의료보장정책의 형성과 문제점 (Policy Formulation of Health Insurance and Its Problems in Korea)

  • Korea's social health insurance system was introduced in 1977, which has made a universal coverage possibly by July 1989. Korean government had pursued a single objective for the last decade to put the whole population under the coverage of medical security, and the objective was achieved within 12 years. The rapid accomplishment is primarily due to such factors as limited benefits, high copayment rate, low contributions as well as rapid economic growth. There are several sources of pressure for the implementation of social health insurance such as health professional group, labor unions, politicians, international organizations etc.. However it is important to look at the feasibility of social health insurance. Among other things, it is necessary to identify the administrative infrastructure of insurance system and to assess income for source of fund. As many developed countries, Korea began to apply health insurance to the employees of the large firms, and the expansion based on employment status. Thus the several funds system was inevitable according to the gradual expansion strategy. However many persons had criticized several funds system in respect with equity and efficiency aspects. In the short history of the Korean health insurance, whether one fund or sever or funds had been the most controversial issue. In Febrary 1999, the National Assembly passed the act of one fund system. From July 2000 separate funds will be unifed under new health insurance scheme. In this study we will analyze the policy making process on implementation, expansion and integration of health insurance system of Korea. And also analyse problems related to policy making.

노인장기요양보험제도에서의 가족수발자의 경험 (The Experiences of Family Caregivers under the Long-term Care Insurance)

  • Purpose: The purpose of this study was to explore the experiences of family caregivers who care for the elderly under Long-term Care Insurance. Methods: Data were collected using focus group interviews and analyzed using a phenomenological approach. The four focus groups consisted of eight caregivers, two social workers and three nurses in B city, Korea. Results: Five themes were identified: 'Obtaining a care-helper certification for employment', 'Taking care of the elderly in their homes', 'Difficulties due to life changes', 'Difficulties due to reduced wages' and 'Dissatisfaction with the Long-term Care Insurance operating system'. Conclusion: The results of this study demonstrate that the long-term care system for family caregivers faces many systematic challenges in providing care for the elderly harmoniously in their home. To help them succeed in their tasks, Long-term Care Insurance system must offer respite and support programs to family caregivers.

서비스업 여성 근로자의 근무조건에 따른 직무 스트레스 (Occupational Stress of Women Workers in Service Industries depending on Their Working Conditions)

  • Purpose: The purpose of this study is to discover the influence of the working conditions on women workers' occupational stress. Methods: Descriptive method is used to identify women's stress depending on their general working features and conditions by conducting a survey of them in women-dominated service industries. SPSS 18.0 program is used for data analysis and descriptive statistics is presented with standard deviation, frequency and percentage. ${\chi}^2$-test is used as an analysis tool. To figure out factors that influence their stress, logistic regression analysis is used for general features and working conditions as independent variables, and occupational stress as a dependent variable. Results: As a result, among the independent variables, employment status, weekly working hours, career, shiftwork, and work-family-balance are meaningful factors that influence their stress. Temporary workers' stress is 3.65 times higher (p<.001), and workers working over 48 hours a week have 1.97 times higher stress (p<.003). Workers with over 5 years' career are under 1.73 times higher stress (p<.046) and shift workers are under 3.51 times higher stress (p<.001). Work family balance results in 1.93 times higher stress (p<.009). Conclusion: It is necessary to seek how to prevent and manage women workers' stress considering features.

미국 편모가계의 자녀보육비 지출에 영향을 미치는 요인 (Factors that Influence on Child Care Expenses of Single Mother Families in the U. S.)

  • Using the 1999 National Survey of America's Families(NSAF), this study investigated the factors that affect child care expenses and examined financial and social support of single mother families in the U. S. In this study, 4,676 single mother families with children aged twelve and under are included for the analytical sample. About half of all single mother families with children under age 12 had some amount of child care expenses in 1999. Monthly child care expenses for those who had child care expenses was an average of $255 and it accounted for about 15 percent of their family earnings. The profile of financial and social support showed that about one-quarter of single mothers received financial support for child care, one-fifth received free child care by relatives and about 40 percent received child support payments from nonresident fathers. Tobit analysis results showed that the significant factors that affected child care expenses were mother's age, marital status, educational level, employment status, the numbers of children aged 0-5, family income, the receipt of public assistance and financial support for child care.

  • The SPES (Selective Production of Exotic Species) facility, currently under development at Legnaro National Laboratories of INFN, aims at the production of intense RIB (Radioactive Ion Beams) employing the Isotope Separation On-Line (ISOL) technique for interdisciplinary research. The radioactive isotopes of interest are produced by the interaction of a multi-foil uranium carbide target with a 40 MeV 200 μA proton beam generated by a cyclotron proton driver. The Target Ion Source (TIS) is the core of the SPES project, here the radioactive nuclei, mainly neutron-rich isotopes, are stopped, extracted, ionized, separated, accelerated and delivered to specific experimental areas. Due to efficiency reasons, the TIS unit needs to be replaced periodically during operation. In this highly radioactive environment, the employment of autonomous systems allows the manipulation, transport, and storage of the TIS unit without the need for human intervention. A dedicated remote handling infrastructure is therefore under development to fulfill the functional and safety requirement of the project. This contribution describes the layout of the SPES target area, where all the remote handling systems operate to grant the smooth operation of the facility avoiding personnel exposure to a high dose rate or contamination issues.

우리나라 어선원 수급 전망과 대응방안 (Plan for Countermeasure and Prospect of Demand and Supply about Seamen in Korea)

  • A study on the seamen's demand - supply has been focused on early 1990s, and never studied yet since 2007. Because previous studies related to the seamen are very different in conditions the environments around the fishery recently. It is serious problem that the number of domestic semen are decreasing continually. To depend on foreign seamen because of lacking of domestic seamen should be linked closely with the fishing industry environment in our country. Therefore, this study is limited to domestic seamen. Lacking recent domestic seamen, it is the most likely to rise imbalance between demand - supply in the future. Through medium and long term its analysis based on data, we are willing to discover various political subjects for seamen's welfare, the training of manpower, education, etc. To solve these issues, it can be made institutional changes as follows ; First, it is necessary policy-making related the deteriorating employment situation based on its analysis of medium and long term for seamen. Second, it is necessary to overcome a dual system for its management for seamen. Third, it is necessary to improve problems of statistics data for fishery and build a statistical system for seamen. Also, it will be improved a insurance system under 5 ton and strengthen the function for Korean seamen welfare and employment center and must be urgently countermeasures for the minimal unemployment rate through a content development and management of home page. Finally, it will be supplied reliably seamen to improve a practice system for apprentices according to the international regulations(STCW-F) for fishery students.

주부의 취업여부에 따른 가족구성원의 영양상태 비교: 2001 국민건강영양조사 자료 분석 (A Comparative Study on the Nutritional Status of the Families with Working Housewives and with Non-working Ones: Analysis of 2001 Korea National Health and Nutrition Survey Data)

  • In order to investigate if the employment of housewives may affect the nutritional status of their family members, an analysis was made for the data of 2001 Korea National Health and Nutrition survey. Housewives aged 20 or over were divided into two groups of the working (W, 44.3%) and the non-working (NW, 55.7%), and household income levels were divided into 4 groups of low, middle, high, and high above according to the minimum cost of living in the year of 2001. Nutrient intakes were assessed by using dietary recommended intakes for Koreans of 2005. Working housewives showed similar levels to those of non-working housewives in most nutrients intakes except energy and vitamin C. However their families excluding housewives of W, than those of NW, took less protein, calcium, iron, potassium, vitamin A, thiamin, riboflavin, niacin, and vitamin C when assessed as % of recommended intakes and took more sodium. Such differences were very strong in children and adolescents, and in the middle income households. More % of the families of W than those of NW consumed nutrients below the estimated average requirements. Percents of hypertension classified by both systolic and diastolic blood pressure were higher in adult family members of W than in those of NW. This tendency seemed to be more significant in the family members aged 30 to 49. Both obesity and under-weight rates of school children ($7{\sim}12\;yrs$) in W were higher than those in NW. The above resuIts suggested that employment of housewives could have negative influences on the nutritional status of their family members, especially of their children and in the middle income class.

  • In the case of Korea, both of modern medicine and oriental medicine are admitted as medical practices in the system. In other words, healthcare system is dualized. However, medical practice that corresponds to oriental medicine in Korea is substitution of medical practice in cases of foreign countries. For use of medical devices, it is provided only for doctors and medical technician relevant to use. Particularly, although oriental medicine is recognized as orthodox medicine in terms of the features of Korean medical system, superintendency of oriental doctors is not identical with that of doctors for use of medical devices and superintendency toward medical technicians. Recently, Cheongju District Court decided that superintendency of oriental doctor upon physical therapist is not acknowledged. It can be said that the judgement is opposed to the original verdict which judged that oriental doctors' employment and guidance of oriental doctors upon physical therapist is permissible. Hence this study aimed to review on domestic medical law system, which is dualized, roles of medical professionals, intent of the medical license system, provisions related to medical technician law and relevant precedents. Regulations on practices other than licensed practices by medical professionals are made because medical practices may affect on danger toward life and body of human and public health also. Therefore, the nation regulates medical professionals having licenses to perform medical practices within the range of the licenses. It is clearly prescribed that medical technicians may perform medical practices under instructions of doctors or dentists pursuant to the medical technician law. In addition, the court also judges that it is out of the license of oriental doctors if they use CT devices and limits the use of modern medical devices by oriental doctors. That is to say that it limits oriental doctors' employment of medical technicians and pursuant of oriental doctors on medical technicians as well.

관광종사원의 인턴십 평가가 취업경쟁력에 미치는 영향 (Impact of Internship Evaluation Factors on the Employment Competitiveness)

  • 현재 현장에서 종사하고 있는 정규직 종사자들의 입장에서 인턴십 평가를 통한 인턴십 과정생들이 얻을 수 있는 잠재적 편익을 최대화 할 수 있도록 교육자들에게 기초자료를 제공함은 물론 인턴십 교육방향에 기초자료를 제공하고자 한다. 더 나아가서는 본 연구가 각각의 이해집단에게 인턴십 교육의 운용을 마케팅적 사고로 전환하여 교육 소비자 즉, 학생과 산업체의 의견이 적극적으로 반영된 교육체계로 변신함으로써 급변하는 교육환경속에서 관광산업의 경쟁력을 확보하는데 도움을 줄 수 있기를 기대한다. 이에 본 연구결과를 종합해 보면, 첫째, 인턴십 평가요인으로 규율성, 적극성, 기술력, 업무수행능력, 과제물, 탐구력, 지식성, 협조성, 이해력 등으로 평가요인임을 규명하였다. 둘째, 인턴십 평가요인은 인턴십 교육효과(적극성, 탐구력, 규율성) 및 취업경쟁력(규율성, 적극성, 협조성)에 긍정적 영향력을 미치는 것으로 분석되어 본 연구의 목적을 달성하였다고 볼 수 있다. 마지막으로 산업체에서는 학습능력보다는 조직과 융화할 수 있는 인재를 원하고 있는 것으로 분석되었다.

가계의 세탁기사용방식과 사용정도에 관한 연구 (A Study on the Pattern and Extent of Washer Use in Household)

  • In this study one aspect of consumer behavior in household equipment utilization was investigated the pattern, frequency, rate of washer use and their relation to the following factors a) Washer related factors : extent of the presence of desired characteristics, the evaluation of washer's intrinsic features and related household facilities. b) Psycho-social factors : attitude of energy conservation, preference & ability to wash by hand, standard of washing of the respondent homemaker. c) Socio-demographic factors : age, education level and employment status of homemaker, house-hold income, the presence of children under seven years, size of family, the presence of a paid help. The subjects of this study were 286 homemakers with washer in Seoul. Analysis methods were used to fuequency, one-way ANOVA, Gamma test, Pearson's Correlation Coefficient, t-test and multiple regression of SPSS program. The major findings are the following; 1) The pattern, frequency, rate of washe use appeared various in every household. 2) Extent of the presence of desired characteristics was very low and respondents evaluated their washer's intrinsic features moderate. 3) The pattern of washer use was affected by the evaluation of washer's intrinsic features, preference & ability to wash by hand, wife's employment and household income. The frequency of washer use was affected by family size and preference & ability to wash by hand. The rate of washer use was affected by extent of the presence of desired characteristics, the evaluation of washer's intrinsic features and preference & ability to wash by hand. Therefore, washing by hand is major substitute for washer. If more desired characteristics are added to washer, intrinsic features are improved, and maintenance costs are reduced or household income is raised, every houshold with washer will use washer more than washing by hand in washing ask so that it may gain more utility from washer.
