The choice of suitable hip implant is one of important factors in the total hip arthroplasty (THA). In clinical view point, an improper adaptation of hip implant might induce abnormal stress distribution to the bone, which can shorten the lifespan of replaced hip implant. Currently, interest in the custom-designed hip implants has increased as studies reveal the significance of geometric shape of patient's femur in modeling and designing the implants. In this study, we have developed custom-designed hip implant models with various sizes, and analyzed the stress distribution in the bone and bone cement using the Finite Element Method. It was found that minimizing the gap between implant stem and femoral cavity is crucial to minimize the stress concentration in the bone.
PURPOSE: The aims of this study were to assess the intra-tester and inter-tester reliability and validity of hip range of motion using a smart phone in hip operative patients. METHODS: Twenty-five patients (eight total hip arthroplasty and seventeen femur neck fracture) participated in this study. The range of motions in active and passive hip flexion, abduction and external rotation were measured with a goniometer and smart phone over two times by two observers. The intra-tester and inter-tester reliability were evaluated using the intraclass correlation coefficient (ICC2,1). The validity was measured by Pearson's correlation coefficient RESULTS: The intra-observer reliability was good in all measured items (ICC>0.78). The inter-observer reliability was high with ICC (>0.90). All correlation coefficients of smart phone and goniometer was greater than 0.85 and showed a significant positive correlation (p<0.01). CONCLUSION: The range of motion measurement with a smart phone showed acceptable reliability. Therefore, it could be convenient and have economical benefits to measure the range of motion of the hip joint using a smart phone.
With the goal of increasing the survivorship of the prosthesis and anticipating primary stability problems of new prosthetic implants, finite element evaluation of the micromotion, at an early stage of the development, is mandatory. This allows assessing and optimizing different designs without manufacturing prostheses. This study aimed at investigating, using finite element analysis (FEA), the difference in the prediction of the primary stability of cementless hip prostheses implanted into a $Sawbones^{(R)}$ 4th generation, using the manufacturer's mechanical properties and using mechanical properties close to that of human bone provided by the literature (39 papers). FEA was carried out on the composite $Sawbones^{(R)}$ implanted with a straight taper femoral stem subjected to a loading condition simulating normal walking. Our results show that micromotion increases with a reduction of the bone material properties and decreases with the augmentation of the bone material properties at the stem-bone interface. Indeed, a decrease of the cancellous Young modulus from 155MPa to 50MPa increased the average micromotion from $29{\mu}m$ up to $41{\mu}m$ (+42%), whereas an increase of the cancellous Young modulus from 155MPa to 1000MPa decreased the average micromotion from $29{\mu}m$ to $5{\mu}m$ (-83%). A decrease of cortical Young modulus from 16.7GPa to 9GPa increase the average global micromotion from $29{\mu}m$ to $35{\mu}m$ (+33%), whereas an increase of the cortical Young modulus from 16.7GPa to 21GPa decreased the average global micromotion from $29{\mu}m$ to $27{\mu}m$ (-7%). It can also be seen that the material properties of the cancellous structure had a greater influence on the micromotion than the material properties of the cortical structure. The present study shows that micromotion predicted at the stem-bone interface with material properties of the $Sawbones^{(R)}$ 4th generation is close to that predicted with mechanical properties of human femur.

For developing navigation system of total hip arthroplasty (THA) and evaluating hip joint kinematics, 3-D pose position of the femur and acetabulum in the pelvic coordinate system has been quantified. The pelvic coordinate system is determined by manually indicating pelvic landmarks in multidetector-row computed tomography (MDCT) images. It includes intra- and inter-observer variability, and may result in a variability of THA operation or diagnosis. To reduce the variability of pelvic coordinate system determination, this paper proposes an automated method in MDCT images. The proposed method determines pelvic coordinate system automatically by detecting pelvic landmarks on anterior pelvic plane (APP) from MDCT images. The method calibrates pelvic pose by using silhouette images to suppress the affect of pelvic pose change. As a result of comparing with manual determination, the proposed method determined the coordinate system with a mean displacement of $2.6\;{\pm}\;1.6$ mm and a mean angle error of $0.78\;{\pm}\;0.34$ deg on 5 THA subjects. For changes of pelvic pose position within 10 deg, standard deviation of displacement was 3.7 mm, and of pose was 1.28 deg. We confirmed the proposed method was robust for pelvic pose changes.
The experimental comparison between bonded and unbonded types stem-cement interface was carried out on axisymmetric stem-cement-aluminum model of the femoral component of a total hip replacement. Human femur was modeled in non-tapered and tapered($7.5^{\circ}$) aluminum hollow cylinders to emulate the diaphyseal and metaphyseal segments of the femur. For unbonded type, we tested stems with three different taper angles($5^{\circ},\;7.5^{\circ},\;10^{\circ}$). In every case, the cement-aluminum interface was designed to endure 8MPa shear strength. (a measured value at cement-bone interface) We tested aluminum models under axial loading for both cases. As an experimental result, it was found that unbonded stem sustained more axial load as bonded stem in both cases, diaphyseal and metaphyseal models. The unbonded types failed in cement mantle under axial compressive load, while the bonded ones failed in shear at cement-aluminum interface. These results suggest that a polished stem will sustain much higher axial load than a roughened stem. And a polished stem will make more stable cement-bone interface that may promote better osteosythesis around the stem.
In orthopedic surgery and in particular in total hip arthroplasty, the implant fixation is carried out using a surgical cement called polymethylmethacrylat (PMMA). This cement has to insure a good adhesion between implant and bone and a good load distribution to the bone. By its fragile nature, the cement can easily break when it is subjected to a high stress gradient by presenting a craze zone in the vicinity of inclusion. The focus of this study is to analyze the effect of inclusion in some zone of cement in which the loading condition can lead to the crack opening leading to their propagation and consequently the aseptic loosening of the THR. In this study, the fracture behavior of the bone cement including a strange body (bone remain) from which the onset of a crack is supposed. The effect of loading condition, the geometry, the presence of both crack and inclusion on the stress distribution and the fracture behavior of the cement. Results show that the highest stresses are located around the sharp tip of bony inclusion. Most critical cracks are located in the middle of the cement mantle when they are subjected to one leg standing state loading during walking.
주관절 인공 관절 치환술은 고관절이나 슬관절 인공 관절 치환술에 비해 아직은 발전 단계에 있는 수술이다. 많은 임상 경험을 통해 여러 형태의 치환물이 개발되어 사용되고 있지만, 이에 대한 장기 추시 결과는 여전히 부족한 실정이다. 현대의 주관절 치환물은 크게 두 가지 디자인 범주로 나누어지는데, 연결형 치환물과 비연결형 치환물이 그것이며, 이들은 다른 말로 반구속형과 비구속형으로 불리기도 하며, 이에 추가적으로 비구속형/구속형 전환 가능한 치환물도 사용되고 있다. 연결형 치환물과 비연결형 치환물의 선택은 우선적으로 주관절의 안정성 및 골성 구조물의 적절성에 의해 결정된다. 연결형 치환물의 경우 전통적으로 마모, 조기 이완, 해리 등과 같은 합병증으로 인해 장기 추시 결과가 좋지 않았던 것이 사실이다. 하지만, 최근 들어 적절한 환자의 선택, 새로운 디자인의 개발, 수술 술기의 발달, 그리고 수술 후 적절한 재활 치료 등으로 인해 이러한 문제점이 많이 해결되었다. 하지만, 여전히 신체적인 활동에 대한 요구가 높은 환자에 있어서는 마모나 이완 등과 같은 문제점으로 인해 연결형 치환물이 적절한 선택이 되지 못하는 경우가 있다. 본 종설에서는 연결형 (반구속형) 주관절 인공 관절 치환물에 대해서 자세히 살펴보고자 한다.
Based on the present FEM study for negative Poisson's-ratio UHMWPE, the following conclusions seem expected. 1) Negative Poisson's-ratio UHMWPE transfers less stresses to the subchondral or peripheral iliac bone, compared to the conventional UHMWPE with Poission's-ratio. 2) Negative Poisson's-ratio cup reduces stresses in UHMWPE cup itself as well as metal backing, and subchondral bone. 3) The reduction in periacetabular mechanical stresses would significantly reduce the rate of fatigue failure and consequently reduce the incidence of aseptic loosening of the cup due to wear or bone resorption.
Purpose: Periprosthetic femoral fracture (PFF) is a common complication after total hip arthroplasty, and open reduction and internal fixation (ORIF) is a common surgical treatment. We conducted a meta-analysis to compare the outcomes of ORIF in patients with different fracture patterns (Vancouver B1 and B2). Materials and Methods: We conducted a systematic search of PubMed, Embase, Cochrane Library and KoreaMed from inception to August 2022. We conducted a pair-wise meta-analysis (with a fixed-effects model) on the 10 comparative studies and a proportional meta-analysis on the data from the 39 articles to determine a consensus. The outcomes were the incidence of reoperations that included osteosynthesis, irrigation/debridement and revision arthroplasty. Results: The pair-wise meta-analysis showed similar outcomes between two groups; the risk of reoperation (odds ratio [OR]=0.82, confidence interval [CI] 0.43-1.55, P=0.542), nonunion (OR=0.49; CI 0.22-1.10, P=0.085) and deep infection (OR=1.89, CI 0.48-7.46, P=0.361). In proportion meta-analysis, pooled prevalence of reoperation was 9% (95% CI, 6-12) in B1 and 8% (95% CI, 2-15) in B2 (heterogeneity between two groups (Q), P=0.772). The pooled prevalence of nonunion was same as of 4% in B1 and B2 (Q, P=0.678), and deep infection was 2% (95% CI, 1-3) in B1 and 4% (95% CI, 2-7) in B2 (Q, P=0.130). Conclusion: ORIF is a feasible treatment for B1 and B2 periprosthetic femoral fractures, with acceptable outcomes in terms of, nonunion and infection. The results of this study would help clinicians and provide baseline data for further studies validating PFF.
연구계획: 골반골의 악성골종양의 재건술에서 체외열처리를 이용한 재활용 자가골 이식술 및 인공 관절 치환술을 시행한 3례의 후향적 연구 연구목적: 골반골의 악성골종양의 재건술에서 체외열처리를 이용한 재활용 자가골 이식술 및 인공 관절 치환술이 골반골의 재건에 있어 그 유용성을 알아보고자 하였다. 증례: 증 례 1~20세 여자 환자로 3개월 전부터 시작된 우측 고관절부와 대퇴부 동통을 주소로 외부 병원에서 우측 장골에 소파술 및 골 시멘트 충전술 시행 후 악성 골종양으로 진단하에 술 후 방사선 치료 6회 시행 후 전원되어 우측 장골 광범위 절제술 및 섭씨 132도에서 2분간 열처리 후 자가골 재삽입술 시행하고 우측 고관절의 인공 관절 치환술 (ABG$^{(R)}$)을 병행하였다. 수술 당시 전이의 증거는 없었으며 술후 조직 병리 검사상 고분화 골육종 진단되었으며 술전 또는 술후 화학 요법은 시행하지 않았다. 증 례2~56세 여자 환자로 약 3개월 전부터 시작된 우측 대퇴부 동통으로 방사선 검사상 이상 발견되어 전원 후 절개 생검 시행하였다. 결과상 골육종 진단되어 우측 골반골 광범위 절제술 후 섭씨 6 5도 하에서 3 0분간 저온 열처리 후 자가골 삽입 고정술 시행하고 우측 고관절에 대하여 인공 관절 치환술 (ABG$^{(R)}$)을 병행하였다. 수술 당시 전이의 증거는 없었으며 술 후 조직 병리 검사상 고등급 섬유모세포형의 골육종이 진단되었다. 술후 화학 요법은 HDMTX, ADR, CDDP으로 시행하였다. 증 례 3~46세 여자 환자로 우연히 발견된 좌측 장골의 종괴로 연골육종 의심 하에 좌측 골반골 광범위 절제술 후 섭씨 65도 하에서 30분간 저온 열처리 후 자가골 삽입 고정술 시행하고 좌측 고관절에 대하여 인공 관절 치환술 (Protek$^{(R)}$)을 병행하였다. 수술 당시 전이의 증거는 없었으며 술후 조직 병리 검사상 II/III 등급의 연골육종이 진단되었다. 결과: 최종 추시상 각 증례의 종양학적 및 기능적 결과는 증례 1은 7년 추시상 국소적 재발이 없는 상태였으며 Ennecking 등에 따른 기능적 평가 지수에서 53%로 평가되었다. 방사선 추시상 약 1년에서 1년 6개월 사이에 골유합이 관찰되었으며 최종 추시시 장골의 재건 금속판의 파괴 소견이 보였으나 환자의 증상과 연관되지는 않았다. 증례 2는 3년 6개월 추시상 국소적 및 원격 재발이 없는 상태였으며 기능적 평가 지수는 60%로 평가되었다. 증례 3은 7개월 추시상 국소적 및 원격 재발이 없는 상태였으며 기능적 평가 지수는 63%로 평가되었다. 결론: 체외 열처리를 이용한 재활용 자가골 이식술 및 인공 관절 치환술을 시행한 3례에서 비교적 만족할만한 종양학적 및 기능적 결과를 보였으며 한국에서 동종 골 이식술이 어려운 점을 감안할 때 이 방법은 골반골을 재건하는데 한 방법이 될 수 있을 것이라 사료된다.
