Surgical Treatment of Varicose Vein - TIPP(Transilluminated Powered Phlebectomy) - (정맥류 환자의 수술적 치료 -광투시 전도형 정맥적출요법-)

  • The present study was done to determine the efficacy and safety of varicose vein removal using a minimally invasive, powered vein-extracting device with cutaneous transillumination and tumescent anesthesia techniques and then compared this to a retrospective group of conventional phlebectomy operations. There were 133 limbs in 104 patients(72 women, 32 men) treated with the use of the vein extractor aided by transcutaneous illumination. The hydrodissection was performed with Trivex™ Irrigated illuminator(Smith & Nephe $w^{ R}$) system using normal saline after the 2~3mm sized skin incision. Varicose clusters were extracted by the use of TriveTM esector(Smith & Nephe $w^{ R}$) system under transillumination. After the varisoce vein extraction, the operation area was compressed with surgical pad for bleeding control. The complication rate was 3.84% The mean number of incisions was 3.24 and mean operative time per limb was 65.9 minutes. The mean hospitalization was 1.86 days. The varicose vein extraction using transilluminated powered phlebectomy(TIPP) is a safe, efficacious and cosmetically satisfactory method. The procedure decreases the operating time and the number of incisions required to remove varicose clusters. Further evaluation and long term follow up will be necessary to determine the recurrence rate and long termcomplications.to determine the recurrence rate and long termcomplications.

Establishment of ethanol-pretreating animal model to study Helicobacter pylori infection (Helicobacter pylori의 in vivo 연구를 위한 ethanol-pretreating animal model의 개발)

  • A stable and reliable Helicobacter pylori (H. pylori) infection animal model would be necessary for evaluating vaccine efficacy and helpful for understanding the pathological mechanism of the organism. The aim of the present study is to investigate the effect of ethanol treatment prior to H. pylori inoculation on associated gastric mucosal injury and to establish ethanol-pretreating animal model to study H. pylori infection. Male Mongolian gerbils were used for the study. H. pylori was orally inoculated after 12 h fasting. 3 h prior to H. pylori inoculation, a group of gerbils was orally treated with absolute ethanol, 60% and 40% ethanol respectively. Another group of animals was treated either with H. pylori culture media alone or with different concentrations of ethanol plus culture media. Gerbils were killed 4 or 8 weeks after H. pylori inoculation. The colonization of H. pylori was confirmed by both histological examination and rapid urease test. Mucosal damage was evaluated grossly and histologically according to the criteria. The colonization of H. pylori and pathological changes in gastric mucosa of the animals were also observed. Although no significant change to the gastric mucose was observed in the animals treated either with H. pylori culture media alone or with different concentrations of ethanol plus culture media, persistent H. pylori infection was seen in the mucosa and mucosal leucocyte infiltration and severe epithelial damage was observed in the Helicobacter and ethanol + Helicobacter groups after 4 weeks. The gross and histological scores were higher in the ethanol + Helicobacter than in the Helicobacter alone group. As the results, ethanol-pretreatment with 60% concentration induced severe pathogenic changes by H. pylori infection in 5 weeks-old Mongolian gerbils. These results suggested that ethanol-pretreatment before H. pylori inoculation could increase the severity of gastric mucosal inflammation and enhance the colonization of H. pylori. The established ethanol-pretreating animal model would contribute to screen new drugs against H. pylori and be used as an useful tool for various animal experiments with H. pylori strains.

Non-Steroid Anti-Inflammatory Agents for Management of Cold Sweating in Advanced Cancer Patients (식은 땀을 호소하는 진행성 암 환자에서 비스테로이드성 항염증 제제를 이용한 치료)

  • Purpose: Advanced cancer may accompany cold sweat as paraneoplastic symptom. Few studies have been performed on the efficacy of non-steroid anti-inflammatory drug (NSAID) in advanced cancer patients who sweated without fever. Methods: To select study participants, medical records were retrospectively reviewed for patients who satisfied the following criteria: 1) incurable, advanced solid cancer; 2) Cold sweating of 4 or higher on the numeric rating scale (NRS) 4; 3) No evidence of infection or hypoglycemia; 4) No newly started opioid or anti-hormonal agents within one month; 5) NSAID prescription for the management of cold sweating and 6) Documented NRS information before and after NSAID administration. Results: A total of 13 patients were selected after excluding four patients due to lack of NRS information or fever. The mean age was 59 years old (range: 50~71), and nine patients (69%) were male. Bile duct cancer was the most common primary tumor followed by pancreatic cancer, gastric cancer and prostate cancer. The mean NRS of cold sweating dropped from baseline 6.5 (min-max: 4~10) to 1.9 at the follow-up assessment (min-max: 0~5). The mean follow-up period was 9.1 days (range: 2~30 days) from NSAID treatment to assessment. Conclusion: NSAID was effective medication for management of sweating without fever in patients with advanced cancer.

The Effect of Korean Medical Treatments with Postural Yinyang Correction of Temporomandibular Joint on Bell's Palsy (턱관절 자세 음양 교정술을 병행한 한방치료가 벨마비에 미치는 영향)

  • Objectives : The purpose of this study is to find out the effect of Postural Yinyang Correction of TMJ(Functional Cerebrospinal Therapy; FCST) on Bell's palsy. Methods : We reviewed the medical records of 41 patients who were treated for Bell's palsy at the Dept. of Acupuncture and Moxibustion, Dong-Eui University from January $1^{st}$, 2016 to August $31^{st}$, 2016. The patients were divided into 2 groups: Group A(n=21), and Group B(n=20). Patients in Group A were treated by Korean Medical Treatments with FCST using ABA(Accurate Balancing Appliance), a standard intra-oral appliance. Patients in Group B were treated by Korean Medical Treatments without FCST. To estimate the efficacy in recovering palsy, we analyzed the House-Brackmann Grading Scale and Yanagihara's Unweighted Grading System. Results : In both groups, H-B grades were improved significantly during each period except the period from the first visit to one week later(P01). The improvement of H-B grade during each period in Group A is higher than Group B except the period from one week later to two weeks later, but statistically insignificant. In Group A, Y-score was significantly improved during each period except during P01. In group B, Y-score was significantly improved during each period except during P01 and the period from the first visit to two weeks later. The improvement of Y-score during each period in Group A is higher than Group B, but statistically insignificant. Conclusions : FCST using Standard Intraoral Appliance may be helpful in treating Bell's Palsy, but the further researches are needed.

  • Osteoporosis is a major bone disorder defined as having bone mineral density (BMD) of 2.5 standard deviations or more below the peak bone mass. Osteoporosis will increasingly be a major disorder that faces the aging mankind. It is the result of an imbalance in the bone remodeling system, where bone constantly undergoes a cycle of resorption by osteoclasts and formation by osteoblasts. Estrogen deficiency in women following menopause is identified as the predominant reason that causes disparity in this system. Current medical treatments for osteoporosis include hormone replacement therapy (HRT), biphosphonates, and teriparatide, but have various side effects that raise questions concerning their medical safety and practicality. Alternative treatments involving natural product sources are under study to find a safer therapy. Many natural sources including lactoferrin and isoflavones and numerous traditional herbal medicines exhibit anti-resorptive or anabolic effects on bone and thus show promises to provide therapeutic agents in treating osteoporosis. Unfortunately, the majority of natural product treatments are still in its preliminary stages to prove their efficacy even though the development pace of treatment for osteoporosis is astounding in the past few decades. Further progress in pre-clinical studies and the subsequent clinical studies will someday lead to a breakthrough that takes us another step forward in science.

The Effect of Ion Water and Premixed Mineral Supplementation on the Growth Performance, Carcass, and Meat Quality Parameters in Finishing Pigs (이온수 및 복합광물질첨가 비육돈의 생산형질, 도체 및 육질 특성에 미치는 영향)

  • This study investigated the effects of ion water and premixed mineral supplementation on the growth performance, carcass, and meat quality parameters in finishing pigs ($LY{\times}D$). Each 20 pigs were randomly allotted to three treatments; CON (basal diet), T1(CON diet added with active water), T2 (T1 diet added with 1.0% premixed mineral). Used ion water and premixed mineral were consisted mainly of Zn and Si, respectively. Daily weight gain feed efficacy were the highest in T2, while fat content was the highest in control (p<0.05) than the others. However, the slaughter qualities including dressing percent, backfat thickness, marbling score, meat color, meat texture, drip loss, meat separation and meat qualities including pH, cooking loss, water holding capacity (WHC), shear force, meat color, backfat color, meat texture were not significantly different (p>0.05) among the treatment groups. Panel test scores of tenderness were the highest in T2 compared to the others.

Specificity in Weed Competition and Herbicide Response of Soybean under Foggy Condition (인공안개처리에 따른 콩의 잡초경합 양상 및 제초제반응 특이성)

  • Greenhouse experiment was carried out to investigate the specifics in growth, weed competition, and herbicide response of soybean(Glycine max) under foggy condition. Plant height and leaf stage of soybean under foggy condition were greatly increased through all growing stage comparing with non-foggy condition. At 100 days after seeding, plant height of soybean also was increased by 14% while number of pods, number of grains per pod were reduced by 16, 24 and 23%, respectively comparing with non-foggy condition. On the other hand, plant height of soybean under foggy condition was reduced with increasing the duration of competition and this tendency was similar to non-foggy condition. Plant height was reduced by above 50-day competition. Number of pods per plant of soybean was reduced regardless of foggy condition. Among the herbicides treated, quizalofop caused slightly crop injury under foggy condition, but the other herbicides did not affect growth of soybean. Number of pods and 100-grain weight of soybean by treatment of herbicides were similar to that of the untreated control but those of soybean under foggy condition were significantly reduced. At 40 days after herbicide application, weeding efficacy (based on shoot fresh weight) of herbicides treated under foggy and non-foggy condition, released exellent data(above than 97%) in all regardless foggy condition.

Biological control of Paraconiothyrium minitans CM2 on Lettuce Sclerotinia Rot Caused by Sclerotinia sclerotiorum (Paraconiothyrium minitans CM2의 상추 균핵병균(Sclerotinia sclerotiorum)에 대한 생물적 방제)

  • A mycoparasite, Paraconiothyrium minitans CM2 was selected for biological control of sclerotinia rot of lettuce caused by Sclerotinia sclerotiorum. The experiment was carried out in a lettuce greenhouse in Yangpyeong from March to April.. When lettuce sclerotinia rot showed in the early stage of occurrence, Conidial suspension of the mycoparasite was weekly treated once to three times onto soil surface around lettuce plants. Incidence of sclerotinia rot in the once-application plot of the mycoparasite ($1{\times}10^7$ spores/$m{\ell}$) and in the benomyl(WP)-treated plot was 11.0% and 2.7%, respectively, whereas that of control was 31.0%. Incidence of twice- and three-application plots of the isolate was 7.9% and 12.8%, respectively. For increasing the effect of the mycoparasite, the experiment for the timing of application of P. minitans CM2 was carried out in a lettuce greenhouse in Yangpyeong and Suwon. Control efficacy against lettuce sclerotinia rot in the soil-drenching plots of P. minitans CM2 ($5{\times}10^6$ spores/$m{\ell}$) in the planting was 75.3~84.7%, and control effect by treatment of the isolate at the pot drenching+the soil-drenching plots in the early stage of disease occurrence was 63.8~58.0%. As the results, P. minitans CM2 could be a prospective biofungicide for biological control of sclerotinia rot of lettuce.

Effect of Bordeaux Mixture on Control of Rice Leaf Blast (벼 유기재배에서 석회보르도액을 이용한 벼 잎도열병 방제 효과)

  • Recently organic farming practice of rice has been emerged in Korea, but one of the major limiting factor is the no effective environmental-friendly agro-materials to control major plant diseases. Bordeaux mixture has been used effectively as a preventive agro-chemical. The aim of this study was to investigate efficacy of Bordeanux mixture on control of rice blast caused by Magnaporthe grisea which is one of the disruptive rice diseases in world-wide. In greenhouse experiment, pre-treatment of 6-6 type of Bordeaux mixture before inoculation of spore suspension of M. grasea showed 71 % of control value. In field experiment, preventive applications of 4-8 and 6-6 types of Bordeaux mixture showed over 71 % of the control value. Chemical injury on rice leaves were not found in the application concentrations of all types of Bordeaux mixture, but observed in applications of Bordeaux mixtures between 30 and 100 diluted concentrations. This results indicate Bordeaux mixture can be used as an effective environmental-friendly agro-chemical to control rice blast disease in the field.

Development of Biofungicide Using Bacillus sp. KBC1004 for the Control of Anthracnose of Red Pepper (길항세균 Bacillus sp. KBC1004를 이용한 고추탄저병의 생물학적 방제제 개발)

  • To develop an effective biopesticide to control pepper anthracnose disease, an isolate which showed strong inhibitory effect on the mycelial growth and conidial germination of Colletotrichum acutatum was selected among the antagonistic bacterial isolates collected from pepper grown soil. The bacterial isolate was identified as Bacillus sp. KBC1004 using 16S rRNA sequence analysis. The liquid culture of KBC1004 was freeze-dried and formulated as a wettable powder(WP). The wettable powder form of KBC1004 required at least 24 hours to activate and to inhibit the conidial germination of C. acutatum. In vitro bioassay using the detached green pepper fruits, biocontrol activity of the WP was not recognizable in simultaneous inoculation, but significant disease suppression was observed pre-treatment (24 hr) of the WP before pathogen inoculation. In field experiment, 4 times foliar applications of the 1/500 diluted wettable powder from the end of June showed great control efficacy similar to that of the chemical fungicide application. These results suggest that the formulated WP product could be an alternative mean to control of pepper anthracnose disease in environmentally friendly farming practices.
