Anti-allodynic Efficacy of NMDA Antagonist Peptide and Noradrenaline Alone and in Combination in Rodent Neuropathic Pain Model

  • Background: The present experiment was conducted to identify the cooperative effect of serine histogranin (SHG) and noradrenaline in alleviating peripheral neuropathic pain. Methods: Chronic constriction injury of the right sciatic nerve was used to induce chronic neuropathic pain. For drug delivery, a PE10 tube was inserted into the subarachnoid space. Acetone drops and a $44^{\circ}C$ water bath were used to evaluate the cold and heat allodynia, respectively. Placing and grasping reflexes were used to assess the locomotor system. Results: SHG at 0.5 and $1{\mu}g$significantly (P < 0.05) decreased the thermal allodynia. The cold allodynia was also significantly reduced by intrathecal injections of 0.5 (P < 0.05) and $1{\mu}g$(P < 0.001) of SHG. $1{\mu}g$of noradrenaline, but not $0.5{\mu}g$, significantly alleviated the cold (P < 0.01) and thermal (P < 0.05) allodynia. The ameliorating effect of noradrenaline or SHG disappeared when the two compounds were administrated in equal concentrations. A significant difference (P < 0.01 in the acetone and P < 0.05 in the heat) was observed in the groups under equal doses of the two compounds, with a lower effectiveness of the combination therapy. Conclusions: Our findings suggest that the simultaneous administrations of noradrenaline and SHG do not result in synergistic analgesia, and combination therapy may not be a good approach to the treatment of chronic neuropathic pain syndrome.

Residue analysis of spinetoram and spinosad on paprika leaf using the modified QuEChERS pre-treatment methods

  • Spinosad and spinetoram are widely used insecticides for the control of lepidopteran larvae, leaf miners, and thrips; however, they might also have low toxicity toward beneficial insects like bees. Because these pesticides are easily photolyzed by ultraviolet radiation, the QuEChERS method, with its simple pretreatment procedure, is often used for analyzing residues of spinosad and spinetoram. The present study performed a residue analysis using a modified QuEChERS method by pretreating with ammonium salt. The limit of detection (LOD) of the modified method was 0.05 mg/kg and the limit of quantification (LOQ) was 0.25 mg/kg. The coefficient of determination ($R^2$) for the calibration curve was 0.999. Also, we examined any change in the adhesion of spinosad and spinetoram on the plants depending on a spray volume. The adhesion was approximately 70% when the spray volume was increased from 60 L to 120 L per 10 a whereas the adhesion was approximately 37% when the spray volume was increased from 125 L to 250 L. This showed that the amount of adhesion decreased with the higher spray volume. The efficacy result of spinetoram was that over 90% of Frankliniella occidentalis was controlled with the application volume of 125 L per 10 a. Therefore, the result of this study indicates that control of insects is effective and sufficient with a spray volume of 125 L per 10 a in paprika cultivation facilities.

The Anti-Inflammatory Effect of Lonicera Japonica-Glycyrrhiza Uralensis Decoction on Ulcerative Colitis Induced by DSS in Mice (항염증조절을 통한 금은화-감초 복합 추출물의 DSS 유도 궤양성 대장염 완화 효과)

  • Objectives The purpose of this study is to investigate the anti-inflammatory effect of Lonicera Japonica-Glycyrrhiza Uralensis decoction extracts (LGE) on ulcerative colitis in children and adolescents. Methods Colitis was induced by DSS (Dextran Sulfate Sodium) in C57BL/6 mice. The sample mice were divided into group of four. The mice in the control group were not inflammation-induced. The control group was composed of untreated ulcerative colitis elicited mice. The mice in the experimental group were administered with Pentasa and another experimental group mice were treated with LGE after colitis elicitation. The effects on ulcerative colitis were evaluated by the morphological changes of colonic mucosa, decrease in the effect of pro-inflammatory cytokines ($TNF-{\alpha}$ and $NF-{\kappa}B$) and inflammatory cytokines (iNOS and COX-2) in the mucosa. Results LGE showed protective effects in DSS induced ulcerative colitis. LGE inhibited shortening of colon length and relieved the hemorrhagic erosion in colonic mucosa. LGE decreased pro-inflammatory cytokines ($TNF-{\alpha}$ and $NF-{\kappa}B$) and inflammatory cytokines (iNOS and COX-2). According to the GC/MS analysis, N-methyl pyrrolidone (NMP) was identified. Conclusions The result shows the clinical efficacy of LGE and demonstrates possible treatment options for ulcerative colitis. Further investigations for biological activity and chemical analysis of LGE will be needed.

An analysis of the Gongjindan's ingredients and its efficacy on anti-oxidation (공진단(拱辰丹)의 성분 분석 및 갱산화(坑酸化) 작용에 미치는 영향)

  • Objectives : For the congenital feeble, the Gongjindan is useful medicine. The experiment is to estimate the value of the Gongjindan as therapeutic agent preventing against aging with an analysis of the ingredients and the bio-activating effects by enzymologic methods. Methods : General ingredients' of the Gongjindan's extract were analyzed first and the quantitative analysis of a reducing sugar, a soluble protein, an amino acid and minerals was made. The Gongjindan, which is extracted, concentrated, and freeze drying with water, ethanol and chloroform, have got applied for the experiment. The effects on electronic donating ability, SOD-like activity, nitric oxide inhibition, xanthine oxidase inhibition, whitening effect have been investigated in the physiological activity measurement of function experiment. Results : The contained amino acid, in order of high amount, is Arginine, Alanine, Glutamic acid, Proline and the contained free amino acid is Glutamic acid, Leucine, Lysine, Phenylalanine. The derivative of free amino acid is ${\gamma}-Aminoisobutyric$ acid, Phosphoserine, Taurine. And the Gongjindan is containing 13 species of minerals in order of Ca>K>Na>Mg>Fe>AI>Mn. Then, to assure of the Gongjindan's capability of anti-oxidation, these following subjects-polyphenol, electronic donating ability, SOD-like activity, nitric oxide inhibition, xanthine oxidase inhibition, tyrosinase inhibition- are analyzed and show high activity especially the most in ethanol extracts. Conclusion : With this analysis of ingredients, the Gongjindan is containing many materials functioning as anti-oxidation, anti-aging, anti-fatigue, neurotransmitter and immune agent. Moreover, In every water, ethanol, chloroform extracts, the Gongjindan's capability of anti-oxidation is confirmed so that we can apply to patients' treatment clinically.

Effects of Mixed Pills of Chokong (Pickled Black Soybeans) with Medicinal Herbs on Body Weight Gain and Lipid Profiles in Rats Fed High-fat Diet (한방초콩환이 고지방식이를 급여한 흰쥐의 체중 및 지질 변화에 미치는 영향)

  • Objectives : In this study we investigated the antiobese effects of mixed pills of pickled black soybeans with herbs(herbal Chokong pill, hereafter HCKP) in rats fed high-fat diet. It was evaluated by measuring the changes of body weight, adipose tissues weight and lipid profiles in serum. Methods : Black soybeans were pickled in vinegar for 15 days to prepare Chokong, at room temperature. For treatment group, HCKP was prepared, which five kinds of medicinal herbs(Rhynchosia nulubilis, Glycyrrhiza uralensis, Zizyphus vulgaris, Atractylodes macrocephala K, Astragalus membranaceus and Cornus officinalis) were added to dried Chokong. Four groups of male Sprague-Dawley rats were fed different diets during 9 weeks: normal diet containing 5%(w/w) com oil, high-fat diet containing 10%(w/w) lard plus 5%(w/w) corn oil (HF), high-fat diet supplemented with 1%(T1) and 5%(T5) HCKP powder, respectively. Results : The T5 group had markedly lower body weight gain and weights of epididymal adipose tissue when compared with HF group. There were significant differences in visceral adipose tissue weights, serum total cholesterol and triglyceride concentrations between the HF and T5 group. Then, the efficacy of powered HCKP on body weight and lipid profiles change in rats fed high-fat diet were induced dose dependantly. Conclusion : These results suggest that the possibility of HCKP, as an antiobese functional formula, by suppression of body weight gain and improved lipid profiles.

Interspinous Implant with Unilateral Laminotomy for Bilateral Decompression of Degenerative Lumbar Spinal Stenosis in Elderly Patients

  • Objective : This study assessed the safety and efficacy of one level unilateral laminotomy bilateral decompression (ULBD) with the placement of a device for intervertebral assisted motion (DIAM) compared with one level ULBD only in elderly patients with degenerative lumbar spinal stenosis (DLSS). Methods : A non randomized prospective analysis was performed on 16 patients who underwent one level ULBD with DIAM (Group A) and 20 patients with one level ULBD only (Group B) between February 2007 and March 2008. Radiographic imaging, visual analog scale (VAS) and MacNab outcome scale were obtained before and after surgery at a mean interval of 21 months (range 17-27 months). Results : The disc height, interpedicular distance, slip distance and segmental lordotic angle were similar between two groups. In the group A, there was no significant difference between the pre- and post-operative imaging in terms of the sagittal balance and disc height. Both groups showed significant improvement in the clinical outcomes. In addition, there was significantly less low-back pain in the group A than in the group B at the last follow up, while the clinical improvement of the leg pain and MacNab outcome scale showed no significant difference in the two groups. There were no major complications or DIAM associated complications. Conclusion : ULBD with DIAM is a safe and efficacious treatment for selective elderly patients with DLSS, particularly for relieving low back pain comparing to ULBD. ULBD with DIAM did not alter the disc height or sagittal alignment at the mean 21 months follow-up interval.

  • Interest in application of ultraviolet light technology for primary disinfection of potable water in drinking water treatment plants has increased significantly in recent years. The efficacy of disinfection processes in water purification systems is governed by several key factors, including reactor hydraulics, disinfectant chemistry, and microbial inactivation kinetics. The objective of this work was to develop a computational fluid dynamics(CFD) model to predict velocity fields, mass transport, chlorine decay, and microbial inactivation in a continuous flow reactor. The CFD model was also used to evaluate disinfection efficiency in alternative reactor designs. In a typical operation, water enters the inlet of a UV lamp and flows through the annular space between the quartz sleeve and the outside chamber wall. The irradiated water leaves through the outlet nozzle. In this paper, it describe the how to design optimal ultraviolet disinfection device for ground water and rainwater. To search the optimal design method, it was performed computer simulation with 3D-CFD discrete ordinates model and manufactured prototype. Using proposed design method, performed simulation and proved satisfied performance.

A Bibliographic Studies on the Bufonis Venenum for Clinical Treatment: Important to Toxicity and Processing (섬수의 임상적 활용을 위한 문헌적 고찰 -독성 및 수치를 중심으로-)

  • Objectives : The purpose of this study is to find out the clinical possibility of Bufonis Venenum used without toxicity and side effect. Methods : We investigated the pharmacological effects, toxicity and processing of Bufonis Venenum through the literatures and studies. Results : Bufonis Venenum is made by parotid gland of dermato gland of Bufo bufo gargarizns or B. melanostictus Schneider, and it is dried for using. The medical ation of Bufonis Venenum are cardiotonic, respiration stimulation, anticancer, topical anesthesia. The toxic symptoms of Bufonis Venenum are relative with digestive, circulatory, nervous system similared with digitalis toxicity. It is important to take 0.015-0.03g by mouth, external use about 1-4% 0.5-3ml and 2-8ml injections by 20ml mix to 5% dextrose fluid. Bufonis Venenum is processed to prevent toxicity and evaluate efficacy by alcohol and milk. There are 68 prescription consisted by Bufonis Venenum in KTKP(Korean Traditional Knowledge Portal). They usually use for antiabcess, anticancer with Moschus moschiferus(麝香), Cinnabar(朱砂). Conclusions : The results from above literary studies show that internal, external medicine and Aqua-acupuncture of Bufonis Venenum could be clinically used to sedative, antiinflammatory, anticancer and topical ataralgesia without toxity through optimum dose and processing.

A CLINICAL STUDY OF ANESTHETIC EFFICACY OF ALKALINIZING LIDOCAINE IN INFERIOR ALVEOLAR NERVE BLOCKS (하치조신경 전달 마취 시 알칼리화 된 리도카인의 마취 효능에 관한 임상적 연구)

  • Inferior alveolar nerve block using lidocaine is the most frequent local anesthetic method in the dental treatment, but clinically it is not always successful. The 2% lidocaine cartridge has been used commonly in dental anesthesia. It contains vasoconstrictor and antioxidant, which presents low pH which provides chemical stability and longer shelf life. But alkalinized local anesthetics has less tissue trauma, easier dissociation of the non-ionized base which penetrates nerve sheath, rapid onset and more intensity. In this study, in inferior alveolar nerve block, alkalinized lidocaine using sodium bicarbonate (experimental group) is compared with plain lidocaine (control group) about injection pain, anesthetic onset, duration and postinjection discomfort. In inferior alveolar nerve block, alkalinized lidocaine using sodium bicarbonate showed lower injection pain. There was significant difference statistically from plain lidocaine(p=0.019). Comparing with plain lidocaine, alkalinized lidocaine produced more rapid onset (lip & pulp anesthetic onset), there was no significant difference(p>0.05). but there was boundary significance (0.050.05). These results suggest that addition of sodium bicarbonate to 2% lidocaine(1:100,000 epinephrine) for inferior alveolar nerve block is more effective for reduction of injection pain and onset time.

Survey on the Satisfaction of the Medication Counseling for Outpatient Prescription (원외처방환자에 대한 복약지도 현황 및 만족도 조사)

  • Medication counseling improves patients' compliance, which enhances the effectiveness of treatment and reduces the medical cost consequently. Since separation of dispensary from medical practice took place, most patients have had to go to pharmacy after receiving prescription from hospital. The importance of medication counseling in pharmacy thus has been emphasized. To study the present conditions of medication counseling from the pharmacists and the patients satisfaction with them, we conducted a survey with questionnaires. The subjects were 146 outpatients and 55 pharmacies located in Yeongdeungpo-gu. The research showed that 69.9% of the outpatients had received medication counseling and only 35.5% of them were satisfied with it. The main reason for their unsatisfaction was insufficiency of explanation. A number of patients(75.3%) were thinking that medication counseling from the pharmacists is necessary for appropriate administration and optimal efficacy of the medicine. Among 55 pharmacies involved in the research, 17 of them(30.9%) answered that they have been giving patients medication counseling, which were mostly verbally done. Only 8 of them(l4.5%) were providing medication information sheets for some specific drugs. The pharmacists referred to a few problems disturbing optimal medication counseling like these: 1) Lack of time, 2) insufficient information, 3) inappropriate counseling skills. To improve these problems, they hope to get more information about prescription and specific medicines from pharmacy in hospital. If hospital decides to hold the lectures on medication counseling, about 80% of the pharmacists tend to take part in them.
