The Application of Customer Relationship Management for the Effective Prenatal Care (효과적인 산전관리를 위한 고객관계관리(CRM)의 도입)

  • The prenatal care is the preventive medical service to help the pregnant mother deliver the healthy baby. It's regular examines give some chances to check-up the healthy conditions. This thesis concentrates on the CRM system to support an effective prenatal care system and prove the effectiveness of it. As CRM is the adapted management related to the customer's own information, it is important to develop the CRM model classified by the patients characteristics. A general hospital in Busan operated the CRM system to carry out the effective prenatal care and there is an analysis to ensure the effectiveness of CRM system for the pregnant women in our maternity ward. The results can be summarized as follows: 1) According to the comparisons with the CRM system, we can conclude the system is desirable. (1) Maternal Age : In the age distribution, the prenatal visit frequency, triple marker freqency, oral GTT and targeted ultrasonography in the experimental group in 30 to 34 years old is higher on the whole. For over 35 years old group, the higher frequency comes out in the oral GTT and targeted ultrasonography and for 25 to 29 years old group the different figure shows just in the targeted ultrasonography. (2) Area of residence: There is a clear difference in all the items in Busan and near area but no sign of difference in prenatal visits and oral GTT in other residencial area. Especially in the targeted ultrasonography the higher figure shows in the experimental group located in the both areas. The targeted ultrasonography is known as the specific examination which should be examined by the specialists, on the contrary the other examinations can be operated in the small clinic. So the public information and seminars related with ultrasonography increases the check-up frequency. The clinic requests some ultrasonographical examinations to the specialists in general hospital. (3) Parity: The clear difference shows that the CRM system causes the prenatal visit frequency to become higher in experimental group. The figure is 9.7 times and 8.6 times each. This is opposite that the past study said multiparity reduced the average prenatal visits. But the result of CRM is considered as the method to help the multiparity understand the importance of the prenatal care. (4) Obstetrical history: In the experimental group of the spontaneous delivery group, the figure is higher in the prenatal visit frequency, triple marker, oral GTT and targeted ultrasonography but the Caesarean section delivery group has higher figure in targeted ultrasonography. (5) In the first check-up, the rate of targeted ultrasonography in under 16 week pregnancy, in the 16 week pregnancy to 32 week pregnancy and the over 32 week pregnancy in the experimental group is upper than the compared one. For the oral GTT, there is a difference in under 16 week pregnancy but no difference in prenatal visits and triple marker. 2) The analysis of characteristics of prenatal care through the decision tree resulted in the fact that the most important variable is the residential area. After the delivery frequency is following, the obstetrical history and maternal age are in order. It is the same result in the triple marker and oral GTT. Consequently it is the same order of important variables in CRM system. The effectiveness of CRM system is proved in this study. The CRM system is a marketing method to control and lead the customers through the segmentation of customer data. It increases the new customer aquisition, maintenance of loyal customers, augmentation of customers value, activation of potential customers and creation of life time customers. So eventually it can enlarge the customers value. The medical institution should make efforts to establish the data base enforced by the customer's information on the underlying ordinary data system to carry out the CRM system effectively. In addition, it should develop the a variety of marketing strategy in order to set up one to one marketing satisfying the needs of individual patients.

A Study on Reasons for Planting Abies holophylla by Imperial Japan in Korean Temples (주요 사찰에 일제가 심은 전나무 노거수의 식재의미 연구)

  • This study investigates how Abies holophylla was planted as a tree sacred to the gods by Japan during the late Chosun and Japanese colonial periods. This study surveyed the current conditions of old Abies holophylla geological information, year planted, and the relationship between Abies holophylla and traditional Japanese values. The reasons that Abies holophylla was considered a tree sacred to the gods are as follows. Firstly, trees are planted on the spaces between Iljumun and Numun in Buddhist temples of Korea in the same way as trees are planted on the roads for worship in traditional temples of Japan. Secondly, tree diameter at breast height ranged from 60cm to 100cm. This means that the Abies holophylla trees range in age from 70 years to 138 years, showing that the trees were planted at the period between the beginning of Japanese Buddhism(1877) and the Japanese Colonial Period (1945). Thirdly, conclusions regarding why the Abies holophylla were planted in Korean temples at the beginning of Japanese Buddhism will be drawn based on the records that Jingu-Kogo's Conquest of Silla was taken care of the Suwataisha as a tree sacred to the gods in Japanese tradition. The reason for which Abies holophylla was planted in Korean temples should be evaluated through the remnants of Japan in the past. The issue of the old Abies holophylla in Korean temples should be investigated and further discussed in depth.

Development of prediction model identifying high-risk older persons in need of long-term care (장기요양 필요 발생의 고위험 대상자 발굴을 위한 예측모형 개발)

  • In aged society, it is important to prevent older people from being disability needing long-term care. The purpose of this study is to develop a prediction model to discover high-risk groups who are likely to be beneficiaries of Long-Term Care Insurance. This study is a retrospective study using database of National Health Insurance Service (NHIS) collected in the past of the study subjects. The study subjects are 7,724,101, the population over 65 years of age registered for medical insurance. To develop the prediction model, we used logistic regression, decision tree, random forest, and multi-layer perceptron neural network. Finally, random forest was selected as the prediction model based on the performances of models obtained through internal and external validation. Random forest could predict about 90% of the older people in need of long-term care using DB without any information from the assessment of eligibility for long-term care. The findings might be useful in evidencebased health management for prevention services and can contribute to preemptively discovering those who need preventive services in older people.

The Variation Factors of Severity-Adjusted Length of Stay in CABG (관상동맥우회술 시행환자의 중증도 보정 재원일수 변이에 관한 연구)

  • Our study was carried out to analyze the variation factors of severity-adjusted length of stay(LOS) in coronary artery bypass graft(CABG). The subjects were 932 CABG inpatients of the Korean National Hospital Discharge In-depth Injury Survey from 2004 through 2008. The data were analyzed using $x^2$ test and the severity-adjusted model was developed using data mining technique. The results of the study were as follows: male(71.1%), older than 61 years of age(61.6%), more than 500 beds(92.8%) and admitting via ambulatory care(70.0%) appeared to have higher rate than otherwise. In-hospital mortality of CABG inpatients was 2.8%. In addition, 46.4% of the patients received their care in other residence. The angina pectoris(45.6%) was found to be the highest in principle diagnosis, followed by chronic ischemic heart disease(36.9%) and acute myocardial infarction(12.0%). We developed severity-adjusted LOS model using the variables such as gender, age and comorbidity. Comparison of adjusted values in predicted LOS revealed that there were significant variations in LOS by location of hospital, bed size, and whether patients received the care in their residences. The variations of LOS can be explained as the indirect indicator for quality variation of medical process. It is suggested that the severity-adjusted LOS model developed in this study should be utilized as a useful method for benchmarking in hospital and it is necessary that national standard clinical practice guideline should be developed.

A Review of the List of Plant Diseases in Korea and the Names of Korean Tree Diseases (한국식물병명목록과 우리나라 나무병 이름에 대한 소고)

  • Since the List of Plant Diseases in Korea (DisList) was first published in 1986, the 6th edition appeared 36 years later. In 2023, the 6.1 edition, a revised and improved version of the 6th edition, was released to the public on the web free of charge. The contents of DisList increased, with the number of hosts increasing from 437 taxa to 1,420 taxa and the number of disease species increasing from 1,539 to 6,680. Among these, tree diseases are 3,586 species and their hosts include 504 taxa, providing much help to experts who need them. Meanwhile, the importance of accurate disease names continues to grow with the legalization of tree care, but many disease names are still inappropriate or misused, causing confusion. Disease names that do not follow the naming regulations are still registered, and even if the same pathogen infects hosts of the same taxa, the disease names are given differently, and there are many disease names that do not indicate the characteristics of the disease. Also, there are diseases reported without Korean names. In order to make DisList better, the review committee for disease names should establish the regulations to review and register disease names, and establish a system to review new disease names before publishing papers.

  • It is becoming more important as the growing of health information and increasing in cancer patients diagnose over the time gradually. Among the various types of cancer, we focuses on breast cancer diagnosis. The accuracy of breast cancer diagnosis is increasing when the diagnosis is based on evidence and statistics. To do this we use the weka data mining tools and analysis algorithms significantly associated with the decision tree uses rules. In addition, the data pre-processing and cross-validation are used to increase the reliability of the results. The number and cause of the disease becomes important to increase evidence-based medical doctors. As the evidence-based medical, the data obtained from patients in the past through the disease by calculating the probability for future patients to diagnose and predict disease and treatment plan. It can be found by improving the survival rate plays an important role.

  • The safety-critical software in healthcare systems needs more and more perceptive excess among human observation and computer support. It is a challenging conversion that we are fronting in confirming security in healthcare systems. Held in the center are the patients-the most important receivers of care. Patient injuries and fatalities connected to health information technologies commonly show up in the news, contrasted with tales of how health experts are being provided financial motivation to approve the products that may be generating damage. Those events are unbelievable and terrifying, however they emphasize on a crucial issue and understanding that we have to be more careful for the safety and protection of our patients.

  • 본 연구는 데이터마이닝 기법을 이용하여 건강보험청구료에 있어서 이상정도가 심한 요양기관을 탐지하고, 실제 의료영역에 적용하기 위한 시스템 개발을 목적으로 한다. 현재 건강보험 심사평가원의 이상탐지시스템은 평가대상이 되는 항목을 개별적으로 평가하고, 탐지된 기관의 선정 이유에 대한 근거제시가 부족한 단점을 가지고 있다. 따라서 본 연구에서는 항목을 종합적으로 평가할 수 있는 정량적 지표를 설계하고, 항목들의 상대적 중요도를 파악할 수 있도록 항목들에 대한 가중치 부여한다. 또한 지표에서 얻어진 값으로 등급을 구분하고, 의사결정나무기법(decision tree)를 이용하여 해석력을 높이는 방법을 제시한다.

Missing teeth after maxillofacial trauma: a case report and management protocol

  • Management of maxillofacial trauma includes primary care, in which diagnosis and management of dentoalveolar injury play a vital role. Due to the impact sustained during a maxillofacial injury (whether direct or indirect), dentoalveolar injuries can occur, leading to fracture and displacement of teeth and associated alveolar bone into the surrounding soft tissues and associated structures, such as the maxillary sinus, nasal cavity, upper respiratory tract, tracheobronchial tree, or gastrointestinal tract. Undiagnosed displaced teeth may cause complications such as airway obstruction. This paper reports a case of displaced teeth in the nasal cavity and gastrointestinal tract and highlights the management protocol for displaced teeth secondary to maxillofacial trauma.
