기관기관지 잔유조직에 의한 선천성 식도협착증 수술 치험 -3예 보고- (Congenital Esophageal Stenosis due to Tracheobronchial Remnants - 3 Case Reports -)

  • Congenital esophageal stenosis due to tracheobronchial remnants is a rare anomaly, resulting in dysphagia and recurrent pneumonia, We have experienced three cases of csophageal stenosis due to ectopic tracheobronchial remnants and performed operative correction. Two patients were 20 months and five year old male with a chief complaints of swallowing difficulty from birth and the other was a twenty three year old female with a slowly increasing symptom of dysphagia for twenty years. Esophagogram of the patient with tracheobronchial remnants shows abrupt narrow segment at distal esophagus with marked proximal dilatation, and linear barium collections perpendicularly projecting from the stenotic esophagus. All of them were performed surgical correction by esophagectomy of the stenotic portion and esopahgo-gastrostomy with anti-reflux procedures, The resected specimens of these patients showed ectopic tracheobronchial chondroepithelial tissue within the esophageal wall histopathologically. Postoperative course was uneventful and have been in good condition without any problems.

  • Esophageal stenosis due to ectopic tracheobronchial remnants is an uncommon anomaly and only few cases were reported in the literature. We have experienced two case of esophageal stenosis due to ectopic tracheobronchial remnants and performed operative correction. The one patient was a five year old male with a chief complaint of swallowing difficulty from birth and the other was a twenty-three year old female with a slowly incresing symptom of dysphagia for twenty years. both of them were performed surgical correction by esophagectomy of the stenotic portion and esophago-gastrostomy with anti-reflux procedures.   • Congenital esophageal stenosis due to tracheobronchial remnants is one of main forms of congenital esophageal stenosis.A 19-month-old male was presented an appearence of underdevelopment and mild dehydration state due to persistent vomiting since 5 months after his birth. Esophagogram revealed an abrupt narrowing of lower esophagus with dilatation above it. The operation method was aesection of esohageal stenosis and end to end anastomosis through left seventh thoracotomy. The postoperative course was uneventful.

Rigid Bronchoscopy for Post-tuberculosis Tracheobronchial Stenosis

  • The healing process of tracheobronchial tuberculosis (TB) results in tracheobronchial fibrosis causing airway stenosis in 11% to 42% of patients. In Korea, where pulmonary TB is still prevalent, post-TB tracheobronchial stenosis (PTTS) is one of the main causes of benign airway stenosis causing progressive dyspnea, hypoxemia, and often life-threatening respiratory insufficiency. The development of rigid bronchoscopy replaced surgical management 30 years ago, and nowadays PTTS is mainly managed by bronchoscopic intervention in Korea. Similar to pulmonary TB, tracheobronchial TB is treated with combination of anti-TB medications. The indication of rigid bronchoscopy is more than American Thoracic Society (ATS) grade 3 dyspnea in PTTS patients. First, the narrowed airway is dilated by multiple techniques including ballooning, laser resection, and bougienation under general anesthesia. Then, most of the patients need silicone stenting to maintain the patency of dilated airway; 1.5 to 2 years after indwelling, the stent could be removed, this has shown a 70% success rate. Acute complications without mortality develop in less than 10% of patients. Subgroup analysis showed successful removal of the stent was significantly associated with male sex, young age, good baseline lung function and absence of complete one lobe collapse. In conclusion, rigid bronchoscopy could be applied to PTTS patients with acceptable efficacy and tolerable safety.

  • 기관지내 팽창성 금속 스텐트의 사용은, 아직은 외국의 경우에서도, 추적 관찰기간이 짧고 시술받은 환자의 수도 많지 않으며 장기간 삽입후의 변화에 대하여는 그 결과가 예측하기 힘들다는 점등을 생각할 때는 현재 이의 시행은 아직은 실험적 단계라 할 수 있다. 그러나 내과적 혹은 외과적인 문제로 수술적 기관지 보존술등을 시행받기 어려운 상태에 있는 기관지 협착환자에서는 단독으로, 혹은 laser therapy, electrocoagulation, cryotherapy, balloon dilatation 및 다른 tracheal devices (예(例): Montgomery T-tube)등 의 치료방법들과 같이 병행하여, 팽창성 금속 스텐트의 삽입은 기관기관지 협착증의 치료에 도움이 되리라 생각된다. 또 국내에서는 아직도 유병율이 높은 폐결핵환자에서 충분한 기간동안 약물치료후에도 일부 환자에서는 기관지 결핵에 의하여 기관지 협착이 발생하는데 이때 수술적 치료외에는 적절한 치료방법이 없는 실정이다. 이러한 환자에서 만약 협착부위가 한쪽 주 기관지에 부분적으로 국한 되어 있으며 그 이하의 폐실질파괴가 심하지 않다면 스텐트 삽입은 수술적 치료이전에 시도하여 볼 수 있으리라 사료된다.

  • 본원에서는 우측 흉곽내 폐용적이 감소하면서 심장이 우측으로 이상 회전을 하게 되어 기관기관지의 협착이 유발된 3세 여환아를 우측 폐의 확장을 유도하여 심장의 이상 회전을 교정해 줌으로써 기관기관지 협착을 치료하였기에 이에 보고하는 바이다.

기도질환 환자의 치료기관지경술 (Bronchoscopic Intervention for Airway Disease)

  • Surgical resection and reanastomosis has been the treatment of choice in patients with tracheobronchial stenosis. Recent development of bronchoscopic intervention has been replacing the role of surgery in these patients. After summarizing the upto date data of bronchoscopic intervention, the proper management of tracheobronchial stenosis will be presented. Bronchoscopic intervention would be much effective when performed under rigid bron- choscopy, due to the stable patients' condition and endoscopic view. The usual method of intervention includes ballooning, Nd-YAG laser resection, bougienation, mechanical airway dilatation, stenting and photodynamic therapy. Silicone stents are very effective in patients with tracheobronchial stenosis to maintain airway patency. Bronchoscopic intervention provided immediate symptomatic relief and improved lung function in most of patients. After airway stabilization, stents were removed successfully in 2/3 of the patients at a 12-18 months post-insertion. Less than 5% of patients eventually needs surgical management. Acute complications, including excessive bleeding, pneumothorax, and pneumomediastinum develops in less than 5% of patients but managed without mortality. Stent-related late complications, such as, migration, granuloma formation, mucostasis, and restenosis are relatively high but usually controlled by follow-up bronchoscopy. In conclusion, bronchoscopic intervention, including silicone stenting could be a useful and safe method for treating tracheobronchial stenosis.

Preoperative Diagnosis of Congenital Esophageal Stenosis Caused by Tracheobronchial Remnants Using Miniprobe Endoscopic Ultrasonography in a Child

  • Congenital esophageal stenosis (CES) can be classified into three types based on the etiology of stenosis: tracheobronchial remnants (TBRs), fibromuscular hypertrophy (FMH), and membranous diaphragm (MD). It is important to make a differential diagnosis because the therapeutic plan for CES is determined by its etiology. Most cases of FMH and MD can be managed with balloon dilatation, whereas cases of TBRs require resection and anastomosis. Thus, the preoperative distinction of TBRs is critical. Recently miniprobe endoscopic ultrasonography (EUS) with a maximum diameter of 2.5 mm has been useful for distinguishing TBRs from FMH in pediatric patients with CES. EUS shows hyperechoic lesions indicating TBR cartilage. Miniprobe EUS is recommended for choosing the correct therapeutic method for CES. We report a case of CES due to TBRs in which a preoperative diagnosis was made in a child using miniprobe EUS without any difficulties.

양성 기도협착질환에서 실리콘 기도스텐트의 임상경험 - Dumon 스텐트와 Natural 스텐트의 비교 - (Clinical Experience of Silicone Airway Stent in the Management of Benign Tracheobronchial Stenosis)

  • 연구배경 : 양성 기도협착질환 환자에서 새로 개발된 Natural 기도스텐트의 임상결과를 Dumon 스텐트와 비교하였다. 방 법 : 양성 기도협착으로 스텐트를 삽입한 94명의 환자 (39 Dumon, 55 Natural)를 대상으로 임상증상, 시술 전 검사결과, 시술방법, 그리고 추적관찰시의 임상경과를 후향적으로 조사하였다. 결 과: 원인질환으로 기관지결핵 후 기도협착 (74%)이 가장 많았고, 기관삽관 후 기관협착 (20명), 기관절개술 후 기관협착 (2명)등이 있었다. 스텐트를 삽입한 후, 전체 94명의 환자 중 82명 (87%)에서 주관적으로 호흡곤란이개선되었고, Dumon 스텐트군(35/39, 90%)과 Natural 스텐트군 (47/55, 86%) 사이의 차이는 없었다. 모두 48명 (51%)에서 중앙값 14개월(범위6-37개월)후에 스텐트를 성공적으로 제거할 수 있었고, Dumon 스텐트군 (54%, 16개월 후) 과 Natural 스텐트군 (49%, 12개월 후)간의 차이는 없었다. 스텐트 삽입 후 부작용은 스텐트의 위치 변동이 45명 (48%)에서, 육아종 과형성이 41명 (44%)에서, 점액저류에 의한 스텐트 내경 50% 이상의 감소가 17명 (18%)에서, 그리고 스텐트 삽입 전후부위의 협착이 40명 (43%)에서 발생하였다. Dumon 과 Natural 스텐트를 삽입한 환자들 사이에서 부작용의 분포의 차이는 없었다. 결 론 : 새로 개발한 Natural 기도스텐트는 양성기도협착 환자에서 기존의 Dumon 스텐트와 대등한 임상 결과를 보여주고 있어, 기도 협착이 있는 환자의 치료에 유용하게 사용할 수 있을 것으로 사료된다.

Congenital Esophageal Atresia Associated with a Tracheobronchial Remnant

  • A rare case of esophageal atresia/tracheo-esophageal fistula (EA-TEF) with an associated tracheobronchial remnant (TBR) is reported and discussed herein. A 13-month-old patient was seen with a complaint of vomiting of solid food 1 year after EA-TEF repair. An esophagogram showed a tapered narrowing in the lower segment of the esophagus. A re-operation was carried out and a pathologic examination of the resected stenotic segment revealed the presence of a TBR.
