Apical foramen morphology according to the length of merged canal at the apex

  • Objectives: The aim of this study was to investigate the relationship between the apical foramen morphology and the length of merged canal at the apex in type II root canal system. Materials and Methods: This study included intact extracted maxillary and mandibular human premolars (n = 20) with fully formed roots without any visible signs of external resorption. The root segments were obtained by removing the crown 1 mm beneath the cementum-enamel junction (CEJ) using a rotary diamond disk. The distance between the file tip and merged point of joining two canals was defined as Lj. The roots were carefully sectioned at 1 mm from the apex by a slow-speed water-cooled diamond saw. All cross sections were examined under the microscope at ${\times}50$ magnification and photographed to estimate the shape of the apical foramen. The longest and the shortest diameter of apical foramen was measured using ImageJ program (1.44p, National Institutes of Health). Correlation coefficient was calculated to identify the link between Lj and the apical foramen shape by Pearson's correlation. Results: The average value of Lj was 3.74 mm. The average of proportion (P), estimated by dividing the longest diameter into the shortest diameter of the apical foramen, was 3.64. This study showed a significant negative correlation between P and Lj (p < 0.05). Conclusions: As Lj gets longer, the apical foramen becomes more ovally shaped. Likewise, as it gets shorter, the apical foramen becomes more flat shaped.

근관충전용 실러의 물리적 성질에 관한 연구 (A STUDY ON THE PHYSICAL PROPERTIES OF ROOT CANAL SEALERS)

  • The purpose of this study was to compare and estimate the physical properties of five root canal sealers classified Calciobiotic root canals sealer as calcium hydroxide based sealer, Apatite root sealer type II as calcium phosphate based sealer, AH-26 as resin based sealer, Canals and Pulpdent root canals sealer as zinc oxide eugenol based sealer. The author investigated dimensional change and flow rate of canal sealers, diametral tensile strength and shear bond strength of sealers to dentin to evaluate the physical properties on affect of complete obturation of root canal and performed the total 100 specimens of each 25 sealers under the condition of root temperature according to manufacturer's instructions. All specimens were stored at $37{\pm}1^{\circ}C$ in 100 % relative humidity. A microscope for measurement of micro distance is used for the dimensional change test and evacuation methods using vaccum were used for the flow rate test. The result differed by the storage time measured on the tests of diametral tensile strength and shear bond strength to dentin. The following results were obtained ; 1 On the test of dimensional change, Canals and Pulpdent expanded slightly, AH-26 and Apatite showed the severe shrinkage after 48 hours. 2. AH-26 and Apatite were the excellent with each 24.59mm, 31.19mm after 3 minutes in the aspect of flow property. 3. On the diametral tensile strength, Calciobiotic root canals sealer showed the highest strength with 27.13kg/$cm^2$ after 48 hours, Apatite root sealer type II showed highest strength with 84.57kg/$cm^2$ after 120 hours. 4. On the shear bond strength to dentin, AH-26 was most excellent with 55.73kgf/$cm^2$ after 24 hours and with 134.71kgf/$cm^2$ after 120 hours.

C-shaped canal의 절단면 분석을 통한 근관형태의 변화와 근관과 치아외벽간의 최소거리 분석에 관한 연구 (CROSS-SECTIONAL MORPHOLOGY AND MINIMUM CANAL WALL WIDTHS IN C-SHAPED ROOT OF MANDIBULAR MOLARS)

  • C-shape 근관은 주로 하악 제 2대구치에서 보이는 해부학적 변이 형태로서, 그 특징은 각 근관을 연결하는 fin이나 web이 존재한다. 이런 해부학적으로 복잡함은 근관치료시에 세정과 성형 및 근관충전을 방해하며, 과다한 기구 조작시에 뜻하지 않는 천공을 유발하기도 한다. 이 연구의 목적은 C-shape 치근을 가진 하악 제2 대구치의 해부학적인 형태와 치근의 위치에 따른 최소 근관벽 두께에 대하여 연구하고자 하였다. 발거된 하악 대구치중 C-shape 치근을 가진 치아를 30개 선별하여 방사선 사진 및 협측, 설측, 치근단 부위를 디지털 카메라를 이용하여 촬영하였다. 자가 중합형 교정용 레진 (Orthodontics resin, Densply/Caulk, U.S.A)을 이용하여 치관부를 제거한 치아를 매몰한 후 각각의 시편을 Accutom-50 (Struers, Denmark)에 고정시켜 1 mm 간격으로 절단, 연마한 다음 절단면을 수술용현미경에 연결된 디지털 카메라 (Coolpix 995, Nikon, Tokyo, Japan)를 이용하여 각각의 상을 촬영하였다. 30개 치아에서 촬영한 197개의 치아단면을 Root thickness gauge program을 이용하여 근관형태의 변화를 분류하고 부위별 근관과 치아외벽 사이에 존재하는 상아질의 최소두께에 대해 연구 분석하였다. 1. 근관입구에서 C I이 가장 많이 (73%) 관찰되었으나 치근단부로 갈수록 근관 형태의 다양한 변화를 보여 C II와 C III의 근관형태가 많이 관찰되었으며, 11 개의 (36%)치아에서는 근관입구에서 치근단부까지 근관형태의 변화를 보이지 않았다. 근관입구에서 단지 2개의 치아에서 나타난 C III type은 치근단부로 갈수록 높은 발현빈도를 나타내었다. 2. C-shape 근관은 근관입구 부위에서는 continuous 나 semicolon canal을 가지나 치근단부로 갈수록 2개나 3개의 근관을 갖는 경우가 많다. 3.평균적인 근관과 치질사이의 최소거리는 근첨부의 설측부위에서 나타났다.

Successful nonsurgical treatment of type II dens invaginatus with 5 root canals using a self-adjusting file: a case report

  • The present report describes the endodontic treatment of an Oehlers type II dens invaginatus in a maxillary lateral incisor with 5 root canals, an extremely rare condition. Apical periodontitis and related symptoms were noted. Cone-beam computed tomography was used to aid the diagnosis, reveal tooth morphology, and assist in canal location. The pulp chamber was carefully accessed, and the root canals were explored under magnification. All root canals were prepared with an R25 Reciproc Blue system and sodium hypochlorite (NaOCl) irrigation. After initial preparation, a self-adjusting file (SAF) with NaOCl and ethylenediaminetetraacetic acid was used to complement the disinfection. Additionally, calcium hydroxide medication was applied. Vertical compaction was used to fill the canals with a calcium silicate-based endodontic sealer and gutta-percha. After 12 months, the patient exhibited healing of the periapical region, absence of symptoms, and normal dental function. In conclusion, this nonsurgical treatment protocol was successful in promoting the cure of apical periodontitis. Both complementary disinfection with an SAF and use of calcium hydroxide medication should be considered when choosing the best treatment approach for dens invaginatus with very complex anatomy.

근관벽(根管壁) 유지구(維持溝)가 Post의 유지력(維持力)에 미치는 영향(影響) (THE INFLUENCE OF RETENTIVE GROOVE ON THE CANAL WALL UPON THE RETENTION OF POST)

  • The experimental study was made to evaluate the influence of retentive grooves on the canal wall upon the retention of post, using different type of cements. Ninety periodontally involved anterior teeth were extracted and divided into six groups of 15 teeth. The groups were labeled as follows: Group I: Dentatus post cemented zincphosphate cement into post hole with smooth wall. Group II: Dentatus post cemented with zincphosphate cement into post hole with retentive grooves. Group III: Dentatus post cemented with glass-ionomer cement into post hole with smooth wall. Group IV: Dentatus post cemented with glass-ionomer cement into post hole with retentive grooves. Group V: Dentatus post cemented with composite resin adhesive into post hole with smooth wall. Group VI: Dentatus post cemented with composite resin adhesive into post hole with retentive grooves. The tensile strength was tested by an Instron type tester with cross-head speed of 5mm per minute. The results were as follows: In the group without retentive grooves, three kinds of cement have little effect upon retentive capacity of Dentatus post. The Dentatus post cemented on the canal wall with retentive grooves is more retentive than Dentatus post without retentive grooves.

EXTENSIONS OF EULER TYPE II TRANSFORMATION AND SAALSCHÜTZ'S THEOREM

  • In this research paper, motivated by the extension of the Euler type I transformation obtained very recently by Rathie and Paris, the authors aim at presenting the extensions of Euler type II transformation. In addition to this, a natural extension of the classical Saalsch$\ddot{u}$tz's summation theorem for the series $_3F_2$ has been investigated. Two interesting applications of the newly obtained extension of classical Saalsch$\ddot{u}$tz's summation theorem are given.

C-shaped root canals of mandibular second molars in a Korean population: a CBCT analysis

  • Objectives: The purpose of this study was to investigate the C-shaped root canal anatomy of mandibular second molars in a Korean population. Materials and Methods: A total of 542 teeth were evaluated using cone-beam computed tomography (CBCT). The canal shapes were classified according to a modified version of Melton's method at the level where the pulp chamber floor became discernible. Results: Of the 542 mandibular second molars, 215 (39.8%) had C-shaped canals, 330 (53%) had 3 canals, 17 (3.3%) had 2 canals, 12 (2.2%) had 4 canals, and 8 (1.7%) had 1 canal. The prevalence of C-shaped canals was 47.8% in females and 28.4% in males. Seventy-seven percent of the C-shaped canals showed a bilateral appearance. The prevalence of C-shaped canals showed no difference according to age or tooth position. Most teeth with a C-shaped canal system presented Melton's type II (45.6%) and type III (32.1%) configurations. Conclusions: There was a high prevalence of C-shaped canals in the mandibular second molars of the Korean population studied. CBCT is expected to be useful for endodontic diagnosis and treatment planning of mandibular second molars.

Fabrication and performance evaluation of ultraviolet photodetector based on organic /inorganic heterojunction

  • Organic/inorganic ultraviolet photodetector was fabricated using thermal evaporation technique. Organic/inorganic heterojunction based on thermally evaporated copper (II) acetylacetonate thin film of thickness 200 nm deposited on an n-type silicon substrate is introduced. I-V characteristics of the fabricated heterojunction were investigated under UV illumination of intensity $65mW/cm^2$. The diode parameters such as ideality factor, n, barrier height, ${\Phi}_B$, and reverse saturation current, $I_s$, were determined using thermionic emission theory. The series resistance of the fabricated diode was determined using modified Nord's method. The estimated values of series resistance and barrier height of the diode were about $0.33K{\Omega}$ and 0.72 eV, respectively. The fabricated photodetector exhibited a responsivity and specific detectivity about 9 mA/W and $4.6{\times}10^9$ Jones, respectively. The response behavior of the fabricated photodetector was analyzed through ON-OFF switching behavior. The estimated values of rise and fall time of the present architecture under UV illumination were about 199 ms and 154 ms, respectively. Finally, enhancing the photoresponsivity of the fabricated photodetector, post-deposition plasma treatment process was employed. A remarkable modification of the device performance was noticed as a result of plasma treatment. These modifications are representative in a decrease of series resistance and an increase of photoresponsivity and specific detectivity. The process of plasma treatment achieved an increment of external quantum efficiency from 5.53% to 8.34% at -3.5 V under UV illumination.

Cone-beam computed tomographic evaluation of the root canal anatomy of the lower premolars and molars in a Brazilian sub-population

  • Purpose: This study evaluated anatomical variations in the root canals of the lower premolars and molars in a Brazilian sub-population using cone-beam computed tomography (CBCT). Materials and Methods: In total, 121 CBCT images of patients were selected from a database. All images contained lower first and second premolars and molars on both sides of the arch, fully developed roots, and no treatment, resorption, or calcifications. In each image, the root canals of the lower premolars and molars were evaluated according to the Vertucci classification in On-Demand 3D software in the multiplanar reconstruction with dynamic navigation. Twenty-five percent of the images were re-assessed to analyze intraobserver confidence with the kappa test. Data were statistically evaluated with linear regression to evaluate the correlations of anatomic variations with age and sex, and the Wilcoxon test to analyze the laterality of variations, with a significance level of 5%. Results: The intraobserver agreement (0.94) was excellent. In general, the root canals of lower premolars and molars showed a higher prevalence of type I than other Vertucci classification types, followed by type V in premolars and type II in molars. When the molar roots were evaluated separately, type II was more frequent in mesial roots and type I in distal roots. Although age showed no correlations with the results, sex and laterality showed correlations with tooth 45 and the lower second premolars, respectively. Conclusion: The lower premolars and molars of a Brazilian sub-population showed a wide range of root canal anatomic variations.

안와 외벽 골절의 분류와 임상적 의의 (Classification of the Lateral Orbital Wall Fracture and Its Clinical Significance)

  • Purpose: The lateral orbital wall fractures have been previously classified by some authors. As there are some limitations in applying in their own classifications, we hope to present a refined classification system of the lateral orbital wall fracture and to identify the correlation between the specific type of the fracture and clinical diagnosis. Methods: The facial bone CT scans and medical records of 78 patients with the lateral orbital wall fractures were reviewed in a retrospective manner. The classification is based on the CT scan. In type I, the fracture and its segments are away from the lateral rectus muscle and in type II, they are next to or slightly pushing the muscle in axial CT scan. In type III, the fracture segments compress and displace the longitudinal axis of the muscle or the optic nerve in axial view of CT scan. Type IV fracture includes multiple fractures found around the orbital apex or optic canal in coronal view of CT scans of the type I and type II fractures. Results: The most common fracture pattern was type I(43.6%), followed by type IV(29.5%), type II(20.5%), and type III(6.4%). As diplopia and restriction of extraocular muscles were found in type I and II fractures, severe ophthalmic complications such as superior orbital fissure syndrome, orbital apex syndrome, and traumatic optic neuropathy were found in type III and IV fractures almost exclusively. Conclusion: We propose an easy classification system of the lateral orbital wall fracture which correlates closely with ophthalmic complications and may help to make further treatment plan. In Type III and IV fractures, severe ophthalmic complications may ensue in higher rates, so early diagnosis and treatment should be performed.
