트레드밀 훈련 시 비마비측 하지에 적용한 인솔이 만성 뇌졸중 환자의 균형에 미치는 영향 -예비 연구- (Effects of Less Affected Lower Extremity Insole on Balance in Chronic Stroke Patients during Treadmill Walking -A Preliminary Study-)

  • Purpose: The study investigated the effect on chronic stroke patients' balance of a weight shift to the affected side using an insole on the less affected side during treadmill walking training. Methods: The subjects were 7 patients who had been diagnosed with stroke 6-24 months prior to the study. In each case, an insole was applied on the patient's less affected side during treadmill walking training. Each training session lasted 30 minutes and was undertaken 5 times per week for 4 weeks. Biorescue equipment that measures shifts in center of pressure was used to assess balance ability as measured by the Korea-Berg balance scale (K-BBS) before and after each training intervention. The Wilcoxon signed-rank test was used to evaluate within-group effects. Results: The results revealed statistically significant before and after differences in area, pressure, length, and mean velocity of the balance test and on K-BBS (p < 0.05). Conclusion: In chronic stroke patients, using an insole to adjust the height of the shoe on the less affected side is an effective means of increasing weight-shifting on the paralyzed side during treadmill gait training.

만성 뇌졸중 환자의 트레드밀 훈련이 족저압, 하지 근 기능, 균형에 미치는 융복합적 효과 : 메타분석 (Convergence Effects of Treadmill Training on Plantar Pressure, Lower Limb Muscle Function, and Balance in Chronic Stroke : A Meta-Analysis)

  • 만성 뇌졸중 환자를 대상으로 메타분석 기법을 활용하여 트레드밀 훈련의 융복합적 효과성을 평가하고자 한다. PICOS 기준에 의거하여 문헌을 검색한 후 총 22편의 국내 '뇌졸중', '트레드밀' 관련 연구물을 선정하였다. 메타분석을 위해 CMA 프로그램을 이용하여 효과크기를 산출하였으며, 국내 트레드밀 훈련의 전체 효과크기는 0.661(95%CI: 0.456~0.865)로 중간크기로 유의한 차이가 있었다(p<.05). 트레드밀 훈련의 효과를 족저압(1.147), 하지 근 기능(0.875), 균형(0.664)으로 나누어 효과크기를 비교한 결과, 족저압의 효과크기가 가장 크게 나타났다. 추가로 균형은 하위항목들을 나누어 분석하였고, 균형은 TUG(0.553), BBS(0.760), 정적균형지수(0.654)의 효과크기가 나타났다. 따라서 트레드밀 훈련이 만성 뇌졸중 환자의 삶의 질 향상에 긍정적인 영향을 제공한다. 트레드밀 훈련에 대한 메타분석 연구는 정보, 통신 및 의료기술의 헬스케어 융합을 향한 업계 패러다임의 전환으로 이어질 수 있을 것이다.

  • 본 연구는 뇌졸중 환자에게 하지 근력강화 프로그램이 균형, 보행 및 상지 기능에 미치는 효과에 대해서 알아보았다. 신경계 손상 환자가 있는 병원을 기반으로 해서 하지 근력 강화에 대한 연구 방법을 만들어서 진행하였고, 무작위대조시험을 적용하였다. 24명의 뇌졸중 환자를 두 군으로 분류하였다. 하지 근력 강화 프로그램그룹 (Lower extremity strengthening program group, LESPG) (12명)과 트레드밀 훈련그룹 (Treadmill trainig group, TTG) (12명)으로 실험하였다. LESPG는 마비측에 하지 근력 강화프로그램을 수행하였다. TTG는 하루 30분 동안 트레드밀 운동을 적용하였다. 평가 도구는 일어나서 보행 검사(Timed Up and Go test, TUG), 기능적 보행평가 (Functional Gait Assessment, FGA) 및 뇌졸중 상지 기능검사(Manual Function Test, MFT)를 적용하였다. 균형, 보행 및 상지 기능에 대한 두 군 간의 TUG, FGA 및 MFT 점수는 LESPG가 TTG보다 유의한 차이가 있었다(p<.01). 4주간의 하지 근력강화 프로그램의 효과는 뇌졸중 후 편마비 환자의 균형, 보행 및 상지 기능에 효과가 있었다.

속도-의존적 체중지지 트레드밀 보행이 뇌졸중 환자의 보행에 미치는 영향 (The Effect of Speed-dependent with Body Weight Supported Treadmill Training on the Ambulation of Stroke)

  • This study is the quasi-experimental study on the gait training rehabilitation. The purpose of this study is to prepare the baseline data for most suitable of gait while we were scrutinizing how the walking characters, functional walking ability, gait quality of stroke patients were affected by the gait on BWSTT (Body Weight Supported Treadmill Training) through the change of treadmill velocity and body weight support. To accomplish this purpose, this study used thirty subjects, more than 3 months post stroke, for rehabilitation who were divided between two gait training groups they received the neurophysiological physical therapy. For 6 weeks, 5 times a week for 15 minutes per session, the BWSTT group participated in 30 sessions structured speed-dependent treadmill training with 30% body weight supported, and the ratio of body weight support was gradually decreased as the patients advanced the capability of more self-support. The OGT(Over Ground Training) group received the same quantity of equal sessions like BWSTT. Firstly, we measured the absolute improvement of walking velocity (m/s), capacity(min/m) and cadence(steps/min) among walking characters. Secondly, we measured the functional walking ability such as Functional Ambulatory Category(FAC, score out of 5), Modified Motor Assesment Scale(MMAS, score out of 6) and Gait Quality Chart(score out of 41). Data analysis was performed with using SPSS 10.0 win program. The descriptive analysis was used to obtain average and standard deviation. The independent t-test and the paired t-test were used to compare both the groups about pre and post training test. Treatment effects were established by pre and post assessment. Subjects tolerated the training well without side-effects. Therefore, the results of this study were as follows; 1. There was a more significant difference from the improvement of walking velocity(0.09m/s), endurance(4.53min/m), cadence(4.20steps/min), FAC(0.26score), MMAS(0.33 score) and hip joint and pelvic of gait quality(0.39 score) ever before in the BWSTT group(p<.05). 2. There was a more significant increase from the walking velocity(0.01m/s) in the OGT group(p<.05). 3. There was a more statistical significant increase from comparing the average of walking velocity in both groups ever before(0.42m/s in BWSTT group and 0.31m/s in OGT group)(p<.05). There was a statistical significant difference from the average of cadence in both groups(61.87step/min in BWSTT group and 3.60steps/min in OGT group)(p<.05). As we can see from above, the findings suggest that BWSTT may be more effective than the OGT for improving some gait parameters such as gait velocity and cadency. This conclusion also suggest that BWSTT is more effective for the improvement of gait of stroke patients.

대퇴골 회전방지보조기를 착용한 트레드밀 보행훈련이 뇌성마비 아동의 하지배열 및 보행에 미치는 영향: 단일그룹 반복측정 연구 (The Effects of Treadmill Gait Training with Flexible Derotator of Femur Orthosis on Postural Alignment of Lower Extremities and Gait in Children with Cerebral Palsy: Single Group Rpeated Measure Design)

  • PURPOSE: The purpose of this study was to investigate the effects of flexible derotator of femur orthosis (FDO) during treadmill gait training on the quadriceps-angle (Q-angle), lateral pelvic tilt, gait speed, and number of steps in children with cerebral palsy. METHODS: Seven children with cerebral palsy who had rotational deformity of the lower extremities participated in this study. We used single group repeated measure design. The procedure consisted of baseline phase, intervention phase, and post-intervention phase. The baseline phase consisted of stretching and strengthening exercise and treadmill gait training without FDO. The treatment phase not only included the same procedures as those for baseline, but also included FDO during treadmill gait training. Postural alignment of the lower extremities was assessed with the Q-angle, and lateral pelvic tilt using the Dartfish software program. A 10-m walk test was used to evaluate gait speed and number of steps. RESULTS: For postural alignment, there was significant differences after the application of FDO (p<.05). For gait ability, there was significant differences in all phases (p<.01). CONCLUSION: These finding suggest that the application of FDO during treadmill gait training had a positive effect on the improvement of postural alignment and gait ability in children with cerebral palsy having rotational deformity.

The Effect of Backward Walking Training Methods on Walking in Stroke Patients

  • Purpose: The purpose of the present study was to examine, in stroke patients, differences between backward walking training applied on a treadmill and the same training applied on the ground. Methods: Twenty seven stroke patients were divided into a treadmill backward walking group of 14 patients and a ground backward walking group of 13 subjects. Each group performed their respective training method for 8 weeks (15 min per day, 4 days a week). Walking ability was measured using a 10 m MWS (Maximal Walking Speed) test and the GAITRite system to examine changes in walking. Cadence, stridelength, step time, step length and symmetry index of the less affected side were measured to examine changes in stance phase of the lower extremity of the more affected side. Results: 10 m MWS, cadence, stride length, step time and step length of the less affected side significantly increased and symmetry index significantly decreased after training in both groups. The treadmill backward walking group experienced a significantly greater increase in step time and step length and a significantly greater decrease in symmetry index than the ground backward walking group. Conclusion: The two walking training methods were effective for improving stability in stance phase of the lower extremity of the more affected side, but the treadmill method was more effective. The present study is meaningful in that it analyzed the effects of backward walking training methods on walking and the differences of the training methods to provide information necessary for effective treatment of stroke patients.

  • This paper examines the effects on mental fatigue of changes in the intensity of physical activity. A treadmill-equipped instrument and perception tester were used to attain several levels of physical activity In this paper, In order to determine the individual levels of physical activity of subjects, Borg-RPE scale, Heart Rate(HR) and Respiratory Quotient(RQ) were used. Also, an addition test in whole-body activity on treadmill-equipped instrument as an indicator of mental fatigue were performed. In the above experiments, the scores obtained in addition test administered before and after physical activity at each intensity level used. Restricted within the limits of this paper, the results of these tests showed that mental fatigue decreased after physical activity.

Effects of Korean Ginseng and Wild Simulated Cultivation Ginseng for Muscle Strength and Endurance

  • Muscle strength and endurance activities of Korean ginseng (Panax ginseng C. A. Meyer; KG) were compared with those of wild simulated cultivation ginseng (WCG) in mice. Fifty male ICR mice were divided into five groups: A (vehicle); B (WCG 100 mg/kg); C (WCG 500 mg/kg); D (KG 100 mg/kg); E (KG 500 mg/kg). Subsequently, the mice were subjected to the forced swimming test (FST) and treadmill test at the $4^{th}$ and $7^{th}$ weeks. The glycogen content in the muscle and blood analysis (levels of glucose, triglyceride (TG), IGF-1) were also performed immediately after the last FST and treadmill test at the $7^{th}$ week. Immobility times in FST were shorter in WCG- than KG-treated groups, and the results of the treadmill tests were also significant except for KG-treated at 100 mg/kg. The glycogen content was increased in both groups with a peak at 500 mg/kg of WCG groups. Serum concentrations of TG and glucose were decreased by administration of KG and WCG and all treated groups showed increase in the level of IGF-1 in serum. These results suggest that KG and WCG supplementations are effective in escalating the muscle strength and endurance.

뇌졸중 환자의 보행과 균형에 대한 트레드밀과 에르고미터 자전거 훈련의 효과 (The Comparison of Effect of Treadmill and Ergometer Training on Gait and Balance in Stroke)

  • Purpose : The goal of this study was to identify the effects of treadmill walking training (TW) and ergometer bicycle training (EB) on gait and balance in stroke patients. Methods : The subjects consisted of 42 stroke patients. They were randomly divided to two groups: TW(n=20) and EB(n=22). Each group trained along with the conventional physical therapy, three times a week for six weeks. The ability of gait was assessed by the 10m walk test and Timed Up and Go test(TUG). The ability of balance was assessed by Berg Balance Scale(BBS) and Balance Performance Monitor(BPM). Results : There was no significant difference between the 10m walking test and TUG groups, but there was a statistical difference between before and after the training for all groups. The result of BBS that assessed balance showed a significant reduction between before and after the training for all groups, but there was no difference between the two groups. While the two groups showed no difference in the BPM assessment, only the EB showed a significant improvement of before and after the training in each group. Conclusion : The outcomes suggest that stroke patients can improve their gait and balance performance through the TW and EB trainings. Although dynamic mean balance showed significance from EB, no significant difference was found between two groups. Even though it cannot be determined through this study which training group is more effective among the above mentioned two, it could be suggested that each training is effective to gait ability and the ability of balance of stroke patients.

협심증이 의심되어 운동부하검사를 받은 여성들의 흉통에 관한 내용분석 (Content Analysis of Experiences of Chest Pain in Women Who Underwent the Treadmill Test with an Impression of Angina Pectoris)

  • Purpose: Experience of chest pain may be different in women from that of men and this discrepancy may cause misdiagnosis of angina contribute to mortality by this disease in women. The purpose of this study was to analyze the characteristics of chest pain and responses to the symptoms in women. Method: A content analysis was conducted with interviewed data obtained from women who underwent the treadmill test Result: Seven major categories were identified from this content analysis: attributes of chest pain; accompanied symptoms; precipitating factors; relief strategies; family support; communication with physicians; and effects of chest pain on their lives. Characteristics of pain were described as heaviness, tightness, heating sensation, tearing, and others. Duration and intensity of pain varied in a wide range. Radiating pain presented in 9 patients, and the locations of radiation were throat, neck, shoulder arm and fingers. Women tended not to respond actively to their chest pain, and didn't get appropriate support either from their family or from their physicians when they reported chest pain. Conclusion: Women express non-typical as well as typical patterns of pain when they experience chest pain. Clinicians have to consider the variability of symptoms when they assess women with suspicions of angina.
