A Study on Delivery Environments and Immunization practice of the Maternal St Child Health Services in a Rural Area (일부농촌지역의 모자보건사업중 분만상태와 예방접종실시에 관한 조사 연구)

  • This study was conducted to find out status of delivery environments, delivery attendants, and various immunization practices towards 513 babies who were born during the period from January 1, 1972 to December 31, 1974, in Dongnae Area, Shindong Myon, Chunseong Gun, Ganwon Do. The results and findings obtained from the study are summarized as follows : 1. Of all deliveries, 10.7% of the babies were reported born at the hospitals. 2. Deliveries attended by qualified professionals such as, doctors, midwives or public health nurses, were 22.2% of the total deliveries. Those who delivered alone without having anyone's assistants were as much as 9.7%, The percentage of the deliveries attended by the professional groups were increased year by year among the younger group mothers with the babies birth order being first or second place. 3. Sewing scissors were most frequently used as a tool for cutting the umbilical cord indicating 87.5% in this area. Sterilized tools were used among 50.3% of the women. 4. The usage af vinyl sheet and cement bag paper were frequently used as a delivery sheet, indicating 32.3%. Delivery set which was distributed by the health center was used 18.0%. (excluded the deliveries attended by professional attendants) 5. Immunization practices for the infant were observed as relatively high rate: 92.8% of the infant received D. p. T., 81, 3u/o received against poliomyelitis, 52.8% for small pox, 78.9% with B. C. G., and 18.5%against measles. The percentage af babies complected 3 times shot for D. P. T. and 2 times shot for poliomyelitis vaccine turned out be to 67.8% and 87.5%, respectively.

A Case of Neonatal Tetanus Cured with Neuromuscular Blocking Agent and Ventilatory Support (근이완제 및 인공 환기요법으로 치료한 신생아 파상풍 1례)

  • A neonate born at 38 gestational weeks was admitted due to generalized tonic-clonic seizure and cyanosis. The neonate was born six days previously at home through normal delivery and the umbilical cord was cut using scissors sterilized in boiling water. The neonate weighed 3,180 g at admission. Physical examination revealed cyanosis, opisthotonus, trismus and reactive muscle spasms. Laboratory exam, brain sonogram and EEG showed no significant abnormal findings. Based on her history and physical examination, the neonate was diagnosed with tetanus and put in an incubator isolated in a quiet, dark room. Treatment with tetanus human immunoglobulin along with antibiotics (penicillin G) were started immediately, and mechanical ventilation, administration of neuromuscular blocking agent and muscle relaxant were also started off. The frequency of seizure episode decreased gradually, and on the 32nd hospital day, mechanical ventilatory support was stopped along with extubation two days later. Thereafter, the neonate was in continuous generalized hypertonic state and showed feeding difficulty, but there was gradual improvement. She was dismissed on the 49th hospital day and is currently under OPD follow-up, doing well with no special problems. Respiratory management is critical to neonatal tetanus. We report here a case of tetanus treated with inhibition of self-respiration, neuromuscular blocker and application of ventilator, and present this method as a useful direction for future treatment of neonatal tetanus.

Effect of S-Allyl Cysteine(SAC) on the Proliferation of Umbilical Cord Blood(UCB)-derived Mesenchymal Stem Cells(MSCs) (S-Allyl Cysteine(SAC)이 제대혈 유래 중간엽 줄기세포 증식에 미치는 영향)

  • To improve the growth of human mesenchymal stem cells(hMSCs) under general cell culture conditions(20% $O_2$ and 5% $CO_2$), we examined the effect of s-allylcysteine(SAC), which is known as an antioxidant and the main component of aged-garlic extract, on hydrogen peroxide-induced cellular stress in hMSCs. We found that SAC blocked hydrogen peroxideinduced cell death and cellular apoptosis, but that SAC did not improve the growth of hMSCs during short-term culture. To evaluate the protective effect of SAC, we examined the endogenous expression of the antioxidant enzymes catalase (CAT), superoxide dismutase(SOD), and glutathione peroxidase(Gpx) in hMSCs. Hydrogen peroxide was found to downregulate the expression of CAT, SOD, and Gpx at the protein level. However, in the pre-treatment group of SAC, SAC inhibited the hydrogen peroxide-induced down-regulation of CAT, SOD, and Gpx. Unfortunately, treatment with SAC alone did not induce the up-regulation of antioxidant enzymes and the cell proliferation of hMSCs. Surprisingly, SAC improved cell growth in a single cell level culture of hMSCs. These results indicate that SAC may be involved in the preservation of the self-renewal capacity of hMSCs. Taken together, SAC improves the proliferation of hMSCs via inhibition of oxidative-stress-induced cell apoptosis through regulation of antioxidant enzymes. In conclusion, SAC may be an indispensable component in an in vitro culture system of human MSCs for maintaining self-renewal and multipotent characterization of human MSCs.

G$\alpha$12 and G$\alpha$13 Subunits Modulate $Ca^{2+}$-Induced Histamine Release in Human Umbilical Cord Blood-Derived Mast Cells

  • The role of $G{\alpha}12\;and\;G{\alpha}13$ in modulating the IgE receptor-mediated histamine secretion in the streptolysin-o-permeabilized human cultured mast cell was investigated. The expression of $G{\alpha}12\;and\;G{\alpha}13$ proteins were regulated during human cultured mast cell differentiation, and a significant correlation was observed between the levels of expression of $G{\alpha}12\;and\;G{\alpha}13$ proteins and IgE receptor-mediated histamine secretion capability in human cultured mast cells. Antibodies against $G{\alpha}12\;and\;G{\alpha}13$ effectively inhibited the IgE receptor-induced histamine release, and the concentration of anti-$G{\alpha}12$ antibody used to inhibit histamine secretion was shown to also inhibit the IgE receptor-mediated elevation of intracellular $Ca^2+$. Therefore, the results suggest that $G{\alpha}12\;and\;G{\alpha}13$ play roles in modulating IgE receptor-activated $Ca^2+$ influx, thereby regulating histamine release in cultured human mast cells. This is the first report to show that $G{\alpha}12\;and\;G{\alpha}13$ are involved in the regulation of $Ca^2+$ mediated exocytosis in human cultured mast cells.

Newborn traits associated with pre-weaning growth and survival in piglets

  • Objective: Piglet pre-weaning mortality is an important variable indicating the efficacy of farrowing management and animal well-being during lactation. The present study determined the association of newborn traits measured soon after birth with piglet pre-weaning mortality and growth. Methods: In total, 805 piglets born from 57 multiparous sows were investigated. Their blood oxygen saturation, blood glucose and rectal temperature at 24 h after birth (RT24h) were monitored. Birth order, sex, skin color, integrity of the umbilical cord, attempts to stand and birth intervention were monitored. Piglets were weighed at day 0, 7, and 21 to evaluate average daily gain (ADG). Results: Piglet pre-weaning mortality for lactation period was 12.6% and cumulative mortality during the first 7 days of age was 8.6%. A higher proportion of piglets with pale skin color died compared to piglets with normal skin color (26.7% vs 7.7%, p<0.001). A higher (p<0.001) proportion of piglets that attempted to stand after 5 min (38.5%) died compared to piglets that attempted to stand within 1 min (6.3%) after birth. Piglet body weight at birth ($BW_B$), blood glucose and the number of piglets born alive (BA) were correlated with ADG (p<0.05). Piglets with $BW_B$ <1.30 kg had higher (p<0.001) mortality rate than piglets with $BW_B{\geq}1.80 kg$ (19.0% vs 3.3%) and piglets with $BW_B$ 1.30 to 1.79 kg (4.0%). Piglet with RT24h <$37.0^{\circ}C$ had higher (p<0.001) mortality rate (86.2%) than piglets with RT24h >$38.5^{\circ}C$ (3.9%). Conclusion: Low $BW_B$ and low RT24h compromise piglet survival during the lactation period in the tropical conditions. Piglets in the litters with a high BA, low $BW_B$ and low blood glucose have reduced ADG.

A Study of the Neonatal Care in the hospitals located in Pusan (부산광역시 소재 병원의 신생아간호 실태조사)

  • This study is designed to produce basic data on the nursing intervention between infants and mothers after discharge from hospitals. This research is carried by investigating the nursing intervention during their after-delivery staying in the hospitals located in Pusan. The object of this study includes the neonatal wards of 51 hospitals. Data collection was carried from the 3rd of September, 2001 to the 4th of October in the same year. Tools for measurement using in this research is the nursing checklist designed by these researchers. The data were analyzed by using SPSS and the output was produced in frequency and percentage considering general characteristics and nursing activities. On the course of investigation, it was revealed that the average number of items checked are as follows; total beds in one hospital came up to 360.1, the beds in each newborn infant ward came up to 36.1, in each ward the numbers of incubators came up to 9.2 and intensive care unit to 1.7. In each ward 8.4 nurses and 2.9 nurse aids were working at that time. In each hospital, secure of air way, physical examination, eye care, umbilical cord care, maintenance of body temperature, identification of infant, nutritive condition, safety and danger management, infection management, mother-infantile interaction, medical assistant to doctor, medication and discharge education were comparatively well done. But, in case of emotional, social and cognitive aspect of nursing activities through interactions between newborn infants and nurses as substitutes are not reached to the adequate level. In the end, this study hereby suggests that further investigation on the device to facilitate the interaction between newborn infant and nurse as a nursing intervention of high quality.

  • Gestational diabetes mellitus (GDM), one of the common metabolic disorders of pregnancy, leads to functional alterations in various cells including stem cells as well as some abnormalities in fetal development. Perivascular stem cells (PVCs) have gained more attention in recent years, for the treatment of various diseases. However, the effect of GDM on PVC function has not been investigated. In our study, we isolated PVCs from umbilical cord of normal pregnant women and GDM patients and compared their phenotypes and function. There is no significant difference in phenotypic expression, response to bFGF exposure and adipogenic differentiation capacity between normal (N)-PVCs and GDM-PVCs. However, when compared with N-PVCs, early passage GDM-PVCs displayed decreased initial rates of cell yield and proliferation as well as a reduced ability to promote wound closure. These results suggest that maternal metabolic dysregulation during gestation can alter the function of endogenous multipotent stem cells, which may impact their therapeutic effectiveness.

  • Leukocyte adhesion deficiency is rare inherited defect on phagocytic function resulting lack of leukocyte cell surface expression of $\beta2$ integrin molecule that are essential for leukocyte adhesion to endothelial cells and chemotaxis. Clinical features of patients with leukocyte adhesion deficiency type I include recurrent necrotic infection of the skin mucous membranes, and intestinal tract with septicemia, and omphalitis arising from delayed umbilical cord separation. Oral manifestations are severe progressive periodontitis with alveolar bone loss, periodontal pockets, and partial and total premature loss of the deciduous and permanent dentitions. We report a case of leukocyte adhesion deficiency type I in a 5-year-old child with severe periodontitis. In order to prevent local and systemic infection, we controlled periodontal disease with periodic oral prophylaxis. Oral swabs and blood cultures were perfomed for suspected infection, so that optimal measures were taken through the use of appropriate antibiotics.

Effect of Low Intensity Sound Wave on UC-MSC(Umbilical Cord Mesenchymal Stem Cell Growth (저강도 음파 조사가 중간엽 줄기세포 증식에 미치는 영향)

  • It is known that the low-intensity sound stimulation really affect to grow the cell. The cellular growth mechanism, however, does not been clearly identified even the effect on the low-intensity sound stimulation. The purpose of this study is to investigate the effect of low-intensity sound stimulation on the alveolar UC-MSC proliferation. Before the low-intensity sound stimulation is applied, the UC-MSC are cultured for 24 hours to facilitate their attachments. The cells are divided into two groups. And each was exposed to a medium with or without the low-intensity ultrasound stimulation at 71dB intensity level. The UC-MSC are again divided into three treatment groups of group 1, 2, and 3 and exposed to a frequency at 50Hz, 100Hz, and 1000Hz, respectively. In the results, it is investigated that the growth rates of UC-MSC for the stimulated groups were higher than those of control groups. In 1000Hz frequency, the number of UC-MSC cell is significantly higher than control groups (p>0.05). We would put the hypothesis that the cell growth could be enhanced by an appropriate low-intensity sound stimulation.

  • Tae-yeul is the baby's disorder inheritted by its mother during fregnancy. This is due to its mother's overeating hot, fatty food and taking harmful medicine. So internal physical fever is accumulated in mother's body, and it heats up the placenta and the umbilical cord. Generally, this disorder means the skin disease that is occurred from $2{\sim}5$ months to 1 year after birth. It is specialized by the dirty oozing, a severe itching and the rash at its head and face. It is similar in its symptoms such as atopic dermatitis in western medicine. Recently, this disease has the increasing tendency of prevalence rate because of the bad atmosphere and the invironmental pollution. The results were summerised as follows; 1, 2, 3, 4 1. The causes of Tae-yeul are summarized by fragnat woman's contacting a cool air, overating a hot and toxic food and taking emotional shock. 2. The symptoms of Tae-yeul are the edema on upper eyelids, stifling convulsions, flush, the fever of one side body, the asthma of sputum increase, the yawing of the tired mind, the bowl complaint, the red urine and urination disturbance. 3. In the treatment method of Tae-yeul, its mother feed her milk after taking a madicine. So one must expect to be treated slowly. If one tries to treat it with a cool madicine quickly, it can cause vomitting or a serious disorder. 4. In the prescription of Tae-yeul, they used a Moktongsan, a Chiwhnagtang, an Yangyoubang, a Tojuksan, a Whangyunsan, a Juryungtang and a Kamdutang, etc.. For the method of cleaning face or bathing, they used Sasungsan, Whanbaekpijuntang. 5. Tae-yeul including Naesun, Taelyumchang, yousun, etc. is similar to the atopic dermatitis of western medicine in its symptoms.
