Objectives: The purpose of this study is to develop a clinical practice observation course in a medical school that can give Korean medical students opportunities to experience the treatment environment in western medicine, and to analyze the results of the course. Methods: A total of 47 (collection rate: 85.5%) senior students of the College of Korean medicine in Gyunggi-do replied to the survey. Twenty-seven of them were male and 20 were female. They participated in the clinical practice observation course in the medical school two days a week for two weeks (total: four days) in the academic year 2011-2012. Before the course, an orientation was held; and upon the students' completion of the course, they presented their experiences and replied to the survey. Results: According to the survey, the purpose of the course was well explained (91.5%), and the course was necessary for Korean medical students (97.9%). The four-day course duration was inappropriate, though (83%). The female students were more satisfied with their participation in the fourth year and with the timing of the course than the male students were. The students who finished the course at the surgical departments recognized the necessity of the course more than the students who finished the course at the non-surgical departments. The advantages of the course included opportunities to observe operation scenes and experience clinical treatment processes and systems. The weakness of the course included poor organization due to the short preparation period. Conclusions: The course was confirmed as necessary for Korean medical students so that they would understand the treatment environment in western medicine. Through the course, the students broadened the understanding of the disease and developed a strong sense of identity as Korean medical students. The duration of the course needs to be extended, and its structure should be enhanced. Furthermore, various educational programs that offer Korean medical students opportunities to experience various clinical cases need to be introduced.
An account book of medical treatment is a form of collection materials for diagnostic standardization, and it is a basis of standardization, standardization of medical records is a preconsideration of each standardization. But an account book of medical treatment is only a kind of form for recording medical treatment, therefore standardization of medical treatment eventually holds the key to the standardization of recording charts. However until now we have gradually reformed medical records in accordance with individual characters of medical treatment, and didn't have even standard sheme of medical records, also medical terms for medical records had an inconsistency of redescription and reiterative representation for an identical terms in all parts of the East learning, medical terms for medical records didn't unity. To make better this realities, standardization study used orginated system in the process of existing study, it can get ready the basis of discussion between O.M.D and O.M.D. it can make analysis of diagnostic course and can clearly understand usable information by diagnostic course. for that reason we hope that the basis of standardization is accomplished. And in advance of study for this standardization we have to analysis the course of medical treatment with demonstration of roof, first of all we have to study term definition by diagnostic course and prepare basis by diagnostic course. because this study have limits of indivisual study, it needs to long and synthetic investigation in Association levels. Although we cann't completely alternate with methods of measurement which relyed on individual mastery, if we exclude erroes of individual measurement through mechanization and verify results of diagnosis through keynotes, we can realize standardization of medical treatment with demonstration of proof and in this process we can use medical records as a tool collecting exact data, also we can realize standardization of drawing up medical records.
Crohn's disease is characterized by chronic inflammation involving any portion of the gastrointestinal tract. Treating Crohn's disease is a major challenge for clinicians, as no curative therapy currently exists. Pediatric Crohn's disease is characterized by frequent relapses, a wide extent of disease, a high prevalence of extraintestinal manifestations, and a severe clinical course. The classic therapeutic approach is known as the 'step-up' strategy, and follows a progressive course of treatment intensification as disease severity increases. Although this approach is usually effective for symptom control, many patients become either resistant to or dependent on corticosteroids. The efficacy of infliximab suggests that, rather than a progressive course of treatment, early intense induction may reduce complications associated with conventional treatment and improve quality of life. Intensive early therapy with infliximab is known as the 'top-down' strategy. Such therapy offers the potential for altering the natural history of Crohn's disease, and is changing treatment paradigms. However, the relatively new concept of an early aggressive or 'top-down' treatment approach is not yet widely accepted, especially in pediatric patients. The results of our current study demonstrate that early and intensive treatment of pediatric Crohn's disease patients with infliximab, at initial diagnosis, was more effective for maintaining remission and reducing flares.

Objectives: This article describes the effectiveness of, and student satisfaction with, the medical interview course. Methods: In the years 2010 and 2011, forty-two students took the medical interview course at the School of Korean Medicine, Pusan National University. The evaluation of the course was conducted using student questionnaires. Results: The students believed that learning medical interview skills was very helpful, and there was an increase in the number of students who thought they could improve their skills by learning the medical interview course contents. Also, they have come to feel that 'building a relationship' is the most important skill in the medical interview process. We used experiential teaching methods such as doctor-patient role-playing, and the students rated that method better than others. In addition, the students responded that the appropriate time to take a medical interview course is in the middle of a clinical practice. They also thought that the current credit value of the course was appropriate. Moreover, the students wanted to add some specific contents about specialized communication skills for Korean medical treatment. Conclusions: The medical interview course received a favorable evaluation from the students who finished the course. Among the course contents, they were most concerned with building the doctor-patient relationship. Also, they preferred experiential teaching methods more than didactic methods. The current timing and credit value of the medical interview class was evaluated as appropriate, and the students felt that communication skills specific to Korean medical treatment need to be taught. Therefore, we conclude the medical interview course is important and is worth considering as an independent course.

Objectives: This study aimed to explore the influence of depression severity, disease course, treatment status, and other factors on cognitive function in adolescents with depressive disorders. Methods: Participants who met the inclusion criteria were enrolled in the study. Sociodemographic data of each participant were recorded, including age, sex, and family history of mental disorders. Zung's Self-Rating Depression Scale was used to assess depression status in adolescents. Moreover, P300 and mismatch negativity (MMN) were used to objectively evaluate the participants' cognitive function. Results: Only 26.8% of the adolescents with depression received standard antidepressant treatment. The latencies of N2 (267.80±23.34 ms), P3 (357.71±32.09 ms), and MMN (212.10±15.61 ms) in the adolescent depression group were longer than those in the healthy control group (p<0.01). Further analysis revealed that the latency of MMN was extended with increased levels of depression in adolescents. The MMN latency was short in participants with depression receiving standardized treatment. Furthermore, the latency of MMN was positively correlated with the severity and duration of depression (correlation coefficients were 0.465 and 0.479, respectively) (p<0.01). Conclusion: Receiving standardized treatment and shortening the course of depression can reduce cognitive impairment in adolescents with depression.
44 Temporomandibular Disorders(TMDs) patients with non-trauma and non-iatrogenic origin pressented at Wonkwang University Dental Hospital. They were grouped into experimental subjects and 85 persons without TMSDs were classified into control groups. The objectives of this study was to investigate the effects of recent life changes on the symptom severity and treatment course of TMDs. For that purpose, the author used several scales and indices, namely, Social Readjustment Rating Scale(SRRS) devised by Holmes and Rahe, SRRS-Korean revision by Hong and Jeong, Helkimos Anamnestic index, Clinical Dysfunction index, and Visual Analogue Scale treatment index(VAS Ti) transformed from VAS by the author. Data resulted from the investigation were collected by scale or index and processed with SPSS. The obtained results were as follows : 1. Life change units(LCU) and values of indices of experimental group were higher than those of control group. 2. Life change units recorded with SRRS-Korean revision were higher than those with original SRRS in all cases and significant positive correlations existed, between he two Therefore, clinical use of original SRRS in Korea is reliable and valid. 3. In experimental group, LCU were positively correlated with Helkimos Clinical Dysfunction index and VAS treatment index, but in control group LCU were not correlated with any items. From this, increase of life changes in patient with TMDs is likely to aggravate TMJ dysfunction and more likely to complicate treatment course.

Glutaric aciduria type 1 (GA1; OMIM #231670) is a rare autosomal recessive-inherited neurometabolic disorder caused by the deficiency of glutaryl-CoA dehydrogenase (GCDH), which is encoded by the GCDH gene. It results in the accumulation of glutaric acid (GA), 3-hydroxyglutaric acid (3-OH-GA), glutaconic acid, and glutarylcarnitine (C5DC). These metabolites are considered to damage the striatum through an excitotoxic mechanism. The treatments of GA1 known to date are metabolic maintenance treatment based on a low-lysine diet and emergency treatment during acute illness. However, treatment after the onset of neurological symptoms has limited effectiveness and is associated with poor outcomes, and the effect of treatment and disease course after treatment are not good. After the implementation of newborn screening, the incidence of acute encephalopathic crisis fell to 10%-20% with early diagnosis, preventative dietary management, and aggressive medical intervention during acute episodes. Recently, several cohort studies have been published on the natural course and treatment of GA1 patients. This mini review will cover the clinical symptoms, natural history, and treatment of GA1 through a literature review.

Objectives The purpose of this study is to examine the correlation of antibiotics administration duration and antimicrobial resistance by reviewing domestic and foreign literatures. Methods We searched literatures dated up to 23 February, 2018 in PubMed and Cochrane Library using terms of "Anti-Bacterial Agents", "Carrier State", "Nasopharynx", "Drug Administration Schedule", and also searched via RISS (Research Information Service System), KISS (Koreanstudies Information Service System), DBpia (DataBase Periodical Information Academic) using terms of antibiotics, resistance, and dose. Results In comparison with shortened and standard antibiotic course, longer treatment duration is associated with greater antimicrobial resistance or non-significant difference, but we cannot find literature that shortened antibiotic course increases antimicrobial resistance on human nasopharyngeal flora. Conclusions Currently, there is no evidence that completing the standard antibiotic course reduces antimicrobial resistance. It can be a strategy for reducing antibiotic use to apply Korean medicine treatment, as well as short-course antibiotic therapy or delayed antibiotic prescription. Additional well-designed trials should be conducted in domestic and foreign settings about the appropriate duration of antibiotic therapy.
In order to investigate the relation of subjective complaints to objective course of treatment in pulmonary tuberculosis, one hundred and seventy four cases registered at the University Health Center in Chonnam University were divided into two groups, complaint group and non-complaint group. By analysing the record at the University Health Center during these seven years from 1973 to 1979, following results were obtained: 1. In complaint group who had subjective symtoms, thirty eight cases(77.6%) among forty nine cases were detected. at clinic visit. In non-complaint group who had not any symtoms, one hundred and twenty one cases (96.8%) among one hundred and twenty five cases were detected at compulsive physical examination. 2. Comparisons of complaint group with non-complaint group were not statistically significant in many aspects, but significantly higher cases were discharged at the University Health Center to receive other medical service in complaint group than in non-complaint group. (6.1% vs 0.8%, respectively) On the contrary, non-complaint group refused medication more than two times compared with complaint group in the course of treatment (29.9% vs 12.2%, respectively). 3. In the average time lag from initial diagnosis to initial treatment, comparisons between complaint vs non-complaint group were significant (7.8 vs 28.3 days, respectively). 4. Subjective symptoms were thought to create motivation to receive therapy, and influence of motivation to therapy was strong at initial stage but it become weaker as time passed.
