핵의학 영상 검사 중에서 99mTc-DMSA Renal scan은 비침습적이면서도 신장의 기능과 형태를 동시에 평가할 수 있으며 좌우 양측 신장에 섭취를 분석함으로써 신기능을 정량적이면서 정확하게 분석할 수 있다. 신장의 크기 및 길이 평가도 신장 이상의 주요 평가 항목이며, 99mTc-DMSA Renal scan을 이용한 신장 길이의 측정은 호흡에 의한 흔들림이나 해상력의 한계로 인해서 CT 보다 신장의 길이가 과대 또는 과소평가 될 수 있다. 본 연구는 해부학적인 평가가 우수한 CT 검사와 기능적인 평가가 우수한 99mTc-DMSA Renal scan에서 측정한 신장 길이의 차이 및 상관관계를 분석하고, 99mTc-DMSA Renal scan에서 측정한 신장 길이의 정확성을 예측하는데 그 목적이 있었다. 신장 CT 검사와 99mTc-DMSA Renal scan을 검사한 200명의 환자 자료를 대상으로 연구 분석하였고, 통계 분석을 위해 SPSS Ver. 17.0 프로그램을 사용하였다. 호흡에 의한 영향과 해상력의 차이에 의해서 CT 검사 보다 99mTc-DMSA Renal scan에서 측정한 신장의 길이가 더 큰 것으로 분석되었다. 그러나 감마카메라와 연령에 따른 신장 길의 차이는 없었으나 신장 길이를 분석한 방사선사 별로는 차이가 있었다. 따라서 신장 길이 측정에서 방법의 표준화를 통한 정량 분석 결과의 일반화는 매우 중요할 것이며, 이는 99mTc-DMSA Renal scan에서 츠정한 신장 길이의 정확성을 향상할 수 있을 것이다. 또한 CT 영상과 99mTc-DMSA Renal scan에서 측정한 신장길이는 좌우측 신장 모두 매우 강한 양(+)의 상관관계가 있었다. 다수의 신장 질환에서 신기능 변화뿐만 아니라 길이나 크기가 변화하는 경우가 많으며 신장 길이를 정확하게 측정하는 방법과 표준화는 임상적인 진단에 매우 중요할 것으로 판단된다. 특히 소아의 경우에서는 다량의 방사선 피폭이 있는 CT 검사보다 99mTc-DMSA Renal scan을 이용하여 형태학적인 평가와 기능적인 평가를 모두 시행하는 경우 많으므로 99mTc-DMSA Renal scan에서 신장에 대한 기능적인 정보뿐만 아니라 크기나 길이, 위치 등과 같은 해부학적 정보를 정확하게 제공하는 것은 더욱 중요할 것으로 판단된다.
Purpose: To determine the prevalence, clinical manifestations, and outcomes of renal involvements in pediatric Alagille syndrome (ALGS). Methods: A total of 21 patients diagnosed with ALGS at age under 18 years who visited Samsung Medical Center from March 1999 to March 2022 were enrolled. ALGS was diagnosed either by clinical manifestations, targeted JAG1 sequencing, and/or liver biopsy. Medical records including sex, age, renal manifestations, urinalysis, serum creatinine, JAG1 sequencing, and ultrasonography were retrospectively reviewed. Results: The male to female ratio was 9:12. The mean age of patients at confirmative diagnosis of ALGS was 18.4 months. Sanger sequencing was performed for 17 patients. Sixteen of 21 patients (76.1%) showed JAG1 mutations. Renal involvement was found in 10 patients (47.6%). The most common type of anomaly was renal dysplasia (40%). One patient having renal dysplasia was pathologically confirmed with glomerular lipid deposition. Two patients (20%) manifested nephrocalcinosis/nephrolithiasis. Among eight renal-involved patients who survived, four (50%) progressed to chronic kidney disease stage 3. Two of these chronic kidney disease patients were diagnosed with hepatorenal syndrome. The other four patients had renal functions preserved, including two without any interventions and two who underwent urological interventions. Conclusions: The current study revealed a high prevalence of renal involvement in Korean pediatric ALGS with diverse phenotypes.
The accumulations of mercury, lactate dehydrogenase and antioxidant enzymes activities of which are glutathione peroxidase, catalase and superoxide dismutase, and pathological changes were investigated in liver and kidney of mice which were fed the water supplemented with two levels (0.5 mM and 1.0 mM) of mercury chloride (HgCl$_2$). During the mercury feeding, the weight gain of mice in experimental groups was less than that of control group mice, while no overt signs related to mercury toxicity were noted in any experimental groups. Mercury concentrations in liver and kidney increased significantly in the early period (1~2 weeks) after mercury administration, which were measured as high as 100 times in liver and kidney in comparison to those of the control groups, but there were relatively stable for the levels of accumulation in following periods. The lactate dehydrogenase activities in liver and kidney were relatively increased in the period of 2~3 weeks of mercury administration in the experimental groups, there were normal levels in other periods of administration without the dose-dependencies. The glutathione peroxidase activities were not affected by the dosages of mercury chloride and the duration of ingestion. But the catalase activities significantly increased in 2~3 weeks after ingestion, and the superoxide dismutase activities of kidney also showed a peak in 3 weeks of ingestion while this peak was not found in the results measured in liver tissues.
Purpose: This study was to develop and evaluate an individualized education program based on self-efficacy for patients with kidney transplantation in Korea. Methods: A nonequivalent control group pretest-posttest design was used. The participants consisted of 43 patients who underwent kidney transplants at one hospital in Seoul, from July 2012 to April 2013. The experimental group received an individualized education based on self-efficacy in the hospital and follow-up telephone consultation in the 2nd and 3rd week after discharge. The control group received a routine discharge education. Knowledge, self-efficacy, and compliance related to kidney transplant were measured and analyzed by frequency, average, Mann-Whitney U test, Fisher's exact test, independent t-test, and ${\chi}^2$-test using SPSS WIN 20.0. Results: Significant differences were found in self-efficacy between the experimental group and the control group. But no significant differences were found in knowledge and compliance between two groups. The scores of several items on monitoring health status in compliance were higher in the experimental group than those of the control group. Conclusion: The results of this study demonstrate the fact that an education program based in self-efficacy for patients with kidney transplant would be effective in improving self-efficacy and the ability to monitor their health status.
The walls of guard cells have many specialized features. Guard cells are present in the leaves of bryophytes, ferns, and almost all vascular plants. However, they exhibit considerable morphological diversities. There are two types of guard cells: the first type is found in a few monocots, such as palms and corn, and the other is found in most dicots, many monocots, mosses, ferns, and gymnosperms. In corns, guard cells have a characteristic dumbbell shape with bulbous ends. Most dicot and monocot species have kidney-shaped guard cells that have an elliptical contour with a pore at its center. Although subsidiary cells are common in species with kidney-shaped stomata, they are almost always absent in most of the other plants. In this study, there were many different stomatal features that were associated with kidney-shaped guard cells, but not dumbbell shaped guard cells, which are present in most grasses, such as cereals. Each plant investigated exhibited different characteristic features and most of these plants had kidney-shaped guard cells. However, the guard cells of Chamaesyce supina Mold, were often more rectangular than kidney-shaped. In contrast, Sedum sarmentosum guard cells were of the sink ensiform type and in Trifolium repens, the guard cells exhibited a more rhombic shape. Therefore, kidney-shaped guard cells could be divided into a number of subtypes that need to be investigated further.

Diabetic kidney disease is one of the most serious complications of diabetes. Although diabetic kidney disease can be effectively controlled through strict blood glucose management and corresponding symptomatic treatment, these therapies cannot reduce its incidence in diabetic patients. The sodium-glucose cotransporter 2 (SGLT2) inhibitors and the traditional Chinese herb "Gegen" have been widely used in diabetes-related therapy. However, it remains unclear whether the combined use of these two kinds of medicines contributes to an increased curative effect on diabetic kidney disease. In this study, we examined this issue by evaluating the efficacy of the combination of puerarin, an active ingredient of Gegen, and canagliflozin, an SGLT2 inhibitor for a 12-week intervention using a mouse model of diabetes. The results indicated that the combination of puerarin and canagliflozin was superior to canagliflozin alone in improving the metabolic and renal function parameters of diabetic mice. Our findings suggested that the renoprotective effect of combined puerarin and canagliflozin in diabetic mice was achieved by reducing renal lipid accumulation. This study provides a new strategy for the clinical prevention and treatment of diabetic kidney disease. The puerarin and SGLT2 inhibitor combination therapy at the initial stage of diabetes may effectively delay the occurrence of diabetic kidney injury, and significantly alleviate the burden of renal lipotoxicity.
The change of plasma renin activity is one of the most important parameters explaining the pathological physiology of the hypertension. The relation between renal renin activity and plasma renin activity has not well been documented since last decades. In an attempt to clarify the relationship a series of experiments have been done in rats. The following results were observed. 1) Renal renin activity of clamped kidney increased after silver clipping and the increments were maintained until four to five weeks of operation. 2) Renal renin activity of the untouched contralateral kidney was vulnerable to be suppressed just after clamping, and the activity disappered almost below the limitation of the radioimmunoassay sensitivity up to four weeks. 3) Plasma lenin activity was changed by the renal renin activity, but the regression coefficient from the two kidney one clip Goldblatt hypertensive rats was different from the sham-operated, or age-matched control rats. 4) Plasma renin activity of all the groups tested has the exponential curve in terms of renal renin activity. These data suggest that the renal renin activity is important to control the plasma renin activity in certain experimental condition, especially in chronic status.
Background: In veterinary medicine, previous studies regarding the diagnostic performance of shear wave elastography (SWE) in chronic kidney disease (CKD) are not consistent with each other. Moreover, there has been no study evaluating the relationship between symmetric dimethylarginine (SDMA) concentration and renal shear wave velocity (SWV) using two-dimensional SWE (2D SWE) in dogs with CKD. Objectives: This study aimed to evaluate the diagnostic capability of 2D SWE in dogs with CKD and to assess the relationship between renal SWV and SDMA concentration. Methods: Dogs with healthy kidneys and dogs with CKD underwent 2D SWE and SDMA assay. Renal stiffness was estimated as renal SWV in m/s. Results: SDMA concentration had a weak positive correlation with the left (r = 0.338, p = 0.022) and right renal SWV (r = 0.337, p = 0.044). Renal SWV was not significantly different between healthy kidney and CKD groups in the left (p = 0.085) and right (p = 0.171) kidneys. Conclusions: 2D SWE may could not distinguish between dogs with healthy kidney and dogs with early stage of CKD, but it would be useful for assessing the serial change of renal function in dogs.
BACKGROUND/OBJECTIVES: Increased levels of uremic toxins and decreased antioxidant capacity have a significant impact on the progression of chronic kidney disease (CKD). However, it remains unclear whether they interact with each other to mediate the damage of kidney function. The purpose of this study was to investigate whether uremic toxins (i.e., homocysteine and indoxyl sulfate [IS]), as well as glutathione-dependent antioxidant enzyme activities are dependently or independently associated with kidney function during different stages of CKD patients. SUBJECTS/METHODS: One hundred thirty-two patients diagnosed with CKD at stages 1 to 5 participated in this cross-sectional study. RESULTS: Patients who had reached an advanced CKD stage experienced an increase in plasma uremic toxin levels, along with decreased glutathione peroxidase (GSH-Px) activity. Plasma homocysteine, cysteine, and IS concentrations were all positively associated with each other, but negatively correlated to GSH-Px activity levels after adjusting for potential confounders in all CKD patients. Although plasma homocysteine, cysteine, IS, and GSH-Px levels were significantly associated with kidney function, only plasma IS levels still had a significant association with kidney function after these parameters were simultaneously adjusted. In addition, plasma IS could interact with GSH-Px activity to be associated with kidney function. CONCLUSIONS: IS plays a more dominant role than homocysteine and GSH-Px activity in relation to kidney function.
Currently commercially available phantom can reproduce and evaluate only a static situation, the study is incomplete research on phantom and system which is can confirmed functional situation in the kidney by time through dynamic phantom and blood flow velocity, various difference according to the amount of radioactive. Therefore, through this study, it has produced the dynamic kidney phantom to reproduce images through the dynamic flow of the kidney, it desires to evaluate the usefulness of nuclear medicine imaging. The production of the kidney phantom was fabricated based on the normal adult kidney, in order to reproduce the dynamic situation based on the fabricated kidney phantom, in this study it was applied the volume pump that can adjust the speed of blood flow, so it can be integrated continuously radioactive isotopes in the kidney by using 99mTc-pertechnate. Used the radioactive isotope was supplied through the two pump. It was confirmed the changes according to the infusion rate, radioactive isotopes and the different injection speeds on the left and right, analysis of the acquired images was done by drawn five times ROI in order to check the reproducibility of each on the front and rear of the kidney and bladder. Depending on the speed of injection, radioisotope was a lot of integrated and emissions up when adjusting the pressure of the pump as 30 stroke, it was the least integrated and emissions up when adjusting as 40 stroke. The integration of the left & right kidney was not reached in the amount of the highest when adjusting as 10 stroke. In the changes according to the amount of the radioactive isotope, 0.6 mCi(22.2 MBq), 0.8 mCi (29.6 MBq)was showed up similar tendency but, in the result of the injection 0.8 mCi, it was showed up counts close to double of 0.6 mCi. In the result of the differently injection speed of the left & right kidney, as a result of different conditions that injection speed was 20 stroke through left kidney phantom, the injection speed was 30 stroke through right kidney phantom, it was enough difference in the resulting image can be easily distinguished with the naked eye. Through this study, the results showed that the dynamic kidney phantom system is able to similarly reproduce renogram in the actual clinical. Especially, the depicted over time for the flow to be excreted through the kidney into the bladder was adequately reproduce, it is expected to be utilized as basic data to check the quality of the dynamic images. In addition, it is considered to help in the field of functional imaging and quality control.
