaughter and slaughter was detected for IGF-1, blood urea nitrogen (BUN), IP and HDLC in steers, and creatinine in bulls. A positive correlation between before shipment and slaughter was detected for testosterone, IGF-1, creatinine, triglyceride, HDLC and globulin in steers, and TP, creatinine, HDLC and globulin in bulls.

Influences of Wild Haw (Crataegus pinnatifida BUNGE) on Lowering BUN and Creatinine Concentrations in Dyslipidemia (야생 산사(Crataegus pinnatifida BUNGE)가 이상지질혈증의 BUN 및 Creatinine 농도 저하에 미치는 영향)

  • This study was investigate the influences of wild haw (Crataegus pinnatifida $B_{UNGE}$) on lowering blood urea nitrogen (BUN) and creatinine concentrations in dyslipidemic rats. Four groups of rats were fed different diets for 5 weeks: basal diet (BD group), nondyslipidemic diet+feral haw extract (NDF group), dyslipidemic diet (DLD group), dyslipidemic diet+feral haw extract (DFH group). BUN, creatinine and uric acid concentrations were significantly higher in the DLD group than in the BD group. However, DFH group significantly lowered BUN, creatinine and uric acid concentrations compared to the DLD group. Amylase activity was significantly lower in the DLD group than in the BD group, while DFH group significantly higher compared to the DLD group. Lipase activity was also also significantly lower in the DLD group than the other group. Accordingly, these results suggest that haw extract could be effective for improving kidney function in dyslipidemic rats.

  • The purpose of this study was to investigate the improvement effect of turmeric (Curcuma longa L.) on the hepatic functional enzyme and catalase activity of streptozotocin (STZ)-induced diabetic rats. Sprague-Dawley (SD) male rats were divided into four groups (n=6), and fed experimental diets containing turmeric meal [basal diet+5% turmeric (BT), basal diet+STZ+5% turmeric (ST)], and control (basal diet, BD), BS groups (basal diet+STZ). Serum concentrations of creatinine and blood urea nitrogen (BUN) were significantly decreased (p<0.05) by 5% turmeric supplementation diet. The activities of akaline phosphatase (ALP), lactate dehydrogenase (LDH), aspartate transaminase (AST), alanine transaminase (ALT), amylase and lipase were decreased in the BD, BT and ST group than BS group. The catalase (CAT) activity was significantly increased (p<0.05) in turmeric supplementation diet (BT, ST group) than diabetic group (BS). Furthermore, the activities of amylase and lipase in the sera of turmeric diet group were significantly decreased (p<0.05). In vivo experiments with diabetic rats showed that ingestion of turmeric supplementation diet were effective in creatinine concentration, and hepatic functional enzyme activities.

Deleterious Effects of Hyperoxemic Extracorporeal Circulation during Cardiovascular Surgery

  • Although extracorporeal circulation (ECC) has been routinely used for cardiovascular surgery, hyperoxemia during ECC may produce oxygen toxicity and cellular injury. This study was performed to investigate the clinical influences of hyperoxemic ECC during cardiovascular operation. 40 adult patients scheduled for elective cardiovascular surgery were classified into normoxemic (arterial oxygen tension: 115 mmHg, n=20) and hyperoxemic (arterial oxygen tension: 380 mmHg, n=20) ECC. At preoperative and postoperative period, total leukocyte and neutrophil counts, platelet counts, iron, glucose, aspartate aminotransferase (AST), alanine aminotransferase (ALT), blood urea nitrogen (BUN), and creatinine in peripheral arterial blood, malondialdehyde (MDA) and troponin-T concentration (TnT) in coronary sinus blood, pulmonary vascular resistance (PVR), and postoperative blood loss volume (BLS) were measured and compared between groups. Hyperoxemic group had postoperatively higher total leukocyte and neutrophil counts, MDA, TnT, PVR total BLS, iron, glucose, AST, ALT, BUN, and creatinine than normoxemic group (p<0.05).0 conclusion, hyperoxemic ECC results in greater inflammatory response and oxidative damaging effects on the heart lung, liver and kidney, probably being adverse to postoperative patient recovery. For reducing these deleterious effects and improving postoperative outcomes, management lowering oxygen tension during ECC is recommended.

The Effects of Hyeongbangjihwang-tang and Sibimikwanjung-tang in Glycerol-Induced Renal Failure in Rats (형방지황탕(荊防地黃湯)과 십이미관중탕(十二味寬中湯)이 Glycerol로 유발된 흰쥐의 급성신부전에 미치는 영향)

Objective : The present author have examined the comparative effects of Hyeongbangjihwang-tang and Sibimikwanjung-tang in glycerol-induced acute renal failure, a model for myoglobinuric acute renal failure. 2. Methods : Above two prescriptions were applied to rat ad libitum and serum level of creatinine, glucose and blood urea nitrogen(BUN) were measured. And serum level of $Na^+$, $K^+$ and $Cl^-$, too. 50%(vol/vol) glycerol at a dose of 10 ml/kg of rat body weight intramuscularly and 1% concentration of prescriptions were applied to rats for 7 days ad libitum. 3. Results : Serum level of creatinine and $K^+$ significantly increased by glycerol injection, and these showed pre-renal and renal cause of acute renal failure. Hyeongbangjihwang-tang administered group showed decreasing tendency on serum level of creatinine when compared with control groups, but not significant. Sibimikwanjung-tang administered group showed significant increase of serum level of BUN when compared with control groups. 4. Conclusions : Above two prescriptions showed no significant changes on glycerol induced acute renal failure, but Hyeongbangjihwang-tang and Sibimikwanjung-tang showed a little different manner of response.

Effect of Renal Failure on Pharmacokinetics of Carbamazepine in Rabbits (카르바마제핀의 체내동태에 대한 신장해의 영향)

  • The pharmacokinetics of carbamazepine(100 mg/kg, oral) in the folic acid-induced renal failure rabbits was studied. Renal failure was induced by the i.v. injection of folic acid (50, 100, and 200 mg/kg). At folic acid dose of 100 and 200 mg/kg, the serum creatinine concentration (Scr) and blood urea nitrogen (BUN) increased significantly compared with control rabbits. Plasma concentrations and area under the plasma level-time curve (AUC) of carbamazepine increased significantly at folic acid dose of 100 and 200 mg/kg. The elimination rate constant (Kel) of carbamazepine decreased significantly, and half-life $(t_{1/2})$ of carbamazepine increased significantly at folic acid dose of 100 and 200 mg/kg. The serum creatinine concentration (Scr) correlated well with AUC and elimination rate constant (Kel) of carbamazepine, as well as BUN with AUC and elimination rate constant (Kel) of carbamazepine. These results suggest that adjustment of the dosage regimen of carbamazepine is desirable, and serum creatinine concentration (Scr) as well as BUN can be used for adjusting the dosage regimen of carbamazepine in renal failure.
