Purpose : The purpose of this study was to compare the effects of ankle balance training on unstable and stable surfaces to discover on which surface the proprioception, balance and muscle strength of obese middle aged women improves more. Method : Thirty obese middle-aged women were randomly recruited and divided into two groups (training on an unstable surface, training on a stable surface). The subjects in each group participated in the training for six weeks, three times per week for 30 minutes each session. Proprioception was measured using Dualer IQ digital inclinometer; sway length (SL) and sway area (SA) of center of pressure, and limit of stability (LOS) were measured for balance; muscle strength before and after the training was measured using manual muscle testing. Results : There were significant improvements in the subscales of the proprioception, balance and muscle strength in those who participated in ankle balance training on the unstable surface, and in those who participated on the stable surface. However, before and after the intervention, there were no differences between those who had the balance traning on the unstable surface and those who had the balance traning on the stable surface. Conclusion : Unstable and stable surface ankle balance training are both effective in improving the proprioception, balance and muscle strength of obese middle-aged woman.
In Korea, military authorities have neglected to consider impacts of military projects on local communities and natural environment. Moreover, local communities have had difficulties in dealing with the Ministry of National Defense (MND), which was stubborn enough not to implement environmental assessment on their projects. In this situation, recent case, "EIA of Baekgol Division's Howitzer Fire Training Site" in the Supreme Court-in which judges upheld the Higher Court's decision that the division violated the Environmental Impact Assessment law by ignoring to implement EIA-reveals that military projects can no longer forgo environmental assessment. The decision has serious ramifications on the future of Environmental Impact Assessment in military-led projects. This paper examines the proper scope of EIA in military-led projects and, more specifically, fire training site and searches for how to improve it through 'probable error,' a military training method that is applied to real 'howitzer' fire training. Probable error of the artillery field manual is nothing more than an error that exceeded as often as it is not exceeded and its scientific method was demonstrated through real fire tests in the US. Army. If it is applied to improve assessment methods about the proper scope of EIA in military 'howitzer' fire training site, 'probable error' will improve effect prediction, mitigation and reliability.
Objectives : The purpose of this study was to evaluate the effects of core stability training on postural control and balance of hemiplegia patients who are difficult to control posture due to stroke. Methods : Subjects of the study were consisted of 25 adult hemiplegia patients(experimental 12, control 13) who were receiving rehabilitation therapy in hospital. Its group had a core stability training program by a physical therapists for 40 minutes, five times a week for nine-week period. Measurements of postural assessment scale for stroke(PASS), berg balance scale(BBS) and manual function test(MFT) were evaluated at initial presentation(pretest) and after completion of the each therapy program(posttest). Independent t-test and paired t-test was utilized to detect the mean difference between the groups. Results : Firstly, the result of PASS showed that postural control was significantly increased after the completion of core stability training(p<.01). Secondly, the result of BBS showed that balance control was significantly increased after the completion of core stability training(p<.01) and general physical therapy(p<.01). Lastly, the result of MFT showed that upper extremity's motor function was significantly increased after the completion of core stability training(p<.001). Conclusions : This study showed that core stability training is an effective treatment strategy on postural control, balance and upper extremity's motor function.
목적 : 본 사례연구는 보툴리눔 독소(Botulinum Toxin) 주사 후 과제 지향적 훈련이 뇌졸중 환자의 상지 기능 변화와 일상생활활동에 향상에 영향을 미치는지 알아보고자 하였다. 연구방법 : 실험 대상자는 뇌졸중으로 인해 편마비 진단을 받은 44세 남자 환자로 손상 측 상지에 보툴리눔 독소 주사 후 과제 지향적 훈련을 주 5회 30분씩 8주간 훈련을 받았다. 평가는 주사 전, 중재 4주 후와 8주 후의 상지기능 변화와 일상생활활동 수행변화를 평가하였다. 상지기능 평가는 뇌졸중 상지기능 평가(Manual Function Test; MFT)와 상자와 나무토막 검사(Box and Block Test)로 평가하였고, 일상생활활동 수행평가는 수정된 바델지수(Modified barthel index; MBI)로 측정하였다. 결과 : 상지기능평가에서 손의 조작능력과 기민성이 향상되었고 또한, 일상생활활동 하위 영역 중 개인위생, 식사하기, 용변처리, 옷 입기 항목에서 향상을 보였다. 결론 : 본 연구 결과를 바탕으로, 보툴리눔 독소 주사 후 과제 지향적 훈련이 뇌졸중 환자의 상지기능 및 일상생활활동 수행능력 향상을 위한 치료에 효과적임을 알 수 있었다.
The propose of the study was to evaluate the efficacy of the partial body weight support during treadmill training on the ambulation in elderly with chronic stroke. Fourteen hemiplegic volunteers participated and were divided into an experimental and control groups. In the experimental group, the body weight support during treadmill training was performed 3 times per week for 6 weeks. In the control group, usual treadmill training was applied. Before and after experiments, temporal-spatial gait parameters were measured. The date of 14 patients who carried out the whole experimental course were statistically analyzed. The results of the study were : 1. In the comparison of gait velocity before and after experiment, the gait velocity was significantly increased in the experimental group and the control group(p<.05). In the comparison of difference of the gait velocity between groups, there was not, significant difference between the experimental group and the control group(p>.05). 2. In comparison of gait cadence before and after experiment, the gait cadence was significantly increased in both groups(p<.05). In the comparison of difference of the gait cadence between groups, there was not significant difference between the experimental group and the control group(p>.05). 3. In the comparison of step length before and after experiment, the step length was significantly increased in the experimental group and the control group(p<.05). This study approaches the effects of Balance Training on excercise performance and the prevention from the injuries caused by excercise. The subjects of the Balance Training program are female exercise beginners and the research period covers 8 weeks from January 10 to March 7, 2003. The research objects are 19 female golf beginners in 30s to 40s, who live in Seoul or Seongnam in Gyeonggi province and have played golf less than 6 months. The programs of the Balance Training and exercise performance were conducted to an 11 experimental group among the 19 research objects at the same time, and only the exercise performance program was applied to an 8 control group for 8 weeks. Before and after 8 weeks' application of the research programs to each group, the research subjects were examined, especially the components of their bodies, the balance and the performance capability were measured both before and after the test. The frequency of injuries by exercise was measured after the test, and the difference of the frequency was compared with the frequency before exercise. First, the experimental group, in a measurement of balance, showed that SN, MB, SAr and SAg of static balance decreased in a situation of MEO, MEC, GEO, GEC, TBEO, TBO, FHEO, FEO(p <0.05), but the control group increased. Second, the analysis on the change of exercise performance indicated better improvement in distance, ball speed, and accuracy of the experimental group than the control group(p<0.05). Third, the experience of injuries showed that there were 2 injuries in the experimental group and 11 injuries in the control group. The injured parts were 2 cases in the hands and fingers of the experimental group, and 1 case in the shoulder, 4 in the elbows, 4 in the hands and fingers and 2 in the lumber of the control group. From the above-mentioned results, it is recognized that the Balance Training program improved the exercise performance of female golf beginners and had good effects on the prevention from injuries. Accordingly, if this program is applied to sports-beginners, it will contribute to the improvement of the public health.
An effective method for produce munitions effectiveness data is to calculate weapon effectiveness indices in the US military's Joint Munitions Effectiveness Manuals (JMEM) and take advantage of the damage evaluation model (GFSM) and weapon Effectiveness Evaluation Model (Matrix Evaluator). However, a study about the Range Safety that can be applied in the live firing exercises is very insufficient in the case of ROK military. The Range Safety program is an element of the US Army Safety Program, and is the program responsible for developing policies and guidance to ensure the safe operation of live-fire ranges. The methodology of Weapon Danger Zone (WDZ) program is based on a combination of weapon modeling/simulation data and actual impact data. Also, each WDZ incorporates a probability distribution function which provides the information necessary to perform a quantitative risk assessment to evaluate the relative risk of an identified profile. A study of method to establish for K-Range Safety data is to develop manuals (pamphlet) will be a standard to ensure the effective and safe fire training at the ROK military education and training and environmental conditions. For example, WDZs are generated with the WDZ tool as part of the RMTK (Range Managers Tool Kit) package. The WDZ tool is a Geographic Information System-based application that is available to operational planners and range safety manager of Army and Marine Corps in both desktop and web-based versions. K-Range Safety Program based on US data is reflected in the Korean terrain by operating environments and training doctrine etc, and the range safety data are made. Distribution of this K-range safety pamphlet is available to Army users in electronic media only and is intended for the standing army and army reserve. Also publication and distribution to authorized users for marine corps commands are indicated in the table of allowances for publications. Presently, media for sanitation education consisting of a sanitation manual and a CD-ROM intended for restaurant employers and employees was developed and evaluated. The sanitation manual consisted of five principles: prevention of foodborne illness, personal hygiene, control of food production, instrument and equipment cleaning and sanitation, and management of environmental sanitation. The CD-ROM was composed of animations detailed real-life examples of Salmonella, Staphylococcus aureus, and Norovirus foodborned illness outbreaks; slides summarizing the five principles of the manual; and a poster entitled You can prevent foodborne illness listing and describing the principles. A 15 question evaluation survey was developed to gauge the efficacy of the animations. The survey was divided into five sections on comprehension of the instructions, content organization concerning understanding, content organization concerning the information presented, content organization concerning retention of interest (concentration), and recommendations concerning concentration. Ranked on a 5-point scale the survey produced a mean value of 3.80$\pm$0.39 and individual scores of 3.92$\pm$0.45 (learning instruction), 3.86$\pm$0.48 (understanding), 3.82$\pm$0.52 (information), 3.75$\pm$0.49 (concentration), and 3.67$\pm$0.58 (concentration-recommendation). Overall, evaluation results of the animation were good and easy to understand, with only a few respondents electing to watch the animations more than once. In terms of continuous and recurring education, sanitation training programs should be easy to learn and contain sufficient and specific examples of the importance of sanitation in achieving food safety.

Objective: To evaluate the accuracy of a multi-stage convolutional neural network (CNN) model-based automated identification system for posteroanterior (PA) cephalometric landmarks. Methods: The multi-stage CNN model was implemented with a personal computer. A total of 430 PA-cephalograms synthesized from cone-beam computed tomography scans (CBCT-PA) were selected as samples. Twenty-three landmarks used for Tweemac analysis were manually identified on all CBCT-PA images by a single examiner. Intra-examiner reproducibility was confirmed by repeating the identification on 85 randomly selected images, which were subsequently set as test data, with a two-week interval before training. For initial learning stage of the multi-stage CNN model, the data from 345 of 430 CBCT-PA images were used, after which the multi-stage CNN model was tested with previous 85 images. The first manual identification on these 85 images was set as a truth ground. The mean radial error (MRE) and successful detection rate (SDR) were calculated to evaluate the errors in manual identification and artificial intelligence (AI) prediction. Results: The AI showed an average MRE of 2.23 ± 2.02 mm with an SDR of 60.88% for errors of 2 mm or lower. However, in a comparison of the repetitive task, the AI predicted landmarks at the same position, while the MRE for the repeated manual identification was 1.31 ± 0.94 mm. Conclusions: Automated identification for CBCT-synthesized PA cephalometric landmarks did not sufficiently achieve the clinically favorable error range of less than 2 mm. However, AI landmark identification on PA cephalograms showed better consistency than manual identification.
It is known that chronic pain and injury of upper limb joint tissue in manual wheelchair users is usually caused by muscle imbalance, and the propulsion speed is reported to increase this muscle imbalance. In this study, kinematic variables, electromyography, and ultrasonographic images of the upper limb were measured and analyzed at two different propulsion speeds to provide a quantitative basis for the risk of upper extremity joint injury. Eleven patients with spinal cord injury for the experimental group (GE) and 27 healthy adults for the control group (GC) participated in this study. Joint angles and electromyography were measured while subjects performed self-selected comfortable and fast-speed wheelchair propulsion. Ultrasound images were recorded before and after each propulsion task to measure the acromiohumeral distance (AHD). The range of motion of the shoulder (14.35 deg in GE; 20.24 deg in GC) and elbow (5.25 deg in GE; 2.57 deg in GC) joints were significantly decreased (p<0.001). Muscle activation levels of the anterior deltoid, posterior deltoid, biceps brachii, and triceps brachii increased at fast propulsion. Specifically, triceps brachii showed a significant increase in muscle activation at fast propulsion. AHD decreased at fast propulsion. Moreover, the AHD of GE was already narrowed by about 60% compared to the GC from the pre-tests. Increased load on wheelchair propulsion, such as fast propulsion, is considered to cause upper limb joint impingement and soft tissue injury due to overuse of the extensor muscles in a narrow joint space. It is expected that the results of this study can be a quantitative and objective basis for training and rehabilitation for manual wheelchair users to prevent joint pain and damage.
