기관 게실을 동반한 호산구성 기관지염 1예 (A Case of Eosinophilic Bronchitis Associated with Tracheal Diverticulum)

  • Tracheal diverticulum is relatively rare. It results from congenital or acquired weakness of the tracheal wall. Most cases are asymptomatic, but when symptoms are present, they are usually nonspecific. A 54-year-old man complained of sputum lasting for several months. Chest computed tomography showed an air-containing cystic structure in the trachea. Fiberoptic bronchoscopy demonstrated ostium arising from the right posterolateral wall at the trachea. Reported herein is a case of eosinophilic bronchitis associated with tracheal diverticulum.

  • Tracheal diverticulum is very rare, that is usually an incidental finding at routine chest computed tomography scan. Differential diagnosis of tracheal diverticulum includes pharyngocele, laryngocele, Zenker diverticulum, apical lung hernia, blebs and bulla, and pneumomediastinum. Treatment options can be devided into medical and surgical. The majority of patients is asymptomatic and requires no specific intervention. We experienced one case of tracheal diverticulum in patient with tongue cancer and report it with reviews of literature.

  • 기관기관지비대증 또는 무니어-쿤 증후군은 기관과 양쪽 주기관지의 미만성 비대를 특징으로 하는 원인 불명의 매우 드믄 질환이다. 선천성으로 생각 되나 확실하지 않다. 만성 호흡기계 감염, 기관지 확장증등을 일으키는가 하면 평생 무증상으로 지내는 사람도 있는등 임상양상은 매우 다양하다. 저자들은 최근에 기관 게실이 있는 무니어-쿤 증후군 환자에서 폐암이 동반된 증례를 치험하였기에 문헌 고찰과 함께 보고 한다.

  • 저자들은 과거 결핵을 앓고 진균덩이를 가진 환자로 평소 만성적인 기침과 가래 등 호흡기적 증상을 가진 환자가 객혈로 내원하여 기관지 혈관 조영 색전술을 시행하는 과정에서 촬영한 전산화 단층 촬영과 기관지 내시경에서 발견된 기관 게실을 문헌 고찰과 함께 보고하는 바이다.

  • Paratracheal air cysts are rare lesions and detected incidentally during CT scan or autopsy. Histopathologic diagnoses of paratracheal air cysts include trachocele, tracheal diverticulum and lymphoepithelial cyst. The cysts are lined by ciliated columnar epithelium and have communication with trachea. Previous reports suggested an association with obstructive lung disease because of increased expiratory pressures in these patients. Most of these cysts are asymptomatic but rarely cause productive cough, wheezing, recurrent laryngeal nerve paralysis and difficult intubation. We report a case of paratracheal air cyst causing dyspnea with a review of literature.

  • Paratracheal air cysts are a rare entity in which cystic formation occurs adjacent to the trachea. Most patients with paratracheal air cysts are asymptomatic, and the cysts are detected incidentally on chest radiograph or computed tomography (CT) scan. Most symptomatic patients complain of pulmonary symptoms or repeated respiratory infection. Rarely, the air cysts can lead to paralysis of the recurrent laryngeal nerve as a result of direct compression. We report a case of a 59-year-old male patient who presented with voice change, and the cause was identified as paratracheal air cysts on a chest CT scan. Surgical resection via video-assisted mediastinoscopy was performed, and the voice recovered immediately after the operation.
