Ultrasound-guided Aspiration of the Iatrogenic Pneumothorax Caused by Paravertebral Block -A Case Report-

  • Thoracic paravertebral block is performed for the treatment of patients with chronic pain, such as complex regional pain syndrome (CRPS) and post-herpetic neuralgia. Thoracic paravertebral block can result in iatrogenic pneumothorax. Because pneumothorax can develop into medical emergencies and needle aspiration or chest tube placement may be needed, early diagnosis is very important. Recently, thoracic ultrasonography has begun to be used to diagnose pneumothorax. In addition, ultrasound-guided aspiration can be an accurate and safe technique for treatment of pneumothorax, as the needle position can be followed in real time. We report a case of iatrogenic pneumothorax following thoracic paravertebral block for the treatment of chronic pain due to CRPS, treated successfully by ultrasound-guided aspiration.

Ultrasound Assisted Sn-Ag-Pd Activation Process for Electroless Copper Plating (무전해 동 도금을 위한 초음파 적용 주석-은-팔라듐 활성화 공정에 대한 연구)

  • An ultrasound-assisted Sn-Ag-Pd activation method for electroless copper plating was presented in this study. With this activation process, it was shown that the fine catalyst particles were homogeneously distributed with high density on the entire specimen. In addition, it was observed that incubation period occurred during the electroless plating step was decreased owing to the absorption of Ag which holds high catalytic activity. Resulting from the refinement and high densification of catalyst, the defect-free gap-fill was achieved within the 20x nm trench.

  • Necrobiosis lipoidica (NL) is a rare, idiopathic, chronic granulomatous inflammatory disease of collagen degeneration with the risk of ulceration. Many procedures have been proposed to treat this rare disease. In this study, we applied LDM®-MED for the management of NL, and this condition in our patient was chronic and refractory to other therapeutic options. To the best of our knowledge, no study has explored treatment of NL using ultrasound. Our results suggest that application of LDM®-MED seems to be an effective treatment option for NL. Long-term and systematic studies are needed to determine whether such application of LDM®-MED will be an innovative and effective treatment option for NL and various kinds of chronic wounds.

Simultaneous Combination Treatment Using High-Intensity Focused Ultrasound and Fractional Carbon Dioxide Laser Resurfacing for Facial Rejuvenation

  • Background and Objectives High-intensity focused ultrasound (HIFU) can produce small zones of thermal damage. A HIFU procedure is non-invasive and it can achieve rejuvenation of facial skin. Fractional CO2 laser resurfacing delivers thermal damage to the pixilated columnar zone of the skin and so evoke collagen remodeling, the same as HIFU. In many cases, the patients who want rejuvenation with HIFU are also good candidates for cutaneous photorejuvenation such as can be accomplished via fractional CO2 resurfacing. If patients are treated in a single session by remodeling both the superficial and deep compartments of skin by using both modalities, then improvement in rhytides and tightening of sagging skin will optimize the aesthetic result. Materials and Methods Between May 2014 and January 2018, a total of 44 patients were treated with combination HIFU and fractional CO2 laser resurfacing according to our protocol. First, the HIFU was applied to the entire face with an average of 300 treatment lines. Immediately after HIFU treatment, the ultrasound gel was washed off and then fractional CO2 laser resurfacing was performed. We evaluated the patients using 4-point grading scales. The clinician examined the skin for evidence of complications after the completion of treatment. Results All the patients' skin quality showed improvement. Further. the clinical results after duel modality treatment were substantially better than that after the use of either modality alone. The recovery times and the incidence of adverse events when quickly and consecutively performing both treatments were similar as compared to those with employing stepwise treatment. We encountered no complications whatsoever. Conclusion When compared with stepwise therapy, combination therapy with HIFU and fractional CO2 resurfacing offers better, safer and more effective clinical results. Thus, for targeting multiple layers of aging facial skin, this combination therapy can be safely performed in a single treatment session.

An in vivo electromyographic evaluation of pain relief using different therapies in masticatory myalgia patients

  • Objectives: This study is aimed to evaluate and compare the effect of moist heat fomentation therapy with ultrasound therapy in patients with the masticatory myalgia. Materials and Methods: The study was conducted on 42 patients with masticatory myalgia, dividing them into two groups; Group A (21 patients), received moist heat therapy and Group B (21 patients), received ultrasound therapy for seven effective days. Prior and after the treatment the numeric rating scale (NRS) and the electromyography (EMG) scores were recorded and compared. The observations were analyzed clinically and statistical support was taken to assess the NRS and EMG data. Results: Irrespective of the groups, patients testified a significant reduction in pain after the treatment. From the EMG readings; even though the standard deviation for each group was varied considerably, EMG recorded an improved muscle activity. Statistical analysis was used to assess and identify the best treatment methodology between the two modalities. Conclusion: From the statistical analysis, it is concluded that, though both the therapies had significantly reduced the symptomatic response, it is moist heat fomentation that improved muscle activity both statistically and clinically in comparison to ultrasound.

  • A 60-year-old man presented with pain on the left cheek and lateral nose. The patient had been diagnosed with facial herpes zoster in the left V2 area 6 months previously. Medical treatment was prescribed for 6 months but it had little effect. We blocked the left infraorbital nerve under ultrasound guidance, but pain relief was short term. Therefore, we performed pulsed radiofrequency treatment on the left infraorbital nerve under ultrasound guidance. Six months after the procedure, the reduction of pain was still maintained, and there was no need for further management.

A Study on the Laboratory Scale Ultrasound Treatment System for Methyl tert-Butyl Ether Polluted Groundwater (Methyl tert-Butyl Ether 오염 지하수 처리를 위한 실험실 규모 초음파 분해 시스템 연구)

  • A series of experiments with a laboratory scale ultrasound treatment system for MTBE polluted groundwater was performed to increase the efficiency of MTBE degradation in groundwater. This study evaluated several factors to increase the efficiency of MTBE treatment for artificial and natural groundwater. The treated volume of groundwater, ultrasound frequency and power, and pollutant concentrations have been changed to evaluate its effects on the degradation efficiency of MTBE in batch and continuous flow reactor. For the specific experimental conditions on this paper, MTBE degradations are more efficient at 580 kHz than those at 1 MHz. The efficiency of MTBE degradation is proportional to the intensity of ultrasound power per unit volume of MTBE polluted groundwater. The concentration of ions in groundwater does not much affect the efficiency of MTBE degradation. The $1^{st}$ order degradation constant of MTBE for different power per unit volume at 580 kHz shows linear relationship at same concentration. The $1^{st}$ order degradation constant for 0.1 mM MTBE solution is higher than that for 1 mM MTBE solution. These experimental results could be helpful to seek optimal conditions for relatively large volume of polluted groundwater treatment.

Study on In Vitro Bovine Embryo Production with Follicular Oocytes Obtained via Ultrasound-guided Ovum Pick-up (OPU) and Slaughterhouse-derived(SHD) Ovary Aspiration in Korean Native Cows (한우에서 초음파 유래와 도축장 유래 난포란을 이용한 체외 수정란 생산에 관한 연구)

  • The present study was carried out for the comparative study on the collection of bovine follicular oocytes by ultrasound-guided ovum pick-up(OPU) and slaughterhouse-derived (SHD) ovary aspiration and in vitro production of bovine embryos with the follicular oncytes in Korean native cows. Bovine follicular nocytes were observed with a 6.5 MHz convex-array ultrasound transducer designed for intravaginal use and the oocytes were collected with the aspiration equipment attached to the ultrasonograph. Bovine ovaries were collected and transported in phosphate buffered saline from the local slaughterhouse, the follicular oocytes were collected by the aspiration method. The collected follicular oocytes in good quality were matured, fertilized and cultured in the media. The total number of the visible follicles and the recovery rate of follicular oocytes were increased in ultrasonography following follicle stimulating hormone(FSH) treatment in Korean native cows. The mean recovery rate of oocytes was 66.2, 52.8 and 41.7% in the FSH-OPU, non-treatment-OPU and SHD ovaries, respectively. The mean number of recorved oocytes per cow were not significantly(P<0.05) different between the FSH-OPU(14.0$\pm$11.54) and SHD(17.1i6.21) groups, but the numbers in both groups were significantly(P<0.05) higher than the number in the non-treatment-OPU(3.7$\pm$1.57) group. The mean number of usable nocytes in Grade T /11 per ovary was 6.3, 4.8 and 1.3 in the cows of the SHD, FSH-OPU and non-treatment-OPU groups, respectively. The in vitro developmental rate to the blastocyst was not significantly different between the oocytes obtained via OPU(37.1%) and SHD(29.3%). Therefore, the ultrasound-guided OPU technique can be applied to the production of excellent embryos from the high-quality cows, and for the large scale production of in vitro bovine oocytes and embryos, the SHD ovary aspiration method is valuable.

Inhibitory Effects of Ultrasound in Combination with Ascorbic Acid on Browning and Polyphenol Oxidase Activity of Fresh-cut Apples

  • This study was conducted to investigate the effect of ultrasound with ascorbic acid on the quality of fresh-cut apples. Prepared apple cubes were dipped in distilled water (US) or in 1% ascorbic acid solution (AS), both were treated with ultrasound at 40 kHz, while the other sample was just dipped in 1% ascorbic acid solution for 1 min (AA). All samples were stored at $4^{\circ}C$ for 12 days. AS treatment had an effect on apple cube color as indicated by its significantly higher L values and lower ${\Delta}E$ value whereas AA- and US-treated samples showed a considerable browning. Higher inhibition rate on browning and polyphenol oxidase (PPO) activity of 46 and 98%, respectively, were observed after AS treatment. The highest total phenolic content of AS-treated samples was shown during whole storage periods. This investigation revealed that the use of ultrasound in combination with ascorbic acid had a positive effect on quality maintaining of fresh-cut apples.

A Case Report of Ultrasound-guided Bee Venom Pharmacopuncture on Plantar Fasciitis (족저근막염의 초음파유도하 봉약침 치험1례)

  • Objectives: This study is to report the usefulness of ultrasound in diagnosing plantar fasciitis and the effectiveness of ultrasound-guided bee venom pharmacopuncture as a treatment of it. Methods: A 61-year-old woman suffered from plantar fasciitis on her right foot for 1 month. The sonographic findings were fascial thickening, blurring of perifascial border and perifascial effusion. The ultrasound-guided bee venom pharmacopuncture for 6 times and conventional Korean medicine therapies like acupuncture and moxibustion for 12 times in 5 weeks. Numeric rating scale (NRS) and foot function index(FFI) was used to evaluate the pain and its progress. and thickness of plantar fascia was measured by sonography every week. Results: After treatments, NRS and FFI were reduced from 10 to 1 and from 190 to 72. The thickness of fascia was reduced from 0.43cm to 0.40. Blurring and effusion of perifascial border were also improved. Conclusion: This report suggests that the Ultrasound-guided Bee Venom Pharmacopuncture is effective for Plantar Fasciitis. Ultrasonography could be one of the most valuable items in the clinical practice of Korean medicine doctors who seek minimally invasive treatment.
