사이펀 차단기 시뮬레이션 프로그램의 개발 및 활용 (Development and Application of Siphon Breaker Simulation Program)

  • 일부 연구용 원자로의 설계조건상 사이펀 현상은 배관 파단 사고 시 수조수의 지속적인 방출을 유발할 수 있다. 사이펀 차단기는 이러한 현상을 효과적으로 제한하기 위한 안전장치로, 유체역학적인 특성상 사이펀 차단 현상 해석을 위해 고려해야 할 변수가 많고 계산이 복잡하다. 이에 사이펀 차단 현상을 쉽게 분석할 수 있는 프로그램을 개발하게 되었다. 윈도우8 운영체제에서 비쥬얼 스튜디오 2012를 이용하여 MFC프로그래밍으로 개발되었으며, 사용자가 쉽게 사용할 수 있도록 GUI형식으로 개발되었다. 개발된 프로그램은 사용자가 입력한 값으로부터 유체역학적 관계식을 통해 3단계의 연산과정을 거쳐 시뮬레이션을 진행한다. 베르누이 방정식으로부터 유속과 유량을 구하여 수위, 언더슈팅, 압력, 손실계수, 그리고 이상 유동과 관계된 값들을 연산한다. 프로그램에 적용된 이상유동 해석모델은 Chisholm 모델이며, 실제와 유사하게 시뮬레이션이 가능함을 확인하였다. 시뮬레이션 결과는 그래프를 통해 나타나기 때문에 사용자는 전체적인 차단 현상을 쉽게 파악하는 것이 가능하며, 시뮬레이션 데이터의 저장 또한 가능하다. 따라서 사용자는 사이펀 차단기 시뮬레이션 프로그램의 사용을 통해 사이펀 차단 현상을 쉽게 확인할 수 있으며, 사이펀 차단기의 실제 설계에도 이용할 수 있을 것으로 기대된다.

모의 수출 조건에서 수확 후 CO2와 부직포 패드 처리가 느타리버섯 품질에 미치는 영향 (Effect of Postharvest High CO2 Treatment and Anti-moisture Pad on the Quality of Fresh Oyster Mushroom during Export Simulation)

  • 느타리버섯 수출력 증진방안을 모색하기 위하여 선적전 고농도 $CO_2$ 및 흡습지 처리가 모의 수출과정에서 버섯의 품질에 미치는 영향을 조사하였다. 수출과정의 온도 변화는 표장용기 내부에 응축수를 만들어 품질을 떨어뜨리는 원인이 되며 흡습지는 경도 유지, 갈변 예방 등에서 효과적이었다. 그러나 모의소매과정에서의 고온($15^{\circ}C$) 노출은 그 효과를 크게 감소시켰다. 선적전 고농도의 $CO_2$ 처리는 경도에 대한 영향은 뚜렷하지 않았지만 버섯의 색택변화를 지연시켰다. 총당, 페놀 등 내적성분은 처리간 일정한 경향을 보이지 않았다. 전반적 수출잠재력은 현행 수출용 규격인 작은 버섯에서 높았다. 수출용 버섯의 품질저하는 포장용기내 응축수 발생과 온도변화의 영향을 크게 받아 이를 개선하기 위한 노력이 필요하다.

ADHD 환자에 대한 OROS-Methylphenidate 약물치료의 부작용과 관련요인들에 대한 연구 (The Side Effects and Correlates of OROS-Methylphenidate in the Treatment of Children and Adolescents with ADHD)

  • Objectives : The aim of this study was to investigate the effect of the clinical and demographic variables such as body weight, dosage, family history of attention-deficit hyperactivity disorder (ADHD), and psychiatric co-morbidity on the side-effects of OROS-Methylphenidate (OROS-MPH), and to evaluate the relationship between drug response and side effect severity. Methods : A total of 144 children (ages 6-18) with diagnosed ADHD were treated with OROS-MPH. Children were examined at baseline and after 1, 3, 6, 9, and 12 weeks of each treatment condition. The stimulant drug side effect rating scale (SERS), pulse rate, systolic blood pressure, diastolic blood pressure, and electrocardiogram (ECG) were evaluated to assess side effect profiles. Changes in these parameters from baseline were examined and analyzed. Results : Anorexia (30.95%) and insomnia (13.10%) were the most commonly reported side effects during this study. Insomnia and loss of appetite score increased at one week follow-up, but was sustained or decreased as treatment progressed. Small but significant increases in pulse rate and diastolic blood pressure were observed during treatment ; however, no clinically meaningful changes in ECG parameters were noted during the study. Low body weight, high dosage of OROS-MPH, and family history of ADHD were associated with cardiovascular side effect. In contrast, there was no significant relationship between OROS-MPH treatment response and the severity of side effect and no difference resulted between the responder and non-responder groups with respect to OROS-MPH dosage in the 12 weeks of follow-up. Conclusion : To the best of our knowledge, this study is the first Korean study to investigate comprehensive side effect profiles and their correlates in OROS-MPH treatment for ADHD children. OROS-MPH was well tolerated with no clinically significant side effects during the treatment period. In conclusion, low body weight, high dosage of OROSMPH, and family history of ADHD could be used as predictive factors in increasing pulse rate and blood pressure.

폐경 여성의 내장지방 및 식사의 질에 미치는 영양 교육의 효과 (The Effect of Nutrition Education on Visceral Fat Reduction and Diet Quality in Postmenopausal Women)

  • 본 연구에서는 6개월간 폐경 여성을 대상으로 영양교육과 파워 워킹 프로그램을 실시한 영양교육군, 영양 교육 없이 파워 워킹 프로그램만 실시한 대조군으로 나눠 체중조절 프로그램을 실시 하였다. 그 결과, 두 군 모두 체중과 체질량 지수, 체지방량과 내장지방이 유의적으로 감소하였으나, 영양교육을 실시한 영양교육군에서 체중 감소율이 더 크게 나타났으며, 특히 폐경 여성들에게 문제 시 되고 있는 내장지방의 변화율이 더 유의적으로 감소하는 긍정적인 결과를 보였다. 또한 본 연구에서는 영양교육군 내에서 내장지방변화율에 따라 내장지방 감소율이 큰 군 (HVL group)과 내장지방 감소율이 적은 그룹 (LVL group)으로 나누어 체중조절 프로그램의 효과와 관련된 요인을 분석하였다. 그 결과 내장지방 감소율이 컸던 군에서 체중, BMI, 체지방, 복부지방, 내장지방 등의 체성분들의 변화율이 더 크게 나타났다. 혈압 및 혈액 성분에서는 수축기, 이완기 혈압과 총콜레스테롤, LDL 콜레스테롤, 중성지방, 동맥 경화 지수의 개선 정도가 더 큰 결과를 보였다. 또한 내장지방 감소율이 컸던 군이 내장지방 감소율이 적은 군에 비해 단백질, 칼슘, 비타민C의 INQ와 칼슘, 비타민 $B_1$, 비타민 $B_2$, 비타민 C의 MAR도 유의적으로 증가하여 식사의 질이 더 개선되었음을 알 수 있었다. 또한 단백질의 영양밀도 변화율은 체지방 분포의 변화율과 음의 상관관계를 나타낸 반면, 탄수화물 영양밀도의 변화율은 양의 상관관계를 나타내었고, 비타민 A 영양밀도 변화율은 총 콜레스테롤, LDL-콜레스테롤, 동맥경화 지수 변화율과 음의 상관관계를 보여, 영양교육에 따른 신체계측치 및 체성분의 변화가 영양소 섭취 변화와 관련이 있음을 보여주었다. 이는 영양 교육 및 중재에 있어 실천의 중요성을 강조하는 것으로 이를 위해서는 폐경 여성들의 대사적 특성과 더불어 생활 습관을 이해하고 만성 질병을 예방하고 올바른 식습관을 통해 건강한 삶을 유지할 수 있도록 식사의 균형성, 다양성을 포함한 권장 식사 지침을 제시하고, 적당한 식품 섭취량을 포함한 종합적인 프로그램을 개발하여야 할 것이다. 또한 이러한 페경 여성을 위한 전문적이며 체계적인 프로그램을 통해 폐경 이후 체중 및 체지방 증가를 예방하고, 나아가 만성질병을 예방, 관리할 수 있을 것으로 사료된다.

생사의 포합향상에 관한 연구 (Studies on Raw Silk Cohesion for Promotion)

  • The purpose of this studies is to improve the cohesion of raw silk through various analyses on cocoon drying, cooking, reeling, re-reeling, and on the properties of water. Also we investigated the correlation between silk testing items which we have reached to the following results. 1. Drying of cocoon When cocoons were slowly dried with 100$^{\circ}C$, the results of cohesion became much better. On the other hand, the results were considerably decreased in case the temperature with 115$^{\circ}C$. 2. Cooking of cocoon In case of the cooking of cocoon, we found that the result of cohesion was best with incomplete cooking, that of the control was next, while in over cooking, the results were very low. Also the results of cohesion were much better when using the method of over cooking with sericin arrestive agent than that of incomplete cooking with sericin agglutinating agent. 3. Reeling of cocoon A) When the temperature of reeling bath was 25-45$^{\circ}C$, the results of cohesion test were much better, but at the temperature below 25$^{\circ}C$ or above 45$^{\circ}C$, the results became worse. B) With out the process of croissieur, the results of cohesion were too bad, but in case of croissieur more than 1cm, cohesion became better rapidly. Further more, we understood that the results of cohesion were improving slightly with longer length of croissieur. C) When the velocity of reeling was increased, the results of cohesion also improved. The best results were shown when reeling velocity was 180-220 r.p.m. But when the velocity was increased more than 220 r.p.m., the results of cohesion got worse more or less. D) When the temperature of the drying pipe in reeling machine was raised, the results of cohesion also showed a tendency to improve. 4. Re-reeling A) We could net reach a conclusion as to have correlation between the number of dipping repeat in vacuum tank and the results of cohesion before re-reeling process. B) When we used Seracol 500 as an agglutination protective agent with l/1000 to l/2000 of water, the results of cohesion test were better. C) When we used Pearl-lite as an agglutination protective agent with 1/1000 to 1/2000 of water, the results of cohesion were considerably better. D) We gained tile best results when used Cohesion Improving Chemical, A-80, with 500-1500 times diluted. 1) Results of cohesion was improved when humidity was low or temperature was high in the rereeling machine. 5. Filature water A) The water pH near the isoelectric point of protein showed the best cohesion, but the farther water pH, the worser results were obtained. B) With the increasing of M-alkalinity in filature water, the results of cohesion were worse. Above all, we understood the tendency of the results of cohesion get worse when the M-alkalinity is increased above 200 ppm. C) By increasing the total hardness of the filature water, it improved the results of cohesion. Especially, when the total hardness was above 300ppm, the results were extremely high. 6. Effects combination of each results A) The result of effects combination in filature processes with the obtained best conditions was distinctively improved. But the results could not reach in mathematicaly double effect. When reelied under worse conditions, the results of cohesion test were too bad. There was "effect limit" for the promotion. B) Generally the results of cohesion were bad when the filature conditions(the temperature, pressure and the properties of water, etc) are processed as sericin loss to be high. On the other hand, the results were very good when lower sericin loss was controlled in filature conditions. C) When filature conditions such as reeling velocity and croissieur length provide pysical cohesion ability and when raw silk dry fast during reeling and re-reeling, we found the result of cohesion was better. 7. Correlation of silk testing items. A) A negative correlation exists between the results of cohesion test and cleanness defect. Another word, the result of cohesion test was found to be worse as cleanness defect increased. B) In cleanness, cohesion has negative correlation against the number of slugs, but we could not find any correlation against long loops, loose ends. C) Cohesion has negative correlation against average neatness and low neatness defect. The better the results of neatness respectively, the better the results of cohesion found. D) There is no correlation between tenacity and the results of cohesion test, but there was high positive correlation between the results of elongation and those of cohesion test. The more elongation, the better the results of cohesion was found.

지리정보기반의 재해 관리시스템 구축(I) -민간 보험사의 사례, 태풍의 경우- (GIS-based Disaster Management System for a Private Insurance Company in Case of Typhoons(I))

  • 자연재해 및 인위적 재해는 지리학에서 인문지리와 자연지리를 통합할 수 있는 주제로 기대되고 있으나 실제로 지리정보를 이용한 분석방법에 대한 연구와 시스템이 개발된 사례는 많지 않다. 태풍 루사와 매미가 국내 개인 및 국가에 입힌 손실만큼 손보사에게 끼친 손실이 막대하여, 보다 과학적이고 합리적인 자연재해 피해액에 대한 추정과 재보험 가격산정을 위한 시나리오 구성이 요구되었다. 태풍을 사례로 한 본 연구에서는 태풍경로에 따른 풍속예측모델을 적용하기 위하여 전국단위의 필요한 지리정보를 구축하였다. 1: 5,000 수치 지도를 기본지도로 사용하였으며, 기상자료 및 계약물건의 소재지에 대한 주소자료를 점형 자료로 구축하였으며, 과거 관측된 태풍의 주요 기압의 변화 값을 속성으로 하여 경위도 좌표로 선형 자료로 구축하였으며, 토지피복도는 풍속의 정확도를 높이기 위한 자료로 모델의 변수 조정에 사용하였다. 모든 자료를 전국을 1km 간격의 격자형자료로 변형하여 중첩할 수 있고, 태풍 풍속모델과 격자별 피해가능정도를 구할 수 있도록 하였다. 풍속에 대한모델의 정확도는 실제 기상측정지점의 측정값과 비교하여 검증과정을 거쳤으며(전체 평균 $R^2=0.68$), 변이가 큰 기상측정지점 변화를 준 보정과정을 통해 예측시스템의 정확도를 향상시켰다. 풍속에 따른 피해율을 적용한 피해민감도곡선을 주거지역, 공업지역, 기타지역으로 나누어 적용하고 실제 손해배상액과 비교해 본 결과, 과대평가된 부분과 과소평가된 부분을 동시에 관찰할 수 있었다 본 연구와 시스템 구축으로 민간보험사는 재보험 요율에 근거자료를 보유할 수 있을 뿐더러 유사 재해 시 대응할 수 있는 시나리오를 작동함으로 자원의 배분계획을 수립할 수 있고 대외적 신인도를 제고할 수 있을 것으로 예측된다. 향후 하천범람모형 및 태풍과 지진으로 인한 해일 모형, 내수 침수모형을 추가하여 종합적인 재해모형으로 완성할 예정이다.

만성폐쇄성폐질환 환자의 영양상태에 따른 안정시 폐기능 차이 (The Differences in Resting Pulmonary Function in Relation to the Nutritional status of Patients with Chronic Obstructive Pulmonary Disease)

  • 연구배경 : 만성폐쇄성폐질환 환자의 체중감소는 질환자체의 자연경과로 이해하고 있었으나, 체중이 감소된 환자들은 다른 질환에 대한 이환율이 증가하고, 폐성심이나 심부전으로 진행하는 경우가 많아 환자의 예후를 결정하는데 있어 중요한 요인으로 생각하게 되었다. 저자들은 생체전기 저항법률 이용하여 만성폐쇄성폐질환 환자의 영양결핍 정도와, 영양결핍과안정시 폐기능과의 관계를 알아보았다. 대상 및 방법 : 만성폐쇄성폐질환으로 입원한 중등도 혹은 중증 환자를 대상으로 급성기에 나타나는 증상이 호전된 후 퇴원하기 직전에 안정시 폐기능, 동맥혈 가스분석, 호흡 근육 강도, 그리고 체성분 분석을 시행하였다. 체생분분석 결과를 영양결핍과 정상상태로 구분하고 이 두 집단의 안정시 폐기능을 비교하였다. 결 과 : 대상 환자의 59%가 영양결핍 상태였으며, 저체증과 제지방량 결핍이 동시에 있는 경우가 31%이었다. 저체중만 있는 경우가 22%이였으며 제지방량 결핍만 있는 경우는 6%로, 전체 환자의 53%가 저체중이었고 37%의 환자가 제지방량 결핍이었다. 1초간 노력성 호기량 및 노력성 폐활량은 영양상태에 따라 차이가 없었으나, 최대호기유량은 영양겹핍환자가 유의하게 낮았다(p<0.05). 기도저항 및 폐 확산능, 그리고 최대흡기압 역시 영양결핍환자가 정상 환자보다 훨씬 낮았다(p<0.05). 동맥혈 산소분압 및 이산화탄소분압은 두 집단사이에 차이가 없었으며, 잔가량 및 총폐용량은 영양상태에 따라 차이는 없었으나, 영양결핍 환자들이 낮은 경향이었다. 결 론 : 영향결핍은 폐기능저하와 밀접한 관계가 있으며, 만성 폐쇄성폐질환 환자의 예후를 결정하는데 중요하게 작용할 것으로 생각된다. 또한 만성폐쇄성폐질환 환자의 치료에 기류제한의 완화 및 급성악화의 조절과 더불어 근육량 및 활동능력을 유지하기 위하여 영양학적인 측면을 고려하여야 할 것으로 생각된다.

전척수(全脊髓) 및 경막외차단(硬膜外遮斷)으로 편타성(鞭打性) 손상(損傷)의 통증치험(痛症治驗) (4례(例) 보고(報告)) (Total Spinal Block and Cortical Epidural Block for Whiplash Syndrome and Reflex Sympathetic Dystrophy (Report of Four Cases))

  • For the relief of pain in 3 cases of whiplash syndromes (case I, II and IV) and in one of reflex sympathetic dystrophy (case III), we have carried out six intentional. total spinal blocks (TSB) which attempted two times in case I, three in case II and one in carte III whoso various symptoms were chronically unresponsive to the usual conservative treatments, and a time of cervical epidural and right suprascapular nerve block in case W whose acute symptom lasted 4 drys following the cervical injury (see fables from 1 to 9). During the 753, we have observed clinically the sequential charges of respiration, lid and pupil reflexes, body motion and consciousness. And checked the blood pressure, pulse rate and arterial Pco2. The effectiveness of those blocks has been assessed by using the Visual Analog Scale which is designed to measure the patient$\acute{s}$ subjective intensity of pain and also we have found out the sequelae following those blocks. The methods of the blocks were as the following: 1. Under the N.P.O. for 8~10 hours, the preparations of immediate cardiopulmonary resuscitation and premedication with atropine 0.5mg at thirty minutes before the TSB, it was performed by injecting the mixture of 2% mepivacaine 10 or 15ml and normal saline 10 or 5ml through No. 23 G. spinal needle into the subarachnoid space of $C_7-T_1$ interspinous region with fully flexed neck on the lateral posture. Immediately after the injection of the local anesthetic in the lateral position, the patient$\acute{s}$ were hasten to change Trendelenburg$\acute{s}$ position in order to act the drugs cephalad and to make easy controlled respiration with oxygen. 2. The cervical epidural block was done by injecting the mixture of 0.5% bupivacaine 4ml, normal saline 4ml and triamcinolone 15mg through No. 18 G. Tuohy needle into the epidural space on the same region and posture as the above without premedication.7he suprascapular nerve block was done by injecting of 0.5% bupivacaine 3ml only into the right suprascapular fossa on the sitting posture. The results were as the following: 1. The cessation of respiration was seen within 5 minutes following the subarachnoidal injection of the above 20ml mixture in 2 to 3 minutes and then soon the consciousness began to disappear. The loss of Lid and pupil reflexes noted between 5 to 10 minutes and the size of the dilated pupils was equal between 5 to 20 minutes, but the pupil of the dependent side on tile lateral position was dilated 1 to 3 minutes earlier than that of the independent. The patients had r=ever responded to any stimulations during the TSB except their heart funtion. 2. The recovery of the TSB was as the following, firstly the ankle and lower limb of the independent side began to move slightly with in 34 to 75 minutes after the injection and then that of the dependent Secondly the neck and upper limb moved 6 to 15 minutes later than the lower limb. Thirdly the self respiration began to appear between 40 to 80 minutes from the block. The lid and pupil reacted to touch and light respectively between 40 to 80 minutes but the pupil of the independent side responded earlier than that of the depends. Lastly the consciousness recovered completely between 80 to 125 minutes from the block. 3. In the cardiopulmonary function during the TSB, the blood pressure were stable except the 210/130 tory at the and block of case I. There were bradycardias between 65 to 85 minutes in case I and II but no arrythmia on the EKG. The level of the arterial Pco2 was maintained to 43~45 torr during the TSB. 4. The effectiveness of the above blocks was no pain(0%) in case IV, and light (10~20%) in case I and II but no improvement in case III. 5. The right arm weakness has been complicated as to be Injected accidently the "COLD" local anesthetic at the End block of case I.

조기 후두암 환자에서 보전적 후두수술 후 음성 변화 (The Voice Change after Conservative Laryngeal Surgery)

  • Objectives : The total laryngectomy for laryngeal cancer has made patients be afraid of voice loss. Early staged glottic or supraglottic cancer can be treated with conservative laryngeal surgery which preserve voice, though which was not normal voice comparing before. Voice analysis is used to evaluates objectively the quality of the voice in pre- and postoperation, 4 different types of conservative laryngeal surgery : laser cordectomy, supracticoid partial laryngectomy, vertical partial laryngectomy, and supralottic laryngectomy. Materials and Methods : The patients who received conservative laryngeal surgery(laser cordectomy : 23 cases, vertical partial laryngecotmy : 9cases, supracriocoid partial laryngectomy : 6cases, supraglottic laryngectomy : 8cases) from 1995 to 2001 in the Asan medical center. Fundamental frequency(F0), shimmer, jitter, noise to harmony ratio(NHR), maximum comfortable phonation time and subglottic pressure were used as parameters for voice analysis. Results : The patients who received laser cordectomy(shimmer : 5.26${\pm}$1.12%, jitter : 3.33${\pm}$0.42%, NHR : 0.47${\pm}$0.02, MPT : 9.32${\pm}$3.59sec) and supraglottic laryngectomy(shimmer : 4.39${\pm}$1.03%, jitter : 1.49${\pm}$0.14%, NHR : 0.51${\pm}$0.06, MPT : 8.9${\pm}$0.59sec) showed better results than other two procedures, but differed from normal value. Especially the patients who received supracricoid partial laryngectomy(shimmer : 9.23${\pm}$1.56%, jitter : 5.81${\pm}$1.23%, NHR : 5.89${\pm}$1.13, MPT : 6.3${\pm}$1.18sec, MFR : 632${\pm}$89ml/sec) had poorer quality of voice but presented fast functional recovery time, and the subjective symptom was improved as time goes by slowly. Conclusion : The appropriate conservative laryngeal surgery for each cancers and stage can preserve the acceptable voice for patients. Supracricoid partial laryngectomy for T1b glottic cancer can be used for acceptable voice despite its poor voice analysis.

  • Background: Ma-huang (Ephedra sinica) has been widely used to treat respiratory disease in oriental medicine for over a hundred years. Ma-huang preparations contain approximately 1.25% ephedrine alkaloids. Recently, the ephedra alkaloids have received much press lately due to adverse effects in those using whole extracts as 'dietary supplements' for weight loss or athletic performance enhancement, and these reports are troubling given the increasing use of Ma-huang by the general public. The purpose of this report is to determine the proper dosage to minimize adverse effects and maximize the potential curative value. Objectives : The object of this study was to find an effective yet low risk dosage of Ma-huang. Methods : The study was designed as a double-blind randomized placebo-controlled trial. The subjects of this study were 26 adults between 20 to 40 of age who agreed to participate in this study. They were allocated through randomization into three groups. Each group took three opaque capsules three times a day. A group (N=9) took one Ma-huang capsule and two placebo capsules, B group (N=8) took two Ma-huang capsules and one placebo capsule, C group (N=9) took three Ma-huang capsules. The total trial periods was two days. To compare the adverse effects of Ma-huang according to dosage, blood pressure and pulse were checked, and other adverse effects were assessed using a morning questionnaire, patient's global assessment scale and Wong-Baker faces pain rating. Results : The following result were obtained: 1. After taking 18 g of Ma-huang per day, pulse rate had a significant increase. 2. After taking more than 6 g of Ma-huang per day, palpitation would be increased significantly. 3. After taking more than 18 g of Ma-huang per day, tiredness would be increased significantly. Conclusion: According to the results, 12 g of Ma-huang per day will minimize adverse effects and maximize the potential curative value.
