Near Total Excision in Patients with Angiosarcoma on Scalp (혈관육종 환자에서 두피의 아전절제술)

  • Purpose: Angiosarcoma is a rare malignant neoplasm of endothelial type cells that line vessel walls. It tends to occur in aged male and the prognosis of angiosarcoma is very poor because of frequent local recurrence and early metastasis. The treatment regimen is yet to be established from its rare occurrence but the wide excision in early stage is known to be the most effective. The authors report two cases of near totally excised angiosarcoma with more than a safety margin of 5 cm. Methods: The two subjects were aged male patients, one of the two was diagnosed with angiosarcoma from our institution confirmed by the biopsy. The other one went through the wide excision with a safety margin of 2 cm and split-thickness skin graft but local recurrence was observed. The two patients underwent near total excision with more than a safety margin of 5 cm, leaving only the periosteum. After confirming that the angiosarcoma had not infiltrated the excision margin, reconstruction with split-thickness skin graft was performed. Results: Based on 6 months and 24 months post-surgery assessment, no local recurrence or remote metastasis in the lungs, liver, bones, and lymph nodes at the neck, where remote metastasis is common, was reported by the two subjects who underwent near total excision with a safety margin of 5 cm. Conclusion: Angiosarcoma has very poor prognosis from its frequent recurrence and metastasis. To enhance the survival rate of angiosarcoma patients, early diagnosis, timely surgical treatment, and radiotherapy after surgery are critical. In addition, authors suggest that it is necessary to further study the efficacy of wide excision using a wider safety margin as much as possible, and to apply this to more cases.

Evaluation of Pyunkang-tang® Administration on Hematological, Biochemical, and Protein and Lipid Profiles in Dogs

  • $Pyunkang-tang^{(R)}$ is a kind of herbal medicine and has been used for the treatment of atopy and allergic disease in humans over forty years. The purpose of this study was to evaluate the hematological, biochemical, and protein and lipid electrophoresis profiles after the oral administration of $Pyunkang-tang^{(R)}$ in healthy dogs. Fifteen clinically healthy beagle dogs were selected and orally administered either 33 ml of $Pyunkang-tang^{(R)}$ (group I, n = 5), 16.5 ml of $Pyunkang-tang^{(R)}$ (group II, n = 5), or 33 ml of distilled water (group III, n = 5) 3 times a day for 4 weeks. The results of the hematological, serum biochemical, and urine analysis did not differ significantly among the 3 groups. The oral administration of $Pyunkang-tang^{(R)}$ for 4 weeks was associated with significant changes in the serum globulin levels and an elevation in the high-density lipoprotein (HDL) concentrations in groups I and II (p < 0.05), compared to group III. The ${\alpha}1-globulin$ and ${\gamma}-globulin$ levels were significantly increased in group I, and the ${\alpha}1-globulin$ and ${\beta}-globulin$ levels were significantly increased in group II. The ${\alpha}2-globulin$ levels were significantly decreased in both groups. During the short-term evaluation of $Pyunkang-tang^{(R)}$ administration, we did not detect any specific adverse effects in the dogs. However, further evaluation of the safety and efficacy of $Pyunkang-tang^{(R)}$ for the treatment of atopic disease in dogs is needed.

Clinical Application of Oclacitinib in Dogs with Atopic Dermatitis

  • This study evaluated the efficacy and safety to determine the therapeutic responses of oclacitinib in canine atopic dermatitis (CAD) and identified factors related to the therapeutic response. Overall, 13 dogs with CAD were treated with oclacitinib for 56 days. Owners and veterinarians assessed visual analog scale (VAS) scores of pruritus and dermatitis. The examined dogs were grouped according to treatment success and failure based on changes in the VAS scores. To identify factors related to the therapeutic responses of oclacitinib, signalments (breed, sex, age, and body weight), mean progression time of CAD, mean Owner and Veterinarian VAS scores at day 0, and preexisting infection rate were compared between the two groups. Among the 13 dogs, 7 (53.8%) met the criteria of treatment success. In the success group, the Owner VAS scores were significantly lower from day 6 to 56 compared with the score at day 0 (P < 0.05). Additionally, the Veterinarian VAS scores were significantly decreased at days 14 and 42 compared with the score at day 0 (P < 0.05). There were no changes in hematological indices after the administration of oclacitinib. The most common abnormality reported was otitis externa (30.8%), followed by pyoderma (23.1%), and vomiting (7.7%). Factors related to responses of oclacitinib were not identified. This study demonstrated that oclacitinib was safe and moderately effective in dogs with CAD. This is the first report of the clinical application of oclacitinib in South Korea.

Efficacy of Chlorine for Reducing Bacterial Populations and Bacteriological Contamination on Carcass and Treatment Water at Different Stage of Poultry Processing (도계처리 단계별 도체와 처리수의 세균오염 및 염소처리 효과)

  • This study was carried out to assess the effect of the chlorine treatment into water for processing chicken products in each stage of slaughtering, with a special viewpoint related with reducing the viable number of microorganisms by which the water and the chicken body were contaminated. The mean bacterial number on chicken samples after picking process was log5.37$\pm$0.20~5.84$\pm$0.160CFU/$\textrm{cm}^2$. When assessed by standard plate count method, it was the higher one than any other processing stage in which eviscerating, pinning, packaging, and chilling was followed in order of the mean bacterial number. The coliform bacterial numbers on carcasses after sampling from different processing stages were log2.11$\pm$0.63~2.88$\pm$0.25MPN/$\textrm{cm}^2, which show almost similar numbers in each processing stage. But, after chilling process the number was decreased slightly. The bacterial counts in the water for scalding and chilling showed log3.43 $\pm$ 0.59~5.06$\pm$0.21 and log4.30$\pm$0.21~6.62$\pm$0.33CFU/$m\ell$, respectively. In the coliform counts for the water taken out from the 2nd chilling tank, the number was log1.97$\pm$0.35~2.91$\pm$0.22MPN/$m\ell$ which showed higher than those of the 1st and the 3rd chilling tank water. The effect of chlorination in reducing the bacterial numbers was accepted at the residual chlorine concentration of 1$m\ell$/$\ell$by showing the reduction from $10^8$ to $10^4$CFU level and the numbers were decreased less than 10CFU at the concentration of 5mg/$\ell$, when assessed by viable cell counts. In conclusion, these results suggested that chlorination In chilling water with final concentration of 5mg/$\ell$was strongly recommended to reduce the bacterial numbers on final chicken products.

Effects of Stephaniae Tetrandrae Rhizoma and Sinomenii Caulis on Hyperlipidemia in Rat (분방기(粉防己)와 청풍등(淸風藤)이 고지혈증(高脂血症) 흰쥐에 미치는 효과)

  • To compare the efficacy of Stephaniae Tetrandrae Rhizoma and Sinomenii Caulis which are medicinal plants used as Bang Kee(防己), the plants were combined with Astragali Radix and Lycopii Herba into 6 different medicinal decoctions. The specimen decoctions prepared were each decoctions of Stephaniae Tetrandrae Rhizoma(St), Sinomenii Caulis(Si), Stephaniae Tetrandrae Rhizoma-Astragali Radix(StA), Stephaniae Tetrandrae Rhizoma-Lycopii Herba(StL), Sinomenii Caulis-Astragali Radix(SiA) and Sinomenii Caulis-Lycopii Herba(SiL). They were dosed to rats with induced hyperlipidemia. In the groups of rats with hyperlipidemia induced by high cholesterol diet, in the group which St was dosed to, there was no significant effect on the level of Tc, TG, HDLc. And in the group which Si was dosed to, the level of Tc, TG dropped and that of HDLc rose. In the groups of rats with hyperlipidemia induced by high cholesterol diet, in the group which StA was dosed to, there was no significant change in the level of Tc, TG, or HDLc. But in the group which SiA was dosed to, the level of Tc and TG dropped and that of HDLc rose. In the groups of rats with hyperlipidemia induced by high cholesterol diet, in the group which StL was dosed to, the level of TG dropped, but there was no significant change in the level of Tc or HDLc. And in the SiL group, there was no significant change in the level of Tc or TG, but the level of HDLc rose. On diseases such as hyperlipidemia, when using drug couple of Bang Kee-Astragali Radix as a treatment, Sinomenii Caulis is recommended rather than Stephaniae Tetrandrae Rhizoma, and when using drug couple of Bang Kee-Lycopii Herba as a treatment, Sinomenii Caulis and Stephaniae Tetrandrae Rhizoma neither showed significant effect. And as Lycopii Herba interrupts the effect of Sinomenii Caulis, drug couple of Sinomenii Caulis-Lycopii Herba is not recommended.

  • Cancer is one of the most serious health problem in modern society, and there are many studies to treat this disease. We have performed the study about analyzing papers related with anti-cancerous herbs that is used in clinical treatment recently. We found 257 papers in 'Pub med', the internet site, using the key words 'cancer' and the scientific name of anti-cancerous herbs. We also have compared the herbs' efficacy in oriental medicine with the experimental results in those papers, and thought the practical uses in clinical treatments. However, research studies that reported data on anti-cancerous herbs were wholly lacking. It is recommended that further studies be performed from the point of view of oriental medicine.

Suppression of Transglutaminase-2 is Involved in Anti-Inflammatory Actions of Glucosamine in 12-O-Tetradecanoylphorbol-13-Acetate-Induced Skin Inflammation

  • Glucosamine (GS) is well known for the treatment of inflammation. However, the mechanism and efficacy of GS for skin inflammation are unclear. The aim of this study was to evaluate the effects and mechanism of GS in the mouse 12-O-tetradecanoyl 13-acetate (TPA)-induced ear edema model. TPA-induced ear edema was evoked in ICR or transglutaminase 2 (Tgase-2) (-/-) mice. GS was administered orally (10-100 mg/kg) or topically (0.5-2.0 w/v %) prior to TPA treatment. Orally administered GS at 10 mg/kg showed a 76 or 57% reduction in ear weight or myeloperoxidase, respectively, and a decreased expression of cyclooxygenase-2 (COX-2), NF-${\kappa}B$ and Tgase-2 in TPA-induced ear edema by western blot and immunohistochemistry. Role of Tgase-2 in TPA ear edema is examined using Tgase-2 (-/-) mice and TPA did not induce COX-2 expression in ear of Tgase-2 (-/-) mice. These observations suggested that Tgase-2 is involved in TPA-induced COX-2 expression in the inflamed ear of mice and antiinflammatory effects of glucosamine is mediated through suppression of Tgase-2 in TPA ear edema.

A Novel Urotensin II Receptor Antagonist, KR-36996 Inhibits Smooth Muscle Proliferation through ERK/ROS Pathway

  • Urotensin II (UII) is a mitogenic and hypertrophic agent that can induce the proliferation of vascular cells. UII inhibition has been considered as beneficial strategy for atherosclerosis and restenosis. However, currently there is no therapeutics clinically available for atherosclerosis or restenosis. In this study, we evaluated the effects of a newly synthesized UII receptor (UT) antagonist, KR-36996, on the proliferation of SMCs in vitro and neointima formation in vivo in comparison with GSK-1440115, a known potent UT antagonist. In primary human aortic SMCs (HASMCs), UII (50 nM) induced proliferation was significantly inhibited by KR-36996 at 1, 10, and 100 nM which showed greater potency ($IC_{50}$: 3.5 nM) than GSK-1440115 ($IC_{50}$: 82.3 nM). UII-induced proliferation of HASMC cells was inhibited by U0126, an ERK1/2 inhibitor, but not by SP600125 (inhibitor of JNK) or SB202190 (inhibitor of p38 MAPK). UII increased the phosphorylation level of ERK1/2. Such increase was significantly inhibited by KR-36996. UII-induced proliferation was also inhibited by trolox, a scavenger for reactive oxygen species (ROS). UII-induced ROS generation was also decreased by KR-36996 treatment. In a carotid artery ligation mouse model, intimal thickening was dramatically suppressed by oral treatment with KR-36996 (30 mg/kg) which showed better efficacy than GSK-1440115. These results suggest that KR-36996 is a better candidate than GSK-1440115 in preventing vascular proliferation in the pathogenesis of atherosclerosis and restenosis.

A Clinical Study of Medial Maxillectomy (내측상악부분절제술의 임상적 고찰)

  • Background: Lateral rhinotomy and medial maxillectomy, an en bloc resection of the medial maxillary sinus, ethmoid sinus with the lamina papyracea, medial orbital floor, and lacrimal fossa-duct, have been advocated for lateral nasal wall neoplasms as a standard approach method. Objective: This report was conducted to investigate the clinical efficacy of lateral rhinotomy and medial maxillectomy for lateral nasal wall neoplasms. Materials and Methods: We retrospectively analyzed clinical data of 31 patients who were treated at department of otolaryngology-head and neck surgery, Catholic university of Korea, school of medicine between 1990 and 1996. Results: Twenty five patients had benign lesions(80.6%). By far, the largest percentage was inverted papillomas(80%, 20/25). Of the six malignant lesions(19.4%), 33.3%(2/6) was squamous cell carcinoma and other lesions were metastatic renal cell carcinoma, adecarcinoma, transitional cell cacinoma, and hemangiopericytoma. There were a 4% recurrence for benign tumors(1/25), 5% especially for inverted papilloma(1/20), and 50% for malignant neoplasms(3/6). The overall complication rate was 9.7%. Conclusion: Despite the various approach for treatment of lateral nasal wall neoplasms including inverted papilloma, we continue to advocate a lateral rhinotomy and medial maxillectomy as the treatment of choice.

Is Salvage Vertical Partial Laryngectomy Safe in Patients with Radiation-Failure Carcinoma of the Glottic Larynx? (방사선치료 실패 후의 수직부분후두절제술은 안전한가?)

  • Objectives: Radiotherapy is an effective treatment in the initial management of T1 glottic carcinoma, but local recurrent or residual tumor growth is found in approximately 10% of the patients. Even in recurrence or residual tumor, in highly selected cases, conservation surgery with preservation of a portion of the larynx is feasible. So we investigated the efficacy of salvage vertical partial laryngectomy for recurrent glottic carcinoma after irradiation. Material and Methods: Retrospectively we reviewed a consecutive series of 10 patients treated by conservative vertical partial laryngectomy of the larynx for radiation-failure, recurrent T1 glottic squamous cell carcinoma treated at Severance Hospital from 1994 to 2002. Results: Local recurrence developed in 4 patients (40%). Two patient was salvaged by further totallaryngectomy. Successful salvage was achieved finally in 8 (80%) of the 10 patients. There was no operative mortality. Postoperative perichondritis was developed in two patients. One was resolved by conservative treatment, but the other has underwent the total laryngectomy. Conclusion: A vertical partial laryngectomy in case of recurrent glottic carcinoma after a course of irridation should be the therapy of choice when stringent criteria are observed.
