비특이성 만성요통 유무에 따른 무산소성 역치수준 비교 (Comparison of the Anaerobic Threshold Level Between Subjects With and Without Non-Specific Chronic Low Back Pain)

  • The purpose of this study was to compare the anaerobic threshold (AT) between subjects with and without non-specific chronic low back pain (NCLBP). The patient group included 15 women with NCLBP. The normal group included 15 women without NCLBP who were age-, height-, weight-, and activity level-matched. The subjects performed a Balke treadmill protocol which was symptom-limited progressive loading test. Their heart rate (HR), ventilatory gas and metabolic equivalents (METs) were measured using the automatic breath gas analyzing system. After the test, each subjects' ratings of perceived exertion (RPE) were evaluated. The visual analog scale (VAS) was assessed pre- and post-test. The independent t-test and Wilcoxon's signed-rank test were used for analysis of the data. Time, HR, the volume of oxygen consumption ($VO_2$), relative $VO_2$, and METs at the AT level of the patient group were significantly lower than those of the healthy group (p<.05). However, there were no significant differences in RPE, VAS, and breathing frequency at the AT level (p>.05). The findings of this study indicate that patients with NCLBP had a lower aerobic fitness than healthy subjects. Thus, implementation of rehabilitation program to increase aerobic fitness may be considered in patietns with NCLBP, and further studies are required to determine the etiological factors of decreased aerobic fitness.

  • 연구배경 : 심폐운동검사를 호흡곤란이나 운동제한의 주원인 감별을 위한 진단적 목적으로 이용시, 사용한 진단 알고리즘과 운동방법에 따른 분석결과에 차이가 있는지를 알아보고, 차이가 있다면 그 원인은 무엇인지 알아보고자 하였다. 연구방법 : 안정시 폐기능검사상 이상이 있는 만성폐질환자 66명, 운동시 호흡곤란을 호소하나 안정시 폐기능검사상 이상이 없는 환자 48명을 대상으로 자전거를 이용하여 증상 제한적 최대운동을 시행하고 Wasserman 등과 Eschenbacher 등이 제안한 진단 알고리즘을 이용 하여 분석하여 그 결과를 비교하였고, 건강한 의과대 32명을 대상으로 자전거와 답차를 이용하여 증상 제한적 최대운동을 시행한 후 Eschenbacher 등이 제안한 진단 알고리즘으로 분석후 운동방법에 따른 분석결과의 차이를 비교하였다. 연구결과 : 1) 호흡곤란을 주증상으로 호소하고 안정시 폐기능 검사상 미국흉부학회 기준에 부합하는 폐기능손상을 보이는 환자중 최근 2개월이내에 증상이 악화되지 않은 환자 66명을 대상으로 증상제한적 최대운동후 Wasserman 방식과 Eschenbacher 방식으로 해석후 비교하여 보았을 때 42명에서 일치하여 63.6%의 일치율을 보였다. 폐기능검사소견에 따라 구분하여 보면, 폐쇄성장애를 보인 경우는 43명중 30명으로 69.8%, 제한성장애를 보인 경우 8명중 2명으로 25%, 혼합성장애를 보인 경우 15명 중 10명으로 66.7%의 일치율을 보였다. 2) 임상적으로 심장질환은 의심되지 않으나 활동시 호흡곤란을 호소하고, 안정시 폐활량측정에서 정상소견을 보이는 48명을 대상으로 자전거를 이용한 운동 후 두 방식으로 분석 비교하였을 때 30명에서 일치하여 일치율은 60.4%이었다. 3) 호흡곤란 등의 호흡기계증상이 없고 안정시 폐활량검사상 정상인 건강한 의과대학 남학생 32명을 대상으로 자전거와 답차를 이용한 운동후 Eschenbacher 방식으로 해석한 결과 운동방법에 따른 일치율은 25% 이었다. 결론 : 이와 같은 결과로 진단 알고리즘 혹은 운동방법에 따라 결과가 차이가 있음을 알 수 있었고, 심폐운동검사를 호흡곤란이나 운동제한의 주원인 감별을 위한 진단적 목적으로 이용하기 위해서는 한국성인에 적용할 수 있는 운동방법에 따른 진단 알고리즘을 확립하는 것이 필요하다고 여겨진다.

전투화 맟춤형 발보장구 착용 시 하지 근육 활동의 평가 (The Evaluation of Lower Extremity Muscles in Combat shoes Custom Foot Orthotics)

  • 본 연구는 행군 시 발보장구 착용이 하지 근육 활동에 미치는 영향을 알아보는데 목적이 있다. 과회내족과 정상족을 선정하여 트레드밀에서 4.5km/h의 속도로 걷게 하였고 이때 근전도기를 이용하여 하지근육의 근육신호를 수집, 분석하였다. 수집한 데이터를 평균적분근전도와 스펙트럼 분석을 통해 근활성도와 피로도를 알아보았고 SPSS 12.0을 이용하여 통계분석하였다. 실험 결과는 과회내족이 정상족보다 행군 시 근 활성이 큰 것으로 나타났고, 보장구 착용 시 근 활성과 피로도가 줄어드는 경향을 나타냈다. 특히 전경골근과 장비골근의 근육활성 감소와 피로도 감소가 통계적으로 유의한 차이를 나타냈다. 결과적으로 과회내족은 장시간 행군 시 발보장구 착용이 피로도 감소에 효과적이며 이는 잠재적인 하지 근골격계 상해 예방에 도움이 될 것으로 사료된다.

트레드밀 운동학습 훈련이 경직성 뇌성마비 아동의 기능과 균형에 미치는 영향 (The Effect of Balance and Function in Children with Spastic Cerebral Palsy using Motor Learning training with Treadmill)

  • 본 연구는 트레드밀을 이용한 운동학습 훈련이 경직성 뇌성마비 아동의 운동기능과 균형능력에 미치는 영향을 알아보고자 하였다. 대동작기능분류체계(GMFCS) 제 III, IV 단계의 경직성 뇌성마비 아동 16명을 대상으로 운동학습훈련군과 대조군으로 각각 무작위로 8명씩 분류하여, 대조군은 주 4회 근력강화운동을 30분/1회 적용하였고, 운동학습훈련군은 근력강화운동과 트레드밀을 이용한 운동학습훈련을 주 4회 15분/1회 적용하였다. 뇌성마비 아동의 운동기능은 대동작기능평가를 이용하여 측정하였고, 균형능력은 good balance system(Meitur Ltd., Finland)의 전산화 측정장비를 이용하여 측정하였다. 적용방법에 따른 운동학습훈련군과 대조군의 전 후 유의성 검증은 Wilcox Signed Rank Test와 Mann-Whitney U test를 실시하였다. 그 결과, 운동학습훈련군은 훈련 적용 후 운동기능과 균형이 유의하게 항상되었고(p<.05), 대조군에 비해 운동학습훈련군에서 훈련 적용 후 운동기능과 균형이 유의하게 향상되었다(p<.05). 트레드밀을 이용한 운동학습훈련은 경직성 뇌성마비아동의 운동기능과 균형능력을 향상시키는데 도움을 주고, 소아치료 중재방법으로 유용하게 사용될 수 있음을 알 수 있었다.

착용쾌적성이 향상된 방탄복 개발과 성능평가 (Development and Performance Evaluation of Body Armor for Wear Comfort Enhancement)

  • This study helps develop a cool body armor that maintains a tight-fit configuration to the body surface and evaluates the performance of newly developed body armor in a wear test. Three types of body armor were used for evaluation. One was a tight fitting body armor that was constructed to improve the degree of fit and ease of movement for Korean soldier using 3D technology. Another was ventilating body armor with attached spacers on the shoulder to reduce the thermal stress on the soldier. The third was a prevailing body armor produced by a Korean body armor company. In order to evaluate the performance of the body armor, a human wear test, a thermal mannequin test, and computational fluid dynamics (CFD) were executed. Five subjects participated in the wear test. Subjective wear sensation, total amount of sweat and dynamic change of clothing microclimate were observed during and after exercise on a treadmill; subsequently, it was found that subjects rated tight fitting body armor and ventilating body armor lighter, drier, and easier to move than the conventional body armor (p<.05). Total amount of sweat was the least in the case of ventilating body armor. The thermal resistance and vapor resistance of the ventilating body armor were improved remarkably. In addition, the skin temperature of the ventilating body armor with spacers was lower than the tight fitting body armor by at least $1^{\circ}C$ in the CFD result. It is noted that thermal-wet comfort of the 3D body armor with ventilating feature is superior to the conventional body armor, especially when the ventilating channel is not closed due to a backpack.

운동적용 시간량이 허혈성 뇌졸중 환자의 운동기능회복과 심전도 및 Troponin I 에 미치는 영향 (Effects on the Recovery of Motor Function, Change ECG and Troponin I According to Different Amounts Exercise in Ischemic Stroke Patients)

  • Purpose : The purpose of this study is to investigate the effects of motor function recovery and change of the heart function factors(ECG & Troponin I) with ischemic stroke patients by different amounts(times) exercise. Methods : Forty-six consecutive chronic hemiparetic patients with cerebral infarct were randomly assigned to two groups: Group 1 (exercise time 60 minutes/day) and Group 2 (exercise time 120 minutes/day). Types of exercise included static bicycle, isokinetic exercise, and standing or gait exercise on a treadmill. Outcome measures included the level of motor recovery (Fugl-Meyer Scale, FMS) and heart function (ECG and Troponin I), and measurements were performed three times: pre-test, 8 weeks and 12 weeks. Results : There was a significantly different change of motor function recovery and ECG between two groups during treatment period. Especially there were significantly change period of pre-test to 8 weeks on ECG and pre-test to 12 weeks on motor function recovery. But Troponin I has no significantly different change between two groups during treatment period. Also there was no significantly different change of motor function recovery and ECG and Troponin I with between two groups during treatment period. Conclusion : The exercise program improved motor function and change ECG without Troponin I in two groups. The result of this study shows that no matter how different amounts of exercise to effect of motor function recovery and heart function test in chronic patients with cerebral infarct.

Effects of ball kicking dual task training on gait performance and balance in individuals with chronic hemiparetic stroke

  • Objective: The purpose of this study was to compare the effect of ball kicking dual task gait training with the addition of a cognitive task with general treadmill gait training (TGT) on gait speed, gait endurance, functional gait, balance and balance confidence in patients with chronic hemiparetic stroke. Design: Randomized controlled trial. Methods: Fourteen stroke patients who volunteered to participate in this study were randomly divided into two groups with seven patients in each group: ball kicking dual task training (DTT) group and TGT group. The DTT group received ball kicking DTT with cognitive tasks consisted of three stages and the TGT group received TGT using normal walking speed, respectively, for 30 minutes per day 3 days per week for 4 weeks. Outcome assessments were made with the 10-meter walking test (10MWT), 6-minute walking test (6MWT), functional gait assessment (FGA), Berg balance scale (BBS), timed up and go test (TUG), and the activities-specific balance confidence (ABC) scale. Results: The DTT group showed more significant improvement in the 10MWT, 6MWT, FGA, BBS, TUG, and ABC than the TGT group (p<0.05). In addition, within groups comparison showed significant improvement in all variables (p<0.05). Conclusions: The findings suggest that both ball kicking dual task gait training and TGT improve gait performance and balance in patients with chronic hemiparetic stroke. However, ball kicking dual task gait training results showed more favorable outcomes than TGT for chronic hemiparetic stoke patients.

전정감각 자극훈련이 뇌졸중 환자의 균형과 보행에 미치는 영향 (The Effects of Vestibular Sensory Stimulation Training on Balance and Gait in the Patients with Stroke)

  • Purpose: The aim of this study is to examine the effects of training in vestibular sensory stimulation on balance and gait of stroke patients. Methods: Twenty patients were randomly assigned to either the experimental group (n=10) or the control group (n=10). Patients in the experimental group received rotational stimulation training, vertical-horizontal stimulation training, gait training on a flat surface with vestibular sensory stimulation, and gait training on soft ground with vestibular sensory stimulation. Patients in the control group received general treadmill gait training. The intervention was applied four times per week, 25 minutes each time, for a period of four weeks. We measured Berg Balance Scale (BBS), Biodex Balance System, Timed up to Go (TUG) test and Dynamic Gait Index (DGI) to evaluate balance and gait ability. Results: BBS differed significantly in both groups between before and after the intervention (p<0.05) and changes in BBS after the intervention differed between the two groups (p<0.05). According to the Biodex Balance System test result, only the experimental group showed significant changes in balance in the conditions of static eyes open (SEC), dynamic eyes open (DEO), and dynamic eyes closed (DEC) (p<0.05). TUG test results differed significantly between prior to and after the training in both the experimental group and the control group (p<0.05) and changes in TUG after the intervention differed significantly between the two groups (p<0.05). DGI results showed significant change after the intervention in the experimental group only (P<0.05). Conclusion: Training in vestibular sensory stimulation was effective in improving static-dynamic balance and gait ability of stroke patients.

3축 가속도 센서를 이용한 실시간 걸음 수 검출 알고리즘 (Real-Time Step Count Detection Algorithm Using a Tri-Axial Accelerometer)

  • 본 논문에서는 3축 가속도 센서를 이용하여 사람이 보행 시 발생하는 센서 데이터를 획득하여 실시간 걸음 수 검출이 가능한 웨어러블 디바이스를 개발하였다. 피험자 59명을 대상으로 트레드밀에서 Actical 과 본 연구에서 개발된 디바이스를 착용 후 36분 동안 테스트 프로토콜에 따라 느리게 걷기, 걷기, 빠르게 걷기, 천천히 뛰기, 뛰기, 빠르게 뛰기 등의 다양한 걸음 속력에서 테스트를 진행하였다. 3축 가속도 센서의 X, Y, Z축 출력 값을 하나의 대표 값으로 처리하는 신호벡터크기(Signal Vector Magnitude : SVM)를 사용하였다. 또한 정확한 걸음 수를 검출하기 위해 휴리스틱 알고리즘(Heuristic Algorithm : HA)을 제안하고 적응적인 임계값 알고리즘(Adaptive Threshold Algorithm : ATA), 적응적인 잠금 구간 알고리즘(Adaptive Locking Period Algorithm : ALPA)을 제안한다. 실험결과 제안하는 알고리즘의 걸음 수 인식률은 97.34%로 Actical의 인식률(91.74%)보다 5.6%향상 되었다.

만성 발목 불안정성 환자에서 키네시오 테이핑과 MWM 테이핑 적용이 발목의 근력과 균형능력의 변화 비교 (Comparison of changes in Ankle Muscle Stregth and Balance ability in Patients with Chronic Ankle Instability using Kinesio Taping and MWM Taping)

  • Background: Ankle sprains, and the resulting ankle instability worsen to chronic due to recurrent ankle injuries or sprains, 78% of which are accompanied by posture instability and damage due to changes in the position of the talus of the ankle. The purpose of this study is to investigate the immediate effect of applying MWM taping on the patient's muscle strength and balance ability in patients with chronic ankle instability. Methods: 15 people with MWM taping and 15 people with Kinesio taping were applied, and after applying the taping of the ankle, 10 minutes of walking treadmill and 10 times of forward lunge operation, the change in ankle muscle strength and balance ability was confirmed. The strength test of the ankle was performed using a test device called Biodex system 4 (USA) for the movement of the dorsi-flexion and plantar flexion of the foot, and the balance of the two groups was measured using Biodex balance system (USA) to test balance ability. Results: The comparison of muscle strength changes in the ankle does not show a significant increase in the group applying MWM compared to the group applying kinesio taping (p<.05). In the comparison of equilibrium capabilities, the MWM taping group also showed a significant increase in the MWM taping group compared to the kinesio taping group (p<.05). Conclusion: When applying MWM taping and kinesio taping to patients with chronic ankle instability, there was no significant difference in comparison of muscle strength changes, but there was a significant difference in comparison of balance ability.
