편마비 환자에서 슬링을 이용한 체간-골반 안정성 집중 훈련이 근활성도 및 균형능력에 미치는 영향 (Effect of Intensive Trunk-Pelvic Stabilization Training Using Sling on Muscle Activity and Balance in Hemiplegia)

  • 본 연구의 목적은 슬링을 이용한 편마비 환자의 체간-골반 안정성 집중훈련 시 체간의 근활성도 및 균형능력에 미치는 효과를 알아보고자 하였다. 6개월 이상에서 1년 된 편마비로 진단 받은 입원 환자 20명을 대상으로 무작위 추출하여 두 그룹으로 나누었다. 실험은 4주간, 주 5회, 1회당 30분간 운동치료를 시행하였고, 대조군(n=10)은 일반적 운동훈련만 적용하였고, 실험군(n=10)은 일반적 운동훈련 및 슬링을 이용한 체간-골반 안전성 집중훈련을 실시하였다. 훈련 효과를 알아보기 위해 훈련 전 후에 체간조절능력, 근활성도, 균형능력을 각각 측정하였다. 훈련 후, 유의한 차이는 실험군에서 체간조절능력(TIS)(p<.001), 특히, 정적조절능력(p<.05), 동적조절능력(p<.01), 협응능력(p<.05), 근 활성도는 RA(p<.001), EO(p<.001), ES(p<.001), Multifidus(p<.05), FMA (Balance)(p<.01), MTD-Balance에서는 정적(p<.001), 동적균형능력(p<.001) 모두 각각 유의한 차이를 보였다. 이로써 슬링을 이용한 체간-골반 안정성 집중훈련은 체간의 근활성도와 균형능력 향상에 효과적인 치료임을 증명하였고, 균형에 있어서 체간-골반 안정화 및 조절의 중요성을 확인할 수 있었다.

  • PURPOSE: The aim of this study was to verify the effect of trunk strengthening exercise using oscillation by comparing trunk muscle thickness, as well as balance of healthy adults during exercises performed with an oscillatory device and non-oscillatory device. METHODS: Twenty-two participants were randomly assigned to one of two groups: the trunk strengthening exercise using oscillation (TSEO) group (n=11) or the trunk strengthening exercise using non-oscillation (TSEN) group (n=11). Subjects in all groups performed the exercises three days per week for 6 weeks. All subjects performed four types of exercises: pull over, seated twist, power push, and diagonal power plank. Trunk muscle thickness of the rectus abdominis (RA), internal oblique (IO), external oblique (EO), transverse abdominis (TrA), and multifidus (MT) were measured with an ultrasonography. The balance ability were evaluated using the Romberg test with eyes open, eyes closed, one-leg standing test (OLST), and limits of stability (LOS). All tests were performed before the intervention, as well as after 6 weeks and 8 weeks of exercises. RESULTS: There was a significant difference of RA, IO, TrA, and MT according to the main effect of the time (p<.05). There was a significant difference of IO and LOS according to interaction effect between the time and group (p<.05). CONCLUSION: As intended, the cyclic forces induced by the oscillating device did increase trunk muscle thickness. However, the effect was limited and significant only for the IO muscle. Combining trunk strengthening exercise with oscillation appears to be more effective in improving dynamic balance.

  • Objectives: This study investigated the correlation among Postural Assessment Scale for Stroke (PASS), Timed "Up and Go" Test (TUG) and gait (velocity, cadence, step-length, stride-length and single-limb support). Methods: The 70 subjects were assessed on trunk control measured with the use of the PASS, dynamic balance (TUG) and gait function (by GAITRite). The data were analyzed using Pearson product correlation. Results: The PASS total scores were significantly correlated with PASS-M, PASS-C, and PASS-T (r =.80 ~ .88 p<.01). All items of the PASS were significantly correlated with TUG (r = -.63 ~ -.81 p<.01), velocity (r = .44~.58 p<.01), cadence (r =.38 ~.51. p<.01), affected side step length (r = .44 ~.56 p<.01) and affected side stride length (r = .45 ~.59 p<.01). But affected side single-limb support was lowly correlated with PASS-M, PASS-C, PASS-T and PASS-total (r = .25~.36 p<.05). Conclusions: Measures of trunk control were significantly related with values of dynamic balance and gait. Based on these results, trunk control is an essential core component of balance and gait. Trunk control training programs after stroke should be developed and emphasized.

시각 통제를 이용한 다중감각 훈련이 아급성 뇌졸중 환자의 균형 및 체간위치감각에 미치는 효과 (The Effects of Balance and Trunk Repositioning Sense with Multisensorial Training using Visual Cue Deprivation in Subacute Stroke Patients)

  • 본 연구는 시각 통제를 이용한 다중감각 훈련이 아급성 뇌졸중 환자의 균형 및 체간위치감각에 미치는 영향을 알아보고자 시행하였다. 뇌졸중 환자 30명을 대상으로, 다중감각 훈련군은 시각통제 후 훈련을 실시하였고, 대조군은 안대로 눈을 가린 것을 제외하고는 실험군과 동일한 훈련을 실행 하였다. 두 군 모두 훈련 과정은 4주간 주 5회 60분 동안 실시하였다. 대상자들은 사전, 사후 평가로 정적 균형, 동적 균형 및 체간위치감각을 검사하였다. 본 연구의 결과는 다중감각 훈련군에서 훈련 전, 후를 비교한 결과 정적 균형, 동적 균형, 체간위치감각에서 유의한 향상이 나타났고(p<.05), 두 그룹 사이에서는 동적 균형, 체간위치감각에서 유의한 차이가 나타났다(p<.05). 본 연구의 결과를 통하여 다중감각 훈련은 동적 균형 및 체간위치감각을 향상시키는데 효과적인 훈련방법으로 제시 할 수 있다.

만성 요통환자에 대한 PNF와 체간운동프로그램이 통증, 기능장애 및 균형에 미치는 효과 (The Effects of PNF Technique Versus Trunk Exercise Program on the Pain, Disability and Balance in Chronic LBP Patients)

  • 본 연구는 만성요통환자 30명을 대상으로 PNF와 체간 운동프로그램을 적용한 다음 환자들의 통증, 기능장애 그리고 정적균형과 동적균형에 미치는 효과에 대해 알아보기 위해 실시하였다. 각 군의 운동효과를 측정하기 위해 모든 대상자는 통증과 함께 Oswestry 장애지수와 외다리기립검사를 통해 정적균형을 측정하였고, Balance 측정기를 이용하여 동적균형을 측정하였다. 그 결과 통증, Oswestry 장애지수, 외다리 기립검사 및 동적균형 모두 체간운동군에 비해 PNF 군이 유의한 차이가 있었다(p<0.05). 따라서 이번 연구결과를 통해 알 수 있는 것은 PNF의 내려치기와 들어올리기 패턴이 만성 요통환자에게 효과적이라는 것을 알 수 있다.

정상인과 뇌졸중 환자의 체간 위치감각 비교 및 보행과 균형에 미치는 영향 (Comparison Between Stroke Patients and Normal Persons for Trunk Position Sense and It's Relation to Balance and Gait)

  • Background: Stroke patients have reduced trunk control compared to normal people. The ability to control the trunk of a stroke patient is important for gait and balance. However, there is still a lack of research methods for the characteristics of stroke control in stroke patients. Objects: The aim of this research was to determine whether trunk position sense has any relation with balance and gait. Methods: This study assessed trunk performance by measuring position sense. Trunk position sense was assessed using the David back concept to determine trunk repositioning error in 20 stroke patients and 20 healthy subjects. Four trunk movements (flexion, extension, lateral flexion, rotation) were tested for repositioning error and the measurement was carried out 6 times per move; these parameters were used to compare the mean values obtained. Subjects with stroke were also evaluated with clinical measures of balance and gait. Results: There were significant differences in trunk repositioning error between the stroke group and the control group in flexion, lateral flexion to the affected side, lateral flexion to the unaffected side, rotation to the affected side, and rotation to the unaffected side. Mean flexion error: post-stroke: 7.95 ± 6.76 degrees, control: 3.32 ± 2.27; mean lateral flexion error to the affected side: 6.13 ± 3.79, to the unaffected side: 5.32 ± 3.15, control: 3.57 ± 1.92; mean rotation error to the affected side: 8.25 ± 3.09, to the unaffected side: 9.24 ± 3.94, control: 5.41 ± 1.82. There was an only significant negative correlation between the repositioning error of lateral flexion and the Berg balance scale score to the affected side (-0.483) and to the unaffected side (-0.497). A strong correlation between balance and gait was found. Conclusion: The results of this study indicate that stroke patients exhibit greater trunk repositioning error than age-matched controls on all planes of movement except for extension. And lateral flexion has correlation with balance and gait.

편측 뇌성마비아의 볼기 테이핑이 앉은 자세에서 골반경사 및 몸통 안정성과 균형에 미치는 영향 (The Effects of Gluteal Taping on Pelvic Alignment, Trunk Stability, and Balance during Sitting in Children with Unilateral Cerebral Palsy)

  • Purpose: The aim of this study was to investigate the effects of gluteal taping on pelvic alignment, trunk stability, and balance during sitting posture in children with unilateral cerebral palsy (CP). Methods: Thirteen children with unilateral cerebral palsy (six females. seven males; mean age 8.5) participated in this study. All participants were evaluated before and after gluteal taping using an Inclinometer for pelvic lateral inclination, trunk impairment scale (TIS) for trunk stability, and modified functional reaching test for balance during sitting. The collected data were analyzed using a paired t-test. Results: The results of this study were as follows: 1) Statistically significant decreases in the angle of pelvic lateral inclination were observed after gluteal taping in children with unilateral CP (p<.05). 2) Statistically significant increases in TIS score were observed after gluteal taping (p<.05). 3) Statistically significant increases in the range of reaching during sitting were observed after gluteal taping (p<.05). Conclusion: : In conclusion, this study showed that gluteal taping improves pelvic alignment, trunk stability, and balance during sitting in children with unilateral cerebral palsy. Further studies will be required to determine the short- and long-term effects of gluteal taping on improving postural symmetry, trunk stability, and balance.

  • Purpose : This study aimed to determine whether task-oriented trunk training can improve muscle strength, muscle activity, balance, and gait in stroke patients. Methods : A total of 27 stroke patients who agreed to participate in the study were randomly divided into the following two groups: (1) experimental group 1, task-oriented training applied to the proximal part (n=14) and (2) experimental group 2, task-oriented training applied to the distal part (n=13). Thereafter, task-oriented trunk training was accordingly applied in each group for 60 minutes per session, 5 times per week for 6 weeks. Muscle power, muscle activity, balance, and gait were assessed using a digital dynamometer, surface electromyograph, Timed Up and Go (TUG) test, and gait analyzer (G-WALK), respectively, before and after training. Results : Trunk muscle strength significantly increased in both groups after training (p<.05). and there was a significant difference between the groups. Muscle activity in the stance phase during gait significantly increased in both groups after training (p<.05), and there was a significant difference between the groups. Muscle activity in the swing phase during gait significantly increased in both groups after training (p<.05), and there was a significant difference between the groups. The TUG test values significantly increased in both groups after training (p<.05), and there was a significant difference between the groups. Gait significantly increased in both groups after training (p<.05), and there was a significant difference between the groups. Conclusion : The results of this study show that task-oriented training can improve trunk muscle strength, muscle activity, balance, and gait in stroke patients.

불안정면에서 시각차단이 체간과 하지의 근활성도에 미치는 영향 (The Effect of Visual Deprivation on Trunk and Lower Extremity Muscle Activity on an Unstable Surface)

  • Purpose: Visual information is one of the most important factors for postural balance. The purpose of this study was to investigate the effect of visual deprivation on the lower extremities and trunk muscle activity on an unstable surface during quiet standing. Methods: Fifteen healthy males from a university population participated in this study. Surface electromyography of the rectus abdominis, the erector spinalis, the vastus femoris oblique, the semitendinosis, the tibialis anterior, and the medial gastrocnemius was measured using EMG equipment (Telemyo 2400, Noraxon, USA). The participants were asked to maintain postural balance on an unstable surface (Balance pad, Airex, USA) for 30 seconds with eyes open and with eyes closed during quiet standing. The Wilcoxon test was used to compare the muscle activity of the lower extremities and the trunk between open and closed eyes on an unstable surface. Results: The muscle activity of the trunk and the lower extremities was increased more with eyes closed than with eyes open on the unstable surface during quiet standing. Conclusion: The findings from this study suggest that visual information could alter lower extremity and trunk muscle activity. Therefore, visual deprivation can be used as a method to improve postural balance.

Effects of Sling Exercise on Pain, Trunk Strength, and Balance in Patients with Chronic Low Back Pain

  • Purpose: The purpose of this study was to examine the effect of sling exercise on pain, trunk strength, and balance in patients with chronic low back pain in their 40s and 50s. Methods: Twenty patients with chronic low back pain were divided into two groups, 10 patients in the exercise group using a sling and 10 patients in the waist stabilization exercise group, applying a random assignment, draw-out method, and sling exercise was applied for 50 minutes a day, 3 times a week, for a total of 5 weeks. For data analysis, a corresponding t-test was performed for within-group changes and an independent t-test was performed for inter-group changes, and the significance level was α=0.05. Results: After 5 weeks, there were significant differences in all items of within-group changes, and the inter-group changes after 5 weeks demonstrated significant differences in pain, trunk flexion strength, and balance. Conclusion: The results of this study showed that exercise using a sling had positive effects on pain, trunk flexion strength, and balance changes in chronic low back pain patients. It is suggested that sling exercise can be used as an intervention method for pain reduction and functional improvement of patients with chronic low back pain in clinical practice.
