The Efficacy of Induction Chemotherapy in Stage III Non-Small Cell Lung Cancer (제 3기 비소세포 폐암에서 유도 화학 요법의 효과)

  • Purpose : This study was performed to analyze the efficacy of induction chemotherapy fellowed by radiation therapy in locally advanced non-small cell lung cancer Materials and Methods : Eighty patients with locally advanced non-small cell lung cancer treated from 1989 to 1995 at Pusan Paik hospital were analyzed retrospectively. Twenty-one patients were treated with induction chemotherapy followed by radiation therapy and Fifty-nine Patients were treated with radiation therapy alone. Chemotherapy regimen consisted of cisplatin-based combination (2 or 3 drugs). All patients were treated by Co-60 or 6 MV linear accelerators. Radiation dose ranged from 50 Gy to 80 Gy (median 64.8 Gy). We evaluated response rate, survival rate, and pattern of failure in both treatment groups. Results : Overall response rate in induction chemotherapy group and radiotherapy alone group were 48% and 45%, respectively. Of the 80 patients, 46 patients were evaluable for pattern of failure. Initial failure pattern in induction chemotherapy group was as follows: 8 (67%) at locoregional, 4 (33) in distant metastasis. Radiation alone group was 21 (71%) and 5 (29%), respectively. Results showed no difference of distant failure between induction chemotherapy group and radiation alone group. The 1 and 2 year survival rate in induction chemotherapy group were 43% and 14%, respectively and in radiotherapy alone group, 31% and 7%, respectively (p=0.135). Conclusion : In stage III non-small cell lung cancer, induction chemotherapy and radiation therapy showed increased tendency in survival with no statistical significance Induction chemotherapy seems to have no effect of decreasing distant failure and no survival advantage compared with radiotherapy alone.

A Study of Goiwhasan's Antigastric ulcer and Blood Hemostasis (괴화산(槐花散)이 항소화성궤양(抗消化性潰瘍) 및 혈액(血液) 응고작용(凝固作用)에 미치는 실험적(實驗的) 연구(硏究))

  • The purpose of this research was to investigate the efficacy of Goiwhasan extract powder on the gastric injuries, antiulcer, gastrointestinal tract and blood hemostasis. Animals were used through this studies mice and rats. All animals were divided into 3 groups, contol group(no treatment), sample Ⅰ group(375mg/kg administration), sample Ⅱ group(750mg/kg administration). The gastric injuries and ulcer have been made by using pyloric ligation, indomethacin, HCI-ethanol, acetic acid and then The histological observation was followed. In the gastrointestinal tract, gastric juice secretion, gastric acidity, pepsin output, blood gastrin and secretin level, transport potentials in the small and large intestine were checked. And studies on blood hemostasis were performed on normal hemostatic activities and plasma prothrombin time, plasma recalcification time, plasma fibrinogen levels in the hypoprothrombinemic mice induced by warfarin. The results were as follows: 1. The antigastric ulcer effects on the pyloric ligation, indomethacin, HCl-ethanol, acetic acid induced gastric injuries were shown in Sample Ⅱ group(p<0.05). 2. Through the morphologic examination on the acetic acid induced ulcer, Sample Ⅰ group showed mild regeneration of epithelium and slight decrease of periulcer edema then that of Control group, while Sample Ⅱ group showed more retraction of round ulcer site, remarkable loss of swelling and edema then that of Control group, and revealing the regenerated epithelium in the surrounding ulcer site. Thus it was noted that both Sample groups have antigastriculcer effects on the experimentally induced gastric ulcer. 3. The inhibitory effects on gastric juice were noted in both Sample Ⅰ group(p<0.05) and Sample Ⅱ group(p<0.01). However, only Sample Ⅱ group showed the inhibitory effects on total acidity and pepsin output(p<0.05). 4. The significant inhibition of blood gastrin level showed at 30 min.(P<0.05) and 90 min.(P<0.05) after starting medication in only Sample Ⅱ group, but significance of blood secretin level in both groups was not recognized. 5. Any significant changes in barium sulfate transport in the small intestine of mice was not recognized in both groups, but the significantly inhibitory effect in large intestine was recognized in both Sample Ⅰ group(p<0.05) and Sample Ⅱ group(p<0.001). 6. In hemostatic effect on both normal mice and hypoprothrombinemic mice induced by warfarin, the significantly shortening effect on coagulation time was seen in only Sample Ⅱ group(p<0.01). 7. On plasma prothrombin time in hypoprothrombinemic rat induced by warfarin, Sample Ⅱ group have shortened the prothrombin time significantly(p<0.001). 8. On plasma reclcification time in hypoprothrombinemic rat induced by warfarin, the recalcification time have been shortened significantly in both Sample Ⅰ group(p<0.05) and Sample Ⅱ group(p<0.01). 9. On plasma fibrinogen levels in hypoprothrombinemic rat induced by warfarin, the fibrinogen contents in Sample Ⅱ have been decreased significantly(p<0.01). Overall the above results suggest that Goiwhasan has an therapeutic efficacy on antigastric ulcer and blood hemostasis. Further studies would be needed on the interaction of its herbal medicine and its mechanism in the future.

Efficacy of the Epidural Buprenorphine for Postoperative Pain Control after Upper Abdominal Surgery (상복부 수술후 진통을 위한 경막외 Buprenorphine의 효과)

  • The use of buprenorphine by epidural route in the prevention of postoperative pain has been controversial. High lipid solubility of buprenorphine caused the same parenteral/epidural analgesic dose ratio, and the analgesic effect of epidural buprenorphine possibly due to systemic absorption, which revealed no advantages of epidural administration against parenteral injection. On the contrary, epidural buprenorphine had longer duration of action and fewer side effects than parenteral buprenorphine, which advocated the epidural use of buprenorphine. We studied the efficacy of epidural buprenorphine by comparing epidural buprenorphine with epidural morphine in terms of latency and the duration of analgesic action, and the incidence of side effects. 0.15mg and 0.3mg of epidural buprenorphine had shorter latency than 2mg of morphine. 0.3 mg of buprenorphine had longer duration of action than 4 mg of morphine. The incidence of nausea and vomiting were slightely higher in buprenorphine group than in morphine group. Voiding difficulty and pruritus were little in buprenorphine group, while the incidence of somnolence was markedly higher in buprenorphine group. Form our results we conclude that epidural buprenorphine may be useful in the treatment of postoperative pain, and but recognize both advantages and disadvantages as compared epidural morphine.

Study on Biological Activities of Adventitious Roots Cultured from Echinacea angustifolia (에키네시아 조직 배양체 추출물의 생리활성에 관한 연구)

  • The Echinacea, which has been commonly known as a species of composite herb of dicotyledonous plant, has been used in native American traditional medicine for the treatment of diseases like colds or other infections in North America. We artificially cultured the adventitious roots of Echinacea angustifolia using the bioreactor culture system from Echinacea angustifolia and evaluated the efficacy as a cosmetic ingredient for skin care. Several studies previously have reported neogenesis, wound healing and inflammatory inhibition effect of Echinacea angustifolia but other efficacies were not well known. In the present study, we investigated the cosmetic efficacy to know applicable value of adventitious roots cultured from Echinacea angustifolia as a cosmetic ingredient. The adventitious roots extract of Echinacea angustifolia has superior anti-oxidant effect and matrix metalloproteinase-2 inhibitory effect, compared to natural Echinacea angustifolia. These results indicate that the adventitious roots extract cultured from Echinacea angustifolia presents a new possibility of being applicable to skin care and anti-wrinkle products as a cosmetic ingredient.

Enhanced Pathogenicity of Bacillus thuringiensis Mixed with a Culture Broth of an Entomopathogenic Bacterium, Xenorhabdus sp. (제노랍두스 곤충병원세균 배양액의 비티 미생물 약제 약효증진 효과)

  • The entomopathogenic bacterium, $Xenorhabdus$ sp., was isolated from an entomopathogenic nematode, $Steinernema$ $monticolum$. When these bacteria were injected into the hemocoel of the diamondback moth, $Plutella$ $xylostella$, they caused significant mortality. However, the bacterium was not pathogenic when it was administered orally. This study showed that $Xenorhabdus$ sp. significantly enhanced oral pathogenicity of $Bacillus$ $thuringiensis$ (Bt) against the last instar larvae of $P.$ $xylostella$. Different ratios of culture broth of $Xenorhabdus$ sp. and Bt showed significantly different pathogenicities against $P.$ $xylostella$. In field tests, the optimal bacterial mixture significantly enhanced control efficacy against $P.$ $xylostella$ compared to Bt treatment alone. These results demonstrated that $Xenorhabdus$ sp. culture broth can be developed as a potent biopesticide by enhancing the insecticidal efficacy of Bt.

The Effect of a Multidisciplinary Team Approach on the Rehabilitation of Stroke Survivors (뇌졸중 환자를 위한 팀접근 재활프로그램의 효과)

  • The purpose of this study was to develop a multidisciplinary team approach program for stroke survivors, and to identify its effects on their rehabilitation. The team was composed of 7 members: a rehabilitation nurse, a physician, a physical therapist, an occupational therapist, a therapeutic recreational therapist, a nutritionist, and a researcher. A quasi-experimental study was performed with 36 subjects: 18 in the experimental group and 18 in the control group, using a noneqivalent control group pretest-posttest design. The experimental group participated 4 times in rehabilitation programsfocused on information and emotional support provided by the rehabilation team-and received telephone counseling from the researcher. The control group did not receive any treatment. The selection criteria for the subjects in this study were: (a) patients who were diagnosed as having had an ischemic stroke within the last year, (b) patients free of any communication disorder, (c) and those having a primary caregiver who could assist in filling out the form assessing the level of ADL. The data were collected from patients who had been discharged from a tertiary hospital, between October 1st, 1999 and September 30th, 2000. The data were analysed by $X^2$ test, Fisher's exact test, ANCOVA, Wilcoxon's rank sum test, and Wilcoxon's signed rank test using an SAS program. The results were as follows: 1. In terms of physical variables (blood pressure, grasp power, and ADL) 1) There was a significant difference in blood pressure between the two groups (systolic pressure P= .012, diastolic pressure P= .050). 2) There was also a significant difference in grasp power between the two groups (affected side : P= .012, unaffected side : P= .010). 3) There was no significant change in the level of ADL between the two groups. 2. In terms of psychosocial variables (depression, self-efficacy, self-esteem, and social activities) 1) There were no significant differences between the two groups. However, all four psychosocial variables showed a tendency to improve in the experimental group, while only two variables (depression and self-efficacy) showed a simalar tendency in the control group. 2) The level of social activities in the control group decreased significantly after a month (P= .050). 3. The level of life satisfaction improved in both groups, but no significant difference was found. Stroke has high recurrence rate and requires considerable follow-up care. The program used in this study was developed and designed for stimulting the rehabilitation process of stroke survivors. Through the program period of one month (meetings were held weekly), a positive effect was detected in physical variables, although the psychosocial variables did not improve significantly. In retrospect, a one month period may not be an adequate length of time to improve the psychosocial variables, as the stroke survivors were complicated cases, and most of them were elderly. Further research is therefore recommended by increasing the length of program, so that its effect can be more noticeable.

Browse almost circular in traditional medicine prescription, which is scattered throughout the country is difficult, and because of the loss can not be confirmed even its existence. This study is to establish the analysis and separated by organized to establish a database on the basis of prescription and mixed herbs in a variety of functional foods developed through usability evaluation methods of treatment. Therefore, the management systems of native chickens related to lack of knowledge and information to foster the continued expansion and specialized training and knowledge of the future growth engine industry through the added value of cultural projects of national knowledge resources. Finally, convergence research and evaluate the usefulness of native chickens through interdisciplinary research systems, and practical measures of functional foods and alternative medicine are presented as well.

Nano Capsulization of Ceramide and the Efficacy of Atopy Skin (나노세라마이드의 캡슐화와 아토피 피부의 치료)

  • The nano-ceramide capsulation is a technique that capsulates ceramide III and tocopheryl linoleate at the mono-vesicle to act on the horny layer in skin. In this technique, $0.5{\~}5.0\;wt\%$ of hydrogenated lecithin and $0.01{\~}2.00\;wt\%$ of lysolecithin are used as the membrane-strengthen agents of the mono-vesicle and $5.0{\~}10.0\;wt\%$ of propylene glycol and $5.0{\~}10.0\;wt\%$ of ethyl alcohol are used as solvents. Active ingredients such ceramide III and tocopheryl linoleate are utilized to enhance the moisturizing efficacy and treat atopy skin. These materials do not contain synthetic emulsifiers. The optimal conditions or nano-ceramide capsulation are such that particles pass Microfludizdizer 3 times at 1,000 bar and $60{\~}70^{\circ}C$ and pH of nano capsules is $5.8{\pm}0.5.$ The average size of particles is $63.1{\pm}7.34\;nm$ showing lucid state like water by the laser light scattering. A zeta potential value is $-55.1\pm0.84\;mV.$ Through clinical tests, the moisturizing effect (in-vivo, n=8, p-value<0.05) showed $21.15\%$ of improvement comparison to comparison-samples and $36.31\%$ of improvement compared to the state before treatment. Moreover, the effectiveness of atopy skin showed positive reaction from 10 volunteers.

EFFECTS OF ER:YAG LASER ABLATION ON THE DENTIN (Er:YAG 레이저를 이용한 치아삭제 효과)

  • The purpose of this study was to evaluate the effects of Er:YAG laser on cutting efficacy and its histologic findings. Dentin specimens of human premolars and molars were used and irradiated by Er:YAG laser with noncontact handpiece type delivery system under different treatment condition of irradiation time. Cavity pattern and volume were evaluated to determine the cutting efficacy and following results were obtained. 1. Cutting volume of sound dentin was getting larger with time immersed in water increase 2. With the condition of irradiation (150mJ, 10Hz, 30sec), surface irregularity was more increased in sound dentin comparing to carious dentin. For the light microscopic examination, dentinal tubules were opened and ash flecks and cracks were noted with inconsistence of dentinal tubules. 3. In case of 30 sec. irradiation in carious dentin, dark zone was limited to small focus whereas 1 min. irradiation, more wider, and cracks were noted in the perpendicular to direction of dentinal tubules. For the 2 min. irradiation, cavity was the widest and more cracks were found.

The Efficacy of Inhaled Corticosteroid on Chronic Idiopathic Cough (만성 특발성 기침에 대한 흡입 스테로이드의 치료 효과)

  • Background: The discomfort caused by chronic cough, that is persistent for more than 3 weeks, causes a number of patients to seek medical attention. However, the underlying disorder often remains undetermined despite thorough examinations, and is considered to be idiopathic. This study compared the efficacy of inhaled corticosteroid with conventional cough suppressants on chronic idiopathic cough. Methods: Eligible patients with chronic idiopathic cough were randomly assigned to either the inhaled fluticasone group or the codeine plus levodropropizine oral administration group. The subjects in each group took their planned medication for 2 weeks. After the trial, comparative analyses of outcomes were performed in terms of the remnant cough (%) at the end of treatment, drug compliance, and adverse drug events. Results: Seventy-seven patients were enrolled in this randomized trial; 38 to the inhaled fluticasone group and 39 to the codeine plus levodropropizine group. The remnant cough was 41.0${\pm}$35.8% in the inhaled fluticasone group, and 32.4${\pm}$32.0% in the codeine+levodropropizine group (p=0.288). Drug compliance was 95.4${\pm}$7.4% and 81.8${\pm}$18.6% in the inhaled fluticasone and the codeine+levodropropizine group, respectively (p<0.001). Nine patients had adverse drug events in the codeine+levodropropizine group compared to one in the inhaled fluticasone group (p<0.001). Conclusion: Short-term inhaled corticosteroid is not inferior to conventional antitussive agents in controlling chronic idiopathic cough without significant adverse events.
