임신 중 교통사고로 한방 치료를 받은 입원 환자에 대한 후향적 분석 (A Retrospective Analysis of Inpatients who Received Traditional Korean Medicine Treatment after a Traffic Accident during Pregnancy)

  • Objectives: This study aimed to report the effectiveness and outcome on the prognosis of pregnancy of traditional Korean medicine treatment for inpatients who had a traffic accident during pregnancy. Methods: The study is conducted by analyzing the medical records of hospitalized patients in a Korean medicine hospital. Seventy-six pregnant patients who got into car accidents from March 2015 to February 2020 were included. We performed statistical analysis by using Statistical Package for the Social Sciences(SPSS) ver. 25.0. To verify the effect of traditional Korean medicine treatment for traffic accidents during pregnancy, we analyzed improvement of symptoms. Also, we analyzed the prognosis of pregnancy and signs of premature birth or miscarriage during treatment to verify the applicability of traditional Korean medicine treatment for traffic accidents during pregnancy. Results: Among the symptoms of pain, there was a statistically significant decrease in lower back pain, post neck pain, headache, pelvic pain, knee pain, lower limb pain, wrist pain, and upper limb pain. Other systemic symptoms showed statistically significant improvement in nausea, abdominal pain, dizziness, and lower limb numbness. All confirmed cases reported normal pregnancy maintenance and delivery, and no miscarriage was reported. Two cases that showed signs of premature birth or miscarriage during treatment were unrelated or unclear with traditional Korean medicine treatment. Conclusions: Traditional Korean medicine treatment is effective and applicable for pregnant women injured by traffic accidents. Further studies are needed to identify the effectiveness and safety of traditional Korean medicine treatment for the sequelae of traffic accidents during pregnancy.

퇴행성 추간판 질환의 최신 지견 -종판의 중요성- (Current Concepts of Degenerative Disc Disease -A Significance of Endplate-)

  • 퇴행성 추간판 질환은 전통적으로 수핵과 섬유륜의 변화에 따른 요통으로 생각되어 왔으나, 최근 연구들에서 추간판을 이루고 있는 상·하 종판의 변화가 추간판의 퇴행을 유발하고, 그 결과로 기계적 압박과 염증 반응에 의해 요통이 유발된다고 한다. 또한 최근에는 추체 골수-종판-수핵과 섬유륜을 하나의 단위로 생각하고, 그 연관성을 설명하고 있다. 종판이 손상되면 결국 추체, 수핵과 섬유륜의 퇴행성 변화를 가중시킨다. 이 과정에서 섬유륜에 가해지는 압박력이 증가하고, 염증성 매개 물질들에 의한 염증 반응이 일어나서, 동척추 신경(sinuvertebral nerve)과 기저추 신경(basivertebral nerve)이 자극을 받으면서 요통을 발생시킨다. 이런 변화가 만성화되면, 추체 내 골수에도 Modic 변화 같은 퇴행성 변화가 발생하게 된다. 결국, 퇴행성 추간판 질환은 추체 골수-종판-수핵과 섬유륜을 하나의 단위로 연관지어서 생각해야 할 필요가 있다. 그러므로 만성 요통 환자 진료 시에는 수핵과 섬유륜의 변화 소견뿐 아니라, 종판의 병변에 관심을 가질 필요가 있다고 생각된다.

  • By studing the congestion theory of Choo Dan-kyei mentioned in "DongEuiBoGam", I have reached following conclusions. 1. Dan-kyei's congestion theory influenced greatly on "DongEuiBoGam". The congestion theory of "DongEuiBoGam" is built up with several theories centering round on Dan-kyei's theory and Huh-Jun's own opinion in addition. 2. Dan-kyei's congestion theory has given theoretical foundation to the classification and diagnosis of congestion reported in "DongEuiBoGam"-Congestion Chapter. 3. Dan-kyei's congestion theory offers many examples of symptoms reported in "DongEuiBoGam"-Congestion Chapter. It's shown well through outer symptoms, several congestion disease, congestion mass, and "congestion never sudden death", etc. 4. Dan-kyei's congestion theory has influenced greatly on treatment of congestion reported in "DongEuiBoGam"-Congestion Chapter, and it's well shown through spewing method, grand method in congestion treatment and common medicine of congestion, etc. 5. Dan-kyei's congestion theory offers variety of treating methods to symptoms caused by congestion reported in "DongEuiBoGam". It's shown variously through upper stomachache, stomachache, back pain, side pain, "San", joint pain, Summer disease, internal injury, cough and morning sickness, etc.. 6. Dan-kyei's congestion theory has factors that could be operated to common diseases of mordern people. They can be classified as food congestion, alcohol congestion, obstruction congestion and Qi congestion, and they are clinically very efficient.

외상성 경추 골절 후 발생한 Brown-Sequard 증후군 환자 1례 (A Case Report of Brown-Sequard Syndrome Caused by Traumatic Cervical Fracture)

  • Objectives : The purpose of this study is to report a case of Brown-Sequard syndrome caused by traumatic cervical fracture, presenting pain in the right upper extremity and back of the left hand, motor weakness in the right side and diminished pain and temperature in the left side. Methods : A patient received Korean medical treatment(acupuncture, electroacupuncture, herbal medicine, cupping, moxibustion, Silver Spike point electrotherapy(SSP)) and rehabilitation treatment. We evaluated pain with the Numeric Rating Scale(NRS), motor grade with a Medical Research Council(MRC) scale, sensory function and Modified Barthel Index(MBI). Results : After treatment, the patient showed considerable improvement in NRS, motor & sensory function, and MBI. Conclusions : Korean medical and rehabilitation treatments could be effective for Brown-Sequard syndrome patients. More extensive studies should be carried out.

2,30대 정상 성인남녀의 몸통 굽힘 시 골반의 기울임과 몸통 움직임 및 몸통 폄 근의 상관관계 연구 (Correlation between Pelvic Tilt Angle with Trunk Motion and Trunk Extensor during Trunk Forward Flexion in Adults Aged 2,30)

  • Purpose : The purpose of this study was to examine if there is any correlation between pelvic tilt angle and trunk motion and trunk extensor during trunk forward flexion and to measure trunk motion, onset time of trunk motion, and onset time of trunk extensor activation. Methods : The subjects of this study were 42 healthy adults. The subjects had no back pain due to neurological disease and no experience of back surgery. After pelvic tilt angle was measured, each trunk forward flexion was performed three times. Trunk motion and onset time of trunk motion were measured using Myomotion. Four sensors were used, with one located at the upper thoracic (below $C_7$), the lower thoracic ($T_{12}-L_1$), the sacrum ($S_1$), and at the center of the anterior femur. Onset time of trunk extensors (spinalis, longissimus, gluteus medius, gluteus maximus, biceps femoris, and gastrocnemius) activation was measured using a wireless surface EMG. The EMG amplitude was normalized by using the reference voluntary contraction (RVC). The statistical significance of the results were evaluated using Pearson's correlation test. Results : The correlation between pelvic tilt angle and lumbar motion, onset time of pelvis motion, and onset time of gluteus medius activation was statistically significant in a positive direction (p<.05). The correlation between pelvic tilt angle with pelvis motion, onset time of lumbar motion, and onset time of longissimus activation showed a statistically significant negative correlation (p<.05). Conclusion : The study results provide a significant contribution to our understanding of the lumbar load at the initial stage of trunk flexion. Therefore, it may be possible to provide basic data for evaluation and treatment, such as orthodontic treatment for alignment of the spine and back pain. In addition, it is necessary to focus on normal exercise pattern reeducation as well as pelvic correction during exercise in daily life or in industrial fields.

여대생의 다리길이 차이와 생활습관 자세 및 통증 (Leg Length Inequality, Habitual Posture, and Pain in Women's College Students)

  • Purpose: The purpose of this study was to investigate the leg length Inequality, habitual posture, and pain in women's college students. Methods: The subjects were 281 students, in 8 women's college in Korea. The tapelines were used for measuring leg length Inequality and questionnaires were used for measuring habitual posture, and pain. The data were collected between August and October 2010 and analyzed using SPSSWIN 11.5. length Inequality(<1 cm). The subjects of 2.8% were the length differences of above 2cm. The worst habitual posture were leaning habits. Especially, the habits of crossing her legs were significantly different to leg length Inequality(F=3.342, $p$=.037). The subjects of 84% were felt a severe pain on the upper body such as waist, back, scapula, neck or shoulder. But there is no difference between pain and leg length Inequality. Habitual posture are related to pain(r=.212, $p$=.001). Conclusion: To protect the severe health problem of musculoskeletal system, this study results will be give aid to health education in women's college students.

Kyphotic cervical curvature로 인한 항통(項痛) 4례(例)에 대한 임상적(臨床的) 고찰(考察) (Clinical studies on neck pain 4 cases associated with kyphotic cervical curvature)

  • Kyphotic cervical curvature, straghtening is commonly caused by trauma, muscle spasm without trauma and wrong posture, etc. Objective : This study is performed to evaluate the clinical effect of neck pain associated with Kyphotic cervical curvatre on cervical x-ray lateral view. Methods : One of the many causes, We examined the patients with neck pain & upper back pain who visited to Department of Acupuncture & Moxibustion, Gumi Oriental Hospital of Kyung-San University from 16th June 1999 to 22th June 2000. Pre and post treatment, We evaluated the cervical angle, Jochumsen's method, VAS(visual analogue scale) and effective score of treatment. Results & Conclusion : 1. Kyphotic cervical curvature is mainly caused by wrong posture during long term, sudden trauma, etc. therfore, postcervical muscles and tendon are injuryed by strong stress. So, muscle imbalance and pain is occured. 2. On these cases, The improvement index for pre/post treatment showed 28/42, 10/15, 9/30, 28/42 degree in cervical angle. Jochumsen's method showed -1/+2, -9/-3, -5/-2, -1/+2mm. Afer treatment VAS is 2, 1, 1, 1 and effective score of treatment is above good. The results suggest that treatments of Oriental Medicine(Acupuncture & Moxibustion, Chu-Na, Cupping and Physical therapy) are effective methods for neck pain with kyphotic cervical curvature on cervical x-ray lateral view.

Test-retest Reliability and Concurrent Validity of a Headphone and Necklace Posture Correction System Developed for Office Workers

  • Background: Office workers experience neck or back pain due to poor posture, such as flexed head and forward head posture, during long-term sedentary work. Posture correction is used to reduce pain caused by poor posture and ensures proper alignment of the body. Several assistive devices have been developed to assist in maintaining an ideal posture; however, there are limitations in practical use due to vast size, unproven long-term effects or inconsistency of maintaining posture alignment. We developed a headphone and necklace posture correction system (HANPCS) for posture correction using an inertial measurement unit (IMU) sensor that provides visual or auditory feedback. Objects: To demonstrate the test-retest reliability and concurrent validity of neck and upper trunk flexion measurements using a HANPCS, compared with a three-dimensional motion analysis system (3DMAS). Methods: Twenty-nine participants were included in this study. The HANPCS was applied to each participant. The angle for each action was measured simultaneously using the HANPCS and 3DMAS. The data were analyzed using the intraclass correlation coefficient (ICC) = [3,3] with 95% confidence intervals (CIs). Results: The angular measurements of the HANPCS for neck and upper trunk flexions showed high intra- (ICC = 0.954-0.971) and inter-day (ICC = 0.865-0.937) values, standard error of measurement (SEM) values (1.05°-2.04°), and minimal detectable change (MDC) values (2.92°-5.65°). Also, the angular measurements between the HANPCS and 3DMAS had excellent ICC values (> 0.90) for all sessions, which indicates high concurrent validity. Conclusion: Our study demonstrates that the HANPCS is as accurate in measuring angle as the gold standard, 3DMAS. Therefore, the HANPCS is reliable and valid because of its angular measurement reliability and validity.

스마트폰 사용이 정상인의 시력, 순발력, 균형능력, 통증에 미치는 영향 (Effects on Sight, Quickness, Balance, Pain of Normal Human Body with Using Portable Mobile Phone)

  • Background: The purpose of this study was to find out influence of using portable mobile devices in human body. Method: The subjects were consisted of the normal persons (19 males and 20 females). These subjects were consisted of three groups according to test time. The A group (12 persons - 6 males, 6 females) tests 5 minutes, the B group (14 persons - 7 males, 7 females) tests 10 minutes, and the C group (13 persons - 6 males, 7 females) tests 15 minutes. Before test, sight, quickness and balance were measured, after test revision, quickness, balance were measured and checked pain scale. Results: The results were as follows. 1. Sight, quickness and balance were decreased and pains were increased after using portable mobile devices than before using these. 2. According to duration of using the portable mobile devices, sight, quickness and balance was not a statistically significant difference, the entire upper body pain was a statistically significant difference. When viewed as part of the pain, shoulder, lower back, and wrist pain were a statistically significant difference, but the neck and elbow were not a statistically significant difference. 3. The influence of vision according to distance of using Portable mobile devices was no statistically significant difference. In addition, less than 20cm distance using equipment group ("Ga") and the group ("Na") which exceeded 20cm in between the sight were not statistically significant difference, Influence of pain according to distance of using Portable mobile devices we-re no statistically significant differences. Conclusion: Using portable mobile devices affect human body on the elements of the biomechanics.

Clinical Features and Treatments of Upper Lumbar Disc Herniations

  • Objective : Disc herniations at the L1-L2 and L2-L3 levels are different from those at lower levels of the lumbar spine with regard to clinical characteristics and surgical outcome. Spinal canals are narrower than those of lower levels, which may compromise multiple spinal nerve roots or conus medullaris. The aim of this study was to evaluate the clinical features and surgical outcomes of upper lumbar disc herniations. Methods : We retrospectively reviewed the clinical features of 41 patients who had undergone surgery for single disc herniations at the L1-L2 and L2-3 levels from 1998 to 2007. The affected levels were L1-L2 in 14 patients and L2-L3 in 27 patients. Presenting symptoms and signs, patient characteristics, radiologic findings, operative methods, and surgical outcomes were investigated. Results : The mean age of patients with upper lumbar disc was 55.5 years (ranged 31 to 78). The mean follow-up period was 16.6 months. Most patients complained of back and buttock pain (38 patients, 92%), and radiating pain in areas such as the anterior or anterolateral aspect of the thigh (32 patients, 78%). Weakness of lower extremities was observed in 16 patients (39%) and sensory disturbance was presented in 19 patients (46%). Only 6 patients (14%) had undergone previous lumbar disc surgery. Discectomy was performed using three methods : unilateral laminectomy in 27 cases, bilateral laminectomy in 3 cases, and the transdural approach in 11 cases, which were performed through total laminectomy in 10 cases and unilateral laminectomy in 1 case. With regard to surgical outcomes, preoperative symptoms improved significantly in 33 patients (80.5%), partially in 7 patients (17%), and were aggravated in 1 patient (2.5%). Conclusion : Clinical features of disc herniations at the L1-L2 and L2-L3 levels were variable, and localized sensory change or pain was rarely demonstrated. In most cases, the discectomy was performed successfully by conventional posterior laminectomy. On the other hand, in large central broad based disc herniation, when the neural elements are severely compromised, the posterior transdural approach could be an alternative.
