Flavonol Glycosides of Maesa Lanceolata Leaves

  • An investigation of the methanolic extract of Maesa lanceolata leaves has led to the isolation of four novel flavonol glycosides characterised as myricetin 3-0-2', 3', 4'-triacetylxylopyranoside (1), quercetin $3-O-{\beta}-3'$, $6'-diacetylglucopyranosyl-(1{\longrightarrow}4)-{\alpha}-2'$, 3'-diacetylrhamnopyranoside (2), myricetin $3-O-xylopyranosyl-(1{\to}3)-{\alpha}-rhamnopyranoside$ (3) and quercetin $3-O-{\beta}-ga1actopyranosyl-(1{\to}4)-{\alpha}-rhamnopyranoside-7-O-{\beta}-galactopyranoside$ (4). Also isolated from the same extract were known flavonols; quercetin (5), myricetin (6), quercetin 3-O-xylopyranoside (7), quercetin $3-O-{\alpha}-rhamnopyranoside$ (8), myricetin $3-O-{\alpha}-rhamnopyranoside$ (9), myricetin $3-O-{\beta}-galactopyranoside$ (10) and quercetin 3-O-rutinoside (11).

Bochdalek hernia in adults: a report of 3 cases (성인에서 발견된 Bochdalek 허니아치험 3)

  • Bochdalek hernia is common in infants and children, but extremely rare is adults. So diagnosis and treatment have been many problems. These are case reports of Bochdalek hernia in adults which were evaluated and corrected at the Department of Thoracic and Cardiovascular Surgery, College of Medicine, Chungnam National University during the past 7 years from June 1976 to August 1983. 1.Among the three cases, one was male and the others were female. 2.At first, they were diagnosed as G-I or respiratory disorders and treated symptomatically. But confirmed with UGI and barium enema. 3.All cases had ipsilateral hypoplasia of entire lung or lower lobe as combined anomaly and corrected as simple closure in two cases, pericardial patch closure in one case. 4.Postoperative course was smooth and uneventful.

Traumatic Diaphragmatic Injuries (Report of 22 Cases) (외상성 횡경막 손상 -22례 보고-)

  • 22 Cases of traumatic diaphragmatic injuries treated at the Department of Thoracic and Cardiovascular Surgery in Chon-Buk National University Hospital from Jan. 1979 to Oct. 1988 were reviewed in this study. Of the 22 cases, 18 were male and 3 were female, a ratio of 4.5:1. This ratio revealed high incidence in male patient. The age distribution ranged from 2 to 60 years and mean age was 31 years. The modes of injury were as follows: 11 stab wound, 5 traffic accident, 2 fall down, 2 fighting injury, 1 compression wound by sand bag, and 1 slip down injury. Useful diagnostic tools were chest X-ray with or without radiopaque dye swallowing, which was the most commonly diagnostic, UGI series, and thoracoscope. Operations were performed in 22 cases, and 18 cases were through thoracotomy. The herniated organs through the ruptured diaphragm were stomach, omentum, liver, spleen, colon, and small bowel. There were associated injuries, and the most commonly associated was rib fracture. There was no postoperative death.

  • A simple method has been developed for four-component synthesis of disubstituted 1,3,4-oxadiazoles using (N-isocyanimino)triphenylphosphorane, a primary amine, a carboxylic acid and biacetyl in $CH_2Cl_2$ by the Ugi-4CR/aza-Wittig sequence at room temperature in excellent yields.

A Clinical Observation on Children with Corrosive Esophagitis (소아 부식성 식도염의 임상적 고찰)

  • Purpose: This study was undertaken to evaluate the clinical features and complication such as esophageal stricture in children with corrosive esophagitis. Methods: We retrospectively reviewed medical records of 31 children who accidentally ingested corrosive materials and visited to emergency room of Chonnam National University Hospital from Jan. 1992 to Dec. 1999. Twenty-one children were examined by upper gastrointestinal (UGI) endoscopy to evaluate location and severity of caustic injury. Results: 1) Among 31 patients, there were 20 males and 11 females and the ratio of male to female was 2:1. Average age at diagnosis was 2.3 years (12 months to 9.8 years). Twenty-seven (87.1%) patients were accidentally ingested vinegar. 2) Initial presenting symptoms were dysphagia (54.8%), vomiting (48.3%), chemical burn on lips and skin (45.2%), excessive salivation (45.2%), coughing and respiratory grunting (32.3%) and aspiration pneumonia (9.8%). 3) UGI endoscopic examination showed caustic injury in 17 children: grade I in 8, grade II in 7 and grade III in 2. The region of caustic injury was proximal esophagus in 5, distal esophagus in 3, entire esophagus in 9 and stomach in 6. 4) Corrosive esophageal strictures developed in 6 children (19.4%) and gastric outlet stricture in 1 (3.2%). All of them showed grade II or III caustic injury on endoscopic examination. Conclusion: The development of esophageal stricture was related to the severity of the caustic injury. Early UGI endoscopic examination in caustic ingestion seems to be useful for prediction of development of caustic stricture.

  • Background: This study is to evaluate the effect of systemic corticosteroid on the clinical outcomes and the occurrence of complications in mechanical ventilated patients with severe community-acquired pneumonia (CAP). Methods: We retrospectively assessed the clinical outcomes and complications in patients with severe CAP admitted to ICU between March 1, 2003 and July 28, 2009. Outcomes were measured by hospital mortality after ICU admission, duration of mechanical ventilation (MV), ICU, and hospital stay. Complications such as ventilator associated pneumonia (VAP), catheter related-blood stream infection (CR-BSI), and upper gastrointestinal (UGI) bleeding during ICU stay were assessed. Results: Of the 93 patients, 36 patients received corticosteroids over 7 days while 57 patients did not receive corticosteroids. Age, underlying disease, APACHE II, PSI score, and use of vasopressor were not different between two groups. In-hospital mortality was 30.5% in the steroid group and 36.8% in the non-steroid group (p>0.05). The major complications such as VAP, CR-BSI and UGI bleeding was significantly higher in the steroid group than in the non-steroid group (19.4% vs. 7%, p<0.05). The use of steroids and the duration of ICU stay were significantly associated with the development of major complications during ones ICU stay (p<0.05). Conclusion: Systemic corticosteroid in patients with severe CAP requiring mechanical ventilation may have no beneficial effect on clinical outcomes like duration of ICU stay and in-hospital mortality but may contribute to the development of ICU acquired complications.

How did the peculiar S0 galaxy M85 form?

  • M85 is a merger remnant galaxy in the Virgo Cluster, showing complex merging features. Globular clusters in M85 are a good tracer of its merging history. To investigate globular cluster system of M85, we obtain deep and wide field images of M85 in ugi filters covering one square degree using CFHT/MegaCam. We discover about 1,000 globular cluster candidates in these images. The color distribution of the globular cluster candidates within r < 5' from M85 does not show a clear bimodality and blue globular cluster candidates are more than red ones. These features are different from those in massive early-type galaxies. The spatial distribution of the globular cluster candidates is elongated along the faint stellar light of M85. We also investigate the spatial distribution of sub-populations of the globular cluster candidates with different color and brightness and estimate their ages based on their color. We discuss these results in relation with the formation history of M85.

Esophageal GIST : case report (하부식도에서 발생한 GIST 1예)

  • Currently gastrointestinal mesenchymal tumors are divided into three major categories: myogenic tumors(leiomyoma, leiomyosarcoma), neurogenic tumors (schwannomas) and neoplasms that belong to neither group, which are known by GIST(gastrointestinal stromal tumors). The stromal tumors are hetrogenous, so that they may show myogenic or neurogenic differentiation or both, or no differentiation at all in some patients. The best defining feature for GIST is their expression of KIT-protein(CD117). Leiomyomas are the most common mesenchymal tumor in esophagus. Esophageal GISTS are very rare in comparision to those of the stomach and intestine. Recently we experieneced one case of the esophageal GIST, so that we describe an esophageal GIST on immunohistochemical analysis. A 70 years old woman complained of dysphagia and nausea for 3 days. FGS showed a huge elevated lesion in lower esophagus 33cm distal to incisor, which was covered with normal mucosa. CT and UGI showed the intramural tumor of lower third of the esophagus. The distal esophagectomy and esophago-gastrostomy were performed. The tumor was located in lower third of esophagus and measured as $6{\times}3.7$cm in size. Immunohistochemically, it showed weakly positive CD117 and diffusely positive S-100. SMA, desmin, NES and chromogranin showed negative immune-reaction. The patient was followed for 15 month after operation. There was no recurrence.

Tracing the Giant Metal-poor Halo Around the Sombrero

  • M104 (NGC 4594, the Sombrero) is an intriguing disk galaxy classified as an elliptical galaxy nowadays. It hosts a luminous bulge and a massive disk, but it is still mysterious how M104 acquired such peculiar structures. Globular clusters are an useful tracer to investigate the formation history of early-type galaxies. In this study we present a wide field imaging study of the globular clusters in M104. Using wide ($1^{\circ}{\times}1^{\circ}$) and deep ugi images of M104 obtained with the CFHT/MegaCam observations, we detect a large number of globular clusters. The color distribution of these globular clusters shows that there are two subpopulations: a metal-poor system and a metal-rich system. The radial number density of the metal-poor globular clusters shows a long tail reaching R ~ 30' (~ 80 kpc), indicating clearly the existence of a giant metal-poor halo in M104. This result is consistent with the previous studies on the dual halos of massive early-type galaxies. We will discuss implications of these results in relation with the formation history of M104.

  • 건강검진을 위해 병원을 내원한 수검자 중 위장조영검사를 선택한 환자를 대상으로 서비스 만족도 개선을 위해 이 연구를 시행하였다. 수검자를 대상으로 설문조사를 시행하였으며, 2014년 5월부터 2개월 동안 검진센터를 방문한 수검자 147명을 교육 전, 2014년 7월부터 2개월 동안 내원한 수검자 164명을 교육 후 대상자로 하였다. 교육 전 대상자에게 불안감, 설명필요, 설명방식, 조영검사만족도 등을 조사하여 이를 바탕으로 동영상을 제작하였으며, 교육 후 대상자에게 제공하여 불안감 변화, 동영상 설명 만족도, 도움정도, 재시청의향 등을 조사하였다. 교육 전 측정한 불안감은 2.62, 설명필요성 2.52, 설명방식은 대상자의 78.2%가 설명안 내동영상을 선택하였으며, 조영검사만족도는 1.55로 낮게 조사되었다. 교육 후 측정한 불안감의 변화는 2.55, 동영상설명만족은 2.80, 도움정도 2.79, 재시청 의향은 2.80으로 높게 측정되었다. 교육 전과 교육 후의 집단간 교차분석에서 불안감, 시청의향, 도움정도, 동영상만족도 등에서 유의하게 측정되었다(p<0.05). 위장조영검사에 대한 동영상 교육과 사전 정보제공이 환자의 불안감을 감소시켰고, 검사만족도는 증가시켰다.
